Part I, Line 7g: Subsidized health services:Emergency Services provided on a 24 - 7 basis and Ambulatory Surgery incur operating losses which are considered subsidized health services.
Part II: Part II Community Building Activities:Soldiers & Sailors Memorial Hospital's Community Building Activities promote the health of the communities it serves through:1. Physical Improvements and Housing: N/A2. Economic Development: Soldiers & Sailors Memorial Hospital (SSMH) supports economic development in the community, via its membership in the Chamber of Commerce.3. Community Support: To be equipped for disaster situations, SSMH participates in emergency preparedness/disaster readiness activities, including disaster drills; stockpiling essential supplies for use during a disaster; training staff for disasters; and conducting POD events as training in the event of a disaster requiring mass immunizations. 4. Environmental Improvements: SSMH accepts sharps from community households and provides proper disposal of same at no cost to the households. 5. Coalition Building: Finger Lakes Health, which includes SSMH, Administrative Team members participate in multiple community organizations/partnerships for the betterment of the communities it serves, including ARC, Happiness House, Finger Lakes Visiting Nurse Service, Lakeview Mental Health, Success for Geneva's Children, Hobart William Smith Advisory Board, among others.6. Workforce Development: SSMH provides Career Exploration programs for young people, including:- tours of departments highlighting career options for students in the home-schooled association- Camp MedQuest, a two day healthcare career exploration program for high school students- shadowing opportunities for high school students.7. Negative Margins: SSMH provides Advanced Life Support at an operating loss to improve the well being of the community. Also, SSMH Community Services staff time is devoted to planning and implementing community outreach activities such as the Walk & Talk program and Keeping U in the Game, and completing Community Benefit reports.
Part III, Line 4: Provision for Doubtful Accounts Footnote:The Hospitals, Living Centers and the Homestead estimate the amount to be recognized in the combined financial statements for doubtful accounts by analyzing specific patient and resident accounts for collectability and also by applying historically established percentages for self-pay and various third-party payers to the aging categories in the accounts receivable aging for each facility.Beginning in 2012 bad debts are recorded as a deduction from revenue rather than as an expense.Part III, Section A, 3Costing Methodology:HFMA P&P Board Statement 15, IX. Valuation and Disclosure of Bad Debts, 9.1 has been applied to the amount reported on line 2. The bad debt amount is recorded at the amount that the payer is expected to pay. The amount reported on line 3 is valued at patient care cost, i. e. the expected payment amount included in line 2 has been multiplied by the ratio of patient care cost to charges (54%). Based upon an analysis of income levels for accounts included in bad debts we estimated that 31% were eligible under the financial assistance program.Rationale for including a portion of bad debt amount as community benefit:It is not always possible to obtain sufficient information to make a determination of eligibility for financial assistance or reclassify bad debts as charity care. When income levels are learned we identify individuals unable to pay for services. Having provided services to those individuals we believe represents a community benefit.
Part III, Line 9b: Collection practices for patients who are known to qualify for financial assistance:After the Customer Service Representative's initial determination, or the determination rendered after an appeal review, the patient's hospital account is adjusted to reflect the amount of any financial assistance deduction to be applied to it. The patient is billed for any remaining amount due, and is offered the telephone number of an individual within Patient Accounts who is available to assist the person in creating a self-payment plan that outlines patient re-payment steps over a specified period of time should the patient require such assistance. Individuals with valid requests for financial assistance are exempt from referrals to a collection agency.
Part V, Section B, Line 20d: Maximum amount charged FAP-eligible individuals:Presently we apply a percentage discount to published charges for patients eligible for assistance under our financial assistance policy. Part V, 20Amounts billed greater than insurance coverage:Because we apply a percentage discount to published charges any individual patient's discounted amount may or may not be more than the amounts generally billed to individuals who had insurance covering such care.Soldiers and Sailors Memorial Hospital
Part V, Section B, Line 21: Amounts generally billed:Presently we apply a percentage discount to published charges for patients eligible for assistance under our financial assistance policy. Any individual patient's discounted amount may or may not be more than the amounts generally billed to individuals who had insurance covering such care.Soldiers and Sailors Memorial Hospital
Part VI, 2 (Needs Assessment Continued)Finger Lakes Health carries out various programming, as well as partnering with community agencies to address the needs identified in the Community Health Needs Assessment. To address Access to Care Finger Lakes Health has increased the number and location of health providers and services available to residents in our service area.Success has been achieved with the recruitment of physicians and allied health professionals, as well as the addition of hours by health system specialists in areas outside their main practice locations.Finger Lakes Health also implemented new transportation services in Yates County that to help link Dundee area residents in southern Yates County, to the primary care and specialty services offered in Penn Yan.Specialty services have been expanded to include: - Cardiology services are offered in Penn Yan (northern Yates County), Dundee (southern Yates County), and Clyde (Wayne County), and in the main Geneva office (Ontario County). Our cardiologists provide services to patients on a regular schedule close to home, which is vital in our rural area where transportation is often a barrier to patients receiving medical care in a timely fashion.- Nephrology services are offered in Geneva, Penn Yan and Waterloo. Additionally, FF Thompson Hospital contracts with Finger Lakes Health for these services to be offered in Canandaigua.- Our Physiatrist sees patients in Penn Yan, providing pain management services, in addition to the main practice office in Geneva.