Mission DescriptionAs a Catholic health ministry of the Church, St. Joseph Health System's Mission is to "provide spiritually centered, holistic care which sustains and improves the health of individuals and communities." This includes acting as advocates for a compassionate and just society through our actions and words.A universal tenet of Catholic health organizations' mission statements is care for all persons, especially the poor and vulnerable. The call to bring God's healing to persons in need is what compelled the founders of various organizations to venture out into parts of this nation that, until their arrival, had been sorely underserved. For more than 250 years, (59 for St. Joseph Health System) Catholic health care in the United States has been driven by that same call and has grown in ways that could not have been imagined.MissionDescription

ExplanationExplanationIdentifierIdentifierReturn ReferenceReturnReference
Part I, Line 3c: Other Testing Methods for Free or Discounted Care the organization provides medically necessary care to all patients, regardless of race, color, creed, ethnic origin, gender, disability or economic status. The organization uses a percentage of Federal Poverty Level (FPL) to determine free and discounted care. At a minimum, patients with income less than or equal to 200% of the FPL, which may be adjusted for cost of living utilizing the local wage index compared to national wage index, will be eligible for 100% charity care write off of charges for services that have been provided to them. Also, at a minimum, patients with incomes above 200% of the FPL but not exceeding 300% of the FPL, subject to adjustments for cost of living utilizing the local wage index compared to national wage index, will receive a discount on the services provided to them.
Part I, Line 7: For the information in the table, a cost-to-charge ratio was calculated and applied.
Part II: Iosco Human Services Coordinating Council: The mission of the HSCC is to provide leadership, coordinate policy, leverage resources, build assets, and foster partnerships, which promote healthy growth, learning and development for everyone in our community. Collaborative Partners: It includes Iosco County DHS, St. Joseph Health System, AuSable Community Mental Health, Iosco Transit Corporation, Iosco County Family Court, Alcona Health Center, Catholic Human Services, Tawas and Hale Area Schools, Hospice Angelic Care, Michigan Works and many other community agencies. Iosco County Poverty Coalition: The mission of the ICPC is to provide enrichment and relationship-based opportunities for neighbors living in poverty. Collaborative and coordinated efforts are led by Iosco County DHS, Alcona Health Center, St. Joseph Health System, Hospice Angelic Care, Oscoda FISH, Carol's Baby Pantry, Michigan Works and many other community agencies whose goal is to provide support and resources to those interested in education, employment, healthcare, family dynamics and various resources throughout Iosco County. Neighbors helping Neighbors!
Part III, Line 4: From the consolidated audited financial statements of Ascension Health Alliance (which include the activity of St. Joseph Health System, Inc.):The provision for doubtful accounts is based upon management's assessment of expected net collections considering economic conditions, historical experience, trends in healthcare coverage, and other collection indicators. Periodically throughout the year, management assesses the adequacy of the allowance for doubtful accounts based upon historical write-off experience by payor category, including those amounts not covered by insurance. The results of this review are then used to make any modifications to the provision for doubtful accounts to establish an appropriate allowance for doubtful accounts. After satisfaction of amounts due from insurance and reasonable efforts to collect from the patient have been exhausted, the System follows established guidelines for placing certain past-due patient balances with collection agencies, subject to the terms of certain restrictions on collection efforts as determined by the System. Accounts receivable are written off after collection efforts have been followed in accordance with the System's policies.
Part III, Line 8: Ascension Health and related health ministries follow the Catholic Health Association ("CHA") guidelines for determining community benefit. CHA community benefit reporting guidelines suggest that Medicare shortfall is not treated as community benefit.
Part III, Line 9b: The organization has a written debt collection policy that also includes a provision on the collection practices to be followed for patients who are known to qualify for charity care or financial assistance. If a patient qualifies for charity or financial assistance certain collection practices do not apply.
