DescFor 150 years, NMH and its predecessor institutions, Passavant Memorial and Wesley Memorial hospitals, have served residents of Chicago. The commitment to provide healthcare, regardless of the ability to pay, reaches back to the founding principles of Passavant and Wesley and continues to be integral to our Patients First mission. Importantly, it provides the foundation for our mission-driven commitment to improve the health of the communities we serve. This commitment is advanced by enhancing patient access, safety and quality through community-based primary care, hospital-based primary care, specialty, diagnostic and other services and partnerships with community health centers that date back more than 50 years. NMH serves as the primary teaching hospital for Feinberg. In fiscal year 2014, NMHC integrated its primary care medical group subsidiary into NMFF, the multispecialty group practice for the fulltime faculty of Feinberg (NMFF) and created NMG, which now has more than 1,100 physicians representing virtually every medical specialty. Northwestern Medicine and all of its contributing entities are positioned to advance groundbreaking work that can only be accomplished with the resources of an academic medical center (AMC). Through partnerships with public and private organizations, Northwestern Medicine applies its expertise in research, education and clinical care to begin the process of eradicating the growing and complex public health issues facing America today - from diabetes, obesity, cancer and heart disease to persistent violence in its communities. NMH is among only six percent of the nation's hospitals designated as an AMC hospital, which according to the Association of American Medical Colleges (AAMC), in aggregate deliver a vastly disproportionate share of the nation's trauma, intensive care and tertiary services; provide 28 percent of all Medicaid care; and underwrite 41 percent of all hospital-based charity care. As the only adult acute care hospital located in Chicago's growing downtown area, more than 45,000 adult patients were admitted to NMH as inpatients in fiscal year 2014. As the only adult Emergency Department (ED) in downtown Chicago with 24/7 service, NMH had more than 83,000 ED visits in fiscal year 2014. NMH is also the only AMC hospital in Chicago participating in both city and state Level I trauma networks and as a Level III neonatal intensive care unit, allowing us to provide lifesaving care and treatment to the most seriously injured adults and premature and sick infants. In addition to meeting the needs of the patients and communities in our primary service area, NMH also serves an important role for patients outside of Chicago. As a nationally ranked AMC hospital and a major referral center in the Midwest and beyond, NMH is one of a limited number of places in the region where patients requiring advanced tertiary, quaternary or specialty services can access the care and services they need.Desc

DescNMHC provides access to specialty medical care, clinical trials and a host of other healthcare services for patients in Lake County and surrounding regions through NLFH and NMG. From its founding 116 years ago as the Alice Home on the campus of Lake Forest College, NLFH has upheld the promise to provide Lake County residents with convenient access to quality care supported by advanced diagnostics and technology. NLFH has continually expanded its healthcare services to respond to the growing needs of the community. NLFH provides care for those unable to pay, consistently providing the highest percentage of charity care as a percent of patient revenue among Lake County hospitals. NLFH's board-certified emergency physicians and trauma-trained nurses serve and support the Region 10 Emergency Medical System, providing trauma and emergency care to patients through the Level II Trauma Center at NLFH and emergency services at the Northwestern Medicine Grayslake Outpatient Center.Desc

northwestern memorial hospitalPart V, Section B, Ques 1, Line j, Other The CHNA report also describes the background of Northwestern Memorial Hospital (hereafter NMH), NMH charity Care, NMH Mission, Providing and Ensuring Access to Care, CHNA Goals and Objectives, Public Dissemination and Development of the Implementation Plan.
Northwestern memorial HospitalPart V, section B, Ques 3, persons identified and consulted As part of the CHNA, four focus groups were held among key stakeholders representing public health, physicians, other healthcare professionals, social service providers and other community leaders from throughout Chicago. A list of recommended participants for the NMH focus groups was provided by NMH. Potential participants were chosen because of their ability to identify primary concerns of the populations with whom they work, as well as of the community overall. Focus group candidates were first contacted by letter to request their participation. Follow-up phone calls were then made to ascertain whether or not they would be able to attend. Confirmation calls were placed the day before the groups were scheduled to insure a reasonable turnout. Audio from the focus group sessions was recorded. Findings from the focus group represent qualitative rather than quantitative data. The group was designed to gather input from participants regarding their opinions and perceptions of the health of the residents in the area. Thus, these findings are based on perceptions, not facts. In total, focus groups held as part of this CHNA incorporated