Schedule H, PART V, Section B, LINE 5In 2014, a community needs assessment titled "One Region, One CNA" was conducted by Montefiore Health System, of which Montefiore Nyack Hospital is affiliated, in collaboration with other participating provider systems in the Hudson Valley region, as part of the Delivery System Reform Incentive Payment Program (DSRIP). Rockland County Department of Health (RCDOH) prepared a Community Health Needs Assessment and Community Health Improvement Plan 2014-2017. RCDOH reviewed the health status of the county, current health-related programs addressing the identified needs of their residents, and defined focus areas that align with the New York State Prevention Agenda. During 2016, Montefiore Nyack Hospital worked closely with the local Health Department in identifying the Prevention Agenda Priorities. Montefiore Nyack Hospital is an active member of the Rockland County Healthcare Priorities Committee, which represents 22 community organizations throughout the county. This group meets monthly with its primary objective being to define health care priorities for Rockland County that align with New York State Prevention Agenda Priorites. The Patient and Community Education Department at the hospital also used feedback from its lecture series, website and telephone contacts to identify health concerns of the public. Diabetes, pre-diabetes, cancer, obesity, maternal and infant care, and heart disease was found to be the public main concerns.
Schedule H, Part V, Section B, Line 7aThe Hospital's CHNA report in available at the direct website: https://www.montefiorenyack.org/sites/default/files/2016-2018_CommunitySer vicePlan.pdf vicePlan.pdf
Schedule H, Part V, Section B, Line 10AThe hospital's most recently adopted implementation strategy is posted on the website: https://www.montefiorenyack.org/sites/default/files/2016-2018_CommunitySer vicePlan.pdf
Schedule H, Part V, Section B, Line 11The New York State prevention agenda topics identified by the most recent CHNA are being addressed by Montefiore Nyack Hospital. The two priority areas identified that align with the Rockland County Department of Health, New York State Prevention Agenda and the Montefiore's Hudson Valley Collaborative DSRIP were to Prevent Chronic Disease and Promote Healthy Women, Infants and Children. Preventing Chronic disease is one priority area with the focus of increase access to high quality chronic disease preventive care and management in both clinical and community settings. Montefiore Nyack Hospital conducts several programs that address this priority to reduce cardiovascular disease by reducing obesity and preventing diabetes. Our Cancer Center is targeting colorectal cancer screening for the high risk population of Latino men, 50 to 75 years in age. The New York State Prevention Agenda Dashboard reflects Rockland's County's overall colorectal cancer screening rate at 71.4% based on NYS Expanded Behavioral Risk Factor Surveillance System. However, the Village of Haverstraw, West Haverstraw, and Spring Valley have been identified as areas where according to published data, the screening rate may be as low as 57% for these areas. New York State Prevention Agenda 2016-2018 goal is to increase the percentage of adults 50 to 75 years who receive a colorectal cancer screening based on the most recent guidelines by 5% or 71%. This is aligned with Healthy People 2020 target of 70.6%. Montefiore Nyack Hospital has joined the American Cancer Society's initiative "80% by 2018". Montefiore Nyack Hospital is joining forces with the Cancer Services Program in setting up this screening program. Under the second priority area Promoting healthy women, infants and children, The New York State's Prevention Agenda Dashboard reflects Rockland County's percentage of infants exclusively breastfed in the hospital at 45.4%. In January of 2016, the percentage of women exclusively breastfeeding in the hospital was 14%. Best practices employed by Nyack includes implementing "Ten Steps to Successful Breastfeeding" a WHO/UNICEF initiative with a specific focus on Skin to skin practice and promoting rooming-in exclusivity in the overnight hours. Scripting suggestions were provided to staff for patient education to handle requests for routine formula supplementation to dissuade this practice. Policies were reviewed to clarify RN role in promoting exclusive breastfeeding while hospitalized and to continue for at least the first six months of life. Within 10 months, the rate for exclusively breastfeeding while hospitalized had risen to 28%. The breast center at Montefiore Nyack Hospital continues to offer comprehensive services to women of Rockland County. The Breast Center promotes self-breast and bone-health awareness for both women and men and provides screening for breast and cervical cancer for uninsured and underinsured women and men. Schedule H, Part V, Section B, Line 13H Family size is factored into the eligibility criteria for financial assistance.
Schedule H, Part V, Section B, Lines 16a - 16cThe hospital's Financial aid policy, application and summary is available on the website: https://www.montefiorenyack.org/financial-aid-policy
Schedule H, Part V, Section B, Line 22The hospital determines the maximum amount that can be charged to FAP-eligible individuals for emergency or other medically necessary care based on the type of service rendered. THE MEDICAID DRG RATE is used for admissions while the MEDICARE APC RATE is used for ambulatory services.

1. Part I, Line 7The cost-to charge ratio methodology was utilized to calculate the amount included in the table. The calculation of this ratio was derived from ratio of patient care cost-to-charge.
1. Part III, Lines 2The cost of bad debt expense is estimated based on the bad debt provision at charge, applied to the ratio of total patient care expense to total charges for all services rendered. Any payments or discounts are excluded from bad debt expense. 1. Part III, Lines 4 As Reported in Montefiore Nyack Hospital and Subsidiaries audited financial statements: Bad debt expense is described as follows: "Patient accounts receivable result from the health care services provided by the Hospital. Additions to the allowance for doubtful accounts result from the provision for bad debts. Accounts written off as uncollectible are deducted from the allowance for doubtful accounts. The amount of the allowance for doubtful accounts is based upon managements assessment of historical and expected net collections, business and economic conditions, trends in Medicare and Medicaid health care coverage and other collection indicators". "For receivables associated with self-pay patients which include both patients without insurance and patients with deductible and copayment balances due for which third-party coverage exists for part of the bill, the Hospital records a significant provision for bad debts in the period of service on the basis of its past experience, which indicates that many patients are unable or unwilling to pay the portion of their bill for which they are financially responsible. The difference between discounted rates and the amounts actually collected after all reasonable collection efforts have been exhausted is charged off against the allowance for doubtful accounts".
1. Part III, LIne 8THE AMOUNT REPORTED ON LINE 6 WAS DERIVED USING THE MEDICARE ALLOWABLE COSTS AS REPORTED ON THE HOSPITAL'S NEW YORK STATE INSTITUTIONAL COST REPORT. The Medicare Shortfall should be considered a community benefit, since the organization is rendering much needed health services to a population, for the most part, that is poor with limited means. Over time, The medicare reimbursement has not kept pace with rising medicare costs thus shifting the burden for the difference to the organization for services rendered mainly to a frail, elderly population that has great health needs.
1. Part III, Section C, Line 9BFor patients who are known to qualify for financial assistance, all collection activity is halted and the patient is referred for financial aid. If an account is in collection and the patient requests financial aid or if the agency determines that the patient is eligible for financial assistance, the account is referred back to the medical center where the patient is provided assistance with completing an application for assistance.
2. Needs AssessmentMontefiore Nyack Hospital assesses community needs by working closely with the local health department as well as community partners. Montefiore Nyack Hospital is an active member of the Rockland County Healthcare Priorities Committee, which represents 22 community organization throughout the county. Montefiore Nyack Hospital has a policy that establishes guidelines for the billing office to follow when working with individuals who are having difficulty paying their medical bills. If a referral is made to the hospital's Financial aid office, the patient will be assisted in completing an application for financial assistance. The hospital makes it financial aid policy known to the public by providing all patients information regarding the hospital's charity care policy.
