Unity Health System along with other Health Systems in Monroe County including Rochester General Health System, Lakeside Health System, University of Rochester Medical Center (URMC) Highland and Strong have jointly filed a community service plan to the New York State Department of Health for the past thirteen years, and this year is no exception. | Part VI - Explanation Of How Organization Furthers Its Exempt Purpose | Unity Health System is the conduit for care for thousands of our community members. At Unity Health System, we offer many community based health services. At Unity Health System our mission is to make a positive difference in the health and well-being of those serve. We provide accurate information and quality services, enabling our community to lead healthy lifestyles for themselves and their families. The Health System is committed to giving back to the community by supporting the many organizations that are already working so hard to improve community health. As such, we provide services and staff to local organizations and community events including the American Heart Walk, the American Cancer Society, the March of Dimes, the Juvenile Diabetes Foundation, the American Diabetes Association, Tour De Cure, and the United Way. Unity Health System is also a member of The Rochester Integrated Health Network (RIHN) which works to promote the integration and coordination of quality, accessible and affordable medical and behavioral health care services for low income, uninsured, under insured children and adults in the Rochester, New York region. The Rochester Integrated Health Network collaborates on improvement projects with its member organizations and community partners. Not only do we provide services and staff for these organizations, but we also independently provide community services such as Healthy Start program, Pastoral Outreach program, and Healthcare for the Homeless. Within the Healthcare for the Homeless program, we administer quality care by using a 40-foot Mobile Unit with exam rooms for homeless patients in need of primary, urgent, and dental care. We also offer a designated Call Center to assist our current and prospective patients in needs they may have, such as help finding a qualified physician in the area, questions regarding medications, post-visits with the patient to see how they are doing and patient satisfaction surveys to determine the quality of care. Unity Health System has launched a Let's Talk Health event series. These events offer a new opportunity to engage and educate our community. Let's Talk health events are hour long health fairs followed by an hour long Q&A session with Unity experts on a specific topic. Such topics held for the community were Synthetics Drugs - A Growing Teen Danger, presented by Unity Behavioral Health, No Bones About It - Straight Talk about Joint Replacement, presented by Unity Joint Replacement Center specialist services, "Hot Flashes: Hormones & Hors D'oeuvres", presented by Unity Obstetrics, and Getting better ZZZs,presented by Unity Sleep Disorders. All of these events were well received and attended. Unity Health System has programs that are largely underwritten as part of our commitment to under served and special needs segments of our community. The benefits these programs bring to the community are so great that the Hospital deems it necessary to subsidize them to ensure the existence of these programs. We feel so strongly about this that we have incorporated it as part of our four core values. People above everything else by treating each other and those we serve with compassion, dignity and respect, Excellence by acting with integrity for the highest quality and service, Learning by sponsoring and participating in educational opportunities and Results by meeting the expectations of the people we serve. These programs fall under many areas of the hospital such as OB/Newborn, Chemical Dependency, Mental Health, Walk in Care Clinic, Dialysis, Cardiac Rehab and Spine Center. A good example of a program that underlines Unity Health System's commitment to the community is the Memory impairment at the Villages at Unity. "The Basics: Memory Loss, Dementia, and Alzheimer's Disease" is the first in a series of seven programs presented by the Rochester/Finger Lakes Chapter of the Alzheimer's Association. Other programs include: Managing Diabetes classes, Joint Replacement Center Reunion Luncheons, Nutrition Talk at Fleet Feet, Diabetes Center Community Web Chats, Fleet Feet Sports Speaker Series, Experts Presenting on Insomnia, Synthetic Drugs: An EMS Education Event, Meaningful Activities for People with Dementia, Living with Alzheimer, and Community Flu Clinics. Unity Health System is also proud to be a part of a growing collaborative effort with the Rochester Business Alliance and the Finger Lakes Health System Agency's to raise awareness about High Blood Pressure. The High Blood Pressure Collaborative Work Group is a strategic partnership to address community-wide transformational change in health and in health care delivery. A community-wide initiative by the group was enacted to combat hypertension by raising awareness of the disease and by enabling people to take better control over the disease. Leaders from every segment of the community - such as health care, business, labor, government, education, social service and faith-based organizations, and