PART V, SECTION B, LINE 3JIDENTIFIED AND EVALUATED SOCIAL DETERMINANTS OF HEALTH.
Part V, Section B, Line 5The Community Health Needs Assessment Project team conducted a series of interviews with LHMC staff and a representative set of the leading community health stakeholders. These interviews allowed the project team to engage the community and capture qualitative data related to community health needs, community priorities, social determinants of health, service gaps, barriers to care, and the population groups most at-risk. These interviews were also critical to the development of LHMC's community health improvement plan (CHIP) as they provided important information related to LHMC's strategic response and explored possible partnerships. Overall, nearly 100 people were involved through our interviews, community forums and strategic planning sessions.
PART V, SECTION B, LINE 6ALAHEY CLINIC & MEDICAL CENTER, 41 MALL ROAD, BURLINGTON, MA 01805 LAHEY MEDICAL CENTER, ONE ESSEX CENTER DRIVE, PEABODY, MA 01960 WINCHESTER HOSPITAL BEVERLY HOSPITAL ADDISON GILBERT HOSPITAL
Part V, Section B, Line 7DThe results of the CHNA were made publically available in various ways including on the LHMC website, distribution to internal and external community stakeholders and presentations to hospital leaders and staff. In addition, report out sessions for key stakeholders in each municipality have been scheduled and will be complete by FY17. The goal of the sessions is to present the findings of the CHNA and to engage them in discussion to gain better understanding of their perceptions of the current health needs, social determinants of health, barriers to care in their communities and discuss potential opportunities for collaboration.
Part V, Section B, Line 11In FY16, Lahey Hospital & Medical Center, in conjunction with all four hospitals in the Lahey Health System, completed the required triennial Community Health Needs Assessment (CHNA). The purpose of the CHNA is to inform and guide the hospital's selection of and commitment to programs and initiatives that address the health needs of the communities it serves. The assessment was conducted in partnership with John Snow Inc. public health management consulting and research organization. LHMC Community Benefits Service Area LHMC serves individuals who come from throughout the United States and from nations around the world. With respect to community benefits, LHMC focuses its efforts more narrowly on the communities in its primary, local service area. More specifically, LHMC's community benefits investments are focused on expanding access, addressing barriers to care and improving the health status of residents living in the following 13, mostly contiguous, municipalities located in Middlesex and Essex counties: Arlington, Bedford, Billerica, Burlington, Lexington, Peabody, Reading, Stoneham, Tewksbury, Wakefield, Wilmington, Winchester and Woburn. LHMC also serves patients from Lowell and Haverhill due to long-standing program affiliations in these cities/towns, and as a result has collected health status information from these communities. However, because these communities are included in other hospitals' community benefits service areas, they have not been included in LHMC's Community Health Improvement Plan (CHIP). Methodology The CHNA was conducted in three phases, allowing LHMC to: -Compile an extensive amount of quantitative and qualitative data -Engage and involve key internal and external stakeholdersrs -Develop a report and detailed CHIP -Comply with all state and federal IRS community benefits requirements Data sources included a broad array of publicly available secondary data, key informant interviews, community forums and the 2015 LHMC Community Health Survey, which captured information from hundreds of random households in LHMC's primary service area. Quantitative Data Sources: -Massachusetts Community Health Information Profile (MassCHIP) -U.S. Census Bureau, American Community Survey 5-Year Estimates (2009-2013) -Behavioral Risk Factor Surveillance System (BRFSS) (2012-2013 aggregate) -CHIA Inpatient Discharges -Massachusetts Health Data Consortium (MHDC) Emergency Department (ED) Visits -Massachusetts Hospital Inpatient Discharges (2008-2012) -Massachusetts Cancer Registry (2007-2011) -Massachusetts Communicable Disease Program (2011-2013) -Massachusetts Hospital ED Discharges (2008-2012) -Massachusetts Vital Records (2008-2012) -Massachusetts Bureau of Substance Abuse Services (BSAS) (2013) -Massachusetts Board of Health Qualitative Data Sources: In order to obtain targeted data and understand the health issues currently perceived by the community, interviews and listening sessions were conducted: -Informant interviews with external stakeholders (28 completed) -Random household surveys (1,137 completed in the LHMC service area) -Community listening sessions (two sessions) Priority Target Populations LHMC focuses its activities to meet the needs of all segments of the population with respect to age, race, ethnicity, income, gender