ExplanationSCHEDULE H, PART VI 1 - Required Descriptions Part I, LINE 3 - EXPLANATION OF ELIGIBILITY FOR FREE CARE Explanation of eligibility for free care. As noted, NYEEI utilizes federal poverty guidelines (FPGs) to determine eligibility for providing free care (100%) and up to 300% for providing discounted care to low income individuals who present here. We do not use an asset test or other threshold, regardless of income, to determine eligibility for free or discounted care for any of our approximately annual 250,000 outpatient encounters. Asset levels are documented and submitted as a requirement for New York state (NYS) Medicaid applications. No other factors are used to determine eligibility. PART I, LINE 6a - DESCRIPTION OF ANNUAL COMMUNITY BENEFIT REPORT NYEEI prepares an annual Community Benefit (aka Community Service Prevention Plan-CSPP) Report. The latest version was submitted to NYS DOH on 11-15-2013. It is a comprehensive 28 page report covering the period from 2013-2015. The Community Benefit Report is: A. Sent to the NYS DOH; B. Posted on our website (NYEE.EDU; C. Available on site for patients and community members; and D. Approximately 28 pages long; it includes a summary of the Community Health Needs Assessment (CHNA); An executive summary is also available to patients. PART I, LINE 7 - EXPLANATION OF COSTING METHODOLOGY Financial and certain other Community Benefits at Cost: Explanation of costing methodology. NYEE costing methodology was based upon the 2013 NYS institutional cost report ICR) and the 2013 Medicare (Form 2252) cost report. These reports are filed with the NYS Department of Health and the applicable CMS intermediary, respectively. The cost-to-charge ratio, derived from Worksheet 2 and the ratio of patient care cost-to-charges were used for the various sub-line items of Line 7. We utilized the worksheets from the Sch H instructions to calculate the total amounts. Part I, Line 7(A) - Financial Assistance at CosT See Worksheet 1. PART I, LINE 7(b) - Medicaid See Worksheet 3. There is NO Medicaid shortfall. See attached PDF pages from Audited Financial Statement. PART I, LINE 7(D) Financial assistance and means-tested government programs are the sum of a and b. PART I, LINE 7(E) Community Health Improvement Services (CHIS); support groups for head and neck cancer survivors as well as for individuals with macular degeneration, uveitis, and glaucoma. More than 50 health fairs and CHIS related activities are detailed in the attachments PART I, LINE 7(F) Health Professions Education; GME (interns, residents and fellows); continuing medical education and community health education lectures. We followed the instruction dividing the result (e) by the expense from part IX, Line 25 of Column A, regarding the health professions education training. PART I, LINE 7(G) Subsidized Health Services: We provided pharmaceuticals to needy patients and to others for free or at discounted prices ($109,286). PART I, LINE 7(H) Research totals for 131 subsidized clinical trials open to the community. All were Institutional review board (IRB) protocols approved for the protection of human subjects. All costs are NET-(Total expense minus Revenue). See worksheet 7. PART I, LINE 7(I) In-kind contributions; including clothing and food drives for storm victims, rebuilding efforts, relief efforts for acid burn victims, procuring holiday food vouchers, and, training for donated hearing aids, etc. PART II, LINE 3 Community support and Community Health Information Services (CHIS)-sheltered workshops, youth training, charity walks, medical missions, etc. There were over 4 dozen in total. A more complete list follows. PART II, LINE 4 Environmental improvements and emergency preparedness; community sharps program, smoking abatement program, flu vaccination, disaster preparedness (medications and supplies), notification program, defibrillators and other equipment, radiation detectors, target hardening (terror resistance). PART III, LINE 2 Bad debt OF $4,657,073 IS PER THE AUDITED FINANCIAL STATEMENTS. SEE PART III, LINE 4 FOR BAD DEBT EXPENSE FOOTNOTE PER AUDITED FINANCIAL STATEMENTS. PART III, LINE 3 Estimated amount of bad debt attributed to CC: $15,996. Rationale for including this portion of cc bad debt as a community benefit: Services were provided to 9 patients presumed to be eligible for financial assistance (FA). The estimate was based on record reviews, demographics (28% of the residents of our PSA live below the poverty level