ExplanationHSHS and the Foundation are Illinois not-for-profit organizations as described in Section 501(c) (3) of the Internal Revenue Code (the Code) and are exempt from federal income taxes on related income pursuant to Section 501(a) of the Code. Kiara, Inc. is an Illinois for-profit corporation that recognizes income taxes under the asset-and-liability method. Deferred tax assets and liabilities are recognized for the future tax consequences attributable to differences between the consolidated financial statement carrying amounts of existing assets and liabilities and their respective tax basis and operating loss and tax credit carryforwards. Deferred tax assets and liabilities are measured using the enacted tax rates expected to apply to taxable income in the years in which those temporary differences are expected to be recovered or settled. The effect on deferred tax assets and liabilities of a change in tax rates is recognized in income in the period that includes the enactment date. In assessing the realizability of deferred tax assets, management considers whether it is more likely than not that some portion or all of the deferred tax assets will not be realized. The ultimate realization of deferred tax assets is dependent upon the generation of future taxable income during the periods in which those temporary differences become deductible. Management considers projected future taxable income and tax planning strategies in making this assessment. Based upon the level of historical taxable losses and projections for future taxable losses over the periods for which the deferred tax assets are deductible, management believes it is more likely than not Kiara, Inc. will not realize the majority of the benefits of these deductible differences. Full valuation allowances have been applied against the deferred tax assets attributable to the net operating loss carryforwards not realized as of June 30, 2018 and 2017 in the accompanying consolidated financial statements due to the uncertainty of realization. HSHS recognizes the tax benefit from an uncertain tax position only if it is more likely than not the tax position will be sustained on examination by the taxing authorities, based on the technical merits of the position. As of June 30, 2018 and 2017, HSHS does not have any liabilities for unrecognized tax benefits.ExplanationTxt

Schedule H, Part V, Section B, Line 3EYes, the significant health needs are a prioritized description of the significant health needs of the community and identified through the CHNA process.
Schedule H, Part V, Section B, Line 5 Facility , 1Facility , 1 - HSHS St. Vincent Hospital. In FY2018 (July 1, 2017 through June 30, 2018), HSHS St. Vincent Hospital conducted its CHNA in partnership with representatives from the community. Upon completion of the CHNA, the hospital developed a set of implementation strategies and adopted an implementation plan to address priority community health needs. The population assessed was Brown County, Wisconsin. Data collected throughout the assessment process was supplemented with qualitative data gathered through a CHNA steering committee with broad community representation, secondary data report, and the opinions of community key stakeholders obtained via a "Needs Assessment Summit" held in October 2017 at Lambeau Field in Green Bay. Identification and prioritization of needs: The following health needs were identified based on burden, scope, severity and urgency of the health need; the health disparities associated with the health need; the importance the community places on addressing the health need; the community assets and resources that could be leveraged through strategic collaboration in the hospital's service area to address the health need; the secondary data sources; and local expertise and input. - Alcohol and other drug abuse (AODA) - Mental health - Physical activity, obesity and nutrition Oral health was identified as a priority community health need in Brown County in the FY2015 CHNA process. A consensus was reached that ongoing oral health efforts should be sustained. Process and methods used to conduct the assessment HSHS St. Vincent Hospital collaborated with member health systems that make up Beyond Health (Brown County Public Health, City of De Pere Public Health, Aurora BayCare, Bellin Health, HSHS St. Mary's Hospital Medical Center, and Brown County United Way) to plan, implement and complete the Brown County community health needs assessment. Beyond Health was formed to improve the health of Brown County residents by conducting periodic community health needs assessments and leading community-wide action planning teams. Internal HSHS St. Vincent Hospital spent more than a year developing the CHNA, identifying and prioritizing community health needs for its service area and formulating an implementation plan to guide ongoing population health initiatives with like-missioned partners and collaborators. These planning and development activities included the following internal and external steps: - Convened the HSHS Eastern Wisconsin Division community benefit team - Determined internal capacity for the collaborative process - Allocated monetary resources to support the community-wide effort - Presented to senior leadership to gain permission for colleagues to track and document community benefit efforts in CBISA - Shared the results of the CHNA and obtain feedback for the implementation plan from the Hospital Advisory Committee - Presented CHNA and implementation plan to HSHS St. Vincent Hospital Board for approval. External HSHS St. Vincent Hospital also leveraged existing relationships to gain additional insights into the community health needs in the hospital's service area. External components and steps included: - Identified the methodology to be