NON-PROFIT ORGANIZATIONS WERE ALSO ENGAGED VIA KEY INFORMANT INTERVIEWS INCLUDING LEADERS FROM THE ATLANTA WOMEN'S FOUNDATION, THE COMMUNITY FOUNDATION FOR GREATER ATLANTA, AND THE UNITED WAY OF METRO ATLANTA. KEY INFORMANT INTERVIEWS: MUCH OF THE WORK IN ARCHI (ATLANTA REGIONAL COLLABORATIVE FOR HEALTH IMPROVEMENT), HAS INCLUDED THE INPUT OF COLLABORATIVE PARTNERS, AND CONTRIBUTED TO THE ASSESSMENT OF THE HEALTH NEEDS AND THE IDENTIFICATION OF HEALTH PRIORITIES IN GRADY'S SERVICE AREA. ARCHI RECEIVED COMMUNITY INPUT FROM A VARIETY OF SOURCES INCLUDING STAKEHOLDER MEETINGS, ONE-ON-ONE INTERVIEWS, AND WRITTEN FEEDBACK. MANY DIFFERENT TYPES OF ORGANIZATIONS AND INDIVIDUALS WITH DIFFERENT TYPES OF EXPERTISE WERE REPRESENTED, INCLUDING: BEHAVIORAL HEALTH, BUSINESS COMMUNITY, CIVIC AND ADVOCACY ORGANIZATIONS, FAITH COMMUNITY, FEDERALLY QUALIFIED HEALTH CENTERS, HEALTH PLANS, LOCAL GOVERNMENTS, MEDIA, PHILANTHROPY, PHYSICIANS,PUBLIC HEALTH, SOCIAL SERVICE PROVIDERS, AND UNIVERSITIES. AS PART OF THE DATA COLLECTION PROCESS FOR ARCHI, THE GHPC CONDUCTED SEVERAL KEY INFORMANT INTERVIEWS WITH PUBLIC HEALTH LEADERS, COUNTY GOVERNMENT REPRESENTATIVES, AND REPRESENTATIVES OF THE PHILANTHROPIC COMMUNITY IN FULTON AND DEKALB COUNTIES. FOCUS GROUPS: A TOTAL OF FIVE FOCUS GROUPS WERE HELD IN THE TARGET COUNTIES. ENGLISH SPEAKING AND LATINO GROUPS WERE RECRUITED BY OUTSIDE FOCUS GROUPS FROM FIVE DIFFERENT CENSUS TRACTS, IDENTIFIED AS BEING HOT SPOTS FOR THE KEY INDICATORS OF DEMOGRAPHICS, POVERTY, UNINSURED POPULATION, EDUCATION ATTAINMENT AND ECONOMIC FACTORS. |

OTHER PROVIDERS PARTICIPATING IN ARCHI INCLUDE EMORY HEALTHCARE, PIEDMONT HEALTHCARE, SOUTHSIDE MEDICAL CENTER, SAINT JOSEPH'S HEALTHCARE, SAINT JOSEPH'S MERCY CARE AND THE FAMILY HEALTH CENTERS OF GEORGIA (FORMERLY KNOWN AS WESTEND MEDICAL CENTER). |
| PART V, SECTION B, LINE 6B | MANY HEALTH CARE,PUBLIC HEALTH, ACADEMIC, NON-PROFIT AND PHILANTHROPIC ORGANIZATIONS ARE PARTICIPATING IN ARCHI INCLUDING ATLANTA REGIONAL COMMISSION, KAISER PERMANENTE OF GEORGIA, UNITED WAY OF GREATER ATLANTA, CARTER CENTER, CENTER FOR DISEASE CONTROL AND PREVENTION, GEORGIA DEPARTMENT OF PUBLIC HEALTH, GEORGIA HEALTH POLICY CENTER, UNIVERSITY SYSTEM OF GEORGIA, AMONG OTHERS. |
| PART V, SECTION B, LINE 11 | In 2015, Grady Health System focused on the implementation strategy through both collective impact with our community partners and investments within the health system to increase access and coordination of care. Below are the activities related to the strategies identified in the Grady Health System Community Benefit Implementation Strategy: 1. Effective and efficient patient care coordination among persons served by the Grady Health System. Grady continues to lead in Community collaboration through the Atlanta Regional Collaborative for Health Improvement (ARCHI). Grady has representation on the Steering Committee and has participated in community-wide care coordination planning and implementation. In 2015, ARCHI launched the Tri-Cities Community Pilot Project. With a neighborhood health center in the East Point neighborhood, Grady is a critical partner in care coordination for this pilot. In 2015, Grady opened updated and expanded urology services. These facilities showed improved access and volumes through 2015. Grady continues to invest in the success of these services to ensure community access to high quality, affordable care. Also, Grady finalized the implementation of the CareLink electronic medical record module to extend patient information to the federally qualified health centers and post-acute partners. CareLink allows external providers to access patient information securely i.e., if a patient visits the Grady emergency department and then later visits a federally qualified health center (FQHC), the physician at the FQHC can access information from the patients ED visit. Physicians can also request specialty appointments and follow up on whether or not the patient kept the appointment. Post-acute providers can access discharge information to facilitate activities that support appropriate care and reductions in re-admissions. Throughout 2015, Grady continued several initiatives begun in 2014 including maintaining Level III Patient Centered Medical Home (PCMH) certification to support coordination as a safety net medical home. The guidelines for PCMH continue to evolve to meet research literature and targeted outcomes. Grady continues to evolve our care delivery model to ensure effective care giver roles and optimization of the electronic health record. Grady also continued two community health worker programs. In partnership with the United Way and the federally qualified health centers, the first program targets individuals with high risk for re-admission after a hospitalization and has been shown to decrease re-admissions in this vulnerable population. Similarly, the community health worker and telehealth program supports the congestive heart Failure program, identifying patients at risk in the inpatient setting and aligning them to a nurse practitioner to manage their care. |
