DescLocated in Bedford, Texas, for more than 42 years, Texas Health Harris Methodist Hospital Hurst-Euless-Bedford (THHEB) operates 322 beds. The city of Bedford is located between Dallas and Fort Worth. THHEB offers a broad range of inpatient and outpatient medical services to the communities in northeast Tarrant County with specialized capabilities in many areas, including women and infant services, cardiovascular services, orthopedic services, physical therapy, digestive diseases, oncology services, and behavioral health. The behavior health program consists of three inpatient units (Adult Psych, Adult Chemical Dependency, and Adolescent Psych). The program also offers an extensive partial hospitalization program, seven outpatient satellite clinics, and provides crisis evaluation services to six acute care facilities in Tarrant County. In September 2016, THHEB will be opening a new ICU/High Acuity Tower. This project is an expansion of an 18 universal bed unit for ICU/High Acuity level patients and repurposing the current 14 bed ICU unit for lower acuity need. The expansion will create capacity for growth and adequate resources to provide quality care to patients served. THHEB also holds a number of specialty accreditations and certifications. THHEB is accredited by The Joint Commission for the Hospital Program and the Behavioral Health Program. THHEB holds accreditation in ACR CT, MRI, and Nuclear Medicine. The Clinical, Anatomic, and Ancillary Testing Departments of the laboratory are accredited by the American Association of Blood Banks American College of Pathologist (CAP). THHEB was the first Laboratory in Northeast Tarrant County to be recognized by the International Organization for Standardization (ISO 15189) for quality and competence in medical laboratories. The hospital was also the first in Northeast Tarrant County to earn 3-year accreditation with commendation from American College of Surgeons Commission on Cancer. THHEB is a fully Accredited Cycle IV Chest Pain Center. THHEB is the first hospital in Northeast Tarrant County to be certified as a Primary Stroke Center by The Joint Commission. Also, THHEB is the first hospital in Northeast Tarrant County to be designated as a Trauma Level III Center. THHEB is the third hospital in Texas to earn Baby Friendly designation from the World Health Organization. THHEB is a Center of Excellence in Minimally Invasive Gynecology (COEMIG). THHEB won the Silver Award from TMF Health Quality Institute, Five-Star Hospital Award from the Bureau of Vital Statistics at the Texas Department of State Health Services, Texas Health Care Quality Improvement Award of Excellence from TMF Health Quality Institute and named QUEST Top Performer by Premier Healthcare Alliance. THHEB provides quality medical healthcare regardless of race, creed, sex, national origin, handicap, age, or ability to pay. The hospital provides care to persons covered by governmental programs including Medicare and Medicaid for reimbursement that does not always cover the cost of providing the care. Recognizing its mission to the community, services are provided to both financially indigent and medically indigent patients. Community benefit is provided through many reduced price services and free programs to the community. These services are essential to provide complete healthcare to the communities served. Some of these programs include: Various health screenings were offered to the community free of charge or for a nominal charge such as diabetes, heart disease and breast cancer. THHEB provides a unique, comprehensive healthcare outreach to adult Tongans living in Northeast Tarrant County through consistent utilization of the mobile health unit, clinical volunteers and health educators. A full time health coordinator leads clinical volunteers in providing medical tests, immunizations, screenings and health education to this targeted population. Teen Court is a program where teens convicted of a misdemeanor are required to attend educational offerings related to the offense committed and complete community service hours. THHEB provides classes on the dangers of smoking and drug and alcohol abuse for this program. During the year, THHEB had 65,571 patient days, 14,402 discharges, 83,022 outpatient encounters, 57,449 emergency room visits and 2,086 births. THHEB provides quality medical healthcare regardless of race, creed, sex, national origin, handicap, age, or ability to pay.Desc