used for this update - Utilized the Green Bay Press-Gazette to conduct a community survey - Collaborated with key stakeholders/Beyond Health in the planning of the summit - Analyzed data from 2017 Needs Assessment Summit hosted at Lambeau Field and the minimal responses that were received from the Press-Gazette survey - Worked with the CHNA committee to determine priorities - Worked with CHNA committee and subject matter experts to develop action plans for the community health improvement plan - Worked with CHNA committee to review community health improvement plan. Defining the purpose and scope The purpose of the CHNA was to 1) evaluate current health needs of the hospital's service area, 2) identify resources and assets available to support initiatives to address the health priorities identified, 3) develop an implementation plan to organize and coordinate collaborative efforts impacting the identified health priorities, and 4) establish a system to track, report and evaluate efforts that will impact identified population health issues on an ongoing basis. Data sources The CHNA process utilizes both primary and secondary data. Some members of the various task forces/action teams in Brown County were enlisted to compile a secondary data report to supplement the primary data. This report summarizes the demographic and health-related information for Brown County. - County Health Rankings and Roadmaps - American Community Survey - Census Bureau - Wisconsin Department of Health Services - Data & Statistics - Wisconsin Interactive Statistics on Health (WISH) - Life Study Brown County - Brown County Health and Human Services - Live 54218 - Bureau of Labor Statistics The data was gathered into a written report/presentation and shared with community members at key community leader meeting which is described below. Input from persons who represent the broad interests of the community HSHS St. Vincent Hospital is committed to addressing community health needs in collaboration with local organizations and other area health care institutions. In response to the FY2015 CHNA, the hospital planned, implemented and evaluated implementation strategies to address the top three identified community health needs: mental health, alcohol and other drug abuse (AODA), physical activity and nutrition, in addition to oral health which had been continued from the FY2012 CHNA. The FY2018 assessment expanded on the FY2015 collaboration, and actively sought input from a broad cross-section of community stakeholders. The goal was to reach a consensus on priorities for which to focus human, material and financial resources. CHNA needs assessment summit Beyond Health steering committee brought together community stakeholders to participate in the CHNA process. The community health assessment data report was emailed to community stakeholders prior to the summit on October 17, 2017. The presentation included links to other downloadable reports allowing participants to review data before the meeting. One hundred twenty-three individuals attended the summit held at Lambeau Field. Input from members of medically underserved, low-income and minority populations Hospital Sisters Health System and HSHS St. Vincent Hospital are committed to promoting and defending human dignity, caring for persons living in poverty and other vulnerable persons, promoting the common good and stewarding resources. We believe that the CHNA process must be informed by input from the poor and vulnerable populations we seek to serve. The CHNA process included representatives of organizations who work directly with their constituents, have extensive knowledge and quantifiable data regarding the needs of their service populations. Actively including these organizations in the assessment process was critical to ensure that the needs of the most vulnerable persons in our communities were being shared and addressed in the CHNA process and development of related implementation strategies. * Aging and Disability Resource Center * Brown County - Community Treatment Center * Brown County Board of Health * Brown County Child and Adolescent Behavioral Health * Brown County Health & Human Services * Brown County Public Health * City of De Pere - Health Department * Diocese of Green Bay Catholic Charities * Family & Childcare Resources of North East Wisconsin * HSHS Libertas Treatment Center * HSHS St Vincent Hospital/ St Mary's Hospital * Brown County Mental Health Task Force - Connections for Mental Wellness * NEW Community Clinic * Northeastern Wisconsin Technical College * Oconto County Public Health * Oneida Nation * Oral Health Partnership * Prevea Health * United Way - Brown County * WI DHS Division of Public Health * Wisconsin Infants & Children

Schedule H, Part V, Section B, Line 11 Facility , 1Facility , 1 - HSHS St. Vincent Hospital. The organization's most recently conducted CHNA was completed for the fiscal year ended 6/30/18. The related implementation strategy that was adopted is for the period July 1, 2018 through June 30, 2021. The information below pertains to the 3rd year cycle of the implementation strategy from the CHNA conducted in the fiscal year 2015. In FY2015, HSHS St. Vincent Hospital conducted a Community Health Needs Assessment (CHNA). Primary and secondary data was gathered from multiple sources to assess the hospital's primary service area. Based on the data and the prioritization process, the following priority community health needs were selected: Oral Health Mental Health Alcohol and Drug Misuse Adequate, appropriate, and safe nutrition The FY2015 Implementation Plan outlined the strategies that the hospital would undertake to address the priority community health needs identified through the CHNA process. Evaluation of the impact of the actions that were taken in response to