Facility: CentraState Medical Center - Part V, Section B, Line 5CentraState is an active member of the Health Improvement Coalition of Monmouth County NJ (HICMC). HICMC is comprised of over 70 partners representing a broad range of communities and interests, particularly healthcare. Data and findings from HICMC 2007, 2012, and 2016 Community Health Improvement Plans (CHIP) were made a part of the CentraState CHNA. Additionally, surveys and meetings with local organizations such as the Howell Chamber of Commerce, the Jackson Chamber of Commerce, etc. contributed to the research.Consultation/Meetings with those with special knowledge of the expertise in public health were held with:Thomas Frank, Health Officer, Colts Neck Health DeptMargaret Jahn, Health Officer, Greater Freehold Area Health DeptDavis Richardson, Health Office, Manalapan Twp Health DeptThomas Frank, Health Officer, Colts Neck Health Dept
Facility: CentraState Medical Center - Part V, Section B, Line 7dAvailability of CHNA report was also announced to the public at the Hospital's Annual Meeting in January 11, 2018.
Facility: CentraState Medical Center - Part V, Section B, Line 11CentraState Healthcare System had developed goals and strategies to meet the three significant health needs of our community, namely: Heart Disease, Obesity and Access to Primary Care. Heart Disease - the goal is to decrease the overall risk of heart disease and provide direct prevention education and services to selected high risk populations within the service area. The strategies to meet this goal are to develop and implement programs and services to enhance knowledge of heart disease management and control the risk factors for heart disease by conducting Peripheral Artery Disease (PAD) screening events; offering cardiac support group; participating in an IRB-approved research study on cardiac rehab with focus on obesity, depression and exercise; increasing community outreach efforts including lectures and blood pressure or cholesterol screenings during community events; providing cardiopulmonary resuscitation classes and offering weight management program and classes.Obesity - the goal is to increase awareness on obesity-related diseases and illnesses in targeted populations, expand diet and exercise programming and develop programs for children and families. The strategies to meet this goal are to expand pre-diabetes education programming at the hospital and in the community, like free blood glucose screenings and nutrition consults; to provide increased access of Diabetes Center nurses to primary care physicians for nutrition counseling in English and Spanish; develop a program for high-risk Latina women with a focus on diet and exercise; participate with the Mexican Consulate in providing no-cost weight loss education for Mexican population; and distribute Novo Nordisk funded diabetes prevention DVD for children in both English and Spanish versions.Access to Primary Care - the goal is to optimize the ability of residents to access primary care, including basic and preventive care especially for the uninsured and underinsured and increase the number of primary care providers in the service area. The strategies to meet this goal are to provide low-cost/no-cost laboratory services; provide community education on how to access healthcare in the service area and to introduce a health plan product for small and mid-size employers to meet the growing concerns with being priced out of existing traditional health insurance options. The Implementation Plan Annual Progress Report 2016 describes how the hospital is addressing the significant needs in its most recently completed CHNA. Any such needs that are not being addressed together with any reason why such needs are not being addressed are discussed in the CHNA. Both documents are posted on the hospital website - URL:www.centrastate.com
Facility: CentraState Medical Center - Part V, Section B, Line 16jThe availability of reduced charge or free care was made known to patients, prospective and former patients and families on the facility website, on billing invoices, posted in each emergency patient care room, in the emergency and all outpatient registration areas, in the admissions/cashier office, and in writing to admitted patients. The actual policy was not provided, but the existence of the policy is made clear and the patient is requested to apply, or ask for more information. In addition, if the patient indicates difficulty in paying, the existence of free or discounted care is communicated to them and the patient is financially screened by phone and/or a charity application is mailed to them.

Form And Line ReferenceFormAndLineReferenceDescExplanationExplanationTxt
Part I, Line 3c - Charity Care Eligibility Criteria (FPG Is Not Used)CentraState follows the NJ Administrative Code 8:31B and considers individual assets over $7,500 and family assets over $15,000 in determining eligibility for Financial Assistance. In addition, there is a "spend down" provision allowing Financial Assistance if the amount due from the patient is greater than the assets over the stated thresholds.
