| Description | THE CENTER PROVIDES HIGH QUALITY PRIMARY CARE AND RELATED SERVICES FOR MEDICALLY UNDERSERVED PERSONS AND PROMOTES ACCESS TO HEALTH SERVICES IN NINE HEALTH CENTERS LOCATED IN THE GREATER BALTIMORE AREA.OVER THE PAST YEAR THC SERVICED 32,927 PATIENTS WITH A TOTAL OF 147,783 VISITS. THE VISITS CONSISTED OF 59% MEDICAL, 12% DENTAL, 12% MENTAL HEALTH AND 17% SUBSTANCE ABUSE. OF THE PATIENTS SERVICED, 55% WERE MEDICAID, 12% WERE SELF PAY, 7% WERE THIRD PARTY, 13% WERE PAC, 4% WERE MEDICARE AND 9% WERE PATIENTS SEEN UNDER GRANTS. THC ALSO PROVIDES OUTREACH SERVICES TO ITS PATIENTS AS WELL AS IN THE SURROUNDING COMMUNITIES. THC ALSO HAS A POPULATION OF HIV PATIENTS THAT ARE SERVICED. PATIENTS ARE PROVIDED COUNSELING, MEDICAL SERVICES, OUTREACH SERVICES, HOUSING ASSISTANCE, AND PHARMACY ASSISTANCE. THESE SERVICES ARE CONTINGENT UPON GRANT FUNDS. | Description |