input from 26 key informants (or community stakeholders), with special emphasis on persons who work with or have special knowledge about vulnerable populations in South Chicago, North Chicago, Downtown/West Chicago, as well as throughout Cook County, including low-income individuals, minority populations, those with chronic conditions and other medically underserved residents. A list of these participants is provided below: a. La Rabida Children's Hospital b. Center s for New Horizon c. South East Chicago Commission d. KLEO Center e. North Park University f. Northwestern memorial HospitalPart V, section B, Ques 7 The CHNA report completed in August 2013 identified areas of opportunity for health improvement for which NMH and its External Steering Committee determined it would not prepare an implementation plan and strategy. These areas of opportunity and the reasons for not addressing are below. Cancer (Cancer Deaths): NMH provides a comprehensive range of outpatient and inpatient services to cancer patients, including prostate, breast and colorectal. NMH will continue to sustain these services. The External Steering Committee recommended that NMH focus on strengthening and improving access to medical homes, where access to these services can be effectively coordinated. Chronic Kidney Disease (Kidney Disease Deaths): NMH provides clinical services to treat chronic kidney disease. The External Steering Committee recommended focusing efforts on other health conditions for which NMH could have a greater impact. Family Planning (Births to Unwed Mothers, Births to Teens): NMH provides a comprehensive range of outpatient and inpatient services to expectant women and teens, including family planning services. NMH will continue to sustain these services and work to strengthen community-based medical homes where family planning services can be conveniently accessed. HIV (HIV/AIDS Deaths): NMH provides clinical services to treat chronic HIV/AIDS and collaborates with Feinberg in conducting research to better prevent, detect and treat HIV/AIDS. The External Steering Committee recommended focusing efforts on improving access to medical homes, where access to these services can be effectively coordinated. Maternal, Infant and Child Health (Lack of Prenatal Care, Low Birth Weight, Infant Mortality): NMH provides a comprehensive range of outpatient and inpatient services to expectant women and teens. NMH will continue to sustain these services. The External Steering Committee recommended that NMH focus on strengthening and improving access to medical homes, where access to these services and other prenatal care can be effectively coordinated. Mental Health & Mental Disorders (Mental Health Status, Chronic Depression, and Mental Health Treatment): NMH provides clinical services to treat mental health and collaborates with Feinberg in conducting research to better detect and treat mental health issues. The External Steering Committee recommended that NMH focus on strengthening and improving access to medical homes, where mental health issues can be effectively monitored and coordinated. Oral Health (Recent Dental Care (Adults)): NMH does not provide office-based dental care services. Respiratory Disease (Pneumonia/Influenza Deaths, Pneumonia Vaccinations (65+), Prevalence of Asthma (Adults), Tuberculosis Incidence): NMH provides clinical services to treat pneumonia, asthma and tuberculosis. The External Steering Committee recommended that NMH focus on strengthening and improving access to medical homes, where preventive care and screening services can be effectively coordinated and access to medically necessary specialty care can be facilitated. Sexually Transmitted Diseases (Gonorrhea Incidence, Syphilis Incidence, Chlamydia Incidence): The External Steering Committee recommended that NMH focus on strengthening and improving access to medical homes, where counseling on prevention and screening for disease can be effectively coordinated and access to medically necessary specialty care can be facilitated. Substance Abuse (Cirrhosis/Liver Disease Deaths, Binge Drinking, Illicit Drug Use): The External Steering Committee recommended focusing efforts on other health conditions for which NMH could have a greater impact. Tobacco Use (Exposure to Environmental Tobacco Smoke): NMH supports public policies aimed at reducing tobacco use. The External Steering Committee recommended focusing efforts on other health conditions for which NMH could have a greater impact. Vision (Blindness/Uncorrectable Vision Problems): The External Steering Committee recommended that NMH focus on strengthening and improving access to medical homes, where primary vision screenings can be effectively and conveniently provided. The External Steering Committee recommended focusing efforts on other health conditions for which NMH could have a greater impact. Many health organizations in Chicago were identified as providers of services to treat these health needs (see CHNA Report).

Community benefit ReportSchedule H, Part I, Line 6a NORTHWESTERN MEMORIAL HEALTHCARE AND SUBSIDIARIES (NMHC) SUBMIT A COMMUNITY BENEFIT REPORT TO THE ILLINOIS ATTORNEY GENERAL ACCORDING TO THE REQUIREMENTS FOR THE STATE OF ILLINOIS. NORTHWESTERN MEMORIAL HOSPITAL'S(NMH), NORTHWESTERN LAKE FOREST HOSPITAL'S (NLFH) AND ALL OTHER NMHC NON-PROFIT SUBSIDIARIES' RESULTS ARE INCLUDED IN THIS REPORT. Effective september 1, 2014 such report contains the results of Cadence Health. A COMPLETE COPY OF THE REPORT IS AVAILABLE ON REQUEST.
