| Desc | Reunion House Crisis Shelter for Homeless Teens: 24 hour crisis shelter for teens. Provided a safe homelike atmosphere for 247 abused, runaway, and homeless teenagers. Provided family support services, counseling, and resources to reunite families, re-engage teens in school, and prepare them for independent living. 81% of teens at the Reunion House returned to their families or another safe living environment.Homeless Youth Services: Outreach, housing, and support services for homeless and near homeless youth. Conducted street-based outreach making 1,440 contacts with teens and young adults. Provided shelter, case management, and support services helping 126 young adults and children escape homelessness and begin on paths to self-sufficient adulthood. 85% of these youth transitioned to stable and permanent housing. Family Shelter: Emergency shelter for families, couples, and singles. Provided short-term emergency shelter for 137 homeless children and adults in community-based apartments. Through case management, assessment, and service linkage, 75% of participating families exited Family Shelter to stable subsidized and unsubsidized long-term housing. Family Housing: Rapid re-housing and transitional housing for families. Provided subsidized housing, case management, and support services for up to two years for 134 homeless families including a total of 417 children and adults. The Family Housing programs helped these families increase income, reduce disruption to childrens education, and improve housing, safety, and family stability. Through the program, 88% of the families served escaped homelessness and transitioned to long-term stable housing. Homelessness Prevention: Brief assistance to avoid homelessness. Provided short-term rental and utility assistance, case management, and support services to prevent 133 children and adults from experiencing homelessness. Assisted with negotiation of payment arrangements and long-term financial planning to ensure that acute financial difficulties do not lead to homelessness. 94% of families receiving these services remained stably housed and were able to avoid homelessness. La Promesa: Affordable housing community for low-income families. Provided high-quality rental housing at affordable rates for 65 low- and moderate-income families who income qualified. Offered a safe apartment community and well-maintained homes for a total of 239 low- and moderate-income Tucsonans. Counseling: Individual, family, and group behavioral health services. Provided counseling services helping 256 children, youth, adults, and elders to experience relief from behavioral health challenges, improved well-being, and improved relationships. Through the elder counseling component, in-home counseling services were provided for 26 aging community members assisting them to prepare for and manage a variety of end-of-life issues. Healthy Futures: Behavioral health support services and groups for youth and families. Assisted 187 children, youth, and their parents to experience improved communication, anger management, relationships, parenting skills, and independent living skills by providing in-home family support and independent living skills education and psycho-educational group activities. 81% of participants demonstrated use of skills learned through their individual and group services. Drug Free Communities: Community-based substance abuse prevention. Engaged 344 high-risk youth and adults from Tucsons low-income 29th street corridor in community efforts to discourage alcohol and substance abuse among area teens. Facilitated educational prevention events reaching 1,193 29th street area residents. Mobilized 46 high-risk teens as active leaders in community campaigns to reduce adolescent alcohol and substance abuse. Center for Community Dialogue: Training and services that help Southern Arizonans discuss challenging issues in a skilled, civil, and respectful way. Assisted 647 community members to resolve conflict situations through skilled volunteer mediation. Facilitated conversations to help 517 youth, adults, and elders discuss sensitive topics. Hosted community forums and public workshops preparing 904 Tucsonans to more skillfully engage in civil dialogue and resolve conflict. 85% of training participants reported significant learning and 100% of mediated disputes came to resolution agreements.Senior Companion Program: Provided companionship and respite care empowering 317 area elders to live as independently as possible and out of institutional care. Low-income aging Senior Companion volunteers provided 45,642 hours of one-on-one support for elders living throughout Pima County and on the Tohono Oodham and Pascua Yaqui reservations. 95% of elders served reported improved independence and 100% of caregivers reported continued ability to care for their aging loved ones as a result of participation. Information and Referral: Collected and maintained community resource information for social service providers in Pima, Cochise, Graham, Greenlee, Santa Cruz, and Yuma counties. Published the Pima County Directory of Community Resources. Coordinated with Community Information and Referral in Phoenix to manage the statewide 2-1-1 helpline and online database responding to 65,158 requests for community resource linkage from Southern Arizona. | Desc |