Part V, Section B, Line 3: An important aspect of CHNA completion has included engaging key community collaborators. Collaborators have brought additional expertise to the St. Joseph Health System, Inc. CHNA process through representation of their individual agencies and participation in community coalitions and councils. District Health Department #2: District Health Department ("DHD") #2 has served as an essential collaborator in the CHNA process. The department covers Alcona, Iosco, Ogemaw, and Oscoda counties. DHD #2 has offices in West Branch, Tawas City, Mio, and Harrisville. In cooperation with community resources, the department is responsible for assisting the community and citizens to assume responsibility for their health and the health of the community. The vision statement of DHD #2 focuses on being the primary resource for individual, community and environmental health. The SJHS CHNA process has utilized the public health expertise of DHD #2 Acting Health Officer Debra Baumann. Department of Human Services: The Department of Human Services ("DHS") has also served as a critical collaborator. DHS covers Iosco County with an office in East Tawas. The agency is Michigan's public assistance, child, and family welfare agency. The local agency administers the federal Temporary Assistance for Needy Families grant (Family Independence Program) and Food Assistance program. DHS also takes applications for Medicaid which is administered by the Department of Community Health. A wide range of protective and preventative services are available for families and children who are neglected, abused, or exploited including foster care placement. AuSable Valley Community Mental Health: AuSable Valley Community Mental Health ("AVCMH") serves the residents of Iosco, Ogemaw, and Oscoda counties. AVCMH provides mental health, developmental disability, and prevention services. Offices are located in West Branch, Tawas City, Oscoda, and Mio. Group homes for the care of the developmentally disabled are located within the service area. AVCMH is an affiliate of Northern Affiliation which is a partnership of three Mental Health Boards which include AVCMH, Northeast Michigan Community Mental Health, and North Country Community Mental Health. The affiliation also includes Northern Michigan Substance Abuse Services which is the regional substance abuse coordinating agency. Other Community Collaborators: Other community groups that have been important to the CHNA process have included agencies that focus on those who are poor and vulnerable. Working with the Human Services Collaborating Council and the Poverty Reduction Coalition in Iosco County has supported SJHS initiatives focusing on social and economic issues in our service area. Persons Consulted: Debra Baumann, Acting Health Officer, District Health Department #2; Karin Hobbs, Director, Iosco County Department of Human Services; Diane Pelts, Director of Quality & Managed Care, AuSable Valley Community Mental Health; Debra Miller, Medical Program Director, AuSable Valley Community Mental Health.St. Joseph Health System, Inc.
Part V, Section B, Line 7: While the supporting data identifies SUBSTANCE ABUSE as a critical community need, St. Joseph Health System ("SJHS") does not intend to meet the identified need of substance abuse. SJHS is able to provide a supportive role in this area. Iosco County is fortunate to have AuSable Valley Community Mental Health and Catholic Human Services who provide prevention, treatment and education programs aimed at alcohol, tobacco, prescription medications and illegal drug use. St. Joseph will explore opportunities to compliment these efforts by implementing a smoking cessation program. SJHS Medical Practices are also committed to reducing substance abuse by continued enforcement of its patient/provider controlled substance contract in which continuous review of pain management and medication use is monitored. St. Joseph will continue to provide in-kind meeting room space to support Alcoholics Anonymous. While supporting data identifies ABUSIVE, CONTROLLING and VIOLENT BEHAVIOR as a critical community need, St. Joseph Health System does not intend to fully meet the identified need of abusive, controlling and violent behavior but will provide a supportive role in this area. Building on our past partnership with the dual domestic violence/sexual assault shelter, Inc., SJHS will continue to provide assistance in order to promote a comprehensive community response to sexual assault through education, system change, crisis and primary and secondary victim services. The commitment of SJHS and the local community to providing services specific to rural community issues has been and continues to be clear, established, and ongoing. SJHS is also involved with Bully Free Iosco which is a multi-collaborative group formed to address bullying within Iosco County schools. SJHS will continue to provide support to anti-bullying outreach programs to empower students and communities to change bullying behavior.St. Joseph Health System, Inc.
Part V, Section B, Line 14g: The policy was posted on the hospital facility's website as the financial assistance policy is posted on website and below is the URL for your reference:http://www.sjhsys.org/documents/SJHS_Tawas_CHNA.pdfA summary of the financial assistance policy is posted on the hospital facility's emergency rooms or waiting rooms, admission offices and available upon request. In accordance with our Mission and Values, St. Joseph Health Care System is committed to caring for all people regardless of their ability to pay, with special attention to those who are poor and vulnerable. Help is available by contacting 989 984-3796, emailing, BMcNally@stmarysofmichigan.org or in person at any of our facilities.St. Joseph Health System, Inc.
Part V, Section B, Line 18e: Question 18 is more appropriately answered as not applicable as the Billing and Collections Policy of St. Joseph Health System does not allow a hospital to engage in extraordinary collection actions before the organization made reasonable efforts to determine whether the individual is eligible for assistance under the financial assistance policy. Reasonable efforts taken include but are not limited to: - Notifying individuals of the financial assistance policy on admission- Notifying individuals of the financial assistance policy prior to discharge- Notifying individuals of the financial assistance policy in communications with the patients regarding the patients' billsDocumenting its determination of whether patients were eligible for financial assistance under the hospital facility's financial assistance policySt. Joseph Health System, Inc.
