DescMedical Education: The Winifred Masterson Burke Rehabilitation Hospital has been a longstanding leader in clinical care, research and innovation and is now home to a nationally recognized Physical Medicine and Rehabilitation residency program. The Accreditation Council for Graduate Medical Education (ACGME)-approved program envisions being the future benchmark for all Physical Medicine and Rehabilitation residency programs in the country. Burke's goal is to produce healthcare leaders known for innovation in all branches of physiatry. This will be accomplished by rigorous training and developing highly skilled clinical and academic physiatrists. Our residents will receive a unique training experience and work directly with neurologists, physiatrists, rheumatologists, internists, cardiologists and pulmonologists on Burkes rehabilitation units. They will also have an opportunity to work closely with research scientists from the world-renowned Burke Medical Research Institute, which is located on Burkes 61-acre campus in White Plains. Burke aims to teach residents who will go on to utilize their training to improve patient care, advance the field of Physical Medicine and Rehabilitation and enhance healthcare in the communities they serve. We offer a 3-year Physical Medicine and Rehabilitation residency program, which is affiliated with Albert Einstein College of Medicine (AECOM). We also offer a one-year neurorehabilitation fellowship under the AECOM Department of Neurology. In the future, we plan to expand the program by including fellowships in traumatic brain injury, sports concussion and spinal cord injury.Desc

Schedule H, Part V, Section B, Line 5The process for preparing the 2016-2018 community health needs assessment was an inter-organization and community collaborative process, initiated with the goal of developing an assessment that was reflective of the needs of the community. The process to identify the needs of the community involved the collection of both primary and secondary data. Multiple conversations and meetings were convened internally and with external partners and a thorough review of the data was conducted to develop the implementation strategies. The collection of data from a sample of the Westchester county residents was an important element of the development of the Community Health Needs Assessment. Burke engaged with the Westchester County Department of Health's online community Resident and Provider Health surveys to assist in the facilitation of these community level connections thereby alleviating additional surveying overload. An online survey was collaboratively developed and made available across Westchester County, distributed by the Westchester County of Health, the Westchester County Executive's Office and the local Delivery System Reform Incentive Payment Program (DSRIP) providers over the summer of 2016. Using data collected through these sources, the impact of the community's health by interventions could be measured and analyzed. The priority areas selected and each of the planned interventions focus on specific priority populations and address the ethnic and culture disparity defined in the indicators for the population served by Burke.
Schedule H, Part V, Section B, Line 7aThe direct website where the hospital facility's CHNA report is available is: https://www.burke.org/docs/Burke CHNA 2016_FINAL.pdf
Schedule H, Part V, Section B, Line 10aThe hospital facility's most recently adopted implementation strategy is posted on the website: https://www.burke.org/docs/Burke CHNA 2016_FINAL.pdf
Schedule H, Part V, Section B, Line 11A review of the results from the primary and secondary data collection in the hospital's most recent conducted CHNA illuminated two major categories of health needs that were important across the population surveyed, reflected in the data as critical and in alignment with the New York State Prevention agenda. The two priority areas identified were (1) Preventing chronic disease and (2) Promoting mental Health and preventing substance abuse. Within the prevent chronic disease priority area, the two focus areas selected were (1) reduce illness, disability and death related to tobacco use and secondhand smoke exposure and (2) increase access to high quality chronic disease preventive care and management in both clinical and community settings. One of the Priority areas, Prevent Chronic disease was re-selected in 2016. This was done as it aligns well with both Burke's patient population and creates a better alignment with the initiatives of the New York State Department of Health's Delivery System reform incentive payment (DSRIP) Program. DSRIP has a very strong focus on both the prevention and management of chronic disease and behavioral health issues that include substance abuse. The second priority area, Promote mental health and prevent substance abuse, is a deviation from the area focused on in the previous