Following the summit, recordings of panel presentations and poster abstracts were posted to the OHA website. Facilitated the second year of the OHA Statewide Sepsis Initiative in response to the OHA Board-directed goal of a statewide 30% reduction in sepsis mortality. 125 member hospitals submitted letters of commitment to participate in the ongoing initiative which includes: -Data submission regarding compliance to the elements of the 3-hour bundle -Monthly educational/coaching calls -Dissemination of effective practices Website maintained and monthly educational/coaching webinars posted along with literature and resources. Hand Hygiene (HH): In 2016, the hand hygiene program contracted hand hygiene observations and reporting continued. Certain hospitals and hospital systems saw the need for continuous hand hygiene monitoring on a monthly basis and at higher thresholds of observations than associated with projects typically led by REF. OSU, Cleveland Clinic and Mount Carmel are examples of Systems who made such a request. Other locations within OHA member community hospitals have also chosen to participate, such as Berger and Memorial Hospital of Union County. This program afforded data continuity for dashboard purposes and avoided the loss or lag of data between OHA projects & funding, such as the PfP or HEN programs. REF 2016 innovations in statewide Hand Hygiene included: -Continuation of huddles to include and address HH rates and goals -Updates of HH to performance expectations in job descriptions and unit policies and procedures -Regional retraining and education for ObserversDesc

DescPopulation Health: Infant Mortality: Presented a data analysis of the Ohio Infant Mortality data and rankings at the quarterly in-person meeting of the Ohio Collaborative to Prevent Infant Mortality. Produced an accompanying white paper resource. The REF of OHA served on executive/steering committees for the Ohio Collaborative to Prevent Infant Mortality, Ohio Perinatal Quality Collaborative, and Cradle Cincinnati. The REF of OHA served in an advisory capacity for the Ohio Department of Health in regard to both the Maternal Child Health and Preventive Health and Health Services federal block grants. OHA served as the Fiscal Agent and on the Planning Committee for the 2016 Ohio Infant Mortality Summit held in December in Cleveland. Safe Sleep: The REF of OHA continued its "Safe Sleep is Good4Baby" initiative aimed at promoting the ABC's of safe sleep practices in Ohio. In 2016, the American Academy of Pediatrics updated their 'safe sleep' recommendations. REF worked with member hospitals to share these updates and communicate other related resources and materials around promoting safe sleep. REF worked closely with Ohio birthing hospitals related to mandated reporting to ODH due to legislation passed in 2014. This legislation requires hospitals to report data to ODH regarding new parents who report not having access to a safe sleep environment. In 2016 the state transitioned this reporting into the automated birth registry system, and REF assisted in communicating this transition with the hospitals. On August 2, 2016, REF presented at the Ohio Colleges of Medicine Government Resource Center - Infant Mortality Research Partnership Executive Committee Meeting, providing information about the previous and ongoing work around safe sleep promotion in Ohio hospitals. Early Elective Deliveries: REF continued its "39+ Weeks is Good4Baby" initiative aimed at reducing early elective deliveries (EED) in Ohio hospitals. OHA received a grant for $2,000 in patient brochures related to 39+ weeks from the March of Dimes which were distributed at no cost to Ohio hospitals. Breastfeeding: The First Steps for Healthy Babies, a quarterly recognition program for hospitals implementing the 10 Steps to Successful Breastfeeding, was launched in March of 2015 in partnership with the Ohio Department of Health. In 2016, the program expanded its reach to over two-thirds of the maternity hospitals in the state. OHA received an additional $15,000 contract from ASTHO to support the First Steps program. A portion of these funds were used to develop a flip chart, poster, and patient resource which were printed for hospitals as free resources. Over 1,600 free materials were printed and distributed across the state to hospitals. The First Steps program partnered with the Ohio Lactation Consultants Association (OLCA) and Ohio Breastfeeding Alliance (OBA) to revive a 'Bag Free' recognition program for hospitals. This program launched in early 2016, and will be an annual recognition. 52 hospitals were recognized for their work in 2015. The First Steps program hosted a free webinar on August 25, 2016 for the program that featured local experts and offered continuing education credit for attending hospital teams. The First Steps program was invited to the Annual OLCA conference on March 11, 2016. Ryan Everett presented at the main session about the First Steps program and the progress made in Ohio. This was followed with a recognition of all awarded hospitals for the 'Ban the Bag' program. The First Steps program was accepted to present a session at the Sixth National Breastfeeding Coalitions Conference on the program. The session title was "Ohio First Steps for Healthy Babies -- Collaborating to Promote Breastfeeding in Ohio Hospitals". A poster was also presented at the 2016 Ohio Infant Mortality Summit. Good4You: In 2016, REF released a free set of resources for hospitals aimed at encouraging healthier choices through making positive changes to their food and beverage environments. The voluntary and self-directed initiative, called "Good4You Eat Healthy", provided materials including a step-by-step guide for each of the focus areas in the hospital: cafeterias/cafes, vending machines, meetings/events and outside vendors and franchises.Desc

Form And Line ReferenceSchedule D, Part X, Line 2 FIN 48 (ASC 740) footnoteFormAndLineReferenceDesc
ExplanationAccounting principles generally accepted in the United States of America prescribe recognition thresholds and measurement attributes for the financial statement recognition and measurement of a tax position taken or expected to be taken in a tax return. Tax benefits will be recognized only if the tax position is more likely-than-not sustained in a tax examination, with a tax examination being presumed to occur. The amount recognized will be the largest amount of tax benefit that is greater than 50% likely of being realized on examination. For tax positions not meeting the more-likely-than-not test, no tax benefit will be recorded. Management has concluded that they are unaware of any tax benefits or liabilities to be recognized at December 31, 2016 and 2015. The Foundation does not expect the total amount of unrecognized tax benefits to significantly change in the next 12 months. The Foundation would recognize interest and penalties related to the unrecognized tax benefits in interest and income tax expense, respectively. The Foundation has no amounts accrued for interest or penalties as of December 31, 2016 and 2015.ExplanationTxt

Form 990, Part III, Line 4d Description of other program services(Expenses $ 163,918 including grants of $ 0)(Revenue $ 447,100) Population Health: Infant Mortality: Presented a data analysis of the Ohio Infant Mortality data and rankings at the quarterly in-person meeting of the Ohio Collaborative to Prevent Infant Mortality. The voluntary and self-directed initiative, called "Good4You Eat Healthy", provided materials including a step-by-step guide for each of the focus areas in the hospital: cafeterias/cafes, vending machines, meetings/events and outside vendors and franchises.
