Form And Line ReferenceFormAndLineReferenceDescExplanationExplanationTxt
PART I, LINE 3CThe hospital provides financial assistance to all individuals that have family income less than 200% of federal poverty guidelines, regardless of the amount of assets held by the patient. In addition to providing financial assistance to patients that qualify under the patient's financial assistance policy because their family income is less than 200% of the FPG, THE HOSPITAL PROVIDES A DISCOUNT TO ALL SELF PAY PATIENTS THAT DO NOT QUALIFY UNDER THE HOSPITAL'S FINANCIAL ASSISTANCE POLICY ON THEIR HOSPITAL CHARGES. THE INTENTION OF THE DISCOUNT IS TO LIMIT THE AMOUNT SELF PAY PATIENTS ARE BILLED TO AN AMOUNT THAT APPROXIMATES THE AMOUNT PAID BY CERTAIN COMMERCIAL HMO/PPO PAYERS. USING THE HOSPITAL'S COST-TO-CHARGE RATIO, THE COST OF THE DISCOUNT PROVIDED TO SELF-PAY PATIENTS WAS APPROXIMATELY $1,256,378.
PART I, LINE 7The cost of the self-pay discount was not included in the determination of the organization's financial assistance at cost, as reported in Part I, Line 7.
PART I, LINE 7, COLUMN FThe Hospital utilized a cost-to-charge ratio for reporting program costs. The cost to charge ratio utilized excludes the cost and associated changes related to the Hospital's community benefit activities reported in Schedule H. Bad debt expense OF $15,828,611 IS REPORTED NET OF REVENUES. This amount was excluded from total expense.
PART I, LINE 7, SECTION GThe Planning Committee of the Board of Director identifies critical medical needs of the community and recruits physicians and establishes new physician practices and clinics to meet the critical needs of the community. In 2013, the loss stemming from the operation of the physician practices and clinics, including the Arlington Urgent Care center, by the Virginia Hospital Center Arlington Health System was $7,320,009. Included in the amount of subsidized health services provided by Virginia Hospital Center Arlington Health System is the operating loss incurred by Arlington Pediatric Center, a subsidiary organization of Virginia Hospital Center Arlington Health System that is exempt from federal income taxation under Section 501(c)(3). Virginia Hospital Center Arlington Health System is required to fund the annual operating losses incurred by Arlington Pediatric Center. The operating loss incurred by APC in 2013 was $200,893.
PART III, LINE 4AS STATED IN THE FOOTNOTES OF FY 2013 AUDITED FINANCIAL STATEMENTS: THE PROVISION FOR BAD DEBTS IS BASED UPON MANAGEMENT'S JUDGMENTAL ASSESSMENT OF HISTORICAL AND EXPECTED NET COLLECTIONS CONSIDERING BUSINESS AND GENERAL ECONOMIC CONDITIONS IN ITS SERVICE AREA, TRENDS IN HEALTHCARE COVERAGE, AND OTHER COLLECTION INDICATORS. THROUGHOUT THE YEAR, MANAGEMENT ASSESSES RECEIVABLE PAYOR COMPOSITION AND AGING, TAKING INTO CONSIDERATION RECENT WRITE-OFF EXPERIENCE BY PAYOR CATEGORY, PAYOR AGREEMENT RATE CHANGES AND OTHER FACTORS. THE RESULTS OF THESE ASSESSMENTS ARE USED TO MAKE MODIFICATIONS TO THE PROVISION FOR BAD DEBTS AND TO ESTABLISH AN APPROPRIATE ALLOWANCE FOR UNCOLLECTIBLE ACCOUNTS RECEIVABLE. FOR SELF-PAY PATIENTS, THE PROVISION IS BASED ON AN ANALYSIS OF PAST EXPERIENCE RELATED TO COLLECTION RATE OF SELF-PAY BALANCES. THE CORPORATION FOLLOWS ESTABLISHED GUIDELINES FOR PLACING CERTAIN PAST-DUE PATIENT BALANCES WITH EXTERNAL COLLECTION AGENCIES. AT DECEMBER 31, 2013 AND 2012, THE ALLOWANCE FOR UNCOLLECTIBLE ACCOUNTS REPRESENTED APPROXIMATELY 96%, OF THE PATIENTS DUE ACCOUNTS RECEIVABLE BALANCE. THE PATIENT DUE ACCOUNTS RECEIVABLE BALANCE REPRESENTS THE ESTIMATED UNINSURED PORTION OF ACCOUNTS RECEIVABLE. The bad debt expense reported on Line 2 was the cost to the organization of providing the patient care services that resulted in the bad debt expense. To determine the cost to the organization, the organization multiplied the gross charges that were uncollected