the hospital's FY2015 CHNA follows. Oral Health: The hospital continued support of the development, implementation and/or continuation of strategies to improve oral health. Outcomes: In late 2011, oral health was identified as a top community health priority in Brown County: 32% of adults had tooth loss due to decay or gum disease; 22% of adults did not regularly see a dentist; and only 20% of Medicaid members received dental care. Because of Oral Health Coalition action over the last eight years, oral health is no longer a top community health priority. Systems and resources are now in place in Brown County to better meet oral health needs. During the 2017 Community Health Needs Assessment, it was voted to move oral health to sustainability mode. The Brown County Oral Health Coalition will continue its work to improve oral health and assure access to oral health services with support from community partners. In 2012, the Oral Health Action Group developed and carried out a CHIP with objectives to: * Implement at least three evidence-based tooth decay prevention programs. * Increase access to oral health care for Medicaid members and self-pay patients. * Reduce oral health-related emergency room visits. Since 2012, the Oral Health Action Group, today known as the Brown County Oral Health Coalition has: * Participated in the WI Medicaid Pilot Project to increase Medicaid reimbursement: hired three dentists and reduced appointment wait times two weeks. * Received grant funding from the Midwest Collaborative Initiative to implement oral health programs. * Implemented Healthy Smiles for Mom and Baby to integrate oral health into prenatal and pediatric health care systems. Mental Health and Drug and Alcohol Misuse: The hospital continued support of the development, implementation and/or continuation of strategies to improve mental health. Outcomes: The hospital actively participated in the development of the Connections for Mental Wellness, which is a collaboration of agencies and organizations designed to reform the mental health delivery system in our community. The hospital led the strategic planning process, wrote portions of a grant application that resulted in a major seven-year grant from Medical College of Wisconsin, and hospital colleagues served on the steering committee. Substance Use: The hospital continued support of the development, implementation and/or continuation of strategies to address Alcohol and Other Drug Abuse (AODA). Outcomes: The hospital partnered with Prevea Health to implement the Treatment Intervention Prevention Services (TIPS) program, a program for non-violent opioid abusers that provides treatment and deferred/dismissed prosecution upon successful completion of the treatment process. Actions included subsidizing scholarships for a residential treatment program and arranging to have the first dose of Vivitrol donated. The hospital continued to support the activities of the community action planning team, which has its own three-year plan with goals and outcome measures. The hospital continued to work with the Public Health Department and our community partners to identify and implement at least one new evidence-based community program that addresses one of the identified health priorities.
Schedule H, Part V, Section B, Line 11 Facility , 2Facility , 2 - HSHS St. Vincent Hospital. In October 2017, Libertas Treatment Center, an affiliate of HSHS St Vincent Hospital, unveiled a labyrinth on their grounds. The labyrinth is a walking path to be used for prayerful meditation. It was made possible due to the generous donations and work of the Greater Green Bay Chamber Leadership Green Bay Team 1 Class of 2017 and Fox Cities Landscape Contractors Association, Inc. Like the labyrinths in the great Christian churches of Chartres, Amiens, Reims and Calais, the purpose of this space is for the prayerful meditation on the person of Jesus Christ, who is Himself the way to the Father. This labyrinth is not a maze, a puzzle or mystery. It is, rather, a tool meant to draw us more deeply into the way of Christian discipleship, which requires patience and attention. Physical Activity and Nutrition: The hospital continued support of the development, implementation and/or continuation of strategies to improve physical activity and nutrition. Outcomes: Supported the "Double Your Bucks" program at farmers' markets in Green Bay, De Pere and Oneida to enable EBT recipients to buy double the amount of fresh fruits and vegetables. The purpose is to create a healthy, equitable food environment where all residents have access to fresh fruits and vegetables at Farmers' Markets. Live54218 along with HSHS St. Vincent Hospital works with local partners to develop the infrastructure necessary to accept FoodShare benefits through an EBT machine at local markets in the Greater Green Bay Area. Electronic Benefits Transfer (EBT) is an electronic system that allows FoodShare recipients authorization to transfer their government benefits from a Federal account to a retailer to pay for food. The Double Your Bucks (DYB) incentive program provided more than $12,500 of fresh fruits and vegetables to FoodShare families at local Farmers' Markets in 2016. We've also seen DYB redemption increase by 13% and repeat users increasing to 47%. Word-of-mouth continues to be the primary way individuals learn about EBT and DYB programs at the markets. This year, we were fortunate to hire a community member with lived FoodShare experience to share information about these programs during daily commutes utilizing public transportation, at local food pantries, and at the markets. This valuable opportunity provided new perspective on the process from the population most affected and contributed to increased EBT and DYB redemption Food as Medicine: Continue healthy food