ExplanationProvide a Detailed Description of the Lobbying Activity Sinai Health System engages in education and other activities to influence legislators regarding federal and state policies related to Medicaid, Sinai's major payer and the safety net of many of its patients. The system lobbying activities are coordinated through Roberta Rakove, senior Vice President for Sinai Health System. Lobbying is conducted on behalf of the entire Sinai System, therefore registered state lobbyists are registered to represent Sinai Health System. Some expenses are allocated to Mount Sinai Hospital and Holy Cross Hospital but the expenses are paid by Sinai Health System. At the federal level, Sinai Health System does not utilize contract lobbyists. Sinai Health System collaborates with the American Hospital Association, the Illinois Hospital Association, the Premier Hospital Alliance, and the Jewish Federation of Metropolitan Chicago/Jewish United Fund. A portion of dues from the American Hospital Association and the Illinois Hospital Association support federal lobbying. At the state level, Sinai utilizes two outside lobbying firms.ExplanationTxt

ExplanationIncome taxes: Mount Sinai, Schwab, Holy Cross, SMG, and SCI are tax-exempt organizations under Internal Revenue Code Section 501(c)(3) and each, as required, files a form 990 (Return of Organization Exempt from Income Tax) annually. Progress Health, Inc. files Federal and Illinois forms 1120 (U.S. Corporation Income Tax Return) annually. The Corporation adopted Financial Accounting Standards Board (FASB) issued guidance for uncertainty in income taxes. The guidance prescribes a recognition threshold and measurement attribute for the financial statement recognition and measurement of a tax position taken or expected to be taken in a tax return. Examples of tax positions common to health systems include such matters as the following: the tax-exempt status of each entity, the nature, characterization and taxability of joint venture income and various positions relative to potential sources of unrelated business taxable income (UBIT). UBIT is reported on Form 990T, as appropriate. The benefit of a tax position is recognized in the financial statements in the period during which, based on all available evidence, management believes that it is more likely than not that the position will be sustained upon examination, including the resolution of appeals or litigation processes, if any. Tax positions are not offset or aggregated with other positions. Tax positions that meet the 'more likely than not' recognition threshold are measured as the largest amount of tax benefit that is more than 50 percent likely to be realized on settlement with the applicable taxing authority. The portion of the benefits associated with tax positions taken that exceeds the amount measured as described above is reflected as a liability for unrecognized tax benefits in the consolidated balance sheets along with any associated interest and penalties that would be payable to the taxing authorities upon examination. As of June 30, 2016 and 2015, there were no unrecognized tax benefits identified and recorded. Forms 990 and 1120 filed by the Corporation are subject to examination by the Internal Revenue Service (IRS) for up to three years from the extended due date of each return. Forms 990 and 1120 filed by the Corporation are not subject to examination for the years ended June 30, 2012 and prior.ExplanationTxt

FORM 990, PART III, LINE 4AProgram Service Accomplishments Sinai Health System is the parent organization and provides administrative management support to its affiliated organizations to enable them to improve the health and communities it serves. Located on the west side of the city of Chicago and serving a population base of 1.5 million people in its service area ZIP codes, Sinai Health System (Sinai) consists of six member organizations. They are Mount Sinai Hospital, Holy Cross Hospital, Schwab Rehabilitation Hospital, Sinai Community Institute, Sinai Medical Group, and Sinai Urban Health Institute. Sinai has a number of clinics that ate located on the west and south sides of Chicago. There is also one facility, Sinai's Touhy Clinic on the north side of Chicago, which sees refugees from many nations (for example Sudan, Iraq and Burma) as well as resident Orthodox Jewish and Russian populations. With the exception of the Touhy Clinic, Sinai Health System serves primarily African-American and Latino patients. Sinai's programs and community outreach are based on scientific epidemiological assessments of local neighborhoods done by Sinai Urban Health Institute (SUHI). In addition to the work of epidemiologists, SUHI community health educators work with neighborhood groups and go door to door for further identification of individuals with chronic disease and for intervention in disease processes. SUHI also conducts health disparities research, develops innovative community health interventions, delivers community health worker training and consultation, and provides a broad scope of evaluation services. Part III, Line 4a and 4b describe the top program services that Sinai conducts with SUHI.