Facility #1 -Texas Health Hurst-Euless-BedfordPart V Section B, Ln 5-Facility #1 Texas Health Hurst-Euless-Bedford utilized vast amounts of qualitative and quantitative data to assess the health needs of the persons in the communities served by the hospital. As a means to further understand the needs from those engaged in the community, Texas Health Hurst-Euless-Bedford conducted a series of community feedback sessions. Participants of the community feedback interviews included, but were not limited to: political leaders, faith community representatives, public health officials, lay community members, physicians, school official and city administration. The information gathered from the community feedback sessions helped Texas Health Hurst-Euless-Bedford to understand the health needs from a community perspective and align this with the data analysis. Part V. Section B-7d - Facility #1 Texas Health Hurst-Euless-Bedford produced executive summaries of the completed CHNAs along with other information that is of benefit to community members as well as a comparison between the health needs of the community and the State of Texas statistics. This information is widely available throughout the facility and given out at various community coalition and partnership meetings in an effort to disseminate the information through various community-based outlets. Part V. Section B, Ln 11-Facility #1 CHNA 2013 revealed a vast number of community health issues that require collective action to improve the health of the community. In order to assist the organization in determining which health needs should be addressed, Texas Health Hurst-Euless-Bedford categorized the community needs in alignment with the national objectives of Healthy People 2020 categories. By using a strategic prioritization method to determine the top needs within the community aligned with the ability to address the said issues, two areas were chosen and are being addressed through the 2014-2016 implementation plans. 2014 was the first year of implementation and the focus was to create the systems, structure and interventions that could be consistently deployed at the hospital and at other system hospitals for a consistent impact across the region. The two areas of priority are first, Awareness, Literacy and Navigation ("ALN") and second, Chronic Disease. In addition, our efforts were to clearly align and coordinate services and interventions with community partners for a collective impact to meet these two priority areas defined. 1. Awareness, Literacy and Navigation (ALN) 1a. Community Health Improvement Strategies include collaboration to develop an area resource guide of key services available to the local community, individuals and non-profit partners that address the social determinants of health. Collation of this information began through the community health councils and community partners during 2014. This "easy to understand and use" guide will provide key details including how to access the resource and/or service in the local community to meet the individual's needs. This guide information collected in 2014 will be made available in an online and/or print format during 2015. 1b. Community Benefit Grants: The Texas Health Grant and Sponsorship Program provides community benefit grants, sponsorships and other in-kind resources to improve the health status of the communities served. Grants are limited to the specific areas of health disparities, disease management, wellness, disease prevention and health promotion programs, clinics, charitable care and community health collaborative. 1c. Support/sponsorship of relevant and targeted community programs to advance awareness and health literacy has begun. 1c1. Through a collaborative effort and sponsorship as a system, the 2nd Annual Health Literacy Symposium was held on April 16, 2014, for the DFW region. Other collaborators included United Way of Tarrant County, Area Agency on Aging and UNT Health Science Center. 1c2. Sponsorship and Partnering in Area Collaborations Working to Address ALN continues. Texas Health Hurst-Euless-Bedford has maintained and supported relationships with key partners to address awareness, literacy, and navigation needs. Some ALN Strategies carried out during 2014 include: 1d . Texas Health Hurst-Euless-Bedford is part of the system-wide Child Automobile Safety Initiative (CASI) whose mission strives to educate and raise community awareness about child passenger safety and the risk of unintentional injury from motor vehicle collisions. This program is provided in collaboration with Texas Health Resources' System Services and the First United Methodist Church of Hurst. 1e. Bluebonnet Retreat is a free, three-day, two-night camp for adult cancer patients with a focus on education, support and wellness. Bluebonnet Retreat is held twice a year for 24 campers and 24 counselors. Counselors are members of the Texas Health HEB team, professionals from other areas of Texas Health and community volunteers. The staff includes physicians, nurses, social workers, dieticians, physical and occupational therapists, radiation therapists, chaplains, cancer survivors and volunteers. In 2014, Texas Health HEB logged 458 volunteer staff hours for these retreats. 