DescGlendale Adventist Medical Center (AHGL) provides quality medical health care regardless of race, creed, sex, national origin, handicap, age, or ability to pay.Although reimbursement for services rendered is critical to the operation and stability Glendale Adventist Medical Center, the organization recognizes that not all individuals possess the ability to pay for essential medical services.Adventist Healths vision is to enhance the health of the communities where we live and serve, by engaging our communities and our patients in a new definition of and partnership for personal community health. In keeping with this commitment to serve all members of the community, the following will be considered when individuals who need health care cannot pay:providing free care and/or subsidized careproviding care to persons covered by governmental programs at below costproviding health/wellness activities and community education programsNot only does Glendale Adventist Medical Center provide low cost care to individuals covered by government-programs and those unable to afford healthcare, it also helps patients find and access private and governmental resources for healthcare benefits.AHGL recognizes below-cost reimbursement as charity and uncompensated care in meeting its mission to the entire community. The unreimbursed cost of providing care to these patients in 2019 was $40,023,146.The following Inpatient services that were provided to all our patients. These services included:1,560 Babies delivered3,504 Surgeries performed95,227 Patient days The following Outpatient services that were provided to all our patients. These services included:61,830 Emergency department visits3,496 Outpatient Surgeries performed137,498 Outpatient visits 127,281 Clinic visitsAHGL recognizes it has an obligation to provide human services above and beyond its role as a healing facility. The following community benefits demonstrate the tangible ways in which the organization is fulfilling its mission. The total unreimbursed cost of these community benefits in 2019 was $25,734,664.Adventist Healths mission statement of Living Gods love by inspiring health, wholeness and hope is coupled with a vision to transform the health experience of our communities through collaborative programs, community investments and community outreach. We are inspired by the healing ministry as represented by the life of Jesus Christ and believe we are called to live out our mission intentionally in the communities we serve. In the small towns, suburbs and inner cities we serve, we continue our journey to provide quality healthcare until every person made in Gods image has experienced the best health today, hope for tomorrow, and Gods love that endures forever. As an illustration of how Adventist Health Glendale impacts our community, let us share a story about how our designation as a STEMI (heart attack) receiving center allows us to provide quality care for our community: Jacob Nalbandyn is 34 years old, in good physical shape, exercises regularly, eats healthy and leads what he describes as an average lifestyle. At his age, you wouldn't expect Jacob to have a heart attack. Thats also what he thought. I was having lunch, and all of a sudden I felt discomfort in my chest, sort of weird, recalls Nalbandyan, married and the father of two young children. The Glendale resident and attorney was in San Diego that day on business. But it was a heart attack and Jacob is quick to credit Adventist Health Glendale emergency medicine physician Dr. Anthony Cardillo and interventional cardiologist Dr. Amir Sadrzadeh Rafie, along with the hospitals ER and cardiology teams, for saving his life. The pain wasnt excruciating, it was just discomfort in my chest, Jacob adds. For a classic heart attack, you think of going to your knees, gasping for breath. It might be indigestion, might be heartburn. I thought, lets get back home to Glendale and see what happens. If I feel worse, Ill go to the ER. In retrospect, Jacob shudders at his decision to drive himself back to Glendale. "I was foolish," he says. Fortunately, he made the trip without further incident and arrived home safely. But when he was still feeling a tightness in his chest and asked his wife Armine what she thought, her response was immediate: Grab your things and get to the ER. Since joining the hospitals ER team 13 years ago, Dr. Cardillo has attended to thousands of patients with chest pains. Despite the patient's age and the fact that he didn't have any cardiovascular risk factors, and with a near normal ECG when he arrived in the ER, Dr. Cardillo astutely diagnosed Jacob with a heart attack. Jacob had suffered, in medical terms, a STEMI (ST-Elevation Myocardial Infarction). This is a very serious type of heart attack when at least one of the major coronary arteries that supplies blood to the heart is completely blocked. He was transferred immediately to the hospitals Cath Lab, Dr. Rafie and his team performed successful balloon angioplasty and stenting of Jacob's blocked coronary arteries. The hour-long procedure reopened blood flow to portions of the heart that were restricted. According to Dr. Rafie, there was 100 percent occlusion of one of the major coronary arteries. Jacob also had blockage of the left anterior descending (LAD) artery, known among cardiologists as a widow-maker, and at any moment he could be struck with another attack. The following