DescSpecial Kids Special Families Foster Care Placement Services. This program began in 2002 at the request of El Paso County Department of Human Services. A factor for the therapeutic nature of the foster care program is that there is a significant number of children in the foster homes having intellectual and developmental disabilities. Families are recruited and trained to provide for the significant needs and challenges that the children often present. Foster families are located in the Denver Metro area, south on through Colorado Springs and to the Pueblo area. Referrals are received throughout the State. Staff are assigned specific families to oversee and support. Services provided include 24 hour crisis intervention and assistance, training opportunities, transportation supports, respite care and supervision. The foster care program also contracts with Aspenpointe Behavioral Health to provide planned and crisis respite for families they serve through the use of our foster homes. Program Service Accomplishments:In 2013, the foster care program outcomes achieved a higher than national average standard for the prevention of disruption of placements. This is indicative of the skill and ability of the staff to provide significant prevention and intervention services. Additionally this program experienced an increase in monthly placements in a stable and steady growth of this program. In January 2013, there were 33 children in placement and in December 2013 there were 48 youth in placement. The program improved quality of services through regular foster parent support in transportation services, in consistent monthly trainings and support group meetings. This in turn, has increased foster parent referrals. A specialized component to the foster care program, Benchmarks, recruits and trains families to accept youth and young adults with dual diagnosed. The goal is to stabilize their lives and prepare the youth for community living. A target group of this program is the young adults with developmental disabilities.Desc

DescSKSF Adult Day Services - I-CAN day program is a community based program, offering transportation from the individual's home to the program, as well as to access services in the community for individuals with disabilities. The clients work with an Adult Service mentor withindividual habilitation goals that are addressed in each given day. Clients volunteer in the community, such as overseeing the upkeep of Nancy Lewis Park, helping in the lunch room at Classical Academy School, helping at Care and Share, and more. Additional volunteer opportunities were developed to offer pre vocational training as well as socialization and self-esteem building. Clients are also able to engage in recreation and physical activity by going to such locations as the YMCA, local parks, shopping, and to the zoo, visiting Peak Aviation was a highlight for many clients. Several clients from our volunteer group were very excited to be recognized at a volunteer appreciation breakfast in May. They also enjoyed learning new job skills which included a visit to Channel 11 news to understand how it operates, and the individual positions.Joey's Place is the facility based program which provides clients the opportunity for working on activities such as cooking projects, health and wellness with exercise classes (including Yoga through The Smokebrush Foundation), academic work- including an in depth holistic study of a different country each month to understand their culture, foods, geography and more, they added sign language classes, computer classes, and art classes with Miss Clara. This year they decided to add a Joey's Place garden. The clients were involved from the very beginning starting the seeds inside in trays, and eventually transplanting them outside to watch them grow. They were able to experience the whole gardening process to understand all that it takes to make a garden grow. The clients were so excited to use the harvested vegetables and herbs in many recipes during their cooking classes. At the end of the growing season, the remaining vegetables were harvested and the growers held their first very own farmers market. This program has continued to assist with the transitional process into the adult system including offering classes on money management & budgeting, and job seeking skills, this program was selected by School District #11 as a choice for parents. 50% of all day program clients are transported to the program by SKSF transportation services. This program accepts individuals on Medicaid Waivers, as well as having some clients using the ability to pay to help working parents/guardians with adult children living in the home. Individuals have the option of choosing either the I-CAN program or Joey's Place or a combination of both in their given service plan.Desc

DescZach's Place - Zach's Place Center is the only licensed child care center in the Pikes Peak Region that specializes in serving families who are raising children with disabilities. The Center was licensed by the State of Colorado in 1999, when the Center opened its doors to serve the community. The services include respite and child care, community connection, life skills training, transportation to access the program, home based care and socialization groups that meet three times a month. In 2013 The State of Colorado approved program expansion to serve children from 2 years and up. The program added a staff position to assist with developing an expanded plan of curriculum and activities. The program is available seven days a week, providing overnight care, as well as before and after school weekend and evening care. The primary diagnosis of the children served were on the autism spectrum, other significant diagnosis includes cerebral palsy, downs syndrome, and moderate to profound intellectual disabilities. The staff are trained for such medical interventions as serving children who have seizure disorders, diabetes, gastrostomy services (g-tube feedings). The program features regular monthly socialization activities including fun nights and teen nights to encourage youth to build relationships, and to promote community support and involvement. These nights have offered continuing support to the parents and families even making aIn 2013, Zach's Place provided over 15,000 hours of care to children. The majority of families are considered to be low income families by HUD standards for our area. In October 2013, the Program Director presented Zach's Place at the National Respite Care Conference in Boston MA; as a model for supporting families having children with significant developmental disabilities. and the prevention of out of home care. This year, the Board of Directors agreed in a strategic plan to expand and enhance Zach's Place program through the relocation to a larger and program designed facility. This program continues to collaborate with many organizations in the community, including KIDPOWER, and The Autism and Asperger's Network, Alliance for Kids, and the Child Care Directors Network, The Network, which is our agency's parent educational group. Volunteerism continues to be an important part of this program, various organizational groups contributed including interacting and socializing with the youth which impacted both theyouth as well as the volunteers.Desc