1f. As part of a faith-based health care system, Texas Health HEB is committed to partnering with faith communities to provide health promotion and education. The goal of the program is the integration of the practice of faith with the practice of nursing to care for the whole person - mind, body and spirit - and to improve the health of the communities we serve. A Faith Community Nurse is a licensed, registered nurse who has completed the required education for FCN and is willing to serve the faith community by becoming a liaison to health care. In 2014, Texas Health HEB was in covenant with seven area churches representing Methodist, Lutheran, Presbyterian, Church of Christ and Catholic faiths. In addition to providing 200 flu shots, several programs are ongoing within these congregations including diabetes management, blood pressure checks, English as a Second Language and Walk to Bethlehem (encouraging increased physical activity). 1g. Half of older adults experience fear of falling, and many respond to this concern by curtailing activity. Being inactive results in loss of muscle strength and balance, it can also compromise social interaction and increase the risk of isolation, depression and anxiety. Fear of falling can actually contribute to falling. One third of older adults fall each year. To decrease the risk of falls, Texas Health HEB offers A Matter of Balance, a free award-winning program designed to manage falls and increase activity levels. Participants learn how to reduce fall hazards, increase strength and balance and improve their overall health through increased activity. In 2014, Texas Health hosted two sessions serving a total of 20 participants. 1h. Texas Health HEB, in partnership with Senior Citizen Services and Tarrant County Area Agency on Aging, conducted two A Matter of Balance programs (AMOB). In 2014, 20 participants enrolled in the AMOB programs and of those participants 82% completed the program. 2. Chronic Disease - Community Health Improvement strategies include maintaining existing chronic disease programs provided at the hospital with a focus on self-management programs. Active partnerships and sponsorships aligned with the strategic foci to address chronic diseases remains a top priority. Some Chronic Disease Strategies carried out during 2014 include: 2a. Nearly half of all adults live with one or more chronic disease conditions. Appropriate management of a chronic disease can result in fewer preventable admissions and lower overall mortality, thereby improving and extending life. Chronic Disease Self-Management Program (CDSMP) is a program offered by Texas Health Resources and local community partners. The program goal is to enable individuals to build the self-confidence and motivation they need to manage the challenges of living with a chronic health condition. Participants are adults experiencing chronic health conditions such as hypertension, arthritis, heart disease, stroke, lung disease and diabetes. Their family members, friends and caregivers also participate. The program provides information and teaches practical skills on managing chronic health problems. CDSMP is implemented by lay leaders and is executed in participant education programs. In 2014, Texas Health HEB trained two facilitators and is on track to hosting multiple sessions both on campus and at covenant churches in 2015. 2b. Texas Health HEB provides community benefit in several ways, such as providing support groups and educational classes in asthma, COPD, cancer, diabetes and cardiovascular disease, providing hand hygiene classes for 325 students and hand gel for every classroom at our two adopt-a-schools. 2c. Texas Health HEB is committed to improving the health of its community by focu

As a means to further understand the needs from those engaged in the community, Texas Health Southlake conducted a series of community feedback sessions. Participants of the community feedback interviews included, but were not limited to: political leaders, faith community representatives, public health officials, lay community members, physicians, school official and city administration. The information gathered from the community feedback sessions helped Texas Health Southlake to understand the health needs from a community perspective and align this with the data analysis. Part V. Section B-7a - Facility #2 The CHNA is available at the hospital's website at www.texashealthsouthlake.com/thrinthecommunity.php. Part V. Section B-7d - Facility #2 Texas Health Southlake produced executive summaries of the completed CHNAs along with other information that is of benefit to community members as well as a comparison between the health needs of the community and the State of Texas statistics. This information is widely available throughout the facility and given out at various community coalition and partnership meetings in an effort to disseminate the information through various community-based outlets. Part V. Section B, Ln 11- Facility #2 CHNA 2013 revealed a vast number of community health issues that require collective action to improve the health of the community. In order to assist the organization in determining which health needs should be addressed, Texas Health Southlake categorized the community needs in alignment with the national objectives of Healthy People 2020 categories. Texas Health Southlake has maintained and supported relationships with key partners to address awareness, literacy, and navigation needs. 