afternoon, Jacob was discharged from the hospital to go home. To experience a massive heart attack, get that taken care of with four stents, and be checked out the next day like nothing had happened, it was incredible, Jacob adds. This entire experience was life-changing in many ways. Even within my circle of friends, there was like a shockwave. Everyone was in disbelief. Clearly, I owe everything to Dr. Cardillo. Dr. Rafie and the Adventist hospital staff. Jacob wants to share his story to help educate others. If this happens to you, dont be as foolish as I was, he says emphatically. I was lucky on a number of occasions that same day. It was like I hit the Lotto four times in a row!Compassionate, high-quality patient care; life-saving and life-preserving medical and surgical services; advanced treatments in a community-based hospital; essential emergency services-these are the reasons we are in the Glendale community.Websites for our community benefit information: https://www.adventisthealth.org/about-us/community-benefit/Desc

Facility: - Part V, Section B, Line 5The 2019 CHNA data collection process can be summarized as follows: 1. review of prior CHNA reporting efforts; 2. collection of most recently available demographic, socioeconomic and health indicator data; 3. data gathering from community members. Leveraging its extensive network of community partners, the hospitals, in collaboration with the Glendale Healthier Community Coalition, held a community summit on October 25, 2018, with 95 stakeholders. In compliance with the ACA, participants represented a broad range of geographic, public health, and population interest including health care professionals, social service providers, city and public health officials, members from the local police department, and other community leaders. In addition, a total of 66 community stakeholders was convened April 4, 2019 for a Prioritization Forum with the goal of ranking the identified health needs. Many of the Forum participants had also attended the first coalition summit. After a large group discussion, participants were given the opportunity to provide input via voting and a survey.Secondary data collection included over 300 data indicators that helped illustrate the health states of the community. Secondary data were collected from a wide range of local, county, state and national sources to present demographics, mortality, morbidity, health behaviors, clinical care, social and economic factors, and physical environment. Data sources included US Census, County of LA Public Health Department, OSHPD, California Health Interview Survey, CDC, LA County Department of Health Services and the LA Homeless Services Authority.
Facility: - Part V, Section B, Line 6aAdventist Health Glendale and Glendale Memorial Hospital and Health Center have partnered to conduct the CHNA.
Facility: - Part V, Section B, Line 11Our 2016 CHNA identified three areas of significant needs: Cardiovascular health, stroke education and support and population health for chronic disease. The main goal was to improve the overall health and wellness of our communities through provision of services, community collaboration and innovation. Priority Need 1- Cardiovascular Health to improve aortic stenosis education and support. The Cardiovascular Institute has served over 200 participants in the community in 2019. Interventions:1.Collaborated with the LA County STEMI Center to obtain two standing screens that have the signs and symptoms of acute coronary syndrome and the differences between men and women. It also discusses the importance of calling 911 when a patient is having any of those symptoms in the community. These screens are displayed at events in the community and on our hospital grounds for the community to see.2. Early Heart Attack Education (EHAC) explaining the signs and symptoms of a heart attack, importance of calling 911, usual signs and symptoms are given on flyers, magnets, and on the hospital website.3. Annual senior health fair provided information and CPR demonstrations along with physicians who were available to answer community member questions. 4. Event health screening: The heart institute provided information about heart health, early warning signs of heart attack, and also offered hands on CPR demonstrations. Priority Need 2- Improve stroke education and support. In 2019, AHGL provided stoke education and awareness to the community in on and off-site settings to Live Well Senior Center participants and paramedics. Presentations were offered to increase knowledge of signs and symptoms of stoke, risk factors, and support services for stroke survivors. Blood pressure screening and pre- and post-program survey were also incorporated to assess program effectiveness. Throughout the year, the Neuroscience Institute/ stroke program has provided preventive education to approximately 1,015 participants in the Glendale region and will continue to serve and expand program offerings in the coming year. Interventions:1. Provided community education regarding the signs and symptoms of stroke and the need to call 911 versus driving to the ED.2. The Neuroscience Institute provides two free stroke awareness community presentations. They were able to evaluate the effectiveness of the stroke community education by performing a pre-test and post-test survey. Presentations were offered to increase knowledge of signs and symptoms of stoke, risk factors, and support services for stroke survivors. Blood pressure screening and pre- and post-program survey were also incorporated to assess program effectiveness. 