initiatives at food pantries and continue planting for purpose and build capacity with the local pantries in Brown County to have fresh food. Maintain promotion of healthy donations for food drives HSHS St. Vincent Hospital actively supported the work of Ben's Wish to help feed low-income students in Brown County by serving as a contributing sponsor to their annual fundraiser/dinner. In addition, Prevea Health released a cookbook in partnership with professional athlete, Randall Cobb, Prevea Gets Cooking with Cobb: Randall's Favorite Kid-Friendly Recipes. A portion of the proceeds from sales were donated to, and continue to be donated to, Ben's Wish. Every week, Ben's Wish supports more than 720 students in seven school districts. Ben's Wish supports local pantries, educates the community on hunger awareness and has developed the Weekend Backpack Program to assist local school children with food insecurity on weekends. The following needs were also identified in FY2018 and are not being addressed directly by the hospital for the reasons indicated. Environmental health: Being a provider of health care services through inpatient and outpatient services, the hospital does not have the expertise or resources to take the lead on addressing this priority in the region. However, in our role as an employer, HSHS St. Vincent Hospital is involved in many initiatives that contribute to helping our community members as their health is impacted due to environmental health (e.g. Double Your Bucks program helps community members receive healthy food from a local farmers market; recycling efforts lead to less waste and reduces our carbon footprint; Mission Outreach programs help recycle medical equipment and make it available to those who are in need, but do not have a means of obtaining such equipment). Wellness: This was one of the top priorities for Brown County but ranked below the top three that were chosen plus there are other organizations such as LIVE54218 (now WELLO) that are actively addressing wellness issues in the community that include year-round access to the popular Fox River Trail, Farm to School project (healthy food education and hands on experience in schools) and Everybody on the Bus!

Schedule H, Part VI, Line 5 Promotion of Community HealthHSHS St. Vincent Hospital provides a range of Community Benefit activities and programs that furthers our mission and long-standing commitment to our community. In many cases, these Community Benefit initiatives would not exist without the leadership role played by HSHS St. Vincent Hospital and they often relieve a burden that would otherwise be carried by government. Since HSHS St. Vincent Hospital and HSHS St Mary's Hospital are in the same market, only a few miles apart, they jointly conduct the CHNA along with community partners. They work together on their CHNA the implementation strategy. Indeed, most proactive community benefit projects are paid for 2/3-1/3 by St. Vincent and St. Mary's. Here are examples of strategies implemented during FY2018: - Cover the cost of a nurse employed by the NEW Community Clinic (FQHC) to assist the volunteer physicians who see patients with acute needs at their Bodart Street location. More than 70% of patients indicate the clinic's care prevented a trip to the emergency department: FY18 $97,732 - Supply NEW Clinic with free and discounted laboratory and radiology services: FY18 $100,000. - Provide funds to assist NEW Clinic to pay for specialized dental assistants to enable the oral surgeons to see as many patients as possible during their volunteer time: FY18 $68,900. - Offer the Sexual Assault Nurse Examiner (SANE) program to provide care and assistance for anyone who has been sexually assaulted; available around the clock to the victims of sexual assault, male or female, infant to elderly;, is the regional site for such care: FY18 $114,634 - Collaborate with Medical College of Wisconsin in offering a community medical education program in Green Bay, and have provided financial support to offset operating costs and staff to assist in training: FY18 $60,000 - Cover more than $20,451 in uncompensated ambulance and other vehicle rides for patients, either to or from the hospital or to help them get to their doctor appointments elsewhere. - Provide printing services to local churches, organizations and schools; - Offer free support programs including cancer survivor and grief care: FY18 $11,654 - Provide Eagle III transportation for critically ill and injured patients; provided staff and financial assistance to Eagle III for patients served who were unable to pay their bill: FY18 $41,596 - Provide hospital colleagues to participate in health fairs and events at area schools, technical colleges, and a wide variety of organizations: FY18 $24,179 - Offer student intern programs with local colleges and universities: FY18 $475,686 - Hospital colleagues serve on local health organization boards and donate their time and expertise during and after business hours: FY18 $4,765 - Provide laundry service for the local Altrusa House free of charge: FY18 $547 - Support Unity Hospice, a provider of appropriate and compassionate care for the terminally ill along with St. Mary's Hospital and Bellin Health

Schedule H, Part I, Line 7 Bad Debt Expense excluded from financial assistance calculation5878020
Schedule H, Part I, Line 7 Costing Methodology used to calculate financial assistanceCHARITY CARE IS CALCULATED BY DETERMINING THE TOTAL AMOUNT FROM PATIENT BILLING THAT IS WRITTEN OFF TO CHARITY CARE CHARGE CODES OR ALLOWANCES. THIS AMOUNT MATCHES THE NUMBER THAT IS REPORTED ON OUR FY2018 AUDITED FINANCIAL STATEMENTS. THE TOTAL CHARITY CARE DOLLAR AMOUNT IS THEN REDUCED TO COST BY APPLYING THE HOSPITAL'S COST TO CHARGE RATIO AS CALCULATED ON IRS SCHEDULE H - WORKSHEET 2.