2. Chronic Disease - Community Health Improvement strategies include maintaining existing chronic disease programs provided at the hospital with a focus on self-management programs. Active partnerships and sponsorships aligned with the strategic foci to address chronic diseases remains a top priority. Texas Health Southlake will not adopt new strategies for the following identified needs in the community for the reasons listed. 1) Access: Provider Supply - resource constraints and lack of effective intervention. The facility will continue to address the need at its current level of support. 2) Injury and Violence - lack of expertise, low priority assigned, need is currently being addressed by multiple organizations in the community and the facility will continue to address the need at its current level of support. 3) Maternal, Infant & Child Health - This need is currently being addressed by multiple outside organizations in the community. The facility will continue to address the need at its current level of support. 4) Mental Health - resource constraints and lack of effective intervention. The facility will continue to address the need at its current level of support. 5) Nutrition, Physical Activity and Obesity - need is currently being addressed by multiple organizations in the community and the facility will continue to address the need at its current level of support. 6) Oral Health - lack of expertise. 7) Other Clinical/Preventive - The facility will continue to address the need at its current level of support. 8) Reproductive and Sexual Health - need is currently being addressed by multiple organizations in the community and the facility will continue to address the need at its current level of support. 9) Social Determinants - resource constraints, low priority assigned and lack of effective intervention. 10) Transportation - lack of expertise. Part V Section B-16A&B - Facility #2 The website where FAP and FAP application was widely available: www.texashealthsouthlake.com/charitycare.php Pt V, Sec B-20e - Billing and Collections - Facility #2 A Plain Language Summary (PLS) of the charity policy is provided on the back of all billing statements. Pt V, Sec B-22d - Charges to Individuals Eligible for Assistance under the FAP- Facility #2 The hospital used the Look-Back Method to compute its Amounts Generally Billed (AGB) percentage. The 4 largest managed care plans were combined and averaged to calculate the AGB. The AGB percentage is used to determine the discount taken from the patient's gross charges. Initially, an uninsured patient is sent a bill with charges less the AGB percentage. If a patient becomes FAP-eligible, then charity care is given for the patient balance due. At no time will the patient pay more than the AGB percentage of their hospital bill.

Patient EligibilityPart I, Line 3c Patients with family income at or below 200% of applicable FPG may be eligible for free care if the patient lacks sufficient funds and assets to pay the out-of-pocket portion of their hospital bill. Patients with family income above 200% of applicable FPG who have unpaid medical bills exceeding a specified percentage of the patient's annual gross income, as determined on a sliding scale based on FPG, may be deemed medically indigent and eligible for charity care. The patient may be eligible for a charity adjustment up to 100% of the unpaid balance of their hospital bill in excess of a specified "patient responsible amount" if the patient has insufficient funds/assets to pay his hospital bill without incurring an undue financial hardship. The patient responsible amount is based on a percentage of the patient's annual income in relation to FPG. A determination as to whether or not a patient has insufficient funds and/or assets to pay for purposes of determining both financial and medical indigence is made at the time a patient's charity care application is reviewed. Assets considered when determining eligibility include cash, stocks, bonds and other financial assets that can be readily converted to cash. An additional process to screen for charity patients using publicly available financial information is also in place for patients not submitting a charity care application.
Cost to Charge RatioPt I, Ln 7 A cost to charge ratio is used in computing the amounts reported in Lines 7a-7c. The amounts reported on Lines 7e-7i were obtained using direct costs, as determined by a cash outlay or, in the case of reporting employee volunteer hours, by using an average national volunteer wage rate.
