Provided both a Spring and a Fall conference related to accreditation standards at two locations per offering. Quality Summit Conducted the 8th Annual Quality Summit on June 10, 2015 entitled, "A Century of Health Care Innovation" with over 200 attendees participating. The summit was held at the Hilton at Easton, Columbus, Ohio and featured four panel presentations including: -Sepsis: Improving Outcomes and Decreasing Mortality Through Effective Practices -Reducing All Cause Readmissions through the Lenses of Finance, Quality, and Clinical Practice -Incorporating Patient Family Engagement -Disparities: A Population-Based Approach to Medication Errors Professional poster presentations with ample networking opportunities to disseminate effective practices were coordinated. Following the summit, recordings of panel presentations and poster abstracts were posted to the OHA website. Initiated the REF of OHA Statewide Sepsis Initiative in response to the OHA Board-directed goal of a statewide 30% reduction in sepsis mortality. 75 member hospitals submitted letters of commitment to participate in the ongoing initiative which includes: -Data submission regarding compliance to the elements of the 3-hour bundle -Monthly educational/coaching calls -Dissemination of effective practices Website launched in June, 2015 with monthly educational/coaching webinars posted along with literature and resources. Hand Hygiene (HH): In 2015, the hand hygiene program included the onset of CONTRACTED hand hygiene observations and reporting. Certain hospitals and hospital systems saw the need for continuous hand hygiene monitoring on a monthly basis and at higher thresholds of observations than associated with projects typically led by the REF. Other locations within OHA member community hospitals have also chosen to participate. This program afforded data continuity for dashboard purposes and avoided the loss or lag of data between REF projects & funding, such as the PfP or HEN programs. Rates for all participating hospitals in Hand Hygiene at the beginning of 2015 averaged: 79 to 81% Rates for all participating hospitals in Hand Hygiene at the end of 2015 exceeded 82% REF 2015 innovations in statewide Hand Hygiene included: -Expansion of huddles to include and address HH rates and goals -Addition of HH to performance expectations in job descriptions and unit policies and procedures -Hand Hygiene electronic monitoring trials in select hospitals and units -Regional retraining and education for Observers -Expanded use of non-students as Observers in data collection to optimize resourcesDesc

DescEmergency Preparedness: Regional Initiative Funding: Regional funds in the amount of $556,408 were received to complete required deliverables, provide an initial $1,500 funding for each hospital with the option to submit for additional funds totaling $70,000. These funds provide the opportunity for healthcare facilities to address their preparedness initiatives as well as supporting the regional healthcare coordinator, the regional coalition coordinator, and 14 county coalitions. Drills and Exercises - collaboration with state and regional partners Regional Full Scale Exercise partnership with Public Health and Regional Coalition members to test hospital and community preparedness for an Ebola outbreak situation. The exercise was designed to provide all participants the opportunity to establish incident command, test identification and isolation of suspected Ebola patient, and staff health and safety procedures. Regional SNS Request Exercise partnership with all 17 hospitals, 2 regional PH coordinators, 14 local health departments and 14 county EMAs (hospitals are only present in 14 of the 21 counties). The SNS Request Standard Operating Guide developed for and adopted by the hospitals which outlines how to request and provide information to PH and EMA was tested via a functional exercise conducted in May, 2016. Special Training: SNS requesting/receiving/awareness training was held via webinar in May 2015 and was attended by 42 hospital participants and 21 local public health departments. Training on how to assemble the mobile all-weather shelters (aka BluMed) was held at two locations in September and was attended by 35 hospitals along with public health, and EMA partners. Assets *SE Ohio has always been conscious of the size and rural nature of the region. All assets are stored at no expense to the grant and available to the entire region -Patient Care/Med Surge supply caches are pre-positioned in 10 of the 21 counties and are inventoried and moved yearly -Mass casualty trailers or supplies are prepositioned in 3 counties cross the region -Refrigerated remains trailer or portable morgue units are prepositioned in 2 counties -Bus Fitting evacuation cots are prepositioned in 11 counties -Oxygen administration units are in two counties -Large portable generators are in two counties and two large generators have been installed in senior centers/community centers in two counties. *For FY 2015, we initiated preventive maintenance record submission for all regional assets stored within the hospitals or Pike County Health District warehouse. *Hospital inventories of grant purchased equipment are verified through site vists. Coalitions *Regional - the Southeast Ohio Healthcare Coalition (SOHC) was developed in FY 2013 with by-laws adopted and a Steering Committee set. The Steering Committee approved the FY 2015-2016 budget and activities related to the coalitions: -In FY 2015-16 the SOHC developed both a communications plan and a command and control plan which were tested in the regional functional exercises -Proposed activities for FY 2015 have been cross-walked against the BP4 reporting measures to ensure we are on track *County - Since 2009, SE Ohio has worked to establish county healthcare coalitions consisting of hospitals, public health, EMA, and all other healthcare and other organizations willing to participate. 14 coalitions work on a local/county/multi-county level to improve the healthcare preparedness State Initiative Funding *State funds in the amount of $364,801 were used to initiate and sustain existing programs/initiatives and develop and implement new statewide programs/initiatives as well as provide funding for the Director of Public Health Programs to oversee this grant and the SE Ohio Regional Grant. These initiatives included: -Provided training to all users of the OHTrac (patient tracking/family reunification system) on the new mobile application developed and implemented in November 2015. This new app will allow expansion of OHTrac to the EMS environment to initiate patient tracking from scene to final destination as well as provide hospitals with the opportunity access OHTrac from any location. A narrated power point training was developed and provided to all healthcare facilities, Regional Healthcare Coordinators and other healthcare providers on the use of the mobile application to include downloading procedures, scanning of patient information, and destination location. -Pediatric triage model for crisis standard of care: Triage mode has been completed and is currently available, with ODH permission for peer review at the request of the developers. This triage model will serve as a model not only for Ohio hospitals and healthcare facilities but for healthcare facilities across the nation. The Institute of Medicine and Assistant Secretary for Preparedness and Response are extremely interested in viewing the final product for endorsement. -Implementation of standard data field definitions and process for data entered into Surgenet (bed tracking) by hospitals and healthcare partners was completed and approved by Ohio Department of Health. Webinars are in place to provide Surgenet users with education on the changes along with a web based test site that can be utilized for training of new users, and during exercises. The development of the test site was a direct request from hospitals so that multiple users could conduct training and participating in exercises on the system without affecting the live system or incurring travel or loss of work costs for hospitals. Ebola and Other Special Pathogens Initiatives and Funding -Ebola and Other Special Pathogens healthcare and community response guidelines were established in collaboration with the Ohio Department of Health and the regional healthcare coordinators. These guidelines were utilized by the regional healthcare coordinators, regional public health coordinators, and appropriate partners to establish their regional Ebola and Other Special Pathogens Response Plan. -OHA entered into a five (5) year contract with the Ohio Department of Health in the amount of $1.2 million over the 5-year period. The goal of this contract is to establish a response process to address pathogens that can be transmitted via the contact, droplet or airborne routes. A statewide Ohio Special Pathogens Coalition was developed with representation of all entities responding to or caring for a suspected or confirmed patient with any disease/condition spread by the routes identified. This Coalition developed a statewide training and exercise plan to identify opportunities to test and/or train on their process to identify, transport, and care for the patient. Additionally, the REF developed a web-based Personal Protective Equipment (PPE) Inventory tool that will identify the type and amounts of PPE available across the state.Desc