Schedule H, Part II Community Building ActivitiesThe community building activities that the hospital tracked and reported in our community benefit report are a portion of our total community benefit efforts. They consist primarily of actively participating in and supporting the work of community groups to advance community awareness about health issues while supporting the areas of identified need in the community. Community building activities included attending the Wisconsin Hospital Association Advocacy Day where staff and other local healthcare organizations meet with our legislators to discuss healthcare legislation. HSHS St. Vincent Hospital leaders are asked to either serve on or are be involved with community boards or organizations. This connection with the community provides a great opportunity to not only experience and understand the needs in the community but to provide feedback on what opportunities exist especially for the poor and vulnerable. Examples of these community connections include Community Partnership for Children, Oral Health Task Force, NEW Community Clinic, St Johns Homeless Shelter, Habitat for Humanity, and Unity Hospice. Donations were made to Oconto, Door and Kewaunee counties to help with their economic development.
Schedule H, Part III, Line 2 Bad debt expense - methodology used to estimate amountSelf-pay patients are screened for verification of no insurance coverage. Once verification of no coverage has been completed, the guarantors will be screened for charity or uninsured discounts based on income levels. Those guarantors who do not qualify for charity through the screening process will be asked to provide proof of income in the form of a recent pay stub and/or recent tax return to determine the level of uninsured discounting. If no proof of income is provided or a guarantor's income is greater than 201% of the federal poverty level, the appropriate discount according to the established criteria up to 400% of the federal poverty level will be applied. The amount of money not collected from those self-pay patients who are not eligible for charity care is reported as bad debt. Allowances for Doubtful Accounts have been recorded based on an historical matrix, using collectability percentages by payor and aging category in accordance with HSHS Executive Policy #F-23, Valuation of Net Accounts Receivable. Percentages are based on historical experience at the hospital and adjusted for known limitations. The Hospital reports bad debt in accordance with Generally Accepted Accounting Principles (GAAP). WHILE THE HOSPITAL FEELS THEIR BAD DEBT EXPENSE INCLUDES AMOUNTS ATTRIBUTABLE TO PATIENTS WHO WOULD QUALIFY FOR FINANCIAL ASSISTANCE UNDER THE HOSPITAL'S FINANCIAL ASSISTANCE POLICY, THE HOSPITAL IS UNABLE TO ESTIMATE THIS AMOUNT.
Schedule H, Part III, Line 3 Bad Debt Expense MethodologyHSHS St. Vincent Hospital strongly believes that its charity care, and the related community benefit obtained from such care, is understated because of those patients who potentially qualify for charity care but do not wish to apply for it. In addition, some care is not classified as charity because of missing documentation on patient resources. Thus, the hospital's bad debt includes a portion that could be classified as charity care if application for such care was sought and/or completed.
Schedule H, Part III, Line 4 Bad debt expense - financial statement footnoteThe text of the footnote to the organization's financial statements that describes bad debt expense can be found on page 20 of the HSHS Consolidated Audit Report
Schedule H, Part III, Line 8 Community benefit & methodology for determining medicare costsHSHS St. Vincent Hospital continually strives to provide excellent patient care in the most cost effective fashion. Nonetheless, the Medicare program, in many cases, does not provide payment that covers the full cost of the care provided. Since it is the mission of the hospital to respond to community need, hospital management continually advocates for improved Medicare payment so that the cost of quality care to those patients who are not able to afford it is not compromised and is fairly subsidized by all payers. While this shortfall in Medicare payments is not classified as community benefit by the IRS, we nonetheless believe it is an important contribution made by the hospital to the health and well-being of the community. If the Medicare program did not exist, many Medicare patients would be eligible for charity care or other means-tested government programs. Further, by absorbing this payment shortfall and providing care below cost to these individuals, St. Vincent Hospital is relieving the burdens of the government. Accordingly, this shortfall restricts St. Vincent Hospital's ability to make funds available to provide for charity care and other community benefits. The hospital Medicare shortfall at cost for fiscal year 2018 was $82,655,696.
