DescQUALITY AND PATIENT SAFETY: Completed the optional (third) year of Partnership for Patients, the nationwide public-private partnership to reduce preventable hospital acquired conditions by 40% and preventable 30-day re-admissions by 20% over 3 years with 68 hospitals participating. As of December, 2014, OHA Hospital Engagement Network (HEN) hospitals demonstrated a 55% reduction overall in the ten healthcare associated conditions/re-admissions but specifically: -100% reduction of Ventilator Associated Pneumonia (VAP) -59% reduction in Central Line Associated Blood Stream Infections (CLABSI) -54% reduction in Pressure Ulcers -49% reduction in Falls -39% reduction in C. Difficile -35% reduction in early elective deliveries -20% reduction in Urinary catheter infections (CAUTI) -36% reduction in surgical site infections -25% reduction in VTE -7% reduction in re-admissions This constitutes a reduction of overall costs by at least $16,981,699. In addition hospitals improved patient and family engagement by 39% and improved leadership by 29%. As result of the significant reduction of harm, 67 hospitals received a $10,000 stipend (total of $670,000). For the 68 hospitals in the OHA HEN, 23 student nurse process observers (at no cost to the facility) were deployed to collect real time data related to healthcare worker hand hygiene, head-of-bed elevation, and central line maintenance. Significant improvement in compliance with these measures were key strategies to reduction in hospital related infections. The Leading Edge Advanced Practice Topics (LEAPT) award from Centers for Medicare and Medicaid completed in December 2014 for rapid cycle quality improvement initiatives in the areas of: -Severe sepsis/septic shock - participating hospitals demonstrated a 38.6% reduction in surgical patients and a 10.6% reduction in non-surgical patients. -Hospital acquired acute renal failure - participating hospitals demonstrated a 20% reduction in AKI -Results beyond 40/20 aims in re-admissions, including hospital teaming with their broader community on work to reduce re-admissions -Cost savings calculations for hospital acquired conditions (core and/or above), especially using "actual" cost and volume data (as compared with "projected") - participating hospitals developed a modeling approach using the Medicare spending per beneficiary (MSPB) data -Hospital culture of safety that fully integrates patient safety with worker safety - participating hospitals demonstrated a 42.6% reduction in employee injury As result of the significant reduction of harm, 27 hospitals received a $50,000 stipend (total of $1,350,000). Facilitated subscriptions for continuous survey readiness to member hospitals via subscription to the CSR consultant service for 89 hospitals. Provided both a Spring and a Fall conference related to accreditation standards at two locations per offering. Maintained three regional quality collaborates in the Northeast, Central and Northwest Ohio regions. Each collaborative was designated two co-chairs, updated their participation agreements, and re-visited their desired areas of focus. Conducted the 7th Annual Quality Summit on June 11, 2014 entitled, "Transforming Ohio Through Our Leadership" with over 200 attendees participating. The summit was held at the Hilton at Easton, Columbus, Ohio and featured four panel presentations that addressed the four statewide focus areas and professional poster presentations with ample networking opportunities to disseminate effective practices followed by the posting of panel presentations and poster abstracts to the OHA website.Desc

DescDISASTER PREPAREDNESS: Regional Initiative Funding -Regional funds in the amount of $791,463.00 were received to: complete required deliverables, provide $1,500.00 (or more) funding for each hospital to continue their preparedness issues, and to support the regional hospital coordinator, the regional coalition coordinator, and both local county coalitions, and one regional coalition. Drills and Exercises - collaboration with state and regional partners -Regional Full Scale Exercise partnership with Public Health and Regional Coalition members to test hospital and community preparedness for recovery procedures after an earthquake. The exercise was designed to provide all participants the opportunity to establish incident command and evaluate community preparedness. -Regional SNS Request Exercise - partnership with all 18 hospitals, two regional PH coordinators, 14 local health departments and 14 county EMAs (hospitals are only present in 14 of the 21 counties). The drill tested the SNS Request Standard Operating Guide developed for and adopted by the hospitals which outlines how to request and provide information to PH and EMA. Special Training -SNS requesting/receiving/awareness training was held via webinar in September, 2014 and was attended by 43 hospital participants. -Training on how to assemble the mobile all-weather shelters (aka BluMed) was held at two locations in September and was attended by 35 hospital, public health, and EMA participants. Assets SE Ohio has always been conscious of the size and rural nature of the region. All assets are stored at no expense to the grant and are available to the entire region -Patient Care/Med Surge supply caches are pre-positioned in 10 of the 21 counties and are inventoried and moved yearly -Mass casualty trailers or