Costing methodologySchedule H, Part I, line 7 THE COST OF financial assistance at cost WAS CALCULATED BY APPLYING THE TOTAL COST-TO-CHARGE RATIO FROM EACH HOSPITAL'S MEDICARE COST REPORT (CMS 2552-10 WORKSHEET C, PART 1, CONSISTENT WITH THE STATE OF ILLINOIS ATTORNEY GENERAL OFFICE DEFINITION) TO THE CHARGES ON ACCOUNTS IDENTIFIED AS QUALIFYING FOR CHARITY CARE (AS DEFINED IN THE AMERICAN INSTITUTE OF CERTIFIED PUBLIC ACCOUNTANTS ACCOUNTING AND AUDITING GUIDE - HEALTHCARE ORGANIZATIONS). THE RESULTANT CALCULATED COST WAS THEN OFFSET BY ANY PAYMENTS AND/OR CONTRIBUTIONS RECEIVED THAT WERE DESIGNATED FOR THE PAYMENT OF PATIENT BILLS QUALIFYING FOR A CHARITY CARE DISCOUNT (AS DEFINED IN THE HEALTHCARE FINANCIAL MANAGEMENT ASSOCIATION'S PRINCIPLES AND PRACTICES BOARD STATEMENT 15: VALUATION AND FINANCIAL STATEMENT PRESENTATION OF CHARITY CARE AND BAD DEBTS BY INSTITUTIONAL HEALTHCARE PROVIDERS). THE UNREIMBURSED COST OF BAD DEBT, MEDICAID, MEDICARE OR ANY OTHER FEDERAL, STATE OR LOCAL INDIGENT HEALTHCARE PROGRAM IS NOT INCLUDED IN THE UNREIMBURSED COST FIGURE FOR CHARITY CARE. THE UNREIMBURSED COST OF MEDICAID FOR THE HOSPITALS WAS CALCULATED BY APPLYING THE HOSPITALS' OVERALL COST-TO-CHARGE RATIOS TO THEIR TOTAL MEDICAID INPATIENT AND OUTPATIENT CHARGES AND THEN SUBTRACTING PAYMENTS RECEIVED UNDER THESE PROGRAMS. THE COST-TO-CHARGE RATIOS ARE ADJUSTED TO EXCLUDE MEDICAL EDUCATION AND OTHER COSTS THAT ARE INCLUDED ELSEWHERE ON SCHEDULE H. THE UNREIMBURSED COST OF MEDICAID FOR FISCAL YEAR 2014 IS REDUCED BY $29.4 MILLION OF NET REIMBURSEMENT NMHC RECEIVED UNDER THE ILLINOIS HOSPITAL ASSESSMENT PROGRAM. The costs for OTHER BENEFITs WERE CALCULATED PRIMARILY BASED ON DIRECT COSTING METHODOLOGY CONSISTENT WITH FUNCTIONAL EXPENSE REPORTING IN THE FOOTNOTES TO THE AUDITED FINANCIAL STATEMENTS.
Bad Debt expense removedschedule h, part I, line 7 a thru k The amount of bad debt expenses included in Part IX line 25 is $83,070,628. This amount is subtracted from total costs for calculating the percentages.
Subsidized Health Servicesschedule H, Part I, line 7g THE BENEFITS REPORTED ARE PRIMARILY ASSOCIATED WITH OPERATING LOSSES SUPPORTING NMH'S MENTAL HEALTH PROGRAMS. NMHC DOES NOT INCLUDE COSTS ATTRIBUTABLE TO PHYSICIAN CLINICS AS SUBSIDIZED HEALTH SERVICES.
Community Building activitiesschedule H, part II, Description Line 8: Work Force Development Community Training and Education programs at NMHC are in place to ensure that a highly trained healthcare workforce of adequate capacity is in place to serve the residents of the region. Importantly, these programs create pathways for at risk members of the community to seek jobs within the healthcare system and also are in place for young people to learn about and potentially explore healthcare careers. NMHC is committed to providing training and employment opportunities for residents of the community while addressing the shortage of healthcare workers through a direct, formal training pipeline. Certificate Programs NMH offers 12 to 21-month certificate programs in four areas including nuclear medicine technology, radiation therapy, radiography and diagnostic medical sonography. The programs are available to employees as well as the general public. Many students come from the local community as well as from affiliated colleges and universities. Leaders of these programs visit city high schools, colleges and universities to introduce various medical fields to prospective students and increase their general knowledge of various allied health fields. NMH is an important clinical setting for the education of the next generation of healthcare workers, from physicians to nurses to skilled technicians. Through clinical affiliations with top regional universities and colleges and established clinical rotations, mentoring, clinician shadowing, traditional didactic lectures and other teaching programs, NMH provides a clinical setting for the education of hundreds of students, many of whom will become professionals in fields identified as areas of current or future workforce shortage in the national healthcare system. NMHC has more than 140 Educational/Student Affiliation Agreements in place with public and private colleges and universities across the country, for both clinical and non-clinical students in both undergraduate and graduate-level degree programs. NMH provided education in fiscal year 2014 to: a. More than 700 undergraduate and graduate nursing students b. 400 students from university-based pharmacy programs c. 50 respiratory therapy students d. Graduate social work interns e. Interns in biomedical engineering f. Students in a broad array of clinical programs. On-the-Job Training and Youth Education Programs 1. Since 1997, NMH has partnered with the CARA program to help homeless and other at-risk adults in their efforts to achieve long-term employment success by providing on-the-job training skills that ready them to move into the workforce. NMH has hired more than 120 employees through this partnership since it began, including three in fiscal year 2014. 