Schedule H, Part III, Line 9b Collection practices for patients eligible for financial assistanceEvery effort is made prior to, during, and after provision of medical services to determine whether a patient is eligible for charity/community care and to assist the patient in completing the application and providing adequate documentation. If the patient qualifies for charity/community care for the full balance of their account, the entire amount is written off to charity/community care and hence no debt collection is pursued. If the patient qualifies for charity/community care for a portion of their account balance, that portion is written off to charity/community care, with the patient being responsible for the remainder of the balance. Additionally, reasonable effort will be made to obtain third-party or government payer reimbursement on behalf of the patient. If those efforts are not fruitful, an offer will be extended to the patient to make installment payments on their balance. Only at such point that the patient defaults on installment payments or refuses to cooperate with the hospital's efforts to be reimbursed will the account be sent to collections. The hospital will make reasonable efforts to determine whether a patient is eligible under the financial assistance policy before it engages in an ECA. To the extent a patient is determined to be eligible for financial assistance under the FAP, the hospital will take all reasonably available measures to reverse any ECA taken against the patient to obtain payment for the care.

Schedule H, Part VI, Line 2 Needs assessmentHSHS St. Vincent Hospital is continually assessing the health care needs of the community through several avenues. The Hospital Advisory Committee meets regularly to discuss hospital services and the ability of the hospital to meet community health care needs. Agendas are purposely organized to include time for feedback from members on what they are hearing in the community. Feedback is also solicited through patient surveys and the Patient Family Engagement Council. The hospital maintains a close relationship with our public health department who also is a good resource regarding the health of our community. HSHS St Vincent Hospital leaders serve on many local boards and organizations that have direct link or connection to serving Brown County. Through these roles, direct feedback is given on current needs in the community. The organizations include Catholic Charities, Habitat for Humanity, St John's Homeless Shelter, NEW Community Clinic and WELLO (wellness).
Schedule H, Part VI, Line 3 Patient education of eligibility for assistanceHSHS St Vincent Hospital posts the financial assistance brochure and application on the hospital website. There is also signage in these areas to high light the availability for assistance for patients that need it. In addition, financial assistance applications are available and prominently displayed in waiting and registration areas. Financial counselors are available to meet with patients on site before or at the time of service to assist them in identifying whether they are eligible for government aid or financial assistance, and to assist them in applying for programs for which they may be eligible. After patients receive service, our customer service representatives are also trained to assist them to apply for various government programs or for our financial assistance program. We also have contracted with an outside firm to do active outreach with patients who may need additional assistance or advocacy to apply for benefits, appeal denials, or who need transportation to government agencies to apply for benefits. One-on-one conversations to educate patients about financial assistance often stem from patients receiving a statement or a pre-collection letter. At that point, the customer service representative or cashier, or other qualified staff, will inform/educate either in person or over the phone about financial assistance options. The first pre-collection letter reminds the patient their account is in delinquent status. It also informs them they may be eligible for financial assistance through the organization's charity care program. The letter provides direct contact information to begin the application process for financial assistance and lists the internet address where the policy and an application can be obtained. A system called Search America is utilized after failed attempts made by the hospital and our law firm to make contact or work out payment arrangements. That system accesses credit and other information and allows us to see the probability of a patient being able to pay their bill. After assessing that information, the hospital makes a presumptive determination as to whether the patient meets charity guidelines to grant charity care or continue collection efforts.
Schedule H, Part VI, Line 4 Community informationBrown County, Wisconsin is a primarily urban county, one of 72 Wisconsin counties, located in the northeastern part of the state. HSHS St Vincent Hospital serves a population of more than 950,000 in 13 counties which spans 9,700 square miles. Brown County is home to a population of 260,401 people of which 97.1% are citizens. Brown County contains a dental care HPSA (health professional shortage area) and the metro area is a MUA (medically underserved area) per HRSA. HSHS St. Vincent Hospital does not exclude MUAs, low income or minority populations from its service area. There are three not-for-profit general medical surgical hospitals and one for-profit general medical surgical hospital serving Brown County and the surrounding area. In addition, there is one not-for-profit psychiatric hospital, one for-profit psychiatric hospital and one county operated psychiatric hospital in the area. Since most of the population resides in the Green Bay metro area, plans and services focus on this area.