FORM 990, PART III, LINE 4BProgram Service Accomplishments The Coordinated Healthcare Interventions for Childhood Asthma Gaps in Outcomes (CHICAGO Plan) is a three arm multi-center randomized control trial for pediatric asthma, funded by the Patient-Centered Outcomes Research Institute (PCORI). The Plan is a three year project which began with a planning year in March 2014, and the active intervention phase kicked off in March 2015. The project is led by the University of Illinois Hospital & Health Sciences System, and includes researchers from the Ann & Robert H. Lurie Childrens Hospital of Chicago, the University of Chicago Medicine, Rush University Medical Center, John H. Stroger and Jr. Hospital of Cook County, in addition to those from SUHI and Sinai. The trial is designed to investigate how health outcomes for African American and Latino children with uncontrolled asthma might best be optimized, namely by investigating the effectiveness of two interventions: 1) structured asthma management education provided to the patient in the Emergency Department (ED) by an ED Coordinator (ED Intervention arm); 2) structured education in the ED coupled with five home visits over the course of six months from a trained Community Health Worker (CHW) (CHW Home Visit arm). The third arm of the study, the control arm, receives usual care for pediatric asthma in the ED. Based on its expertise in implementing the CHW model, SUHI serves as the CHW Coordinating Center (CHW CC) on the project. Sinais ED also serves as a clinical site, enrolling eligible children that come through the ED for an asthma exacerbation. As the CHW CC, SUHI is responsible for the hiring, training and on-going supervision of CHICAGO Plan CHWs at all six intervention sites. For children and their families enrolled in the CHW intervention arm, CHWs conduct five home visits over the course of six months. Throughout the course of the visits, CHWs provide individualized, comprehensive asthma management and healthy homes education to each family. Enrollment into the study was initially supposed to end in May 2016, but PCORI approved a 3 month no cost extension, therefore enrollment into the study continued through August 2016. A total of 371 children ages 5-11 years were enrolled into the study between March 2015 and August 2016. The majority of participants were black (63%) and male (67%) children, and most caregivers were single mothers (63%) with a high school education or less. Enrolled children had a mean cACT score of 16.2 (a score of less than 19 on the cACT indicates poor asthma control). Outcomes analysis is ongoing and results will be available after the study follow-up ends (April 2017). Through Sinais and Holy Cross EDs, a total of 124 children were screened (87 at Sinai and 37 at Holy Cross) of which 40 were enrolled into the study and randomized: 13 in the usual care, 13 in the ED intervention only arm, and 14 in the CHW home visit arm. SUHIs CHW CC Supervisors meet regularly with all CHICAGO Plan CHWs to provide case management support for each family, navigate complex home environmental issues, and ensure all necessary topics are being covered in each home visit as well as the quality of data entered by CHWs. Supervisors also conduct on-going random shadowing of CHWs for quality assurance purposes, and schedule monthly on-going continuing education opportunities.
