Schedule H, Part V, Section B, Line 5:The assessment was conducted utilizing the MAPP model for assessments (Mobilizing to Action through Planning and Partnerships), a model designed by the Centers for Disease Control in conjunction with National Association of City and County Health Officials. This type of assessment is designed to include the eight dimensions of wellness, and incorporate both leadership and input from broad interests of the community we serve. Specifically, our assessment was spearheaded by South Peninsula Hospital, Alaska Department of Public Health Homer Office, South Peninsula Behavioral Health, Sprout Family Services, Kachemak Bay College, Haven House, Sustainable Homer, City of Homer, Seldovia Village Tribe FQHC, Family Planning and Cook Inlet Council on Alcohol and Drug Abuse. The input for the assessment came from staff and clientele of these organizations, key informant interviews in subject areas, focus groups through LPHA assessment, general population through surveys and specific outreach to the underserved.
Schedule H, Part V, Section B, Line 10a:The location of the implementation strategy is: https://www.sphosp.org/wp-content/uploads/2017/08/2017-SPH-Implementation- Strategy-APPROVED.pdf
Schedule H, Part V, Section B, Line 11:ACES (Adverse Childhood Experiences): - SPH is one of the five local agencies working to adopt Trauma Informed Care practices, and support the goal to become Alaskas first trauma-informed community. - SPH Health and Wellness RN serves on the MAPP Resilience Coalition leadership team. - SPH helps implement Resilience Coalition work group strategies. - SPH provides administration for the local MAPP health coalition. - Clinic intake incorporates ACE (Adverse Childhood Experiences) screening. - Trauma-informed care practices are being adopted, including the addition of Trauma Informed Care to the annual clinical skills fair and manager trainings. - SPH has an active task force working to address affordable day care - SPH is a lead agency in the community Safe & Healthy Kids Fair. - SPH offers spiritual care for patients and staff. - Attends Kindergarten Roundup to connect parents and guardians with the necessary care for the newly enrolled students. - SPH offers quarterly Safe Sitter babysitting classes to youth in grades six through eight. - Worked with high school to provide a special free sports physical clinic to make it easy for high school students to enroll in activities. - Underwrote "Week of Mindfulness" with Jerry Braza, leader in teaching mindfulness practices. Offered three trainings, one for healthcare professionals, one for community/parents and one for train the trainer. Access to Care: - Work with local agencies to support a wheel chair accessible taxi van available in the community, and have a representative on the Kenai Peninsula Borough Transportation Planning Committee to advocate for affordable local transportation. - Offer taxi vouchers for patients who do not have transportation, or coordinate with Medicaid for patient travel. - Maintain all elements of Patient Centered Medical Home Level II, which supports evening and weekend appointments and 24/7 online access. - We promote our clinic electronic health patient portal which offers immediate messaging to providers, health information, data tracking and more. - We expanded the Homer Medical Center primary care facility by more than 5,200 square feet, and have added seven additional exam rooms and onsite ancillary services to allow for additional providers and improved access to primary care. - We have added two new full time providers to our Primary Care clinic, and have a contract for a third to begin this summer. - We offer certified enrollment and financial counselors seven days a week to help patients qualify for programs or enroll in the insurance marketplace or other qualified insurance programs. - Our financial assistance information and application is readily available to patients at time of registration, and is posted on the hospital website. - We offer a self-pay discount price for high-dollar advanced imaging procedures. - We offer promotions on vital screenings or services including colonoscopy, PSA, cholesterol, vitamin D, thyroid, prostate, A1C, swallowing screening and heart health labs. - We offer Saturday appointments (when fully staffed) in our Physical Therapy department. - We offer same-day appointments in primary care, and a twice-weekly evening walk-in clinic. - We are exploring opportunities for home-visiting. - Our follow-up care phone call ratio is greatly improving, which is critical in connecting patients to the care they need post-hospital stay. - Wrote off $2,220 in uncompensated diabetes education not covered by Medicaid. - Provided 57 free sports physicals to uninsured or underinsured high school students. - Attends Kindergarten Roundup to connect parents and guardians with the necessary care for the newly enrolled students. - Participated in the annual health fair, with numerous reduced rate and free screenings, including free flu shots, reduced rate blood tests, fitness tests, swallowing