Schedule H, Part VI, Line 5 Promotion of community healthAs a healing ministry of the Catholic Church and an affiliate of Hospital Sisters Health System (HSHS), HSHS St. Vincent Hospital is committed to delivering high quality, compassionate, and cost-effective health care services to all. The hospital was founded more than 130 years ago to bring a healing presence and improve the health of our community, especially for those persons who are sick, poor, and disadvantaged. Because of the hospital's purpose and tradition, it is organized to promote the health of Brown County, Wisconsin and surrounding areas. The hospital is governed by a Board of Directors, most of whom reside in the hospital's primary service area and who are neither employees nor independent contractors of the hospital (nor family members thereof). The Board ensures that HSHS St. Vincent Hospital is responding to community need. In FY2018, the Board dedicated its May 2018 educational session to better understand the recent Community Health Needs Assessment (CHNA) and Community Health Improvement Plan (CHIP). Consistent with its exempt purpose, HSHS St. Vincent Hospital has an open medical staff with privileges available to all qualified physicians in the area. The hospital also operates an emergency department that is open 24 hours to all persons regardless of their ability to pay. As a not-for-profit hospital, HSHS St. Vincent Hospital reinvests surplus funds to support the mission of the organization and health of the community rather than distributing surplus funds as profits to shareholders or individuals. Funds not committed to ongoing operations are generally used to upgrade facilities, secure new technologies, improve patient care, and support initiatives designed to promote health and ensure access for all. HSHS St. Vincent Hospital also conducts medical research through the Cancer Research Institute and serves as a site for health professionals education Examples of the reinvestment of HSHS St. Vincent Hospital's surplus funds includes: * Welcome Baby Program: More than 3,000 babies are born each year in Brown County, Wisconsin. For some families, the joy of welcoming a new child into their family can be overshadowed by hardships. The goal of a county-wide program called, Welcome Baby, is to make sure those adversities don't lead to struggles for the children. The program serves as an early-warning system to help prevent child abuse and neglect in the community. All resident families giving birth in Brown County are screened for risk factors. Those factors include financial hardships, education level of the parents, and the disability status of the mother. Based on the screening, the family may be connected to community-based resources. Between January and September of 2018, 725 parents received a Welcome Baby visit at HSHS St. Vincent Hospital and HSHS St. Mary's Hospital Medical Center. Of those parents, 188 were identified as being "at-risk." Family Services of Northeast Wisconsin runs the program, which was created as part of a United Way need assessment in the community. HSHS St. Vincent Hospital and HSHS St. Mary's Hospital Medical Center are committed to helping the program continue by offering financial support. * HSHS St. Vincent Hospital recognizes the critical role dental care plays in overall health and has partnered with OHP for years to provide this public health service. Through the support of HSHS Eastern Wisconsin Division, the Oral Health Partnership (OHP) continues to expand to communities throughout Brown County, providing dental care for infants to 19 years, who are uninsured and underinsured. HSHS Eastern Wisconsin Division hospitals provide public benefit health care and funding to address identified areas of health concerns to people throughout Wisconsin. Though the need is highest in the city of Green Bay, many children on Medicaid or without insurance, but who qualify for free and reduced lunch, require treatment. In 2016, a special effort was made to see these children. Meetings were held at each school district, and the response from those communities has been positive: "The recent expansion of Oral Health Partnership into the De Pere schools has significantly benefited the health of the children in our community. The De Pere Health Department is grateful to have these services provided in our schools to serve children that otherwise may not receive dental care," said Erin Bongers, RN, BSN, public health nurse. * HSHS St. This costs more than $70,000 each year to support, and the program already has a request to double the number of weekend backpacks sent home with children in our largest school district for 2018-19. * Mission Outreach: SVGB contributed more than 15 tons of supplies and furniture worth more than $100,000 to our System's Mission Outreach program. The donations included beds, nebulizers, chairs, IV poles and more items that would otherwise have been thrown away. Items not able to be used by missions in Third World countries are contributed to other agencies such as St. Vincent de Paul, House of Hope, Habitat for Humanity and the Curative Workshop. * SVGB ICU Expansion: In response to the growing needs of critical care patients, the investment was made to take the director and managers offices, as well as the colleague lounge, and renovate the space into two patient rooms in SVGB sixth floor ICU. Construction kicked off in December 2017, and the project was complete in April 2018. Prior to the project there were 19 patient rooms, now there are 21 rooms. To better meet the needs of bariatric patients, these two new patient rooms were designed with special enhancements made to a bariatric