plan. As a hospital dedicated solely to adult rehabilitation, Burke's programs and services often differ from those offered at an acute care/community hospital. Since Burke does not have a labor and delivery/post-partum unit, it was difficult to implement the breastfeeding objective set forth in the promote healthy women, infants and children priority area, though we did our best in creating objectives that we were able to meet. Therefore, Burke has elected to change the second priority area to promote mental health and prevent substance abuse with a focus on increasing the use of evidence-based policies and evidence-based programs which are grounded on healthy development of children, youth and adults. Burke has a longstanding commitment to the community and has developed innovative programs and services throughout the years to better meet the evolving needs of residents. These programs and services address a majority of the significant community health needs identified in the Community Health Needs Assessment. The breadth and depth of the programs and services vary, but each addresses a need indentified in the community. Some of the programs that align with the needs outlined in this plan include the Memory Evaluation and Treatment Service, an outpatient program that evaluates memory problems and works directly with caregivers and family members to ensure they can meet the needs of the diagnosis; an Arthritis Center, a comprehensive, multi-disciplinary medical and therapeutic program for patients with various types of arthritis and a Cardiac Rehabilitation program, a multidisciplinary program to help patients with heart disease recover faster and regain full, productive lifestyles through exercise, education and counseling. Additionly, Burke works with various partner organizations to facilitate monthly support groups on campus, such as the Hudson Valley Chapter of the Alzheimers Association, the NYC-SNY Chapter of the National Multiple Sclerosis Society and the Brain Injury Association of New York State. Burke will be engaging additional partner organizations to both continue to expand support group offerings, such as a tobacco cessation group, and to create additional community education events. Burkes inpatient and outpatient services work in collaboration to best meet the needs of its patient population, and the community at large, to ensure that needs are being met and that Burke is offering the best care possible. Community education programs, and events such as Burkes annual Wheelchair Games and Heels & Wheels 5K, are created and presented as a way for patients and the community to learn more about what is offered at Burke and how the work being done at the hospital impacts their lives. Burke will build upon these and offer additional events, such as COPD Day and Stroke Awareness Day in the coming years. Furthermore, as a member of the Montefiore Health System since 2016, Burke patients also have access to the vast number of programs and services available through the System and are encouraged to take advantage of them whenever needed.

1. Part I, Line 7THE COST-TO-CHARGE RATIO METHODOLOGY WAS UTILIZED TO CALCULATE THE AMOUNT INCLUDED IN THE TABLE. THE CALCULATION OF THE RATIO WAS DERIVED FROM RATIO OF PATIENT CARE COST-TO-CHARGEs.
1. Part III, Section A, Line 2The cost of bad debt expense is estimated based on the bad debt provision at charge, applied to the ratio of total patient care expenses to total charges for all services rendered. Any payments or discounts are excluded from bad debt expenses.
1. Part III, Section A, Line 3The estimated amount of the organization's bad debt expense (at cost) attributed to patients under the organization's charity care policy was based on the results of predictive analysis. Bad debt should be included as a community benefit because the organization provides much needed health care service indiscriminately to the community-at-large without regard to whether or not the patient has insurance or if the bill will ever be paid. Part III, Section A, Line 4 AS REPORTED IN MONTEFIORE HEALTH SYSTEM'S AUDITED CONSOLIDATED FINANCIAL STATEMENTS, BAD DEBT EXPENSE IS DESCRIBED AS FOLLOWS: "THE COLLECTION OF PATIENT SERVICE REVENUE DUE FROM PATIENTS, INCLUDING COPAYMENTS AND DEDUCTIBLES, FROM THOSE WHO ARE INELIGIBLE FOR CHARITY CARE, IS SUBJECT TO UNCERTAINTY. THE Health System RECORDS BAD DEBT EXPENSE IN THE PERIOD SERVICES ARE RENDERED BASED ON PAST EXPERIENCE, TO ACCOUNT FOR AMOUNTS THAT PATIENTS MAY ULTIMATELY BE UNABLE OR UNWILLING TO PAY. FOR SELF-PAY PATIENTS, WHICH INCLUDES BOTH PATIENTS WITHOUT INSURANCE AND PATIENTS WITH COPAYMENTS AND DEDUCTIBLES AFTER THIRD-PARTY COVERAGE, THE HOSPITAL RECORDS AN ESTIMATE FOR BAD DEBT EXPENSE IN THE CURRENT PERIOD BASED ON PAST EXPERIENCE. AMOUNTS ULTIMATELY WRITTEN OFF AS UNCOLLECTIBLE AND RECOVERIES OF SUCH AMOUNTS ARE DEDUCTED FROM, OR ADDED TO, THE ALLOWANCE FOR DOUBTFUL ACCOUNTS".