- Orthopedic services are now offered in Geneva, Clifton Springs and Penn Yan.Primary Care services: - In addition to the extension of specialty services to more areas, Finger Lakes Health recruited multiple primary care physicians to continue and grow access to those services. A Family Medicine physician began practicing in Penn Yan, replacing a retiring practitioner who maintained the largest family practice in the area; two family medicine physicians began practicing in Geneva, and an internist joined the Finger Lakes Health family of providers in Clifton Springs.- Additionally, Finger Lakes Health continues to provide access to primary careservices through our three Article 28 clinics in Clyde, Dundee and Waterloo. These clinics accept all insurances, including Medicare, Medicaid, Medicaid Managed Care, and a sliding scale for self-pay patients.- Limited transportation services in Yates County have created a barrier to Dundee area residents getting to medical services. A shuttle bus service was implemented in the fall of 2012 to transport Dundee residents to primary care, specialists, and medical testingservices in Penn Yan.- To provide access to services when primary care offices are closed evenings and weekends, a second Urgent Care Service was opened in Clifton Springs. Additionally, both the Geneva and Clifton Springs Urgent Care programs increased the number of days each week the service is offered. To address Chronic Disease, one of our key approaches relates to education, screening, and diagnosis. Therefore Finger Lakes Health has coordinated several important free health screening and public education events.- Heart disease being the number one cause of premature death, Finger Lakes Health initiated a new program to help prevent heart disease through promotion of health and fitness. Cardiac Rehab to PRE-Hab was conducted in Seneca County, because of exceptionally high risk factors for heart disease in that county. Participants spent one session per week for 10 weeks learning about risk factor modification, learning about the benefits of physical activity and healthy nutrition/cooking, and strategies for encouraging others to lead a healthy lifestyle. The program provided screenings, along with the educational and fitness components. Finger Lakes Health plans to introduce the program in other parts of its service area in the future.- Finger Lakes Health's Diabetes Services program offers "Living with Diabetes", a comprehensive class for diabetics and their families several times during the year. Additionally, the program has certified diabetes educators (BS, RN and dietician) who conduct one-on-one counseling with diabetes and their families, as well as offering a free support group to diabetes and their families monthly throughout the year in two communities.- We offer a Stroke Support Group monthly for stroke survivors, their family and caregivers.- A blood pressure screening is held in conjunction with the Stroke Support Group, as well as by our Cardiac Rehab staff on a regular schedule for community members, and at the Community Lunch Program in Geneva on a monthly schedule, as a way to provide the free screening to community members who are uninsured or underinsured. - A community CPR class is held annually during February - Heart Month.- Finger Lakes Health staff participated in a regional Breastfeeding Partnership Coalition to promote the nutritional and health benefits of breastfeeding for both infants and mothers.- Finger Lakes Health sits on a regional Dental Steering Committee that works to promote the importance of regular dental check ups as a way to prevent other health issues. The group also works toward increasing access to dental services in local communities.- Finger Lakes Health's Dine & Discuss lecture series covers many topics, including several directly related to chronic disease. For instance, during 2012, physicians presented programs to the community on: Chronic Obstructive Pulmonary Disease, Stress Management, which can lead to and/or make many chronic diseases worse, Arthritis, Making the Most of Your Doctor's Visit, and the Whether or Not Vitamin Supplements are Necessary.- Education is offered through Finger Lakes Health' community health magazine "Thrive". In 2012, features included tips on getting healthy from the CEO - "Advice to Thrive On"; information on kidney failure and dialysis; a Turn off the TV challenge to encourage less TV and more fitness. To impact Fitness & Nutrition:- Geneva General Child Care Center holds an annual 5K Cubby Chase and Kid Fun Run. The event promotes a fun way for families to work together to achieve improved fitness. - Keeping U in the Game was offered by Finger Lakes Health to young athletes, their families, athletic trainers, coaches, athletic directors, and the community at large. The program featured three physicians speaking on concussion prevention, diagnosis and treatment; the athlete's heart, and preventing sports injuries.- Finger Lakes Health had staff trained in the Cornell Nutrition Works program Preventing Childhood Obesity: An Ecological Approach. That staff then developed a plan to train high school age students on preventing childhood obesity. Those students in turn will serve as mentors for elementary-age students to teach the good habits that will prevent obesity in the future.Covering all of the needs is Finger Lakes Health's "Walk & Talk for Health" program. Aimed at getting community members to participate in an easy exercise regimen to improve their overall health and fitness, Walk & Talk provides and encourages regular fitness and good nutrition. Additionally, regular exercise is known to help reduce the occurrence of many chronic diseases. And, each walk in the Walk & Talk program is led by healthcare professionals. Thus, walkers were offered increased access to care through the opportunity to meet healthcare providers who led the walks and answered questions from participants. Walks are held in three communities (Geneva, Waterloo, and Penn Yan).- As one of the largest employers in some of our counties and in the region, Finger Lakes Health also initiated a "Walk at Work" program to engage health system employees in improving their health.- Diagnosis being one of the keys to chronic disease prevention and control, the Walk & Talk and Walk at Work programs also included a screening component. All participants (200+ community members and 300+ employees) were invited to have blood pressures taken, BMIs calculated, and blood draws done to test cholesterol and A1C levels. We are continually addressing the needs identified in the CHNA. The challenge lies in having a significant impact on the health-related behaviors of the population in our service area. For instance, we get them walking in the Walk and Talk program, held every other week and encourage them to count steps in between each walk and to form their own walking groups outside of our program. We have found some individuals and small groups are doing that, but we continue to struggle with impacting the masses.Additionally, the screenings are offered to all participants. Many do the first blood draws and get a baseline reading, but few complete the second blood draw to see how exercising and improved nutrition have impacted their risk factors.