Part III, Line 9b - Provisions On Collection Practices For Qualified PatientsDuring financial counseling, the patient is screened for eligibility in public assistance programs, including Medicare, Medicaid, the Catastrophic Illness Fund for Children, and the Violent Crime Compensation Board. The patient is assisted with the enrollment process for any programs for which the patient has probable eligibility. Bills may be reduced from 20% to 100% based upon Financial Assistance criteria. The patient responsibility portion of 20%, 40%, 60% or 80% is based upon rates established in P.L. 1971 c. 136. Spend down assets, through partial payment of the hospital bill is allowed to enable the patient to qualify for Financial Assistance. The amount for which the patient is responsible after partial Financial Assistance shall be limited to 30% of income, per NJAC 8:31B. Uninsured patients with income between 301%-500% of Federal Poverty Levels will be provided a significant discount, in accordance with P.L. 1971 c. 136. Patients are billed routinely for unpaid balances and shall be notified of any impending collection or legal action. Patients are provided with contact information to discuss any questions or problems. Patient bills indicate the availability of payment schedules. Upon patient request, the following payment schedule for balances of $100 or more may be agreed upon: Balance of $100 - $450 - Monthly payment = 1/4 of the balance, $451 - $3,000 = 1/12 of the balance, over $3,000 = minimum of $250 per month or 1% of gross income/month, whichever is more. If the patient and Medical Center representative mutually agree to a payment schedule and payments are received as agreed, further collection action will not be taken. Exceptions to the above schedule may be made by Patient Financial Services Management upon the request of the patient, and documentation by the patient of inability to pay according to the established schedule. If, at the end of the financial screening attempts and billing cycle, the patient has not been approved for Financial Assistance and/or is not on a payment plan, or after there has been a default in payment, the account will be assigned to a collection agency and/or attorney for collections.
Part VI, Line 2 - Needs AssessmentCentraState Healthcare System continues to be a charter member of the HICMC, formerly known as the Monmouth County MAPP Coalition. The CentraState representative (Community Relations Coordinator) serves on the steering committee and is a member of one of three identified-needs work groups. This collaboration completed the original CHIP for Monmouth County in April, 2007. A new/revised CHIP was completed in 2011 and was released in April 2012 at a county-wide public meeting. Findings from this county-wide plan were adopted by the Strategic Planning committee of the CentraState Board of Trustees so as to be formally incorporated into the Systems Community Health Needs Assessment (CHNA) which was completed in November 2013 and was filed with tax year 2013 Form 990. Progress of the Implementation Plan was reported with the 2014 and 2015 Form 990 filing. Continued progress of the Implementation Plan is being reported with the 2016 Form 990 filing. In 2010, the coalition began gathering secondary data as part of a community health needs assessment. The planning process for a new/revised CHIP began in 2016.In 2010, needs were identified and in 2011 were prioritized. During 2012, strategies to address the prioritized needs were developed and implemented by the HICMC members including CentraState in its service area. A new cycle began in January of 2016.CentraState developed a plan to complete a CentraState Community Health Needs Assessment as required by the Affordable Care Act and the informal CHNA process began in January 2012 and was completed and implemented in November 2013. The plan is widely available through the CentraState website as well as distribution of printed copies upon request. A new cycle began on January, 2016.Additionally, the Freehold Community Alliance for the Prevention of Alcoholism and Drug Abuse (Freehold Twp and Freehold Boro) conducts a needs assessment every three years in which CentraState participates. CentraStates representative (Community Relations Coordinator) is Co-Chair of the organization (Freehold Twp Mayoral appointment.) Research for development and betterment of the community cost $58,566. Bad Debt expense of $9,598,587 was excluded from total expense when calculating percentages.Part III, Line 2 & Line 4 - Bad Debt ExpenseThe amount of the allowance for doubtful accounts is based on management's assessment of historical and expected collections, business economic conditions, trends in health care coverage, and other collection indicators. Additions to the allowance for doubtful accounts result from the provision for bad debts. Accounts written off as uncollectible are deducted from the allowance for doubtful accounts.Part III, Line 8 - Explanation of Shortfall As Community BenefitCentraState Medical Center feels that Medicare underpayments (shortfall) are community benefit and associated costs. CentraState believes that these services and related costs promote health of the community as a whole and are rendered in conjuction with the organization's charitable tax-exempt purposes and mission in providing medically necessary health care services to all individuals in a non-discriminatory manner without regard to race, color, creed, sex, national origin, religion or ability to pay and consistent with the community benefit standards promulgated by the IRS. The American Hospital Association ("AHA") feels that Medicare underpayments (shortfall) are community benefit thus includable on the Form 990, Schedule H, Part I. CentraState agrees with the AHA position for the following reasons:Providing care for the elderly and serving Medicare patients is an essential part of the community benefit standard. Medicare, like Medicaid, does not pay the full cost of care, and many Medicare spending is for beneficiaries, like Medicaid counterparts, are poor. More than 46% of Medicare spending is for beneficiaries whose income is below 200% of the Federal poverty level.Medicare underpayment must be shouldered by the hospital in order to continue treating the community's elderly and poor. These underpayments represent a real cost of serving the community.Part III, Section B - Medicare Line 7Management has reported $16,334,588 as a shortfall for 2018. Management also believes that an additional $6,161,801 should be included on this line due to Managed Medicare losses. See additional schedule attached labeled as Medicare Managed Care P&L. Therefore the Medical Center's total shortfall would be $22,496,386.