supplies are pre-positioned in 5 counties cross the region -Refrigerated remains trailers or portable units are pre-positioned in 5 counties -Bus Fitting evacuation cots are pre-positioned in 11 counties -Oxygen administration units are in two counties -Large portable generators are in two counties and large generators have been installed in senior centers/community centers in two counties For FY 2014, we are initiating preventive maintenance record submission for all regional assets. Hospital inventories of grant purchased equipment are verified through site visits every two years Coalitions Regional - the Southeast Ohio Healthcare Coalition was developed in FY 2013 with by-laws adopted and a Steering Committee set. The Steering Committee approved the FY 2014 budget and activities related to the coalition, both regional and local -In FY 2014 the SOHC will develop a communications plan and a command and control plan which will be tested in drills and exercises -Proposed activities for FY 2014 have been cross walked against the BP2 reporting measures to ensure we are on track Local - since 2009, SE Ohio has worked to establish local healthcare coalitions consisting of hospitals, public health, EMA, and all other healthcare and other organizations willing to participate. 14 groups work on a local/county/multi-county level to improve the healthcare preparedness State Initiative Funding State funds in the amount of $560,000.00 were used to initiate new statewide programs/initiatives and to sustain existing programs/initiatives as well as to provide funding for the Director of Public Health Programs, to oversee this grant and the SE Ohio Regional Grant. These initiatives included: -Provided training to healthcare facilities, Regional Healthcare Coordinators and other healthcare providers on the family reunification system via development of webinar links that can be viewed at the leisure of the member user. This provides our members with the opportunity to provide training to a large number of individuals within the facility with no travel, and no loss of work costs -Pediatric triage model for crisis standard of care. Triage mode has been completed and is currently out, with ODH permission for peer review at the request of the developers. This triage model will serve as a model not only for Ohio hospitals and healthcare facilities but for healthcare facilities across the nation. The Institute of Medicine and Assistant Secretary for Preparedness and Response are extremely interested in viewing the final product for endorsement -Four provider sessions were held throughout the state of Ohio. Sessions were well received with useful direction received in return. The final report has been compiled with results and recommendations identified and shared with ODH leadership and Crisis Standards of Care Steering Committee. -Expansion of the Ohio Burn Surge Plan was completed with the identification of a lead state burn coordinating center. It was determined by the Burn Surge Committee that OSU Medical Center would serve as the primary state coordinating center, with Metro Health and University of Cincinnati Hospital serving as secondary coordinating centers. Work will continue in the next grant period on training and exercising these facilities with state agencies to include Ohio Department of Health, Ohio Emergency Management Agency, and the American Burn Association. A burn surge website was developed, which provides "just-in-time" training. -Development of standard data field definitions and process for data entered into Surgenet (bed tracking) by hospitals and healthcare partners was completed and approved by Ohio Department of Health. Webinars are in place to provide Surgenet users with education on the changes along with a web based test site that can be utilized for training of new users, and during exercises. The development of the test site was a direct request from hospitals so that multiple users could receive training and participate in exercises on the system without affecting the live system or incurring travel or loss of work costs for hospitals.Desc

ExplanationThe Internal Revenue Service has ruled that the REF and FHC are tax exempt organizations as defined under Section 501(c)(3) of the Internal Revenue Code. Accordingly, no provision for federal income taxes has been made in the financial statements. Additionally, the REF and FHC have been determined not to be a private foundation under Section 509(a) of the U.S. Internal Revenue Code. Accounting principles generally accepted in the United States of America prescribe recognition thresholds and measurement attributes for the financial statement recognition and measurement of a tax position taken or expected to be taken in a tax return. Tax benefits will be recognized only if the tax position is more likely- than-not sustained in a tax examination, with a tax examination being presumed to occur. The amount recognized will be the largest amount of tax benefit that is greater than 50% likely of being realized on examination. For tax positions not meeting the more-likely-than-not test, no tax benefit will be recorded. Management has concluded that they are unaware of any tax benefits or liabilities to be recognized at December 31, 2014. The Foundation does not expect the total amount of unrecognized tax benefits to significantly change in the next 12 months.ExplanationTxt
