Description	Mosaic Medical is an innovative safety net provider of medical and behavioral health care services for Central Oregon. The organization provided 53,872 primary care visits fiscal 2011-2012 as well as thousands of additional connections to dental, medication assistance and case management services. In fiscal 2011-2012, Mosaic's clinics in Prineville, Bend and Madras were some of the first primary care clinics in the region to attain the state's highest designation for Patient-Centered Primary Care Homes. Additionally, Mosaic was awarded a competitive federal grant to open a Redmond clinic within the next year.	Description

Explanation	The Organization is exempt from taxation under Section 501(c)(3) of the Internal Revenue Code and has been classified as an organization that is not a private foundation under Section 509(a)(2). No provision for income taxes is made in the accompanying financial statements, as the Organization has no activities subject to unrelated business income tax.The Organization files income tax returns in the U.S. federal jurisdiction as well as the state of Oregon jurisdiction. Generally, income tax filings are subject to federal and state examination for three years after they are filed. Tax returns filed for periods ended May 31, 2008 and prior are considered closed. In the event penalties and interest are assessed by income taxing authorities, it is the Organizations policy to include these in operating expenses.