patient. There is a larger door, a bariatric lift and a larger entryway into the bathroom. * Practice Greenhealth extended its warmest congratulations to SVGB as a winner of this year's Partner for Change Award. Selection as an Environmental Excellence Award winner recognizes SVGB was recognized for their ongoing commitment to improving its environmental performance and their efforts in achieving a top standard of excellence in sustainability. SVGB was also honored to receive the Greening the OR Recognition Award. The Greening the OR Initiative of Practice Greenhealth recognizes SVGB for making substantial strides in environmental performance in the operating room. * HSHS St. Vincent Hospital contributes $25,000 annually to the Altrusa House in Green Bay, a non-profit organization that offers a "home away from home" to those who have a loved one in the hospital and need a place to stay. Guests of the Altrusa House generally live greater than 50 miles from Green Bay, are in financial need or require an extended stay in Green Bay due to medical treatment. The 12-bed facility is located just two blocks from SVGB. * Surgical Robots: In February 2018, SVGB launched use of the ExcelsiusGPS - one of the most advanced robotic surgery technologies available in the world today for back and neck surgery. The robot is designed for minimally-invasive and complex spinal fusion in the back and neck and allows for: smaller incisions, decreased blood loss, less exposure to radiation, a shorter hospital stay and quicker recovery, and less risk for complication following surgery. The investment into this technology was made to most importantly; provide our patients an enhanced level of care. However, it was also made to provide a service to our community that was not yet available. In fact, SVGB was the first hospital in the state of Wisconsin to offer this service to patients. Spinal fusion is a delicate, complex procedure that many people with spinal conditions and/or injuries need to undergo to relieve significant pain and regain quality of life. This new robot enhances the way our surgeons perform spinal fusion surgery and provides our patients a better chance at recovery. HSHS St. Vincent Hospital also devotes significant resources to access for patients who cannot afford care, along with other community benefits. In FY2018, HSHS St. Vincent Hospital provided more than $41 million in Community Benefit services, including charity care at cost, unpaid costs of Medicaid and other public programs, and a range of diverse programs designed to enhance access and improve community health. Additionally, in FY2018, HSHS St. Vincent Hospital more than $82 million (at cost) in excess of Medicare payment for health care services.
Schedule H, Part VI, Line 6 Affiliated health care systemHeadquartered in Springfield, Illinois, Hospital Sisters Health System (HSHS) is a highly integrated, multi-institutional health care system comprised of 15 hospitals and more than 200 physician practice sites in Illinois and Wisconsin. HSHS is sponsored by Hospital Sisters Ministries (HSM), which is a public juridic person of the Roman Catholic Church. HSHS continues the health care ministries begun by its founders, the Hospital Sisters of St. Francis, more than 140 years ago, in rural and midsized communities throughout Illinois and Wisconsin. HSHS provides compassionate, holistic, high quality, and cost-effective health care through its acute care hospitals, physician practices, home health programs, palliative and hospice programs, and community outreach services. In addition, HSHS affiliated hospitals include critical access facilities that offer essential health services that otherwise would not be available in many communities. Our mission calls us to reveal and embody Christ's healing love for all people, and we extend our high quality Franciscan health care ministry beyond the walls of our hospitals by providing community health improvement services with special emphasis on the poor and vulnerable. In addition to providing community health improvement services to the broader community, HSHS cares for everyone who comes to its facilities, regardless of their ability to pay. In tandem with a variety of cross sector community partners, HSHS's Community Benefit initiatives are strategically planned and professionally implemented to improve access to health care services; enhance the health of the community; advance medical or general health care knowledge; and relieve or reduce the burden of government to improve health. In FY2018, HSHS collectively provided $198.4 million in Community Benefit (9.1% of total hospital expenses). This amount included $25.7 million provided for Financial Assistance (a.k.a. Charity Care) and $133.7 million for unreimbursed care provided as part of the Medicaid program. In addition, HSHS hospitals committed significant resources to treat Medicare patients. The cost of providing services to primarily elderly beneficiaries of the Medicare program - in excess of governmental and managed care contract payments - was $221.5 million. HSHS hospitals also recorded $45.9 million in uncollectible accounts. While HSHS does not count the latter two amounts as Community Benefit, they nonetheless reflect our commitment to serving all persons in need of care. In addition to the dollars invested in our Community Benefit programs, HSHS continues to reinvest any surplus revenue from operations and investments into new medical technology, facility infrastructure and health care services in our communities. By doing so, we ensure our ability to meet the ongoing demand for high quality, efficient and easily accessible health care.