This legislation requires hospitals to report data to ODH regarding new parents who report not having access to a safe sleep environment. Early Elective Deliveries: The REF continued its "39+ Weeks is Good4Baby" initiative aimed at reducing early elective deliveries (EED) in Ohio hospitals. OHA continued the EED work as a part of the HEN 2.0 contract. OHA received a grant for $2,000 in patient brochures related to 39+ weeks from the March of Dimes which were distributed at no cost to Ohio hospitals. Breastfeeding: The First Steps for Healthy Babies, a quarterly recognition program for hospitals implementing the 10 Steps to Successful Breastfeeding, was launched in March of 2015 in partnership with the Ohio Department of Health. OHA received a $15,000 contract from the Association of State and Territorial Health Officials (ASTHO) to support the First Steps program. A portion of these funds were used to develop a certificate, small poster, and larger progress poster which were printed for hospitals as free resources. Two rounds were received in 2015, as the submissions began in July 2015. 36 hospitals, including 8 Baby Friendly USA hospitals, were recognized. All recognized hospitals received free materials. Presented the First Steps program with co-lead from ODH on national ASTHO virtual learning session in December 2015. Walk With A Doc (WWAD): As a part of OHA's centennial celebration, OHA covered the start-up cost and two-year fees for hospitals who wished to begin or continue the Walk With A Doc community walking program. Funded 20 hospitals and health systems to support the WWAD program ($10,000). Energy Foundation: The energy program increased energy efficiency and renewable energy resources through new policy initiatives and improved air quality to improve the health of all Ohioans. It includes a strong network for sustainable healthcare including engagement of hospitals in PUCO rate cases, collaboratives and state policy, creating a powerful learning community, supporting hospitals to achieve high levels of energy efficiency, emergency preparedness and other climate mitigation and sustainability strategies.Desc

Form And Line ReferenceSchedule D, Part X, Line 2 FIN 48 (ASC 740) footnoteFormAndLineReferenceDesc
ExplanationAccounting principles generally accepted in the United States of America prescribe recognition thresholds and measurement attributes for the financial statement recognition and measurement of a tax position taken or expected to be taken in a tax return. Tax benefits will be recognized only if the tax position is more likely-than-not sustained in a tax examination, with a tax examination being presumed to occur. The amount recognized will be the largest amount of tax benefit that is greater than 50% likely of being realized on examination. For tax positions not meeting the more-likely-than-not test, no tax benefit will be recorded. Management has concluded that they are unaware of any tax benefits or liabilities to be recognized at December 31, 2015 and 2014. The Foundation does not expect the total amount of unrecognized tax benefits to significantly change in the next 12 months. The Foundation would recognize interest and penalties related to the unrecognized tax benefits in interest and income tax expense, respectively. The Foundation has no amounts accrued for interest or penalties as of December 31, 2015 and 2014.ExplanationTxt