| PART V, SECTION B, LINE 11 (CONTINUED) | 2. Increase opportunities among persons served by the Grady Health System to access appropriate health care services. Grady has engaged a number of resources to assist patients in gaining access to Medicaid and Marketplace health insurance. In 2015, Grady partnered with Enroll America to complete outreach to former patients. Enroll America contacted over 2,000 people of which nearly half were uninsured and 502 were successfully scheduled for in person assistance. To assist with individual enrollment into Marketplace insurance, Grady previously partnered with Seedco for patient navigators and RING volunteers for patient education. After the grant supporting these navigators expired in 2015, Grady hired the navigators to continue enrollment assistance. In 2015, the navigators helped 163 patients successfully gain health insurance through the Marketplace. Grady also has dedicated employees who work with patients to ensure they have the right information to support the Medicaid application, and we facilitate them through this process to ensure access. In 2015, nearly 6,000 individuals (4,825 adults and 1,051 children) were approved for Medicaid. 3. Improved sexual health in high risk populations in DeKalb and Fulton counties. Grady Health System operates the largest HIV program in Georgia, providing treatment TO over 5,800 people, WHICH IS approximately 25% of people living with HIV in the state. Because strict adherence to antiretroviral therapy (ART) keeps people healthy and significantly reduces risk of transmission, Grady is committed to providing excellent care to all HIV positive patients. 4. Evidence based interventions to reduce obesity in residents of DeKalb and Fulton counties. Grady continues to partner with the Atlanta Beltline to increase access to safe parks, walk/run/bike pathways and health education. Grady is currently on the Beltline Health Steering Committee which provides guidance on a range of health programming. 5. Reduced risk of homicide and injury in target populations in Fulton and DeKalb counties. In 2015, the Injury Prevention and Trauma Research Program facilitated 35 injury prevention presentations and educational activities across the state. Topics included safe driving, fall prevention, violence prevention, child physical abuse prevention, and general trauma awareness. More than 1,500 individuals representing the general public (teens, parents, older adults), nurses, doctors, EMS providers, leadership, and others attended the events. Partnering agencies included the Shepherd Center, Georgia Department of Public Health, Childrens Healthcare of Atlanta, Atlanta Public Schools, DeKalb School System, Georgia EMS, and local senior centers. The lower priorities, which included low birth weight, STDs and high school education non-attainment, were not included as areas of focus for several reasons. Other organizations around Atlanta focus on low birth weight prevention. Similarly, the Georgia Department of Public Health Office of Sexually Transmitted Diseases funds screening and treatment services throughout the state, and other coalitions and non-profit organizations are diligently addressing STD prevention. Likewise, various initiatives led by the Georgia Department of Education and the United Way of Greater Atlanta are underway to address high school graduation non-attainment. PART V, PART B, LINE 15, BOX E The Financial Counselors will advise eligible patients verbally as to where and how to apply for Food Stamps and determine if they meet criteria for Medicaid. Social Workers will typically provide information regarding housing, Food Stamps, and other services to patients. A Financial Counselor may determine if a patient meets criteria for Presumptive Medicaid, Women's Health Medicaid, Women Medicaid Wavier, Cancer State Aid, Crime Victims Compensation Program, Right From The Start Medicaid for newborns, Emergency Medical Assistance - Medicaid for undocumented women who deliver their newborns, Presumptive Medicaid for Pregnant women, Low Income Medicaid, etc., and complete the application as appropriate. The Financial Counselor may also refer a patient to apply for Medicaid with the Department of Family and Children Services Grady Outreach Unit. GMHC haS vendor partnerships on campus whereby representatives are completing applications for eligible patients for various Medicaid Programs, e.g., Low Income Medicaid, SSI, SSD, etc. |
| PART V, LINE 20A: | ADMISSIONS REPRESENTATIVE USUALLY NOTIFIES PATIENTS OF FINANCIAL ASSISTANCE ON ADMISSION. NONE OF THE ACTIONS LISTED IN LINE 19 ARE FOLLOWED. PART V, LINE 22D: AFTER INITIAL MEDICAL EVALUATION, FAMILY COMMUNICATES/PROVIDES FINANCIAL PROOF, RESIDENCY INFORMATION, ETC. SO THAT THE ELIGIBILITY REQUIREMENTS FOR FINANCIAL ASSISTANCE ARE MET.