2 NMH offers ongoing, comprehensive youth programs that expose Chicago students to potential healthcare careers. - For 13 years, through the Medical and Health Careers Academy, NMH has partnered with high schools in the Chicago Public School (CPS) system to promote interest in post-high school education and healthcare careers. At CPS's Westinghouse College Preparatory High School (Westinghouse), a selective enrollment high school located in Garfield Park on the city's west side, NMH employees and physicians talk with students about jobs in healthcare and also invite students and their parents to the hospital, where they receive a behind-the-scenes understanding of clinical areas and potential careers. Several years ago, NMH and Feinberg developed the Northwestern Medicine Scholar's Program at Westinghouse. Through the program, talented students who wish to become physicians or biomedical researchers are provided learning opportunities. A group of high-achieving high school freshmen are selected each year to participate in the four-year program which includes mentoring by senior faculty members, an intensive three-week summer program, distance learning, ACT test preparation and leadership and life skills development. Twenty-four students participated in fiscal year 2014. - The Cristo Rey Internship Program, run collaboratively with the Cristo Rey Jesuit High School in the Pilsen neighborhood on the city's near west side offers students from a primarily Hispanic community an opportunity to work one day a week in an administrative role with the Human Resources division or within the Information Services department at NMH to gain valuable work experience and learn time management and organizational skills in a corporate setting. Nine students participated in this program in fiscal year 2014. - NMHC hosted one high school student intern in fiscal year 2014 through the Urban Alliance program, which empowers under-resourced youth to aspire, work and succeed through paid internships, formal training and mentorship. - NMH has hosted Medical Explorers Post 9766 since 1996. Students participate in a variety of activities designed to encourage their expressed interest in healthcare careers and expose them to the field. The program emphasizes career exploration, life skills, service learning, character development and leadership. Students participate in tours, hear guest speakers and join in discussions and projects. The program offers internships, mentorship, tutoring, networking, community service opportunities and scholarships. To date, more than 800 high school and college students have participated in NMH's Medical Explorers program. Since the program began, many Medical Explorers have pursued careers in nursing and medicine and several are now employed at NMH. 3. NMHC continues to offer comprehensive internships and fellowships for college students and post-graduates. - NMH sponsors the INROADS program, which provides progressive internships, year-round academic instruction and summer workshops to prepare minority college students for the corporate work setting. Initially developed under the federal "Hire the Future" program, NMH was the first Chicago hospital to participate in this program. Students benefit from mentoring and leadership training to prepare them for future positions in a healthcare career. - Chicago Scholars is a not-for-profit organization that provides a comprehensive five-year program of mentoring, internship placement, networking, college admission assistance and scholarships to college-bound and college-level Chicago youth from primarily low-income backgrounds. NMHC is a "High Five Partner" of the program, contributing $25,000 each year for four years. Members of the Human Resources department at NMHC provided support to this program by participating in the applicant interview and selection process and volunteering at the career fair. In fiscal year 2014, NMH provided a paid internship to a college-level student participating in the Chicago Scholars program. - NMHC offers administrative fellowships and internships to help introduce select post-graduate students to various aspects of leadership within an AMC hospital.
Bad debt expense footnoteSchedule H, part III, Line 4 PART III LINE 2: This statement is extracted from the NMHC consolidated financial statements. Net patient service revenue, net of contractual allowances and discounts, is reduced by the provision for uncollectible accounts, and net patient accounts receivable are reduced by allowances for uncollectible accounts. These amounts are based primarily on management's assessment of historical and expected write-offs and net collections, along with the aging status for each major payor source. Management regularly reviews data about these major payor sources of revenue in evaluating the sufficiency of the allowance for uncollectible accounts. Based on historical experience, a portion of Northwestern Memorial's self-pay patients who do not qualify for charity care will be unable or unwilling to pay for the services provided. Thus, a provision is recorded for uncollectible accounts in the period services are provided related to these patients. After all reasonable collection efforts have been exhausted in accordance with Northwestern Memorial's policies, accounts receivable are written off and charged against the allowance for uncollectible accounts. Northwestern Memorial has determined, based on an assessment at the reporting-entity level, that net patient service revenue is primarily recorded prior to assessing the patient's ability to pay, and as such, the entire provision for uncollectible accounts is recorded as a deduction from net patient service revenue in the accompanying consolidated statements of operations and changes in net assets.
Medicare shortfallSchedule H, part III, line 8 THE UNREIMBURSED COST OF MEDICARE IS DEFINED BY THE STATE OF ILLINOIS ATTORNEY GENERAL'S OFFICE ANNUAL NONPROFIT HOSPITAL COMMUNITY BENEFITS PLAN REPORT AS A COMMUNITY BENEFIT. THE HEALTHCARE FINANCial MANAGEMENT ASSOCIATION ALSO VIEWS THE UNREIMBURSED COSTS OF MEDICARE AS PART OF A HOSPITAL'S COMMUNITY BENEFIT PROGRAM. NMHC PROVIDES MEDICAL CARE TO MEDICARE PATIENTS AT A COST HIGHER THAN THE REIMBURSEMENT IT RECEIVES FROM MEDICARE. THE AMOUNTS LISTED FOR PART III, LINES 5 THRU 7, ARE CALCULATED CONSISTENT WITH THE METHODOLOGY DESCRIBED FOR CALCULATING UREIMBURSED COST OF MEDICAID FOR FISCAL 2014.