identity and sexual orientation to ensure that all residents have the opportunity to live healthy lives. However, based on the assessment's quantitative and qualitative findings, there was broad agreement that LHMC's CHIP should target low-income populations (low-income individuals/families, older adults on fixed incomes, and homeless), older adult populations (frail, isolated older adults), youths/adolescents (13-18-year-olds, those in middle school and high school) and other vulnerable populations (diverse racial/ethnic minority and linguistically isolated populations) that are more likely than other cohorts to face disparities in access and health outcomes. Community Health Priorities In response to the priority health needs identified in the 2016 CHNA, LHMC developed a three year Community Health Improvement Plan (CHIP). The CHIP was developed with input from the LHMC Community Benefits Advisory Board and approved by the LHMC board of directors. In addition, feedback from community report out sessions was taken into consideration when developing the plan. The community benefits programs offered in FY16 were designed to address the priority health needs identified in the 2016 CHNA, along with the Massachusetts Statewide Health Priorities identified by the Executive Office of Health and Human Services. Priority Community Health Needs Identified in the 2016 CHNA: -Behavioral Health/Substance Abuse -Elder Health Social Isolation, Depression, Care Management -Wellness, Prevention & Chronic Disease Management Statewide Priority Needs: -Address Unmet Health Needs of the Uninsured -Chronic Disease Management in Disadvantaged Populations -Promoting Weliness of Vulnerable Populations -Reducing Health Disparities -Supporting Health Care Reform FY16 Key Community Benefit Programs & Outcomes In collaboration with various community partners, listed below are some of the key programs LHMC provided in FY16 in response to the needs identified in the CHNA: -LHMC partnered with Mill City Grows, an organization that fosters food justice by improving physical health, economic independence and environmental sustainability in Lowell through increased access to land, locally grown food and education, to offer two five-class Farm to Table Workshops in fall 2015 and spring 2016. Over the course of the series, 19 families attended the cooking classes and took home an average of six pounds of produce per week so they could re-create the healthy recipes at home. -LHMC continued our successful partnership with Minuteman Senior Services to provide a Serving Health Information Needs of Everyone (SHINE) counselor at the Burlington, Arlington and Winchester Councils on Aging, who provided insurance counseling to 747 people, increasing access to care. -LHMC successfully grew our Senior Farmers Market partnership with World PEAS CSA to enable 50 seniors in Burlington and 70 seniors in Arlington to receive free fresh fruits and vegetables once a week for 20 weeks. -LHMC successfully partnered with the North Suburban YMCA and the Burlington and Arlington Council on Aging to offer a free low-impact aerobics class for 24 seniors once a week. -LHMC allocated $40,000 in mini-grant funding to local community health, social service and municipal partners to address health disparities identified in the LHMC Community Health Needs Assessment. Programs included: --Wilmington Public Schools Mental Health First Aid Training --REACH Against Domestic Violence counseling program --Burbank YMCA PINK Program --Minuteman Senior Services Chronic Disease Self-Management Programs --Domestic Violence Services Network (DVSN) RESIST training for Bedford High School and Middlesex Community College students -LHMC partnered with DVSN to provide 75 female students at Bedford High School, Middlesex Community College and Lexington High School and young female airmen at Hanscom Air Force Base in Bedford with RESIST (Realistic Escape Strategies & Instinct-based Self-defense Training). This succinct, well-designed program provides women and girls with practical, concrete actions they can take to recognize abusive behavior, resist and defend against it, and develop a stronger sense of self-regard and self-reliance. -LHMC partnered with Wilmington Public School District to build the capacity of Wilmington school and municipal staff to educate staff, students, parents and community members on how to recognize warning signs and risk factors for mental health disorders, feel comfortable offering help to someone experiencing a mental health crisis, and connect them to places that can provide emergency assistance through the highly successful, evidence-based Mental Health First Aid program. -LHMC colleagues implemented 11 eight-week A Matter of Balance sessions for the Billerica and Woburn Councils on Aging. This program was also offered at Lahey Medical Center, Peabody, and Lahey Hospital & Medical Center, Burlington. -LHMC provided over 100 hours of clinic time at our free weekly blood pressure clinics at the Burlington Mall.