Part VI, Line 2: St. Joseph Health System relies on information from: St. Joseph health System Emergency Department patients; Michigan Statistics - www.aecf.org/kidscount; www.michigan.gov/mdch; http://www.census.gov; www.michigan.gov/mdch; www.aecf.org/kidscount; http://www.diabeters.org; www.michigan.gov/mdch; Community Health Assessment (Alcona, Iosco, Ogemaw, and Oscoda Counties) February 1996 - Demographics: pp.11-22 Key Findings, Population, Educational Attainment, Employment and Income, Poverty, Housing, and Public Assistance, Conclusion; market share data-theidsonline.com; Community Needs Assessment Alcona 2009.
Part VI, Line 3: Upon admission to St. Joseph Health System, all self-pay patients speak with an associate regarding financial assistance that is available to them. Associates help the patient(s) determine eligibility for various government benefits as well as St. Joseph's internal financial assistance program. Should the patient(s) not be consulted upon admission, a letter is sent with their first itemized billing detailing the available financial assistance programs.
Part VI, Line 4: St. Joseph Health System serves five rural counties in Northeast Michigan. The counties of Alcona, Arenac, Iosco, Ogemaw and Oscoda have a total population of approximately 83,000 residents (2010 U.S. Census Bureau). Three zip codes (East Tawas, Tawas City and Oscoda) comprise our primary service area with approximately 18,500 residents. St. Joseph Health System is located in Tawas City, Michigan and remains one of the largest employers within Iosco County. Several automotive parts suppliers, aircraft maintenance and refurbishing facilities are prominent employers within Iosco County. Within the primary service area 96.5% of residents are Caucasian, with 1.7% Hispanic, 0.7% African American, -1% Indian and Asian. Of the approximately 18,500 residents in the primary service area approximately 81% are 45 years of age or older. Payer Mix:- Medicare - 54.65%- BlueCross - 13.73%- Medicaid - 14.73%- Commercial - 7.44%- Self Pay - 3.92%- Other - 5.53%. St. Joseph Health System owns seventeen provider-based and rural health clinics located throughout the full service market area. The scope of service includes primary care, obstetrics, gynecology, pediatrics, general surgery, internal medicine, after hours and specialty care. Additionally, there are two federally qualified health centers located in Iosco and Arenac counties respectively. There are two hospitals located approximately one hour travel time north and west of St. Joseph Health System. In an effort to frame our community demographic related to important factors such as unemployment and uninsured data affecting overall health, we reflect back on our 2013 CHNA. The CHNA revealed the following statistics for Iosco County. Michigan Department of Technology, Management and Budget - Labor Market Information indicates significantly higher unemployment rates affecting Iosco County residents. Review of historical Iosco County data reveals consistently higher than Michigan average; trends as high as 20.9% in January 2010 to lower rates of 8.4% in October 2012. These figures shed light on a community's overall economic situation and provide information about the percentage of the population that may be at risk for various health concerns associated with unemployment. Unemployment has been linked with declines in health status and may lead to physical health responses ranging from self-reported physical illness to mortality, especially suicide. It has also been shown to lead to an increase in unhealthy behaviors related to alcohol and tobacco consumption, diet and exercise, which in turn can lead to increased risk for disease or mortality. Uninsured rates in Iosco County are in the 15% range as compared to Michigan as a whole at 14%.
Part VI, Line 5: Alcoholics Anonymous - While St. Joseph Health System has identified heavy alcohol consumption and substance abuse as growing health issue during the process of our Community Health Needs Assessment, we have agreed to provide a supportive role in this area. Our community includes several organizations whose primary responsibility is to provide prevention and education programs as well as treatment to those affected by alcohol or substance abuse. Although choosing a supportive role, SJHS is committed to addressing the problem and providing support where possible. In order to address this issue, St. Joseph offers In-Kind space to the local Alcoholics Anonymous group. The group meets Wednesday and Thursday evenings each week of the year for approximately 2 hours. Alcoholics Anonymous is a fellowship of men and women who help each other maintain sobriety and who offer to share their recovery experience freely with others who may have a drinking problem. Support Groups - St. Joseph Health System offers several Support Groups with the purpose of addressing social, psychological or emotional issues related to specific diagnoses or occurrences. Diabetic Club - It is a program that is open to the community at no charge and includes a short lecture about general health issues and how those issues affect diabetes. Topics related to lifestyle modifications and tools for diabetes management are taught by a certified Dietician or other health care provider from the Health System. The group also offers peer to peer support. Grief Support Group - It is designed to provide a safe environment to talk about feelings as they relate to loss, learn to help oneself, discover new ways to cope with these feelings and changes in one's life, and identify new sources of strength and hope. Ostomy Support Group - This support group is designed for anyone who has a stoma (an alternate exit of their bowel or bladder), as well as their caregivers or loved ones. The meetings offer information on community resources available for ostomy patients, as well as tips on the latest equipment and ways to cope with an ostomy. Survivors of Suicide - It is a peer facilitated support group with the goal of providing understanding and support to those who have lost a loved one to suicide and to increase awareness so future acts can be prevented. The group's intent is to provide a safe place to share feelings and provide support towards healthy resolution of grief.