Part VI, Line 2: Needs assessment:It is vital that we focus our resources and efforts on areas of health and wellness that matter most in our communities. Therefore, we conduct needs assessments, includingpatient and community surveys and reviewing research data on disease prevalence. In addition, our long-standing Community Advisory Committee, a sub-committee ofour Board of Directors, engages community members to identify community needs and solutions. In 2009, along with other health systems and public health departments in New York State, we used the framework of the NYS Health Commissioner's "Prevention Agenda" as a basis for our community health planning. By drawing upon the public health's prevention agenda, studying data collected through primary and secondary research, and partnering with local health departments in Ontario, Seneca and Yates counties, the health priorities for 2010-2013 were identified. Our community needs outreachefforts for 2010-2013 were aligned with these priorities.Members of the Finger Lakes Health Community Services Department staff serve as representatives at meetings in each of our partner counties (Choose Health Ontario, Yates Health Planning Council, and Seneca County Health Improvement Group) to conduct the needs assessments, review the information gathered from the assessments and in turn determine the health priorities. Those same teams also work together to implement plans to meet the needs identified in the assessments. The priorities established through this process for 2010-2013 that are reflected in our current report, were:-Access to quality health care-Chronic disease management, with emphasis on cardiovascular health-Physical activity and nutrition educationIn 2012-2013, Finger Lakes Health, along with other health systems and public health departments in New York State, again used the framework of the NYS Health Commissioner's "Prevention Agenda" as a basis to conduct another needs assessment to inform our community health planning for the coming years. By drawing upon the public health's prevention agenda, studying data collected through Tools used to conduct the CHNA and get it to the largest group possible, included:-An online survey available to anyone in the community. Promoted via newspaper, email blast to employees, Constant Contact email blast to community members on the email list, in our Spring 2012 Thrive community health magazine, circulated to 90,000 households in our service area.-Focus groups done in cooperation with our county partners targeting specific populations, including the African American Men's Society of Geneva, a Head Start parents' group, the Office for the Aging, and human service agencies, such as ARC, FLACRA, etc.-Distribution of printed copies of the survey via waiting rooms in our facilities and to individuals without Internet access.-Survey of area healthcare providers and human services agencies via email distribution of survey and promotion at various health planning group meetings in the various counties.Once the survey process was completed, the results were tabulated by the S2AY Rural Health Network, which works with multiple counties in the Finger Lakes. Then, individuals who are part of each county - Ontario, Seneca and Yates, steering groups (mentioned above) ranked the issues which were identified through data review, surveys to the public and through focus groups. The ranking was completed using the Hanlon Method. After the issues were ranked, we determined the two health prevention priorities that our Community Health Improvement Plan will be based on.Those priorities by county in Finger Lakes Health's service area are:Yates County:-Obesity prevention-Hypertension-Unintentional InjuryOntario County:-Hypertension/stroke-Obesity, with a focus on childhood obesitySeneca County:-Prevention of Chronic Disease-Promotion of mental health-Prevention of substance abuse A draft Community Health Improvement Plan is currently being developed, which will outline how we intend to meet the priority needs. Implementation of programming to meet those needs will follow.(Description Continued)
Part VI, Line 3: Patient education of eligibility for assistance:Financial assistance under the organization's policy is available to assist uninsured or underinsured members of our community who are willing but unable to pay in full for their care. The program allows a person to receive medically necessary and emergency services at no charge or reduced charges when they meet certain eligibility requirements. Information about the services is available through signage and applications placed in the emergency department, registration areas, and the patient accounts office. We have also published this information in direct mail sent to area homes and also promoted the program on our local radio show. The hospital's website contains information on financial assistance.
Part VI, Line 4: Community information:Soldiers & Sailors Memorial Hospital provides a full range of acute care services to residents of the Finger Lakes region in Upstate New York. The hospital reviews demographic data from sources such as the US Census to identify trends. The hospital uses health indicators and utilization data from numerous sources and receives community input from public surveys. We have a Community Advisory Committee which serves as a regular forum for community input and fostering collaboration. We gather opinion related data from focus groups, satisfaction surveys, and employee opinion surveys. We participate in a Needs Assessment Process and participate as an active member of the community through board participation on numerous local and state-wide health related organizations.
