DescGoverned by a volunteer community board of trustees, Tucson Medical Center is closely tied to and focused on the community. Tucson Medical Center is dedicated to delivering "caring, personalized, quality healthcare to patients and their families in an environment that is supportive, education-focused and compassionate." Improving the healthcare of the community is the core intent of all our efforts and the basis of our mission. Earnings are used to improve our facility, patient care, medical education, training and research. TMC provides care to those with acute health care needs regardless of their ability to pay for the services needed. Likewise, Tucson Medical Center provides extensive outpatient clinical services such as laboratory, radiology and therapies to name a few. Many of these services are provided to patients who are unable to pay for services. TMC also participates in the Pima Community Access Program that provides access to health services at significant discounts to low income families. Unpaid Costs: TMC provides services to patients covered by AHCCCS and other public programs for low income people. This is the shortfall created when a facility receives payments that are less than the cost of caring for public-program beneficiaries. Uncompensated Cost of Care for Charity Care and Bad Debt: Charity care is free or discounted health services provided to people who can demonstrate that they do not have the means to pay the full cost of care and who meet the organizations financial-assistance policy criteria. Bad debt consists of services for which the hospital anticipated but did not receive payment. For purposes of community benefit reporting, charity care and bad debt are reported in terms of costs, not charges. For more information about TMCs charity care policy, visit www.tmcaz.com. Outreach & Education: This figure includes community benefit programs and activities that provide treatment or promote health and healing as a response to identified community needs. These programs and activities help improve access to health care services, enhance the health of the community and advance medical knowledge.DISCOUNTS FOR UNINSURED PATIENTS Self-pay patients who do not have health insurance are not in the same position as insurance companies to negotiate reduced rates. In an effort to address this disparity, TMC reduces a patients bill by half if the patient is without any insurance coverage. Depending on a patients income, other discounts may apply as part of the TMC Community Care program.HEALING CHILDRENThey say laughter is the best medicine. So its no wonder 14-year-old Silvia was doing remarkably well as she recovered from major spine surgery she had at Tucson Medical Center. The Honduras native is described as the most energetic, outgoing person you will ever meet. She has an infectious giggle that fills the house of her host family a house that was her home for more than three months. Silvia suffered from a severe case of scoliosis and required a spinal fusion with a rod placement, but didnt have access to the treatment in Honduras. Her widowed mother, who sells bread for a living, didnt have the money or access to seek out the medical attention her daughter so desperately needed.Healing the Children is a nonprofit charitable organization that helps underserved children throughout the world secure urgently needed medical assistance they otherwise are unable to obtain. As part of the program, Silvia was flown to Arizona in October 2013 along with another little girl named Doris who underwent a similar surgery at a Phoenix area hospital. The girls bonded with their host family, the Shoemakers, for a few weeks before their operations.Leah and Dave Shoemaker both work full time in addition to raising their three young children, 9-year-old Aston, 4-year-old Bryton, and 2-year-old Caelyn. As you can imagine, it was a huge adjustment for all involved. The dynamics of the house changed instantly. When they first came into our house, it was unbelievably difficult. You want to do everything you can to make them feel welcome. But its amazing how quickly everyone adjusts, said Leah.Silvia lives with her 1-year-old niece and 3-year-old nephew in Honduras, so she was used to having little kids around, and was very helpful around the house. She was very eager to blend in with our family, said Leah. One little problem, however was the language barrier. Silvia speaks Spanish, and speaks very fast. Leah admits her Spanish is rusty at best. My Spanish has gotten better, and she has learned to speak slower, so were finally at a point where she finishes my sentences, she laughed. Silvia has become part of our family.Leading up to surgery day, Silvia was fearless and incredibly grateful, almost as if she was on a mission to get better. The face-to-face medical interpreter at TMC was absolutely amazing and made all the difference for Silvia. Everyone at TMC was extraordinarily welcoming. They did everything they could to make Silvia comfortable, and they treated me as her mom, said Leah.Silvia was in TMC for Childrens Pediatric Intensive Care Unit for five days following her surgery something that was tough to prepare her for. Despite telling her its going to hurt, youre going to be sick there really was no way to prepare her for that, said Leah.Silvia was fortunate to be under the care of Dr. Luis Piedrahita, a surgeon