Financial Assistance collection practicesSchedule H, part III, line 9b NMHC's Credit and Collection Policy contains a provision for financial counseling. tHE POLICY STATES THAT patients with self-pay balances and without the resources to pay their obligations will be assessed FOR FREE AND DISCOUNTED CARE eligibility By the Financial Counseling Departments. The assessment involves an evaluation of all levels of assistance including governmental assistance, extended pay alternatives, and free or discounted care. If THE PATIENT QUALIFIES FOR free care, THE ACCOUNT IS ADJUSTED TO ZERO SO NO COLLECTION ACTIVITY OCCURS. If financial assistance results in a discounted or reduced balance, only the reduced balance will be subject to the collection practices.
Needs assessmentschedule H, part VI, Line 2 Our hospitals' mission statements set forth a commitment to improve the health of the communities we serve and to advance medical research and education. The Community Benefit Plan describes the organizational goals that support this commitment and address our responsibility as a tax-exempt organization. With the organizations that comprise Northwestern Medicine, we bring to bear the resources of a world-class academic medical center to benefit our communities in ways that could not be achieved as stand-alone hospitals: enhancing access to healthcare, improving clinical quality, advancing medical innovation, and ensuring that a highly skilled healthcare workforce is in place for decades to come. The goals of NMHC's Community Benefit Plan include the following: A. Provide quality care based on our clinical and academic strengths B. Train the next generation of healthcare professionals for our hospitals, communities and industry C. Support the discovery of new knowledge through research that can cure disease and reduce suffering D. Develop programs to address the affordability and accessibility of healthcare E. Make a measurable, positive impact on the health of medically underserved Residents in our communities F. Through information, empower community residents to make proactive healthcare decisions G. Provide local youth with education, mentoring and exposure to the healthcare industry for potential careers H. Identify and address community needs within available resources I. Promote strong and lasting relationships with our communities NMHC believes that these goals are best accomplished in collaboration with partners both in the community and within Northwestern Medicine. In accordance with the requirements of the Patient Protection and Affordable Care Act, NMH and NLFH each worked with these organizations to complete a comprehensive Community Health Needs Assessment (CHNA) that identified the highest priority health needs of residents of their communities. The most critical health needs of the residents of Chicago and Lake County were identified as: Chicago: Access to health service Heart Disease and stroke Nutrition, physical activity and weight Injury and violence prevention Lake county: Access to health service Heart Disease and stroke Mental health disorders and substance abuse The CHNA and corresponding implementation plans represent a continuation of the NMHC Community Benefit Plan, building on the experience and success of the community services activities and established chronic condition programs. Healthcare Needs Considered in Developing the Plan Our commitment to improve the health of the communities we serve is advanced through the collaborative work we do as Northwestern Medicine, with community-based public and private organizations in both the city of Chicago and Lake County. Together with our community partners, we continue to bring the expertise and resources of an AMC to the important work of responding and working to eradicate significant public health issues such as diabetes, obesity, cancer and heart disease as well as the persistent violence in our communities. The CHNA and corresponding implementation plans were developed collaboratively with a range of healthcare and social service organizations that understand and can represent the healthcare needs of community residents and share our commitment to improving the health status of our communities. The CHNA implementation plans are grounded in a healthcare model that we have worked with our community partners to establish, in which residents of our community are informed and able to make healthy lifestyle choices, manage their chronic health conditions and receive medically necessary healthcare services in the most appropriate setting. The model is based on the belief that healthcare services are optimized when they are coordinated through a "patient-centered medical home." The patient-centered medical home provides health information and resources, assistance in navigating the healthcare system, primary and preventive care at a location convenient for patients and facilitates access to more specialized hospital-based diagnostic and treatment services. We have implemented large-scale programs using this framework to target high priority conditions identified by our community partners and will continue to use this model to address priority health needs identified through the CHNA. Ongoing efforts draw on our AMC-based strengths in communication and education and include programs to address obstetrics/gynecology health, diabetes, breast care and cancer. NMHC has formal and longstanding affiliations with two Federally Qualified Health Center partners based in the community, Near North and Erie. Near North, a community health partner for more than 40 years, provides neighborhood-based care and support services through nine primary healthcare sites and two ancillary sites to more than 46,000 primarily low-income uninsured or underinsured residents who live in some of the city's most impoverished communities. These communities are home to Black, Hispanic, and other racial and ethnic groups that experience health disparities in diabetes, heart disease, asthma, HIV, depression, tobacco use and infant mortality. Near North operates primary care clinics on Chicago's South, West and Near North sides. Erie was founded in the 1950s as a project of volunteer physicians from NMH and Erie Neighborhood House. Erie provides a variety of primary care and case management services through 13 sites, including a new facility in Lake County, five school-based health centers and the only freestanding comprehensive teen health site in Chicago. More than 57,000 patients receive care annually from Erie sites, which serve the Chicago neighborhoods of Albany Park, Avondale, Belmont Cragin, Hermosa, Humboldt Park, Irving Park, Logan Square, Lincoln Square, North Center, North Lawndale, Uptown and West Town along with Lake County in northern Illinois. Erie serves a population that is primarily Hispanic, the majority of which come from households with incomes that fall below the federal poverty level. As discussed further in this report, Erie opened a new location in fiscal year 2014 in Waukegan to serve uninsured and underinsured residents of Lake County. NMHC has worked with CommunityHealth, the largest free medical clinic in Illinois with locations in Chicago's West Town and Englewood neighborhoods, to replicate some of the disease management programs that have been successfully implemented at Near North and Erie. Near North and Erie work with NMH and NLFH to identify community needs and to develop programs and strategies to address those needs and expand access to care. Through formal affiliation agreements, processes have been put in place to ensure a flow of information among NMH, NLFH, Near North and Erie. Members of NMH's senior management team have served for many years as board members at Near North and Erie. Through charity care, outreach services and health education programs, NMHC further increases access to healthcare services and responds to the priority health needs of the residents of Lake County, especially among the uninsured or underinsured. NMHC supported Erie in planning for and obtaining grant funding to open a new Federally Qualified Health Center for medically underserved residents in Lake County in 2014, as described further in this report. Staff at NLFH also work closely with the Lake County Department of Health and area school districts to address priority health needs of the community, including recent efforts to address high school-age substance abuse and suicide and expanding programs to promote an active, healthy lifestyle among residents of Lake County.