and for 52% of our patients-English is NOT their primary language) and an assessment of incomplete documentation on 6 applications for Financial Assistance. Two were clinic visits at $198 each and seven were ambulatory surgery cases at an average of $1800 each. The organizations collection policy applied to the largest number of patients during 2013 contained provisions on the collection policies to be followed for patients who are known to qualify for financial assistance. PART III, LINE 4 - BAD DEBT EXPENSE The hospital calculated its bad debt expense examining net patient service revenue, accounts receivable and allowance for uncollectible accounts: billings related to services rendered are recorded as net patient service revenue in the period in which the service is performed, net of contractual and other allowances which represent differences between gross charges and the estimated receipts under such programs. Net patient service revenue is reported at the estimated net realizable amounts from patients, third-party payors, and others for services rendered, including estimated retroactive adjustments under reimbursement agreements with third-party payors. Retroactive adjustments are accrued on an estimated basis in the period the related services are rendered and adjusted in future periods as final settlements are determine. Patient accounts receivable are also reduced for allowances for uncollectible accounts. The process for estimating the ultimate collection of receivables involves significant assumptions and judgments. Account balances are written off against the allowance when management determines they are not collectiblE. THE FOOTNOTE TO THE AUDITED FINANCIAL STATEMENTS THAT DESCRIBES BAD DEBT EXPENSE IS AT PAGE 11. PART III, LINE 9B - PROVISIONS ON COLLECTION PRACTICES FOR QUALIFIED PATIENTS The hospital had a written bad debt collection policy that applied to the largest number of its patients throughout 2013 that contains a provision on the collection practices to be followed for ALL patients who are known to qualify for financial assistance. The billing and follow-up process begin with the review and correction of pre-bill edit reports and conclude with the resolution of accounts through regular follow-up activities of the transfer to an outside billing service. 2 - COMMUNITY HEALTH NEEDS ASSESSMENT In addition to the CHNA reported in Part V, the hospital assessed the community's health needs by completing each of the last five years-The Community Service & Prevention Plan (CSPP). It included: a: An examination of the Infirmary's primary service area (PSA) in 2013 which was comprised of the five contiguous zip codes to the south, east and west of the hospital, while we serve the entire metropolitan area. b: A detailed analysis of the entire area, which includes Chinatown on individual Census Tracts. The PSA has been designated by HRSA as a Health Personnel Shortage Area. The majority of these local residents do not consider English their primary language. The percentage of elderly is well above the Manhattan average. Per the US Census, 28% live below the poverty level. c: A listing of all local community health centers (3). The New York Eye and Ear Infirmary is the only specialized hospital and there are no acute care hospitals in the Infirmary's immediate PSA d: a full description of how data on needs assessment were obtained. e: Chronic diseases in the community include asthma, drug abuse, diabetes and numerous ear & eye diseases. The full length Community Service Prevention Plan (and a full and shorter version of the Community Health Needs Assessment, aka the CHNA) including the detailed survey results of the needs assessment, can be found on the hospital's website (NYEE.EDU). The infirmary examined distinct constituencies for both their short-term and long-term specialty care health needs. The Infirmary involved 50 community groups and local governmental authorities, including the Gay Men's Health Crisis and Manhattan Community Planning Board #3, in developing the needs assessment. The community assisted in prioritizing the needs. The survey was administered in Spanish, Russian, Chinese and English-the 4 languages spoken by 1% or more of our patients. 2010 US census data were utilized in addition to NYC DOH community health profiles to assess the health care needs of the local community we serve. Other health care providers, including neighboring acute care hospitals, have helped to identify inExplanationTxt