screenings and more. Substance Abuse & Misuse, Mental Health and Domestic Violence: - SPH co-sponsored two community forums on addiction - Supported the development of a community-wide task force for Opioid crisis. Three hospital employees serve on the task force, and several more on sub-committees. - Continue to provide SART/SANE nursing services through the Emergency Department. - There is growing community opinion that inpatient treatment is needed locally, but gathering the EXACT data to prove this is challenging. A defined formula or needs assessment specific to "inpatient treatment" that would address both need and feasibility would be very helpful for rural communities. SPH, South Peninsula Behavioral Health and Public Health meet regularly to share data in the exploration of an inpatient treatment center. - There is a lack of sober living options in the community, mostly due to inadequate funding or reimbursement for this type of service. We continue to work with the opioid task force local officials to help change this, otherwise treatment is not effective when forced back into the same living environment. - Provided training for medical and nursing staff on the science of addiction to better reduce the stigma associated with addiction. - Ensure ER staff are trained with up to date information on available local resources. - Host the community needle-exchange program bi-weekly in the hospitals training center. - Serve as the lead agency for the At Risk Maternal and Newborn working group which is working to address addiction-related concerns in this population and improve related processes. - Offer a CDC approved pain-management program at Homer Medical Center with the goal of empowering patients to successfully manage their pain with alternate therapies to narcotics. - Sponsored numerous community trainings for responding to overdoses. - Distribute drug-disposal bags with narcotics prescriptions in clinic and ER setting. - Work with MAPP of the southern Kenai Peninsula to help fill identified gaps in the local pathway to recovery. Aging Population/Physical Health: - Offer specialty care in cardiology, urology, oncology, pulmonary, dementia, cholesterol, geriatric psychiatry and depression, circulatory, gastrointestinal, wound care, ophthalmology, podiatry, medication management, and other age-related health concerns. - Majority of community health and wellness offerings focus on chronic disease prevention and management; We offer a quarterly 6-session disease management class. - Sponsored a community-wide steps challenge to encourage physical activity. - We offer promotions on vital screenings or services including colonoscopy, PSA, cholesterol, vitamin D, thyroid, prostate, A1C, swallowing screening and heart health labs. - Our Diabetes ed program is now certified with the American Diabetes Association; Appointments are now offered onsite at specialty clinic and evenings at Homer Medical Center, in addition to regular outpatient visits and inpatient care. - Added neurology to our specialty clinic services. - Offered 1,000 reduced rate lab screenings for the Rotary Health Fair. - Offer cancer care with a part time oncologist and fully functional infusion clinic. - Addressing chronic illness was identified specifically as an FY 18 Strategic Action Item in support of the five year strategic plan. - Renovated the surgery department to modernize the operating rooms to better accommodate growing demand from an aging population and the procedures they require. - Expanded the primary care clinic to be able to grow provider capacity to better meet the demand of an aging population. - Prioritized Swing Bed resources to best meet the demand for this level of care. - Sponsored senior day at the county fair to encourage multi-generational socializing; subsidized nearly 300 seniors admission. - Had a booth at the county fair to promote Strong Women strength building practices for senior injury prevention and improved health. - Participated in the annual health fair, with numerous reduced rate and free screenings, including free flu shots, reduced rate blood tests, fitness tests, swallowing screenings and more. - Provided stroke awareness education at local Rotary club and on local radio. - Sponsored two visiting "Chefs at the market" at our local farmers market to promote nutrition and the importance of eating local. - Specialized care for Alzheimers and dementia, both outpatient and residential, is an unmet need; The sustainability of this specialized care in a small community isnt feasible. Subsidized facility development (or adaptation) and subsidized care would be necessary. Healthcare Professionals Recruitment and Retention: - Work with local providers to share recruiting strategies and costs, including advertising and job fair booth costs. - Support a nurse residence program. - A lack of licensed RNs and MDs continues to be a problem. Expedited licensing when recruiting from another state (or reciprocity) is necessary. Poor local economy / Lack of jobs: - Information on self-pay and prompt pay discounts, and financial assistance is readily ava