3. Adventist Health Glendale provided stroke risk assessments including blood pressure screenings at two community events. 4. For National Stroke Month, AHGL celebrated stroke awareness with a wear purple day and a Stroke Awareness event with purple super foods, stroke screenings, public education on stroke identification and prevention, exercise demonstrations, and the presence of our mascot the stroke avenger. These activities increased the number of patients identified as at risk and directed them to stroke prevention and intervention resources.5. Community Mobility program is operated in partnership with the Department of Motor Vehicles for those community members who have had a stroke and are experiencing neurological deficits that may impair driving ability. This program evaluates the ability from a clinical and an on the road perspective to determine driving ability. Priority Need 3 -Population health for chronic disease. Champions for Change has served over 8,680 participants in the Glendale region in 2019. Interventions: 1. AHGL implemented the Smarter Mealtimes program to role model healthy eating and increase healthy cues to action. We conducted nutrition education classes in three schools and implemented a sustainability plan for teachers and school staff and conduct 90 nutrition education classes. The Youth Nutrition and Physical Activity Survey was conducted. Statistical analysis indicated that students in this cohort experienced a 46% increase in their consumption of whole grains and an 8% increase in access to vegetables at home. Students also reduced their consumption of soda, French fries, and cookies or cake by 50%, 45%, and 53%, respectively.2. AHGL continued community Zumba classes at Pacific Park and Community Center. Classes were conducted to improve fitness among community residents. We sustained 30 participants per class, reached 5-7 new participants, and demonstrated / promoted fitness activities at five community events. A sampling of participants showed improved fitness outcomes3. AHGL conducted public opinion surveys and educated community leaders about exposure to secondhand smoke, aerosol vapors, and the need to reduce youth access to tobacco. Outcomes included reduced exposure to tobacco by youth and community members.4. AHGL collected 1,000 public opinion surveys and conducted 100 one-on-one coalition building meetings, provided outreach and education at 30 events, conducted 30 group presentations, and convened 50 coalition meetings. This included a presentation to a city council requesting the adoption of city policy changes. 5. At the hospital campus, AHGL hosted a diabetes support group serving 92 adults.6. Established an additional partnership with the Glendale Unified School District after school sports program in order to reach as many children and adults as possible in the community. Eight elementary schools participated in a range of sports, to which Champions for Change added nutrition classes. We also trained district coaches in healthy options, and as a group, the coaches pledged to drink only water and healthy drinks for the length of the program in order to set a good example for the children.7. Provided a nutrition and gardening program for the students of Thomas Edison Elementary school. Students learned about plant care, nutritional labels, and the health benefits of drinking water instead of sugary drinks. 8. Champions for Change added a garden to the three it was already providing with Pacific Clinics Head Start, via a grant from AHGL, which helps sustain the other gardens as well.9. Held 446 health promotion events through the course of the year in the categories of community gardens, exercise/sports, health education and nutrition education. 10. Direct education classes by Adventist Health Glendale staff that teach nutrition education to members of the community include reading food labels, choosing foods from all the food groups, reducing sugar and sodium, and increasing water consumption. These classes were held at community centers, elementary schools, Head Start sites, and homeless shelters.11. Participation at community events entailed giving information about the Champions for Change program and its benefits and took place at the Cesar Chavez Festival, Glendale Fire Pancake Breakfast, Glendale Cruise Night, Edison Elementary School Carnival, Edison Elementary Back to School Night, and After School Sports Championship EventsTwo at Thomas Edison Elementary School, one at Pacific Clinics-Head Start Lexington, and one at Pacific Clinics-Head Start GlendaleAHGLs Live Well Senior Center served 45,976 with health education and prevention in 2019. Part III, Line 2 - Methodology Used To Estimate Bad Debt ExpenseUncollected patient accounts are analyzed using written patient financial services policies that apply standard procedures for all patient accounts. The result of the analysis is what is recognized as bad debt expense. For example, all self-pay patients receive a discount. If the discounted account is unpaid after collection efforts, the unpaid balance is classified as bad debt. The cost-to-charge ratio is multiplied times the hospital's bad debt expense. The resulting figure has been reported as bad debts at cost on Part III, Line 2.