by the hospital's cost to charge ratio. The organization estimates that a negligible amount of bad debt expense is attributable to patients eligible under the hospital's financial assistance policy. The organization utilizes a software program to determine if patients have certain indicia of qualifying for financial assistance, such as being homeless or possessing very limited assets, before writing off the patients' accounts to bad debt. If the patient does have certain indicia of qualifying under the organization's financial assistance policy, then regardless of whether the patient fills out a financial assistance application, the patient will be approved for, and will receive, financial assistance.
PART III, LINE 8The organization's Medicare Shortfall, as reported in Schedule H, Part III, Section B, 7, should be considered a community benefit. Although providing the best possible care to our Medicare patients creates a financial loss, part of the mission of our organization is to serve elderly patients within the community, and we believe providing such care has contributed to the improvement of health overall within the community. By providing the best possible care to our Medicare patients, we relieve the government and surrounding other hospitals of the financial burden of treating these patients insured by the Medicare program. Furthermore, the Medicare shortfall reported is likely understated because Medicare allowable costs were determined utilizing the cost to charge ratio as set forth in the Medicare Cost Report, and, as a result, do not reflect the true, full cost of delivering care to Medicare patients.
PART III, LINE 9BAs soon as a patient qualifies for financial assistance under the hospital's financial assistance policy (even if after the billing and collection process has commenced),THE COLLECTION POLICY STATES THAT ALL Billing and COLLECTION EFFORTS will CEASE immediately AND COLLECTION EFFORTS will NOT TO BE PURSUED. THE ACCOUNT IS IMMEDIATELY written off and RECOGNIZed as charity.
PART V, SECTION B, LINE 3The CHNA took into account input from persons who represent the broad interests of the community served by the hospital facility, including those with specialized knowledge of or expertise in public health and community leaders of minority and underserved groups. In particular, the organization received input from 31 community leaders from Arlington County, law enforcement, school representatives, Arlington Free Clinic leadership, Virginia Hospital Center medical staff leaders, religious leaders, health systems agency, and community non-profits focused on the health and welfare of members of the community. An updated CHNA is scheduled for 2014.
PART V, SECTION B, LINE 6In support of VHC's ongoing commitment to addressing the community health needs identified in VHC's Community Health Needs Assessment, the following services were undertaken in 2013: - Expansion of inpatient and outpatient addictions services - Received full accreditation from American College of Surgeons for Bariatric Surgery Center - Began providing Home Health Services to the community - Primary care centers established in primary and secondary service areas. - Construction for the state of the art Linear Accelerator for cancer treatment - Offered regular ongoing community education classes for weight management and exercise classes"
PART V, SECTION B, LINE 7THE ORGANIZATION HAS NOT ADDRESSED THE HEALTH NEED OF DENTAL CARE/ORAL HEALTH. THE ORGANIZATION MADE A DETERMINATION THAT IT WOULD BE A MORE EFFECTIVE USE OF RESOURCES TO FOCUS ON ADDRESSING THE 4 COMMUNITY HEALTH NEEDS OF HEALTH CARE COVERAGE, SOCIAL SERVICES, AGING SERVICES, AND SUBSTANCE ABUSE, RATHER THAN ATTEMPTING TO ADDRESS DENTAL CARE/ORAL HEALTH AS WELL. THE ORGANIZATION LACKS THE REQUISITE EXPERTISE AND EXPERIENCE IN THIS AREA TO EFFECTIVELY ADDRESS THIS HEALTH NEED.