neighborhood groups - are actively engaged in this effort. Individual project teams addressing different aspects of care, from health plan design and best practices, to community awareness and behavior change. Beyond medical services and financial underwriting, Unity Health System offers its expertise and resources to benefit the community at large in the form of cash and in-kind donations. The majority of these come in the form of labor cost on work time associated with community health-related boards and other community involvement such as costs of coordinating community events not sponsored by the health care organization. Examples of such events include Chase Corporate Challenge, United Way Campaign and several other Home & Community based events. | Part VI - Patient Education of Eligibility for Assistance | Financial Counselors make every effort to interview all inpatients without full insurance coverage to determine insurance and/or Charity Care eligibility during the patients stay in the Hospital. In addition, any patient may contact a Financial Counselor or the Patient Accounting Department to learn more about the Charity Care Program. The procedure for contacting either of these areas is outlined in all published materials. In addition, all registration representatives are trained on how to direct patients to the appropriate area when questions arise related to Charity Care.Upon receipt of the referral, the Patient Accounting Dept/Financial Counselor will conduct a financial assessment of the patient. If appropriate, the patient will be referred to a Medicaid Liaison/Facilitated Enroller to complete an application for Medicaid, Child Health Plus, or Family Health Plus.Medicaid Liaisons are available to assist patients through the Medicaid application/determination process. If necessary, Medicaid Liaisons are also available to assist with the fair hearing process.Patients who are not eligible to apply for the above insurance or patients who are unable to obtain sufficient insurance coverage, are eligible to apply for Charity Care.Charity Care applications may be submitted by patients, guarantors, guardians, or Powers of Attorney. Applications will be accepted prior to, during, or within six months of service. Exceptions are made in appropriate circumstances.Once an application is received, the Financial Counselor/Patient Account Representative will determine eligibility and the qualifying discount amount. Additional documentation or information may be requested during this phase of the process.Patients may be approved for Charity Care on an account by account basis or for a period of time up to six months. Patients will be required to update their financial information after six months if the need for Charity Care continues. Charity Care applications are processed by the Patient Accounting Department timely and determinations are communicated to the patients within 10 business days of submission. A patient or their representative may request consideration either by phone or in writing of a Charity Care determination if additional information is available that would change their application. A letter indicating the outcome of the reconsideration will be sent out within 10 business days of the request.Bills will continue to be generated and mailed to patients during the application process, however, payment will not be expected until the Charity Care determination is made as long as the patient submits required information within set timeframes.Patients or their representatives who are unwilling to provide required documentation or comply with other aspects of the process are informed that they will not be eligible for Charity Care and that they are responsible for all Hospital charges related to their care. | |

The Unity Hospital of Rochester is currently working with the group to develop the CHNA report.The Charity Care program has been developed to help the Hospital meet the needs of our patients who are either uninsured or under-insured while maintaining the financial viability of the Hospital. The following principles and guidelines explain how the Hospital assists those patients in obtaining access to the Charity Care Program.Principles:-The Unity Hospital of Rochester believes that "fear of a hospital" bill should never get in the way of essential health services. The message will be proactively conveyed to prospective patients and local community service agencies.-We are committed to creating financial aid policies that are consistent with the mission and values of the Hospital, that take into account each individual's ability to contribute to the cost of his or her care, and the Hospital's financial ability to provide the care.-We will communicate our Charity Care policy in a manner that is clear, understandable, sensitive to the patient's dignity, and in a language appropriate to the communities and patients served. Designated staff is provided detailed training so that they are able to provide information and answer questions related to this program.-We are committed to implementing debt collection policies that reflect the mission and values of the organization.-The Unity Hospital of Rochester will work with government, payers, consumer groups, employers, and others to find solutions to expand access to health care coverage to all New York residents.-The Charity Care program does not eliminate personal responsibility.