Part VI, Line 6: St. Joseph Health System is the member of Ascension Health. Ascension Health Alliance became the sole corporate member and parent organization of Ascension Health, a Catholic, national health system consisting primarily of nonprofit corporations that own and operate local health care facilities, or Health Ministries, located in 23 of the United States and the District of Columbia. In addition to serving as the sole corporate member of Ascension Health, Ascension Health Alliance serves as the member or shareholder of various other subsidiaries. Ascension Health Alliance, its subsidiaries, and the Health Ministries are referred to collectively from time to time hereafter as the System. Ascension Health Alliance is sponsored by Ascension Health Ministries, a Public Juridic Person. The Participating Entities of Ascension Health Ministries are the Daughters of Charity of St. Vincent de Paul in the United States, St. Louise Province, the Congregation of St. Joseph, the Congregation of the Sisters of St. Joseph of Carondelet, the Congregation of Alexian Brothers of the Immaculate Conception Province-American Province, and the Sisters of the Sorrowful Mother of the Third Order of St. Francis of Assisi-US/Caribbean Province. Marian Health System, which was previously sponsored by the Sisters of the Sorrowful Mother of the Third Order of St. Francis of Assisi US/Caribbean Province, became part of Ascension Health on April 1, 2013. In addition, Alexian Brothers Health System, which was previously sponsored by the Congregation of Alexian Brothers of the Immaculate Conception Province, Inc. American Province, became part of Ascension Health on January 1, 2012. The 49-bed hospital, staffed by more than 460 employees, is located in Tawas City, Michigan and is a nonprofit acute-care hospital. The hospital maintains two wholly owned subsidiaries: St. Joseph Health Enterprises, a home medical equipment business, and St. Joseph Health System Foundation, a charitable foundation created to receive and administer funds for the hospital for the purpose of acquiring or improving hospital facilities and technologies, and the establishment, support, and maintenance of community health education programs. The hospital provides inpatient, outpatient, and emergency care services for the residents of Iosco County and the surrounding area. Admitting physicians are primarily practitioners in the local area. St. Joseph Health System offers access to a large board-certified medical staff, with more than 17 primary care and specialty physicians. Through community health services, professional education, subsidized health care, financial contributions, community-building activities, and other community benefit activities, St. Joseph Health System contributed $1.6 million to the community. Included in this figure are programs such as:- Charity Care and unreimbursed Medicaid- Community health-education lectures- School health-education programs promoting health care careers & workforce enhancement- Free health screenings- HealthKey- SART (Sexual Assault Response Team)- Community-based chaplaincy and spiritual care- Sponsorships and contributions to various community events.Ascension Health Alliance directs its governance and management activities toward strong, vibrant, Catholic Health Ministries united in service and healing, and dedicates its resources to spiritually centered care which sustains and improves the health of the individuals and communities it serves. In accordance with the System's mission of service to those persons living in poverty and other vulnerable persons, each Health Ministry accepts patients regardless of their ability to pay. The System uses four categories to identify the resources utilized for the care of persons living in poverty and community benefit programs:- Traditional charity care includes the cost of services provided to persons who cannot afford healthcare because of inadequate resources and/or who are uninsured or underinsured.- Unpaid cost of public programs, excluding Medicare, represents the unpaid cost of services provided to persons covered by public programs for persons living in poverty and other vulnerable persons.- Cost of other programs for persons living in poverty and other vulnerable persons includes unreimbursed costs of programs intentionally designed to serve the persons living in poverty and other vulnerable persons of the community, including substance abusers, the homeless, victims of child abuse, and persons with acquired immune deficiency syndrome.- Community benefit consists of the unreimbursed costs of community benefit programs and services for the general community, not solely for the persons living in poverty, including health promotion and education, health clinics and screenings, and medical research.Discounts are provided to all uninsured patients, including those with the means to pay. Discounts provided to those patients who did not qualify for assistance under charity care guidelines are not included in the cost of providing care of persons who are poor and community benefit programs. The cost of providing care of persons who are poor and community benefit programs is estimated using internal cost data.