Bad Debt ExpensePart III, Lines 2, 3 & 4 Bad debt expense is not included as a community benefit for purposes of reporting community benefits to Texas. We treat each patient qualifying for charity care as a charity patient and no charges related to that patient are included in bad debt expense. The hospital is part of a consolidated system of hospitals which operate under the name Texas Health Resources (THR). THR's annual audit is conducted on a system-wide basis with a single consolidated audit report issued for THR and its affiliates. The audited financial statements of THR, which includes the activity of the organization, contain a footnote regarding Accounts Receivable and Allowance for Doubtful Accounts that reads as follows: Patient accounts receivable are reported net of estimated allowances for doubtful accounts and contractual adjustments in the balance sheets. The allowance and resulting provision for bad debts is based upon a combination of the aging of receivables and management's assessment of historical and expected net collections considering business and economic conditions, trends in health care coverage and other collection indicators for each of its major payor sources of revenue. Management assesses the adequacy of the allowance for doubtful accounts based upon historical write-off experience and payment trends by payor category. Patient accounts are also monitored and, if necessary, past due accounts are placed with collection agencies in accordance with guidelines established by management. For receivables associated with services provided to patients who have third-party coverage, the System analyzes contractually due amounts and provides an allowance for doubtful accounts and a provision for bad debts, if necessary (for example, for expected uncollectible deductibles and copayments, or for payors who are known to be having financial difficulties that make the realization of amounts due unlikely). For receivables associated with self-pay patients (which includes both patients without insurance and patients with deductible and copayment balances due for which third party coverage exists for part of the bill), the System records a significant provision for bad debts in the period of service on the basis of its past experience, which indicates that many patients are unable or unwilling to pay the portion of their bill for which they are financially responsible. The difference between the billed rates and the amounts actually collected after all reasonable collection efforts have been exhausted is charged off against the allowance for doubtful accounts. The System's allowance for doubtful accounts for self-pay patients (including allowances for charity care) increased from 96.0% of self-pay accounts receivable at December 31, 2013, to 96.1% of self-pay accounts receivable at December 31, 2014. In addition, the System's self-pay write-offs for bad debts increased from approximately $294,476,000 for fiscal year 2013 to approximately $310,000,000 for fiscal year 2014. The increase in write offs was the result of an overall increase in patient revenue in fiscal year 2014. The System does not maintain a material allowance for doubtful accounts from third-party payors, nor did it have significant write-offs from third-party payors.
Medicare ShortfallPt III, Ln 8 The state of Texas treats any Medicare shortfall as a community benefit for meeting the state statutory requirements for charity care & community benefit. For state purposes, the shortfall is computed by comparing actual Medicare reimbursements with the estimated cost the hospital incurs in providing these services to Medicare patients. Cost is determined by applying a cost-to-charge ratio (with costs determined in accordance with generally accepted accounting principles) to billed charges. The shortfall amount reported to the state of Texas for 2014 is $39,732,086. Sch H requires a different method of calculating the Medicare shortfall shown on Part III, Lines 5-7.
Debt CollectionPart III, Line 9b During the year, standard collection procedures were in place and uniformly applicable to all patient accounts. Except to the extent a patient receives a recovery from any third party or other source, no attempts are made to collect unpaid charges from patient accounts approved for adjustment under the Charity Care Program
Needs AssessmentPart VI, Line 2 The organization is part of the Texas Health Resources (THR) healthcare system. In addition to the organization's Community Health Needs Assessment (CHNA), THR developed and implemented, as a healthcare system, a Community Benefit Framework (CBF), based on the Public Health Institute's best practices ASACB program. The CBF systematically integrates internal practices into the community to improve the efficiency and effectiveness of THR's community health improvement (CHI) strategy, to address disproportionate unmet health needs (DUHN), to address primary prevention and to improve community capacity. CBF provides a quantitative approach to parallel the CHNA in identifying key communities within THR's service area, and the means to align them with a THR entity. The CBF contains five phases: assessment/profile, infrastructure, Community Health Council, programmatic review and strategy development. THR works with each entity community health representative to examine its structure and programs for gaps, needs, strengths, and alignment with the CBF, CHNA and ASACB standards.
Patient EducationPart VI, Line 3 A description of both the Charity Care program and application are available on the THR website at www.TexasHealth.org in English and Spanish. Signs alerting patients of charity care assistance and related contact information are posted in various areas of the hospital including admissions & registration, the emergency room and other outpatient departments. Additionally, financial assistance information is provided verbally and in writing to self-pay patients by financial counselors. As part of the financial counseling meeting, patients are screened for potential eligibility in other governmental assistance programs. If it is determined that the patient is potentially eligible for governmental assistance, the counselor will assist the patient in completing forms necessary to apply for the assistance. At the same time, patients are provided with a Charity Care application and information flyer. The patient is informed that the hospital is a non-profit organization offering financial assistance to patients deemed medically or financially indigent. A Plain Language Summary titled "Getting Help Paying Your Bill" is included on the back of our patient statements. The plain language summary informs the patients of how to qualify for charity, how to apply for help, necessary documentation, and informs of THR's collection activities. The onsite financial counselors may assist the patient in completing the application and obtain available verification information. Once the patient is discharged, the counselors will continue to contact the patient to ensure all forms and verifications needed to file an application for governmental assistance and Charity Care have been provided. In certain cases, self-pay patients who are discharged before seeing a financial counselor receive both a phone call and a letter to alert them to the existence of the financial assistance program. Each bill sent for hospital services also contains contact information alerting patients that Texas Health has a charity care program.