1. Part III, Section B, Line 8Medicare allowable costs reported on Part III, line 6, per Schedule H instructions, were derived using the Medicare cost report cost-finding methodology, which apportions routine costs to medicare using days and ancillary costs to medicare using departmental ratios of costs to charges (RCCS) and does not include the non-allowable Medicare costs.
1. Part III, Section C, Line 9BFor patients who are known to qualify for financial assistance, all collection activity is halted and the patient is referred for financial aid. If an account is in collection and the patient requests financial aid or if the agency determines that the patient is eligible for financial assistance, the account is referred back to the hospital where the patient is provided assistance with completing an application for assistance.
2. Needs AssessmentThe organization assesses community needs by: A) Community Advisory Boards - The Health System works entensively with representatives of the communities to identify health care needs and determine the appropriate configuration of services. B) Community Service Committee - Montefiore Health System, the parent company, has a board committee that is focused on community services. It is responsible for overseeing the Health System's community services and community benefit activities to ensure they are formulated to facilitate the fulfillment of the organization's mission and meet the needs of the community. C) Partnership and Collaborations - The health system participates in a variety of organized partnerships and collaboratives, working with other providers, the New York Department of Health and Mental Hygiene, community-based organizations and members of the community in planning and developing initiatives aimed at improving the health of the communities served.
3. Patient Education of Eligibility for AssistanceAll intake, registration and collection agency staff are trained on the Hospital's financial aid policy and how to provide patients with assistance. The organization has a policy that establishes guildlines for the billing office to follow when working with individuals who are having difficulty paying their medical bills. A referral is made to the financial aid office and a financial counselor will help the patient apply for free or low-cost insurance. If the financial counselor determines that the patient does not qualify for low-cost insurance, the counselor will help the patient apply for a financial aid discount based on income level. The organization makes its financial aid policy known to the public by providing written information including brochures at the admission area, information posted on the intranet and internet, and information sent out on patient's bills.
4. Community InformationBurke Rehabilitation Hospital has identified Westchester County as its primary service area. Westchester County is a large county located just north of New York City in the Hudson Valley. Westchester covers an area of 450 square miles and consists of 48 municipalities. The county includes urban, suburban and rural geographies. The estimated population of Westchester County is made up of more than 976,000 residents. The county seat of Westchester is the city of White Plains. Other major cities include Yonkers, New Rochelle and Mount Vernon. The median household income for Westchester was $85,688, the fourth highest in New York State, after Nassau, Putnam and Suffolk Counties. Westchester County is the fifth healthiest county in New York State, according to the County Health Rankings, produced by the Robert Wood Foundation and the University of Wisconsin. Despite its overall high ranking, there is considerable room to both improve population health and reduce health disparities in Westchester County.
5. Promotion of Community HealthBurke Rehabilitation has a longstanding commitment to the community and is at the forefront of rehabilitation medicine. Burke's continuum of care extends beyond its inpatient and outpatient programs and impacts people in the surrounding communities and throughout Westchester County. Burke regularly holds community education programs and events for the community, with a focus both on the illnesses and injuries treated at Burke and the health and wellness of its community members. The Burke Fitness Center, which is open to the community and serves adults over age 40 and those with a qualifying disability, is a place where people from throughout the area can focus on their health is a setting in which they feel safe and comfortable. Burke's commitment to the community extends to our employees and the medical community as well.