with Tucson Orthopaedic Institute. We are very fortunate to have one of the most advanced health care systems in the world, he said. The great majority of the world population has access only to basic medical care. Complex musculoskeletal problems are not treated and even ignored. Usually very complex surgeries like this one can only be performed here because of the limitations of instrumentation and lack of postoperative care. Bringing the patients here is the ideal thing to do, but requires a lot of non-medical work. I was very pleased with how Healing the Children was able to accomplish this efficiently, and I look forward to continuing to provide my support in any way I can.Anesthesiologist Dr. Luis Esparza from Old Pueblo Anesthesia was asked to participate since he speaks Spanish, and could therefore help her feel a little more comfortable so far from home. I had the opportunity to be involved in providing care for her that she could never have obtained at home. I think that we sometimes lose sight of the fact that what seems routine to us can have such a huge impact on others. Being involved in cases like hers helps to remind me of that. I was happy to be part of the team that cared for her.A bright future aheadThe experience led to the creation of some big goals for Silvia. She wants to learn English, move to America, become a nurse and be able to support her family back home. She knows that the one way to get there is through education.Silvia goes to school in Honduras, whereas most children do not. During the three short months she was here, she made it clear she wanted to go to school. The Shoemakers enrolled her in Emily Gray Junior High. She found a bilingual student who helped her quite a bit, said Leah. She has really embraced this experience and wants to see everything America has to offer. At night, Silvia would spend three to four hours figuring out her homework.Silvia headed back to Honduras in January, but not without leaving her mark in the States. We will keep in touch with her forever, said Leah. These kids become part of your family, and we will never forget them.HEALTHY LIVING CONNECTIONSGone are the days when older adults sit back and wait for dis-ease to take its toll in an inevitable journey of decline. Instead, one of the major health trends amongst older adults is proactively seeking community resources to help stay healthy and prevent or manage conditions. TMC launched Healthy Living Connections seven years ago and its wellness programs this past year to respond to the growing trend of healthy lifestyles. And what better partner for the community than Canyon Ranch Institute, a 501(c)3 non-profit public charity led by the wellness leaders who founded Canyon Ranch Institute.Last year, TMC and Canyon Ranch Institute teamed up to provide the Canyon Ranch Institute Life Enhancement Program to members of our community at no cost. The CRI Life Enhancement Program, also known as the CRI LEP, focuses on self-discovery, preventive care and positive lifestyle habits that enable the participants to experience a healthy transformation through personal empowerment and behavioral changes. The CRI LEP is a 12-week program delivered by a core team of TMC health professionals with expertise in behavioral health, integrative health, nutrition, physical activity and pharmacology. The program includes weekly classes as well as one-on-one appointments with members of the core team.At the core of the program is the philosophy that lasting change is not a one-size-fits-all proposition. It is very important for peopleDesc

Part V, Line 3 - Account Input from Person Who Represent the CommunityIn spring 2012, Tucson Medical Center joined forces with Tucsons nonprofit hospitals on a Pima County Community Health Needs Assessment. The Affordable Care Acts requires tax-exempt, nonprofit hospitals to complete a Community Health Needs Assessment every three years and to implement strategies to address the needs identified in the report. The 121-page assessment was completed with information obtained through written questionnaires and focus groups attended by key informants and community leaders representing the various needs and interests of Pima County. The assessment draws from county health rankings, the Arizona Department of Health Services Vital Statistics, Primary Care Area Statistical Profiles and the U.S. Census Bureau.TMC combined the information from the CHNA with the 2011 Pima County Community Health Assessment and the Arizona Child Fatality Review Program as it developed its Implementation Plan. With input from staff, physicians, and hospital and community leaders, TMC published its Implementation Plan in October 2012. TMC Implementation PlanThe TMC Implementation Plan is based on data from the following:The 2012 Pima County Community Health Needs Assessment 2011 Pima County Community Health AssessmentArizona Child Fatality Review Program
Part V, Line 4 - List Other Hospital Facilities that Jointly Conducted Needs AssessmentTucson Medical Center joined with University of Arizona Medical Center and Carondelet Health Network to produce a Community Health Needs Assessment. This unique collaboration will help guide each nonprofit hospital system develop individual implementation plan. With input from staff, physicians, and hospital and community leadership, TMC released its Implementation in October 2012.
