Form990 Schedule H Part VI Form990ScheduleHPartVI (12 entries)

ExplanationExplanationIdentifierIdentifier
Part I, Line 7: The cost of providing charity care, means tested government programs, and community benefit programs is estimated using internal cost data, and is calculated in compliance with Catholic Health Association ("CHA") guidelines. The organization uses a costing accounting system that addresses all patient segments (for example, inpatient, outpatient, emergency room, private insurance, Medicaid, Medicare, uninsured, or self pay). The best available data was used to calculate the amounts reported in the table.
Part I, Line 7g: The organization has not included costs attributable to a physician clinic as part of subsidized health services.
Part I, L7 Col(f): The amount of bad debt expense reported on Form 990, Part IX, Line 25, column (A), but subtracted for purposes of calculating the percentages in Part I, Line 7, column (f) is $9,962,464.
Part II: Nationally, Tennessee ranks 47th for its infant mortality rate, with more than 700 babies dying each year. A baby born in north Nashville (one of the poorest neighborhoods) has the same chance of survival as a baby born in Sri Lanka. In the last 10 years, low birth weight and prematurity have become the leading causes of infant mortality.Every week, 234 babies are born premature in Tennessee. Even if a child survives a premature birth, the effects can last a lifetime. In an effort to address the high infant mortality and prematurity rates in Tennessee and Davidson County, Baptist Hospital has collaborated with the March of Dimes, Nurses for Newborns and the Public Health Department. We continued this year to build a relationship with the March of Dimes to provide education and information to improve the health of mothers and babies around the areas of preventing birth defects, premature birth and infant mortality. This is accomplished through support of community gatherings and educational endeavors, leadership from the hospital serving on March of Dimes board and committees, leadership and sponsorship of fundraising event activities.Nurses for Newborns of Tennessee exists to provide a safety net for families most at-risk to help prevent infant mortality, child abuse and neglect through in-home nursing visits (up to 2 years) that provide healthcare, education and positive parenting skills. In addition to providing financial support to the program, Baptist Hospital works closely with an RN to promote visits in the hospital to moms prior to going home so that a relationship can be developed to ensure an easy transition. Various members of the hospital staff serve on NFN board and the hospital does a collection each year for items needed for infants of vulnerable families. The Davidson County public Health Department is leading the efforts in terms of awareness of infant mortality disparity between white and African American babies and gathering of coalitions to provide research and solutions. Various members of Baptist Hospital serve on these committees and participate in the gathering of mortality data for the department. In addition to infant mortality issues, Baptist Hospital joined with the American Red Cross to conduct a Latino CPR Saturday. Aimed at reaching the Spanish speaking population in Nashville/Davidson county, the hospital provided funds and communication support to reach 160 people in improving their life saving skills and further improving the quality of life/health for this minority population.
Part III, Line 4: The provision for bad debts is based upon management's assessment of expected net collections considering economic conditions, historical experience, trends in health care coverage, and other collection indicators. Periodically throughout the year, management assesses the adequacy of allowance for uncollectible accounts based upon historical write off experience by payor category, including those amounts not covered by insurance. The results of this review are then used to make any modifications to the provision for bad debts to establish an appropriate allowance for uncollectible accounts. After satisfaction of amounts due from insurance and reasonable efforts to collect from the patient have been exhausted, the System follows established guidelines for placing certain past due patient balances with collection agencies, subject to the terms of certain restrictions on collection efforts as determined by the System. Accounts receivable are written off after collection efforts have been followed in accordance with the System's policies.
Part V, Section B, Line 11h: We use presumptive scoring to identify charity accounts.Baptist Hospital
Part V, Section B, Line 19d: The State of Tennessee legislates that billings can not exceed 175% cost to charge on most recent Joint Annual Report.Baptist Hospital
Part VI, Line 2: Our community needs assessment is conducted every 3 years, most recently completed in January 2010. The assessment was a collaborative effort across the facilities of Saint Thomas Health Services including Baptist Hospital to provide a comprehensive review of available data and studies around: demographic trends, socioeconomic and health status indicators, maternal and infant health issues, major disease prevalence, and health resource utilization and needs. It consists of a compilation of public health data as well as the results of a study of the safety net providers in Nashville that included focus groups of 67 key community informants. The assessment is focused on the counties in which STHS hospitals are located and helps provide focus and direction for our collective efforts to be responsive to the key health concerns and needs of the communities we serve.
Part VI, Line 3: Education of eligibility for assistance at Baptist Hospital begins at registration with signage displayed at all registration points notifying patients that we have a Financial Assistance program, as well as contact information for the patient to use should they need assistance. Brochures containing this information are also available for patients at all registration points. Baptist Hospital registration associates discuss the estimated balance that will be due with both insured and self-pay patients. Should the associate identify that the patient may need assistance with that balance he/she will provide the patient with a Financial Assistance application and help them complete it, if needed. All self-pay and underinsured patients are screened by Baptist Hospital for alternative coverage including Medicaid, SSI/SSD, Cobra, student coverage, and crime victims' compensation. If no alternative is identified then an associate will work with the patient to complete a Financial Assistance application. Also, contact information is listed on Baptist Hospital billing statements for the patient to use in the event they need assistance with their balances, and collections associates have been trained to offer patients a Financial Assistance application if needed by the patient. Baptist Hospital works with third party collection vendors who are required to follow the hospital's Financial Assistance policy. Third party vendors are also used to perform "charity scoring," a process that screens incomplete Financial Assistance applications to qualify patients for presumptive charity.