Community InformationPart VI, Line 4 Texas Health Harris Methodist Hospital Hurst-Euless-Bedford (THHEB) is a suburban hospital in a service area with a population of 559,419. The average household income in THHEB's service area is $88,247. During the year, 15.3% of THHEB's patients were uninsured and 16.0% were Medicaid recipients. THHEB's service area has a rate of uninsured of 11.2% of the population. There are two for-profit acute care hospitals, one other not for profit acute care hospital and one not for profit children's hospital in THHEB's service area.
Promotion of Community HealthPart VI, Line 5 The board of trustees is comprised of community members including medical and business professionals. A majority of these trustees reside in the hospital's service area and volunteer their time to serve on the board. The hospital has an open medical staff and privileges are offered to qualified physicians in the community. The organization utilizes surplus funds to maintain access to patient services and to expand access to healthcare. The uses of these surplus funds include providing community benefit programs and expended capital improvements to the healthcare facilities of the organization. Of the $3,608,189 in contributions to community groups reported on Sch H, Part I, line 7i, $3,483,643 was contributed to or at the direction of Tarrant County Indigent Care Corporation, a charitable organization that provides various local organizations with funds to cover healthcare provided to indigent patients.
Affiliated Health Care SystemPart VI, Line 6 - Affiliated Health Care System The organization is part of the THR healthcare system. THR is one of the largest faith-based, nonprofit healthcare delivery systems in the U.S. and the largest in North TX in terms of patients served. The system of 14 wholly controlled hospitals includes an organization for medical research and education. The system also includes a foundation that fosters philanthropic relationships which support the programs and services of the hospitals it supports. The mission of the hospitals in the THR system is to improve the health of the people in the communities we serve. THR takes its responsibility to its communities seriously and invests charitable resources to promote good health and prevent disease. Not only does THR provide health care to those who do not have the means to pay, its hospitals also conduct a variety of programs designed to improve health and prevent illness in the community. The community health strategies of THR and its affiliated healthcare organizations are driven by community health needs are community-based, include confronting health problems at the source and emphasize health promotion, disease prevention, and early treatment of illness.
State Community Benefit ReportPart VI, Line 7 The organization files a Texas Annual Statement of Community Benefit Standard Report. Texas has established a minimum level of charity care and community benefit that must be provided by nonprofit hospitals in order to retain their Texas tax-exempt status. The standard set by the state is met when charity care and community benefits are provided in a combined amount equal to at least five percent of the hospital's or hospital system's net patient revenue, of which charity care and government-sponsored indigent health care are provided in an amount equal to at least four percent of net patient revenue. The hospital is part of the THR system of healthcare organizations. During the year, the THR system, as a whole, reported to Texas total charity care and community benefits of $652,986,757, in the following categories: $165,719,172 in patient charity care,$49,022,054 in support provided through others,$21,946,439 in unreimbursed Medicaid, $22,545,619 in other community benefits and $393,753,472 in unreimbursed Medicare.
Community BuildingPart II The organization participates in community building activities that aim to address the socio-economic factors that influence the overall health of individuals in the communities served. As identified in the community health needs assessment, there are areas of need that are not in alignment with the operations of a hospital system. Our volunteerism, collaborations and partnerships with various aid organizations and various area coalitions aid in addressing the socio-economic factors that are the backbone for many underlying health concerns.
Workforce DevelopmentPart II The organization also partners with local universities to increase promotion and training of diverse candidates into the healthcare workforce which in turns increases access to healthcare for many in the community.

ExplanationTexas Health HEB (THHEB) is a member of the Texas Health Resources (THR) healthcare system. The majority of the grants distributed by the system are to related tax exempt organizations. The hospital does not specifically monitor the use of grants paid to related organizations since THR administration monitors the use of these grants on a system basis for all members of the THR healthcare system. Grants to non-related tax exempt organizations are generally given to entities that are in the hospital's community and/or service area or to certain organizations which will specifically impact the hospital's community.
