| Form 990, Part VI, Section B, line 11 | Information to complete the Form 990 is gathered by the Director of Finance and provided to the Foundation's accounting firm for preparation of the Form 990. The Director of Finance reviews the return and requests any changes, if necessary, from the Foundation's accounting firm. Once the Form 990 has been reviewed and corrected as needed, it is then presented to the Finance and Audit Committee for review. If there are any concerns from the committee, they are addressed prior to filing with the Internal Revenue Service. |
| Form 990, Part VI, Section B, line 12 | The Foundation has a written conflict of interest policy that has been given to each employee. Employees are required to read and sign a statement in the beginning of each year signifying that they have read and understood the policy. The copy of the signed statement is maintained within the employment records. |
