DescInland Northwest Health Services, a not for profit corporation, provides collaborative, community-benefitting health care. With the guidance of its sole Member, Providence Health & Services-Washington, INHS provides unique, affordable services using collaborative and innovative approaches for the benefit of the entire health care continuum. These local, regional and national solutions improve efficiencies and health outcomes in rehabilitation and clinical services, critical care, transportation, information technology, health care education and other health care services, and incorporate the highest ideals from its Member's Christian heritage in the provision of medical care.St. Luke's Rehabilitation Institute specializes in comprehensive inpatient and outpatient therapy sessions for children and adults and provide rehabilitation treatment for stroke, spinal cord injuries, orthopedic issues and brain injuries. Conveniently located in Spokane, Washington, St. Luke's Rehabilitation Institute serves patients throughout the Inland Northwest including patients in central and eastern Washington, northern Idaho and western Montana. As a division of INHS, St. Luke's Rehabilitation Institute offers high quality outpatient rehabilitation services at several convenient locations. Committed to providing hope, quality of care and recovery, St. Luke's is the state's largest provider of rehabilitation services for stroke, spinal cord injury, head injury and injury or diseases to bones, joints or muscles. After illness or injury, there is a need to gain strength, skills and function to allow people to return to the highest quality of life possible. St. Luke's works as a team with patients and their families to achieve this goal. With a main campus located in downtown Spokane, St. Luke's combines compassionate care with expertise to provide inpatient physical rehabilitation services to patients of all ages.During 2014, the organization had 20,600 resident days and 57,960 outpatient visits.Northwest MedStar, a non-profit organization, is the region's premier critical care transport service dedicated to the safe, compassionate care and transport of critically ill or injured patients to health care facilities throughout the Pacific Northwest. From its bases in Spokane, Tri-Cities, Moses Lake, the Palouse area, Brewster and Missoula, Northwest MedStar provides thousands of helicopter, fixed wing and ground-based transports each year with the highest level of advanced care.In addition to its air ambulance service, Northwest MedStar provides ongoing outreach education for emergency medical workers throughout the region. With a fleet of helicopters and fixed wing planes, Northwest MedStar is accredited by the Commission on Accreditation of Medical Transport Systems (CAMTS), the "seal of approval" for the air medical industry.During 2014, MedStar provided 1,795 airport shuttles, 2,303 ground ambulance transports, 1,216 fixed wing aircraft flights, and 1,930 helicopter flights. Inland Northwest Health Services (INHS) is committed to helping health care professionals maintain skills and stay abreast of the latest recognized medical advances that are vital to quality care and improved patient outcomes.INHS provides certification and continuing education opportunities for EMS and health care providers on-line, in the classroom and at your location.Formerly called the EMS Training Center, INHS provides and coordinates medical training for health care providers throughout the Inland Northwest. With more than 1000 American Heart Association instructors and 39 participating agencies, our training is unsurpassed in quality and availability, with a variety of class locations as well as custom courses designed for specific educational needs.A community's health resources are often scattered across different entities, making it difficult for community members to easily access all that is available to them. Inland Northwest Health Services Community Wellness remedies this challenge by providing a central location for public and professional health education, screenings, interventions and wellness assessments.Northwest TeleHealth provides access to consultations with health care specialists through a robust video-conferencing network of more than 100 units throughout the Northwest. This technology facilitates live, confidential interaction for two or more locations involved in patient consults, rural medicine, health education, medical training, business administration and other related activities. The Center of Occupational Health & Education (COHE) Community of Eastern Washington is a public-private collaboration between the Washington State Department of Labor & Industries and St. Luke's Rehabilitation Institute in Spokane, WA. COHE is an alliance unlike any in the nation and the results are impressive, far exceeding expectations for patient outcomes - including cost savings and satisfaction by providers, patients, and employers.Desc

St. Luke's Rehabilitation InstitutePart V, Section B, Line 3j: Availability of St. Luke's Community Health Needs Assessment.