Patient education of financial assistance eligibilitySchedule H, part VI, Line 3 There are many ways that patients of the Hospitals are informed or made aware of the availability of the Hospital's various financial assistance programs: a. To increase awareness of its financial assistance programs, the Hospitals have developed brochures (in English and Spanish) that are provided to patients upon admission and available at registration points-of-entry. b. English and Spanish-language signs notifying patients that financial assistance is available are present at every patient registration area, including the emergency department. c. As part of the registration process, patients are provided with a financial assistance information brochure which describes the types of assistance available and how to qualify for one or more of the programs. d. The general consent form that every patient signs contains information about the NMHC financial assistance programs, and is available in English, Spanish, Russian and Polish at NMH, while NLFH has programs in english and spanish. e. Inpatients receive a Patient Welcome Package that includes the financial assistance information. f. Patients can learn about and assess their eligibility for the Hospital's financial assistance programs with the help of the Hospital's team of financial counseling and patient inquiry representatives. These representatives are available on a walk-in basis or through a toll-free number. g. Processes are in place to link patients with financial counselors and patient inquiry representatives when financial hardship is identified as a concern during social services assessments. h. The entry portal to the NMH and NLFH websites contain a prominent link to information about NMH's various financial assistance programs, the financial assistance brochure and downloadable applications in multiple languages. i. Working in conjunction with clinical staff, financial counselors visit inpatients not enrolled in government or private health plans while they are still in the hospital to assist them in determining their eligibility for both government health programs and for Hospital Free and Discounted Care programs. j. The Hospitals inform uninsured patients, and patients with an outstanding balance after insurance, of the availability of various financial assistance programs, including the free care and discounted care program, and the catastrophic program offered by the Hospitals, in written correspondence sent to those patients. This information includes the toll-free phone number to the team of patient account representatives. k. The Hospitals have on-site patient account staff who are trained and available to assist patients with financial assistance. l. The Hospitals provide proactive financial counseling for self-pay patients who have a scheduled inpatient admission. Financial counseling includes assessment for publicly or privately funded insurance and the Hospitals' financial assistance programs. Financial assistance programs, includes the free care and discounted care programs, and the catastrophic program offered by the Hospitals, in written correspondence sent to those patients. This information includes the toll-free phone number to the team of patient account representatives.
Community InformationSchedule H, Part VI, Line 4 NMHC's patient care, education and research programs provide broad benefit to Chicago, the region, nationally and internationally. Patient care is provided at both NMH and at NLFH, each serving surrounding communities. Northwestern Memorial Hospital Service Area NMH divides its total service area into three geographic areas: the primary service area (PSA), the city of Chicago and the surrounding seven-county area. The PSA, which is defined by the 22 zip codes surrounding NMH, accounts for 39 percent of inpatient admissions. The city of Chicago in total accounts for 65 percent of inpatient admissions The community in NMH's PSA has a large population that is growing at a rate exceeding that of the overall city of Chicago and NMH must continue to grow to ensure that it can provide quality healthcare services, especially those only available in an AMC environment. Between 2014 and 2019, the population in NMH's PSA is projected to increase by 3.1 percent, and the population of Chicago is projected to increase by 1.0 percent. Chicago is a diverse city, with a large Black population and growing Latino and Asian populations. NMH is committed to providing culturally competent care that is responsive to the needs of all our patients. NMH works with community health centers in some of Chicago's medically underserved areas to identify priority health concerns and jointly develop community based health initiatives designed to address healthcare disparities. Northwestern Lake Forest Hospital Service Area NLFH primarily serves Lake County. Lake County has approximately 703,000 residents and is defined by 28 zip codes and accounts for 89 percent of inpatient admissions at NLFH. Lake County's population is neither growing nor declining. Between 2014 and 2019, the population of Lake County is projected to remain nearly the same; however, the demographic makeup of the population is expected to change, with an increased percentage of Hispanic, Black and Asian residents.
