Group Southwest General Health Center Line Part V, Section B, Line 5Southwest General Health Center created a Community Health Needs Assessment Advisory Council which includes leaders within Southwest General and representatives from local community organizations that represent the general community and those at risk. In addition, community health needs assessments were sent to residents in order to obtain self-reported information related to numerous health indicators.
Group Southwest General Health Center Line Part V, Section B, Line 6bSouthwest General Health Center collaborated with The Center for Health Affairs and multiple other partners to form Cuyahoga County Health Partners. Partnering members included representatives from area hospitals, mental health, public health, schools, universities and agencies that represent the community. Included agencies represented low income and minority populations and the medically underserved UnIted Way, The Salvation Army and Neighborhood Family Practice to name just a few.
Group Southwest General Health Center Line Part V, Section B, Line 11Southwest General Health Center reviewed all the data and prioritized community health needs based on the hospitals scope of practice and expertise. Many of the health indicators identified for adults were currently being addressed through Southwest Generals acute, outpatient and community programs. A decision was made to focus on the following four priorities cardiovascular health, mental health and chemical dependency, cancer, and access to prescriptions. Within these four priority areas, many of the other health indicators i.e. diabetes, womens health, mens health and hypertension also fell within these priorities and will continue to be addressed internally or with our collaborative partners. Needs not addressed are related to safety and social indicators/issues, such as issues pertaining to guns and violence, helping residents pay rent and/or utilities, providing emergency shelter, clothing, legal services, and tax preparation. Also not being addressed at this time, will be oral health. All of these services are available within the community and, as needed, referrals can be made to the appropriate resources.
Group Southwest General Health Center Line Part V, Section B, Line 13bPatients provide the necessary financial information to demonstrate financial need to medical indigence or have high deductible plans are eligible for financial assistance based on the family income. Southwest will discount all applicable patient balances income is between 251 and 400 of currently applicable FPG so that patient bill not be billed gross charges and will not be obliged to pay more than the Medicare reimbursement amount for such care in compliance with the Patient Protection and Affordable Care Act.

Part I Line 3cThe amount of charity care discount granted was based on the following guidelines. Patients attest they have no health insurance, provide the necessary financial information to demonstrate financial need, and are at or below 400 of the currently applicable Federal Poverty Guidelines. Patients who apply but do not qualify for a third party payment program and whose family income is less than 250 of the current Federal Poverty Guidelines will qualify for free care.
Part I Line 6aCommunity Benefits are reported in the annual report sent to the Board and members of the community.
Part I Line 7A cost-to-charge ratio was used to complete the charity care Line 7a and means-tested government programs Line 7b. The cost- to- charge ratio was derived from Worksheet 2 that accompanies the instructions to this schedule.
Part I Line 7The hospitals accounting records were used to complete the community health improvement services and community benefit operations Line 7e and health professions education Line 7f.
Part I Line 7g Subsidized health services include costs and direct offsetting revenue for the following services Coumadin Clinic, Hospice, Residential Hospice, Inpatient Mental Health, and Geriatric Mental Health. The hospitals cost accounting system was used to determine revenue and costs on all services.
Part I Line 7column f. In calculating the percent of total expense reported in column f, the total expense as reported on Form 990 Pt IX, Line 25, column a was reduced by 16,553,888 - the amount of bad debt reported on line 24 a.
Part III Line 2A ratio of patient care cost to charges is used on the bad debt reported on the Health Centers financial statement,
Part III Line 3The Health Centers financial statement bad debts and collection indicators are used along with the ratio of patient care cost to charge ratio.
Part III Line 4The Health Center maintains an allowance for bad debts based on the expected collectability of patient accounts receivable. The allowance is based upon managements assessment of historical and expected net collections considering historical business and economic conditions, trends in health care coverages, and other collection indicators. Periodically through the year, management assesses the adequacy of the allowance for bad debts based upon historical write-off experience by payor category. The results of this review are then used to make modifications to the provision for bad debts to establish an approriate allowance for the uncollectible receivables. After satisfaction of amounts due from insurance, the Health Center follows established guidelines for placing certain past due patient balances with collection agencies, subject to the terms of certain restrictions on collection efforts as determined by the Health Center.
Part III Line 8Medicare shortfall is considered a community benefit of the Health Center. Medicare shortfall uses Medicare revenue and allowable costs as reported in the Medicare Cost Report less net subsidized health services reported in Part 1 line 7g and health professions education reported in Part 1 line 7f.