PART V, SECTION B, LINE 20THE HOSPITAL DOES NOT CHARGE any PATIENTS THAT QUALIFY FOR financial assistance UNDER THE ORGANIZATION'S FINANCIAL ASSISTANCE POLICY FOR EMERGENCY OR OTHER MEDICALLY NECESSARY CARE. All patients that qualify for financial assistance receive free care.
NEEDS ASSESSMENTTHE ORGANIZATION UNDERTOOK A COMMUNITY HEALTH NEEDS ASSESSMENT ("CHNA") DURING THE 2011 TAX YEAR. ASSOCIATED WITH THE COMPLETION OF THE CHNA, THE ORGANIZATION'S BOARD OF DIRECTORS ADOPTED AN implementation strategy to meet the health needs identified in the CHNA. THE CHNA TOOK INTO ACCOUNT INPUT FROM PERSONS WHO REPRESENT THE BROAD INTERESTS OF THE COMMUNITY SERVED BY THE HOSPITAL FACILITY, INCLUDING THOSE WITH SPECIAL KNOWLEDGE OF OR EXPERTISE IN PUBLIC HEALTH. IN PARTICULAR, THE ORGANIZATION RECEIVED INPUT FROM 31 COMMUNITY LEADERS FROM Arlington county, law enforcement, school representatives, Arlington Free Clinic Leadership, Virginia Hospital Center medical staff leaders, religious leaders, health systems agency, and community non-profits. IN ADDITION, THE CHNA DEFINED THE ORGANIZATION'S COMMUNITY, PROVIDED A DESCRIPTION OF THE PROCESS AND METHODS USED TO CONDUCT THE CHNA, AND DESCRIBED HOW THE ORGANIZATION TOOK INTO ACCOUNT INPUT FROM PERSONS WHO REPRESENT THE BROAD INTERESTS OF THE COMMUNITY. THE CHNA IDENTIFIED 5 COMMUNITY HEALTH NEEDS, INCLUDING: HEALTH CARE COVERAGE, DENTALCARE/ORAL HEALTH, SOCIAL SERVICES, AGING SERVICES, AND SUBSTANCE ABUSE. THE ORGANIZATION SET FORTH AN IMPLEMENTATION STRATEGY, ADOPTED BY THE BOARD OF DIRECTORS, PURSUANT TO WHICH THE ORGANIZATION MAINTAINED, EXPANDED, AND ESTABLISED PROGRAMS TO ADDRESS 4 OF THE COMMUNITY HEALTH NEEDS: HEALTH CARE COVERAGE, SOCIAL SERVICES, AGING SERVICES, AND SUBSTANCE ABUSE. THE ORGANIZATION MADE A DETERMINATION THAT IT WOULD BE A MORE EFFECTIVE USE OF RESOURCES TO FOCUS ON ADDRESSING THE 4 COMMUNITY HEALTH NEEDS NOTED ABOVE, RATHER THAN ATTEMPTING TO ADDRESS DENTAL CARE/ORAL HEALTH AS WELL. THE ORGANIZATION LACKS THE REQUISITE EXPERTISE AND EXPERIENCE IN THIS AREA TO EFFECTIVELY ADDRESS THIS HEALTH NEED. In support of VHC ongoing commitment to community health needs, the following services were undertaken in 2013: - Expansion of inpatient and outpatient addictions services - Received full accreditation from American College of Surgeons for Bariatric Surgery Center - Began providing home health servcies to the community - Primary care centers established in primary and secondary service areas - Construction for the state of the art linear accelerator for cancer treatment - Offered regular ongoing community education classes for weight management and exercise classes