Promotion of Community healthschedule H, Part VI, Line 5 As described in earlier sections, NMHC believes that its mission to improve the health of the communities it serves is best accomplished in collaboration with partners in the community. The CHNA process and ongoing input from community health partners inform how NMHC prioritizes community health and wellness programs. NMHC's affiliations with Near North and Erie enable the organizations to meaningfully improve access to healthcare through utilization of high quality medical homes and pathways to access medically necessary hospital-based care. NMHC provides grant funding to support these community health centers, underwrites the cost of medically necessary diagnostic and medical specialty services for patients referred from Erie and Near North and with ongoing support from Feinberg researchers, continually shares knowledge and expertise. NLFH works with Erie and other community-based organizations to support efforts to reach the most medically underserved residents in Lake County. Erie HealthReach Waukegan Health Center In response to a critical need for expanded primary care services for uninsured and underinsured residents of Lake County, NHMC participated with leaders from community-based healthcare programs in Lake County to plan for a new primary care health clinic. NMHC linked these organizations with Erie and provided substantial administrative support to Erie over the past several years to complete the planning and gain federal grant funding to open a new satellite facility in Lake County. Erie HealthReach Waukegan Health Center (EHWHC) opened in 2014 as a partnership between Erie and the former HealthReach organization of Lake County, vastly expands access to primary care for those with no insurance or unable to pay for healthcare in Lake County. The site serves as a patient-centered medical home, providing primary, preventive and dental care. Health education and chronic disease self-management programs established at other Erie sites will be replicated over time at the Lake County facility. The 25,000 square foot building will have the capacity to treat more than 10,000 patients and up to 40,000 patient visits per year. NMHC continues its support through a multi-year donation of operational funds. Plans are in place for McGaw Family Medicine residents to gain training and provide care at EHWHC beginning in 2015. Family Medicine residents provide primary, preventive, acute and chronic healthcare services across all ages and in both office-based and hospital settings, ensuring that the right care is given in the most appropriate setting. Following the model in place between NMH and its affiliated community healthcare partners in Chicago, EHWHC patients will receive medically necessary services at NLFH. Taking Action to Reduce Violence in the Bronzeville Neighborhood of Chicago In Chicago neighborhoods with persistent, exceptionally high rates of violence, many of the typical health concerns facing Americans may not rise to a priority concern. Addressing chronic health concerns or adopting healthy lifestyle measures takes on little significance when injury or death by violence is commonplace. Studies completed by researchers at both Emory University and Yale University found that rates of post-traumatic stress disorder (PTSD) are higher in some high-crime neighborhoods than among veterans of combat. Prolonged exposure to violence leads the part of the brain that triggers the primal "fight or flight" response to remain in hyper-vigilant state, which can result in long-term psychological dysfunction. Those suffering from PTSD experience profound challenges in completing school, holding jobs and avoiding substance addiction and when high rates of PTSD are present, entire communities suffer. Healthcare organizations have long treated victims of violent trauma in emergency departments and have participated in anti-violence advocacy and policy initiatives, but have largely considered addressing the rate of violence in their communities as being outside of their role. The CHNA process conducted by NMH identified prevention of violence and injury as a top concern of surveyed residents and community-based healthcare organizations alike. NMHC made a commitment to participate in efforts to reduce rates of violence, applying resources and expertise of Northwestern Medicine. In fiscal year 2014, leaders from NMHC joined with other healthcare leaders and the United Way of Chicago to help to convene and launch a community-based effort to reduce the rate of violence in the Bronzeville neighborhood on the south side of Chicago. The initiative is based on a model created and successfully implemented by NATAL-Israel Trauma Center, that acknowledges and treats the depression and PTSD that plagues communities impacted by chronic terrorism and violence. Residents of the Bronzeville community routinely turn to faith leaders in the aftermath of violence and the program will be built around the trust established through these relationships. These faith leaders will be trained in mental health counseling, provided resources to help avert the cycle of violence and retaliation following the NATAL model and linked to follow-up healthcare services. NMHC has played an active role in convening members of the community and will continue to dedicate public health and mental health resources to the effort, as well as underwrite costs associated with bringing the NATAL program and its tools to the Bronzeville community. Diabetes Collaborative Model Several years ago, a study conducted by the Sinai Urban Health Institute (SUHI), found that 14 percent of residents of the Humboldt Park neighborhood in Chicago's northwest side reported being diagnosed with diabetes, a rate almost triple that for the city of Chicago as a whole. This incidence of diabetes is the highest reported community incidence rate in the nation and is comparable to the reported incidence in many third-world countries. NMHC worked with Near North, Erie and researchers from Feinberg to create a comprehensive, replicable program to address diabetes in medically underserved communities in Chicago, now known as the Diabetes Collaborative. The program helps patients in medically underserved areas better manage their adult type 2 diabetes and the program is now in place at both Near