St. Luke's Rehabilitation InstitutePart V, Section B, Line 5: A Planning and Implementation Committee was formed to guide the St. Luke's CHNA and implementation strategy process. The Planning and Implementation Committee (PIC) consisted of individuals from St. Luke's and its parent company, Inland Northwest Health Services, with evaluation, community benefit, public relations, and communications experience. The PIC established the scope, timelines, methods, principles, and guidelines for conducting the CHNA and developing the implementation strategy. The PIC, in conjunction with St. Luke's administration, adopted the 2012 CHNA of Spokane County, sponsored jointly by the Spokane Regional Health District, Providence Health Care, and Empire Health Foundation. The PIC and St. Luke's administration endorsed the plan to augment the 2012 Spokane County CHNA with additional information from further community input on health issues affecting the community that St. Luke's serves. St. Luke's sought input from organizations serving and representing the interests of our defined community patients across the lifespan from Spokane County with rehabilitation needs arising from neurological or orthopedic injuries or conditions. Input was sought from public and private agencies providing health or related services within the County, including staff from the Spokane Regional Health District, and societies and support groups. The inclusion of societies and support groups was deemed critical since it allowed, in many cases, for us to hear directly from patients and family members of the vulnerable populations St. Luke's serves. Personal invitations were sent to specific members of these organizations to attend one of six focus groups, with 6-12 people in each group. To assure representation by a full spectrum of community members serving and representing the interests of our defined community, focus groups were held with three audiences:-Patients/family caregivers and support groups;-Community partners who work with St. Luke's to meet the needs of our patient populations;-Community program services that overlap with St. Luke's, but who don't necessarily have direct interaction with St. Luke's
St. Luke's Rehabilitation InstitutePart V, Section B, Line 11: Given the four primary health needs identified by our Community, St. Luke's has elected to address each of the four health needs as we feel that we have the expertise and competency to effectively address each need. In addressing these needs, St. Luke's is committed to working with other facilities and organizations in the community who are also addressing these needs. The four significant health needs identified through the CHNA are:1. Care coordination and management of services for patients;2. Access to services/providers;3. Mental health services;4. Education and resources for patients and their informal caregivers.

INHS oversees a variety of health care divisions and services that work together to improve outcomes, lead the way in health care innovation and create healthier communities:-St. Luke's Rehabilitation Institute: Provides accredited inpatient and outpatient physical rehabilitation services at the state's only freestanding hospital.-Engage: Sets the national standard in how health information technology improves a community's patient care and lowers costs.-Northwest MedStar: Provides safe, compassionate care and air transport of critically ill or injured patients to health care facilities throughout the Northwest.Community Wellness: Improves the health of the community through screenings, assessments, involvement and education.-Northwest TeleHealth: Links the region to a variety of resources at 115 locations over a robust telemedicine, video-conferencing network.-Health Training: Provides and coordinates medical training for health care professionals throughout the Inland Northwest.-COHE Community of Eastern Washington: Works cooperatively in developing programs to improve the quality, cost effectiveness and consistency of care for injured workers.
Part III, Line 2:Bad debt expense represents the amount of gross charges for patients who do not have insurance and which the organization was unable to qualify for assistance under either government programs or our internal charity care policy.
Part III, Line 4:The Health System provides for an allowance against patient accounts receivable for amounts that could become uncollectible. The Health System estimates this allowance based on the aging of accounts receivable, historical collection experience by payor, and other relevant factors. There are various factors that can impact the collection trends, such as changes in the economy, which in turn have an impact on unemployment rates and the number of uninsured and underinsured patients, the increased burden of copayments to be made by patients with insurance coverage and business practices related to collection efforts. These factors continuously change and can have an impact on collection trends and the estimation process used by the Health System. The provision for bad debts in 2014 has decreased from 2013 as a result of the expansion of Medicaid programs and initiatives to assist patients in their Medicaid enrollment. The Health System records a provision for bad debts in the period of services on the basis of past experience, which has historically indicated that many patients are unresponsive or are otherwise unwilling to pay the portion of their bill for which they are financially responsible.
