Document verification reviews are held at least annually verifying client eligibility for Title III services. Additionally, grantees may also be asked to provide verification of other items such as: in-kind match and volunteer hours. Financial reviews include an in-depth review and evaluation of an organization's financial management activities including documentation to support its accounting entries and program reports.ExplanationTxt

Form 990, Part I, line 12:Line 12 total revenue from prior year (2016) to current year (2017) decrease of $550,195: - Less $516,928 grants & contributions - Less $32,731 in program service revenue - Innovations for Aging, LLC less program revenue of $42K - Other income was higher by $9.3K - Less $536 investment (interest) income - To properly collateralize deposits, moved savings to repurchase account in 2017 - Reference 2017 audit pages 7-8 and 24
Form 990, Part I, line 19:Line 19 revenue less expenses shows "positive" change in net assets in prior year (2016) and a "negative" change in net assets in current year (2017): - Revenue recognition impacts this figure. In 2016 and prior years, more revenue was received, but not expended. In 2017, more was expended than received. - 2016 revenue over expenditure = $218K - 2015 revenue over expenditure = $1.02M - Reference 2016 990 line 19
Form 990, Part I, line 22:Line 22 net assets or fund balances shows $974,972 less net assets in current year (2017) compared to prior year (2016): - Assets are about the same from 2016 to 2017 - However, current year (2017) liabilities increased by $883K due to unearned grant revenue which carries with it an "obligation" to pay it back - All other liabilities are comparable - Reference audit page 6
Form 990, Part VI, Section B, line 11bManagement staff and the executive committee review the draft Form 990 with discussion reflected in the meeting minutes. Board members receive a copy of the document one week prior to their regular meeting where they review the 990 and approve its submission.

The Executive Committee resolves any real or apparent conflict(s) and, in the absence of resolution, refer the matter to the full Board of Directors. During meetings, Board members are required to declare any real or apparent conflict of interest. To determine if a conflict of interest exists, the "interested person" discloses the material facts, and the Board, or the Committee there of, determines whether a conflict exists. Board members do not vote in any deliberation relating to policies, issues, or selection of vendors, and/or funding proposals in which a conflict of interest has been determined by the Board. Any proceedings related to conflicts of interest are documented in the meeting minutes, as appropriate. Key Employees: Employees receive a copy of the agency's Employee Handbook at the time of hire which delineates the conflict of interest policy.