North and Erie healthcare sites, with more than 2,000 patients from each organization are participating. The Diabetes Collaborative model emphasizes: a.Culturally-sensitive education emphasizing the patient's role in controlling diabetes along with self-management tools all in both English and Spanish b.Following evidence-based clinical practice guidelines c.Utilizing clinical information systems to support providers in using practice guidelines, including implementing reminder systems d.Providing feedback to physicians so they can track their performance in managing their patients on chronic illness measures e.Using patient registries to support monitoring of patients compliance and outcomes Diabetes is often the result of social and environmental determinants including lack of safe exercise options, inadequate access to fresh fruits and vegetables, traditional diets low in nutrition, and insufficient knowledge of causes and consequences of diabetes. Building on the success of the Diabetes Collaborative, NMHC worked with leaders from Humboldt Park to create a neighborhood-specific program to focus on these challenges. NMHC staff worked with leaders of Humboldt Park through the Kelly Hall YMCA, the West Humboldt Park Development Council, a nonprofit community revitalization organization and the Greater Humboldt Park Diabetes Empowerment Center to create a wide-ranging diabetes prevention and incidence reduction program tailored to the identified needs and challenges of residents of Humboldt Park. The program includes: a.Expanded primary care capacity at community health centers b.A diabetes awareness and education campaign that has reached more than 39,000 residents c.Development of the Community Health Education Center (CHEC) in the Kelly Hall YMCA, considered a neutral safe haven in the high-crime community d.Free health programs at the CHEC, including health education programs focused on prevention and disease management, as well as exercise and cooking classes e.Access to "Diabetes Link," a web-based tool developed by Feinberg experts in population health, linking healthcare providers with diabetes management resources available in their immediate geography The Diabetes Collaborative offerings and capacity have grown each year, with more classes and access points added ongoing. The continuous presence of the Diabetes Collaborative program and services over the past several years has symbolized the commitment among the partnering organizations to the health of the community. FORM 990 SCH J PART I QUEStion 7The bonus and incentive compensation amounts listed in column (B)(ii) for all listed individuals were nonfixed amounts. Incentive compensation amounts are at risk and are not paid unless there is exceptional individual and organizational performance in accordance with substantial pre-approved goals. The incentive compensation listed for certain physicians is for personal professional productivity and for performance in improving the quality of patient care.
FORM 990 SCHEDULE J PART I QUESTION 4 bThere are two different nonqualified deferred compensation plans sponsored by Northwestern Memorial Healthcare, which provide supplemental, competitive retirement benefits. The employer pays the cost of participation, and the benefits and contributions are subject to a substantial risk of forfeiture based on the completion of substantial service requirements. The amounts earned by participants fluctuate from year to year based on a variety of factors including changes in market interest rates. Julia creamer completed her substantial service requirements during the reporting period, which caused her to become vested in and taxable on supplemental retirement benefits that had been earned over many years of executive employment. Of the amount included in "other reportable compensation" most of the benefit amount had been reported on prior form 990s and is now included in column (f) showing the multiple disclosure of the same benefit amounts. Participants in one or both of the plans who are listed on the schedule are Julia Creamer, Dean Harrison, Michelle Janney, Dean Manheimer, Thomas McAfee, Peter McCanna, Daniel Derman, Stephen Falk, Carol Lind, Dennis Murphy, and Douglas Young.
Schedule J part I lines 5a and 6a5a Revenue: Certain listed individuals are employed as physicians. The compensation listed in Schedule J is provided solely in connection with their employment as physicians, and is in part based on revenues associated with their personally performed services. The compensation listed is for the clinical and administrative services provided within the Northwestern memorial HealthCare group. The majority of these physicians are also compensated by an unrelated organization (Northwestern University Feinberg school of medicine) through a common paymaster for their academic and research efforts. The compensation listed in schedule J does not include academic and research compensationfrom the unrelated organization.
schedule J, part 1, question 1Employees of Northwestern Lake Forest Hospital are offered discounted health and fitness club dues at Lake Forest Health and Fitness Institute. The amount of the discount is treated as taxable income for each of the employees.
Schedule J, part IINorthwestern Memorial HealthCare (NMHC), is the direct parent organization for Northwestern Memorial Hospital (NMH), Northwestern Memorial Foundation (NMF), Northwestern Medical Faculty Foundation, doing business as Northwestern medical Group (NMG), and Northwestern Lake Forest Hospital (NLFH). NMHC is also the indirect parent for Northwestern Memorial Physicians Group (NMPG) which was merged into NMG on May 1, 2014, Northwestern Medical Group management services (NMNMS) and Lake Forest Health and Fitness Institute (HFI). No organization in this Group Return compensates its directors for services performed as directors. Where compensation is reported for a director, the compensation is associated with another position held within the eight corporations. Certain individuals hold multiple positions throughout these eight corporations. In order to simplify the reporting, their names are listed only once per Form 990, Part VII and Schedule J. Each individual listed has his or her organization's initials listed next to their respective name and the box checked for their position at that corporation.