FORM 990, PART III, Line 4CProgram Service Accomplishments There is no question regarding the deleterious acute and long-term consequences of diabetes and uncontrolled hyperglycemia. The multitude of severe long-term complications causes more adult cases of blindness, renal failure, and amputations than any other disease in the U.S. In addition, individuals with diabetes have a 2- to 4-fold increased risk for cardiovascular disease and stroke. Similarly, a large body of research has documented increased incidence of cardiovascular and peripheral vascular disease. Data from the Mount Sinai Hospital and its clinics show large number of patients with uncontrolled hyperglycemia. The Controlling Hyperglycemia among Minority Populations (CHAMP) study is a randomized clinical trial led by Sinai Urban Health Institute (SUHI) that has been funded with generous support from Blue Cross Blue Shield of Illinois. The purpose of the study is test the efficacy of two interventions designed to lower blood glucose levels among adults with diabetes. Through this trial, SUHI hopes to determine whether diabetes education provided by a trained community health worker or diabetes education provided by text message is more effective in controlling blood sugar levels compared to standard care. In addition, we are testing the impact of these interventions on health care access and utilization; diabetes knowledge, beliefs, and self-efficacy; and behavioral outcomes. We believe this study could have important implications for health care providers, payers, and other practitioners who are implementing community-level interventions for people living with diabetes, especially in low-income communities. Since January 2016, we have assessed 934 Sinai patients for study eligibility and successfully enrolled 241 participants, each randomly assigned to one of three interventions (Text Message, Community Health Worker (CHW), or Usual Care (Control)). Patients are recruited from three Sinai Health System sites: Mount Sinai Hospital, Holy Cross Hospital, and Sinai Community Institute. There are 75 participants in the CHW Group to date. Each of these participants is visited by CHWs who provide home-based, individualized diabetes education using a standard curriculum, up to six times during the course of the intervention. Each visit lasts approximately one hour. After every session, the participant sets 1-3 SMART (specific, measurable, attainable, relevant, and time-based) wellness goals to work on before the next session CHWs follow-up with the participants by phone in between the monthly home visits to check their progress. During these contacts, they address the participants' self-management activities. There are 78 participants in the Text Message Group. These participants are sent text messages 3-4 times per week through the CareMessage platform (www.caremessage.org). The 25-week text message program was created by our research team and it echoes the Diabetes Learning Circle curriculum. There are 88 participants in the Control Group. These participants receive standard education and medical care as determined by the hospital's diabetic educator, dietitian, or their clinical provider.
FORM 990, PART VI, LINE 1AExplanation of Delegated Board Authority to Committee The Executive Committee consists of the Board Chair, Vice Chair(s), Secretary, Treasurer and President. The Executive Committee has the power to transact all regular business of the Sinai Health System during the period between meetings of the Board, subject to any prior limitation imposed by the Board or by law. When action is taken by the Executive Committee, it will be reported to the Board at the next meeting of the Board.
FORM 990, PART VI, LINE 7ABoard members are nominated by the Board Recruitment, Education, and Nominating Committee of Sinai Health System, and elected by the members from among those persons approved for appointment by Sinai Health System.
FORM 990, PART VI, LINE 11BThe Form 990 information was initially provided by the finance staff with multi-disciplinary input from Public Affairs, Corporate Integrity and other appropriate staff of the organization. The Form 990 was then prepared by Ernst & Young, LLP and reviewed by senior finance staff and other members of senior leadership. Prior to the filing, the Form 990 was made available to the full board of directors.
FORM 990, PART VI, LINE 12CThe Conflict of Interest Disclosure Form is completed and signed annually by all board members, employed physicians, management personnel as well as other employees who are in a position to influence purchasing decisions, affiliations or referrals, hiring decisions or contracts. The form is completed on appointment to the board of directors of Sinai Health System or any one of its entities, upon hire for any relevant job category or at the time of appointment to the medical staff. In addition to the annual filing of the form, an update is required to be filed any time there has been a change. Conflicts disclosed on the Conflict of Interest Disclosure Form are taken into consideration when making board committee assignments. In addition, individuals who have a conflict of interest must abstain from participating in decisions affecting the interested parties and make it clear why they are abstaining. If the potential for conflict of interest exists, employees and physicians are required to discuss the situation with management. Board members of the Sinai Health System or any of its entities are required to report potential conflicts to the Chief Integrity Officer who will review potential conflicts with the Sinai Health System Chief Executive Officer and Chairman of the Board of Directors.
FORM 990, PART VI, LINE 15A & 15BSinai Health System has an Executive Compensation Committee to support the organizational performance of Sinai Health System and its related entities through the alignment of executive compensation with system strategies and programs and ensure compliance with applicable law. The voting members of the Committee are independent members of the Board of Directors. The Committee meets twice annually, or more frequently, as circumstances require. The Committee establishes detailed goals annually for the President and CEO and other executives, and reviews performance against these goals on an annual basis.