Part III, Line 8:It is organization's policy to exclude any Medicare shortfall from Community Benefit information. The amount reported on Part III, Section B, line 6, was determined by applying the Cost-to-Charge Ratio to the Medicare revenue.
Part III, Line 9b:Patients who are known to qualify for charity care or financial assistance are contacted by the Patient Accounts Patient Advocate. The Patient Advocate will work with the patient and/or family to qualify the patient for charity care or discounted care.INHS provides medical care to the community regardless of the patients' ability to pay. The Mission of INHS includes charitable activities such as free care to the needy and care at discounted charges to below cost for Federal and State government sponsored programs.
Part VI, Line 2:For the 2013 St. Luke's CHNA, the following questions were considered by the community focus groups when prioritizing community health needs and services to meet the needs of the population served by St. Luke's:1. Does the community recognize the issue as a need?2. Can existing community programs be leveraged to help more people?3. Would a project/program address the needs of the disabled, vulnerable, underserved?4. How many people would benefit from the project/program?5. Would a project/program be feasible and effective?6. Would the project/program improve or maintain individual health outcomes?7. Does the project/program duplicate services that are already available in the community?8. Does the project/program further St. Luke's goal of getting patients back to living their lives as independently as possible?Persons representing the community's interests were engaged in the focus groups. Following the community focus groups, a summary of all the focus groups was delivered to each attendee with an invitation to provide us with feedback about whether the summary captured the essence of the conversation correctly, whether pertinent features of the focus group conversation were missing, and whether further input would help define or clarify issues of importance.
Part VI, Line 3:Information regarding the INHS charity care policy, including the appropriate forms, is posted on the website for patients to access. In addition, hard-copy versions are available at the point of service; information about payment options, scholarships and charity care are included in the information sent to patients regarding their bill and staff members are trained to work with patients who need assistance in determining how they can apply for government assistance, scholarships and/or charity care.
Part VI, Line 4:St. Luke's Rehabilitation Institute, located in Spokane, Washington, is a private, not-for-profit, 102-bed post-acute care hospital specializing in medical rehabilitation for people with neurological or orthopedic injuries and illnesses. It is the only free-standing inpatient rehabilitation facility in Spokane County. In 2012, more than 1,532 patients received inpatient care through St. Luke's with 93% of these patients coming from Spokane County. Nearly half of these patients received care for stroke, traumatic brain injury, or spinal cord injury. Of the 399 patients receiving inpatient care for stroke at St. Luke's in 2012, one was younger than 18, 128 were between the ages of 18-64, and 270 were age 65 or older. In 2012, of the 217 patients receiving inpatient care for brain injury at St. Luke's, 11 were younger than 18, 114 were between the ages of 18-64, and 92 were age 65 or older. Of the 96 patients receiving inpatient care for spinal cord injury at St. Luke's in 2012, 2 were younger than 18, 56 were between the ages of 18-64, and 38 were age 65 or older. The remaining inpatients received rehabilitation services for a variety of medical conditions, such as cardiac conditions, pulmonary conditions, orthopedic and other major trauma injuries, post-surgical care, amputations and other limb deficiencies, and other neurological conditions.Given the primary geographic area we serve and the specific patient populations for which St. Luke's provides services, the 2013 St. Luke's Community Health Needs Assessment (CHNA) and Implementation Strategy defines the community served by St. Luke's to be patients across the lifespan from Spokane County with rehabilitation needs arising from neurological or orthopedic injuries or conditions. This definition includes the needs of those with vulnerabilities due to disability orsocioeconomic challenge.According to the US Census, Spokane County's population in 2010 was 471,221, with 44% of the residents living in the City of Spokane, 19% living in the City of Spokane Valley, and the remaining 37% living in suburban and rural areas. In 2010, the county population was comprised of:-23% youth (0-17 years of age)-14% seniors (65+ years of age)Among county residents in 2010, the racial and ethnic make-up was:-2% African American-2% American