Part V, Line 7 - Explanation of Needs Not Addressed and Reasons WhyChanges in implementation strategy goals: Agility trumps rigidityGiven the dynamic and complex nature of health care, change is inevitable as we adapt to fluctuations in the health care environment. In recalibrating our plans to move forward as new information and opportunities surfaced, TMC did not undertake some of the strategies outlined in the initial implementation plan.At the time of the report, TMC was exploring the creation of the Green Valley Medical Center, a 32-bed acute care hospital. After due diligence, we determined that the required investment was too much for a community hospital to bear.At the time of the report, TMC was planning to develop its own infrastructure that would enhance continuity of care by allowing information to be shared with providers across the continuum, from primary care doctors, to hospitals, nursing homes and rehabilitative facilities. After much analysis, it was determined that putting efforts into a statewide exchange system would be more valuable to the community than developing our own internal exchange, which would have had more limited communication power. As outlined in the Access to Care section above, TMC continues to be on the leading edge of health care information technology and remains committed to ensuring we have the right tools in place to create a 21st century health care delivery system.Palo Verde Behavioral Health: The implementation plan stressed Palo Verde Hospitals longstanding history of providing mental health services in the community. As outlined in the Mental Health section above, although TMC has long valued the service provided at Palo Verde, we also recognized that selling Palo Verde would allow it to thrive in the hands of a company that specializes in behavioral health services. ConclusionAs a non-profit, community hospital that is locally governed and celebrating its 70th year of serving Southern Arizona, TMC is proud of its important role in strengthening healthcare throughout the region. Whether setting patients on the road to recovery or providing tools to community members to help them retain their good health, TMC is committed to acting with purpose to fill gaps in service and address community needs.
Part V, Line 17e - Other Collection Actions by Facility or Third Party EngagedHospital Leins arising out of accidents (insurance) under state law.
Part V, Line 20d - Other Billing Determination of Individuals Without InsurancePatients determined to be FAP eligable or otherwise self pay recieve a 50% discount and may qualify for additional assistance.

Part VI - Community InformationI. PIMA COUNTY GEOGRAPHY, DEMOGRAPHICS AND SOCIAL STATISTICSNote: With the exception of the Primary Care Area Statistical Area data, much of the following geography, demographic and social statistics profile was provided for use in this assessment courtesy of the Pima County Health Department on Dec. 14, 2011.Geography and History:Pima County, located in Southern Arizona, is adjacent to six other Arizona counties and shares an international border with Mexico. The counties are Cochise to the east, Graham to the northeast, Maricopa to the north, Pinal to the north, Santa Cruz to the southeast, and Yuma to the west. The international border to the south is with the State of Sonora, Mexico. The border is approximately 123 miles long, with approximately 62 miles of that border located on Tohono Oodham Nation land. Ports of entry are Sasabe, AZ (Sasabe, Sonora Mexico) and Lukeville, AZ (Sonoyta, Sonora Mexico).Pima County, the second largest of the four original Arizona counties, was created in 1864 and included approximately all of Southern Arizona acquired from Mexico by the Gadsden Purchase. Settlement of the region goes back to the arrival of the Spanish, in the 1690s, who encountered indigenous people already living there.Around the middle of the 18th century, silver and gold were discovered and prospectors from Mexico entered the area in droves. The latter part of the century saw an expansion of mining and ranching in Pima County and an increase in population. The Royal Presidio de San Augustn del Tucson was completed by 1781, and it remained the northern-most outpost of Mexico until the arrival of American soldiers in 1856. From a population of 395 in 1820, Tucson has grown to become the second largest city in Arizona. It has always served as the Pima County seat and was the Arizona Territorial capital from 1867 to 1877.