Part VI, Line 4: Located in Nashville, Tennessee, Baptist Hospital primarily serves residents of Davidson County and adjacent surrounding counties of Williamson, Sumner, Rutherford, Cheatham, Wilson, Dickson, and Robertson in Middle Tennessee. More than half of Baptist's inpatients are from Davidson County. In 2010, Davidson County's population is estimated to be 655,406 with 17% of residents living below the poverty level; median household income of $46,413; uninsured comprising 15.2% of the population; and racial mix that is predominately Caucasian (59%) followed by 27% African-American, 8% Hispanic 3% Asian, and 3% other categories. Baptist Hospital is the largest not-for-profit community hospital in Middle Tennessee and delivers more babies than any other hospital in the area. Overall, Tennessee residents are rated less healthy than national averages with unfavorable rankings for death rates for cancer, diabetes, heart disease and stroke and unfavorable rankings for risk factors. In fiscal year 2010, Baptist served just over 24,000 inpatients and nearly 7,000 newborns and provided $19.2 million in traditional charity care, unpaid cost of public programs and other community programs and services.
Part VI, Line 5: Baptist Hospital continues to work in collaboration with other community agencies to address the health and well being of our community especially the most vulnerable. Other partnerships include:- Faith Family Medical Center: We provide a building located on the hospital campus, at no cost, for a full clinic which addresses the holistic needs of uninsured working poor.- The Comprehensive Care Center: We provide funds to promote the comprehensive care of the people suffering with HIV/AIDs and have staff members serving on the board of directors.- The Interfaith Dental Clinic: We provides funds to support the mission of the Interfaith Dental Clinic to restore, protect, and improve the oral health of uninsured, low-income, working people, their children and the elderly in the greater Nashville area through access to affordable quality dental care, oral disease prevention services, and oral health education. When needed, the hospital provides the medical services for their patients requiring dental surgery through the existing hospital financial assistance policies.- Hospital Hospitality House: We provide funds and a member of the hospital staff serves on the board to directors to support its mission to serve as a home away from home for patients and caregivers seeking medical treatment in Nashville hospitals, providing lodging, meals and other supportive services.- The Sexual Assault Center: We provide funds and engage with the center in community awareness and educational endeavors to further their mission to end sexual violence and provide healing for children, adults and families affected by sexual assault through counseling and education. Some of the other collaborative efforts in the community include bi-monthly blood drives in conjunction with the local American Red Cross; free cardiac screenings; military packages collected and prepared for our men and women overseas; a baby fair providing infant safety, education seminars and health in nutrition for approximately 4-5,000 attendees; sponsoring walks for Heart, Diabetes, and Cancer; and the Diabetes Extravaganza free community event providing health screening and cooking and exercise demonstrations.
Part VI, Line 6: Baptist Hospital is part of the Nashville Health Ministry of Ascension Health, the largest Catholic and not-for-profit health system in the United States and is known locally as Saint Thomas Health Services (STHS). STHS is dedicated to providing accessible, quality healthcare to all patients regardless of their ability to pay. STHS owns and operates 4 hospitals in Middle Tennessee including Baptist Hospital and Saint Thomas Hospital in Nashville, Middle Tennessee Medical Center in Murfreesboro, and Hickman Community Hospital in Centerville. In addition, STHS provides a variety of ambulatory services through the operation of an urgent care center and community primary care clinics, employment and close alignment with physicians, and partnerships to provide diagnostic imaging, sleep lab, and ambulatory surgery services. As a system, STHS supports a variety of efforts specifically aimed at providing services for persons who are poor, vulnerable or uninsured. These include focused medical mission events in low income communities (where associates volunteer their time to provide direct healthcare services), primary care clinics that provided 49,000 patient encounters in the last year, and a national distribution center and pharmacy to support a network of dispensing sites and direct-to-patient prescription solutions that provided $5.6 million in available medications for patients unable to afford their prescriptions. STHS actively advocates for healthcare access and coverage for all through ongoing meetings with elected officials (State, Local, and Federal levels) and sponsorship of community events and presentations to increase awareness and educate the public regarding healthcare reform and access. STHS helped to form the Safety Net Consortium of Middle Tennessee to address the needs of the uninsured residents of Middle Tennessee. Saint Thomas Health Services Fund serves as the legal and fiduciary agent to the Consortium. The Consortium is governed by a Board of Directors comprised of health care providers and consumer organization members that serve the poor and uninsured. STHS System wide, for fiscal year 2010, STHS provided more than $44 million in traditional charity care, unpaid cost of public programs and other community programs and services.