Indian or Alaskan Native-3% Asian, Native Hawaiian/Pacific Islander-5% Hispanic-86% Non-Hispanic WhiteThe Spokane County high school graduation rate was greater than 80% in 2010, which exceeded the Washington State high school graduation rate of 77%. The 2010 Census estimated the median household income for Spokane County to be $46,320. Spokane County's unemployment rate has exceeded 9% since 2010.The 2010 Washington State Population Survey estimated that 20% of Spokane County adults under age 65 were uninsured, while 6% of children (under age 18) were uninsured. The Spokane Regional Health District indicated that the top three causes of death among all deaths in Spokane County in 2010 were cancer at 22%, heart disease at 20%, and stroke at 6%. Combined, these conditions made up almost half of all deaths in Spokane County. St. Luke's sees survivors of each of these conditionsin its patient populations. The 2011 American Community Survey conducted annually by the US Census Bureau estimated that among those age 18-64 in Spokane County, 6% were living with ambulatory difficulty (walking or climbing stairs), 2% were living with self-care difficulty (dressing or bathing), and 4% had difficulty living independently (difficulty doing errands alone such as visiting a doctor's office or shopping). Among people age 65 and older in Spokane County in 2011, an estimated 24% were living with ambulatory difficulty, 8% were living with self-care difficulty, and 17% had difficulty living independently. Therefore, the elderly bear an inequitable share of disability-related outcomes in our community. At the conclusion of skilled inpatient care services (and often during receipt of home health services) the primary responsibility for providing care to a person with disability falls to a family member or friend. In Spokane County, the 2011 Behavioral Risk Factor Surveillance Survey estimated that more than 16% of county residents had provided care to a family member or friend in the last month. This represents an extended health care need to assure that family caregivers are educated and trained for their responsibilities, and not burdened to such a degree that they, themselves, become the patients.Falls were the leading cause of fatal and nonfatal unintentional injury for those 65 and older in Spokane County between 2007 and 2011 according to the Washington State Department of Health. During that period, 858 deaths per 100,000 population were attributed to falls among those 65 and older in Spokane County. This exceeded the death rate from falls for the state of Washington (525 deaths per 100,000). Also during that period, 8,492 hospitalizations per 100,000 population were attributed to nonfatal falls among those 65 and older in Spokane County. This exceeded the rate of hospitalizations from nonfatal falls for the state of Washington (7,538 hospitalizations per 100,000). The local public health authority, the Spokane Regional Health District (SRHD), reports that 1 in 6 seniors (16%) in Spokane County reported falling in the past three months. SRHD also reports that seniors with a disability were 2.5 times more likely to report a fall than were seniors without a disability. Due to these high rates of death, injury, and healthcare use arising from falls, especially in seniors, falls have been recognized by the Spokane Regional Health District as a major public health problem in Spokane County for the elderly. Falls are also a major concern in the population served by St. Luke's.Our assessment identified a number of strong community assets available to respond to the health needs of Spokane County residents, especially those with vulnerabilities due to disability or socioeconomic challenge. This includes acute care hospitals in addition to St. Luke's as a medical rehabilitation hospital, ambulatory surgical facilities, a child birth center, Medicare assignment skilled nursing facilities, home healthcare agencies, health centers and clinics (including several federally qualified health centers), disease or medical condition-specific societies/associations and support groups, community transportation services, and public or private community partners. The list is not exhaustive, and does not contain reference to the hundreds of primary and specialty health care providers, mental health care providers, and dentists practicing in Spokane County.
Part VI, Line 5:INHS provides medical care to the community regardless of the patients' ability to pay. The mission of INHS includes charitable activities such as free care to the needy and care at discounted charges to below cost for Federal and state government sponsored programs. INHS promotes the health of the community by offering many health education programs and support groups. A division of INHS is MedStar which operates an air ambulance service to improve the outcomes of emergency medical situations.
Part VI, Line 6:At the end of 2013, INHS affiliated with Providence Health & Services. Providence Health & Services owns or operates 32 general acute care hospitals, three ambulatory care centers, five medical groups, six long-term care facilities, seven homecare and hospice entities, five assisted living facilities, a children's nursing center, a high school, a university, 13 low-income housing projects, the Health Plan, a health services contractor, two programs of all inclusive care for the elderly, and 22 controlled fundraising foundations. The Health System provides inpatient, outpatient, primary care, and home care services in Alaska, Washington, Montana, Oregon and Southern California. The Health System operates these businesses primarily in the greater metropolitan areas of Anchorage, Alaska; Everett, Seattle, Edmonds, Issaquah, Spokane and Olympia, Washington; Missoula, Montana; Portland and Medford, Oregon; and Los Angeles, California.

Form 990, Part VI, Section B, line 11The Form 990 is prepared internally by experienced Providence Health & Services staff and reviewed by the internal Director of Taxes of Providence Health & Services and external tax advisors. The Board of Directors reviewed the Form 990 prior to filing with the IRS.
Form 990, Part VI, Section B, line 12cConflict of Interest policies are updated annually at the board level and updated annually for all employees during their evaluation.
Form 990, Part VI, Section B, line 15For officers and key employees paid by INHS, compensation packages are calculated by an outside third party and then reviewed and approved by the Board of Directors. For officers and key employees paid through Providence Health & Services, the following discusses the process utilized. Providence has a single fiduciary Board, with responsibility for financial oversight associated with fulfillment of the Providence Mission, developing system policies, protecting the assets entrusted to the organization and overseeing the strategic and operational affairs of Providence's legal entities. Providence also maintains a network of community ministry boards with responsibility for quality of care oversight, community relations, advocacy and community needs assessments. Providence has a consistent compensation philosophy for all of its employees, including our senior executives. Salaries for senior executives are reviewed by the Providence Board's Human Resources Committee and approved by the full Board of Directors, none of whom is a Providence employee. The Board retains an independent consultant each year to review salaries of those in the most significant leadership roles in the organization. Part of the consultant's role is to review an extensive array of compensation surveys of large, not-for-profit health care systems in the United States. Providence is one of the larger health systems in the country, and as such, the Board benchmarks executive compensation against other large, not-for-profit health systems whose revenue is similar to that of Providence. Base salaries for Providence executives are set at the median level of the market, as identified by the independent consultant and reviewed with the Human Resources Committee. Each year, the Board Chair conducts a formal performance evaluation of the President/CEO that considers input from the other directors and senior leaders reporting to the President. The evaluation is discussed with the Human Resources Committee and then a recommendation is made by the committee to the full Board. The Board Chair and the Chair of the Human Resources Committee also meet with an independent consultant to develop a salary recommendation; which is reviewed and approved first by the committee and then by the Board of Directors. Additionally, the President/CEO utilizes the market information provided by the consultant along with formal performance evaluations, to determine salary recommendations for other senior executives. This process includes a rigorous analysis of those recommendations with the Human Resources Committee as a part of the review and approval process. Performance incentives allow executives to earn additional compensation if they achieve specific organizational goals for furthering Providence operating commitments and strategic objectives - advancing the Providence Mission and core values, meeting benchmarks for charity care, achieving quality targets, delivering top-rated patient satisfaction, meeting employee satisfaction goals and reaching financial performance objectives. The Board of Directors conducts a thorough process to ensure performance incentives are aligned with appropriate practices for not-for-profit health care systems. The Board's process for executive compensation fully complies with IRS standards and mirrors the best practices recommended in the "Report to Congress and the Nonprofit Sector on Governance, Transparency, and Accountability" submitted to the Senate Finance Committee by the Panel on the Nonprofit Sector.