Part III, Line 3 - Methodology of Estimated Amount & Rationale for Including in Community BenefitThe portion of the bad debts attributed to charity care as reported on Part III, Line 3 was calculated by an independent third-party consulting firm. This is an estimate of additional charity care that would have been granted if patients had cooperated by furnishing family financial information. A statistically valid sampling of patient accounts written-off was evaluated. The evaluation used various factors to determine which patients would have been eligible for charity care. Had the hospital obtained sufficient information from all patients who qualified for financial assistance, these additional accounts would have been recorded as charity care instead of bad debt.
Part III, Line 4 - Bad Debt ExpenseThe system-wide combined audited financial statements do not contain a footnote describing bad debt expense.
Part III, Line 8 - Explanation Of Shortfall As Community BenefitThe Medicare cost report apportions the hospitals costs on the basis of inpatient days and ancillary and outpatient charges to establish the costing methodology.Healthcare delivery by hospitals is a complex, highly regulated business in the United States. Healthcare unit cost inflation is driven by compliance with ever-expanding regulatory requirements, shortages of highly skilled labor and evolving medical and information technology. The health care market basket is unrelated to that of the average individual consumer. Since the 1997 Balanced Budget Act, Medicare annual payment updates have fallen behind actual healthcare cost inflation to the point that Medicare payments to many U.S. hospitals are well below the cost of providing care. These unreimbursed costs are a community benefit for seniors and others in the community as these individuals are continuing to receive care without which many would become dependent on other governmental resources such as Medicaid. The benefit to the community for healthier Medicare recipients is no different than those benefits the community realizes for uninsured and underinsured patients who are eligible for partial and full charity care. Medicare is a safety net for seniors and others. Without Medicare coverage, many individuals would undoubtedly qualify for charity care.In addition to the mismatch between Medicare payment increases and healthcare cost inflation, the highly complex Medicare payment systems and formulas produce disparate payment levels from one hospital to another for the same service. These disparate payment levels create disparate results within groups of hospitals.Reconciliation of Medicare Revenue from the hospital's Medicare Cost Report to GL: Revenue Rec. *Medicare Cost Report Revenue $145,140,386 Prior Year Settlements 1,424,064 Cost Report Reimbursable Bad Debts 2,530,373 Estimates and Accrual Variances 86,353 Other 12,116,528 ___________ Total Medicare Revenue $161,297,704*Note: The Medicare Cost Report revenue does not include the bad debt reimbursement. The Cost Report revenue does include the patient co-pay and deductible amounts. Adding the bad debt reimbursement would have duplicated the revenue already accounted for in the co-pay and deductible amounts.
Part III, Line 9b - Provisions On Collection Practices For Qualified PatientsWhen a patient has requested screening for charity care, the hospital must immediately cease collection activity and place the account in a charity pending status. If 100% charity is approved, the entire account balance is written off to charity care. If the patient has a sliding scale liability based on the federal poverty guidelines, they are billed only for that liability. If the patient fails to pay their after-charity liability, they are assigned to a collection agency with an identifier that indicates to the agency that the patient is "low income" and the following criteria must be followed by the agency:1. They may not report the patient to a credit bureau2. They may not file a lawsuit to recover the outstanding liability3. They may not charge interest
Part VI, Line 2 - Needs AssessmentFor the 2016 CHNA, three Glendale hospitalsAHGL, Glendale Memorial Hospital and Health Center, and Verdugo Hills Hospitalcollaborated, as they have in the past, to work with the Center for Nonprofit Management consulting team in conducting the CHNA. During the initial phase of the CHNA process, community input was collected during a focus group with key stakeholders, including health care professionals, government officials, social service providers, community residents, leaders, and other relevant individuals. Previous CHNAs were reviewed to identify trends. Participants completed an online questionnaire about health needs, drivers, and resources, and ranked each health need according to several criteria including severity, change over time, resources available to address the need or driver.
Part VI, Line 3 - Patient Education of Eligibility for AssistanceFor scheduled cases, hospital staff run patients eligibility and informs patients of their benefits and financial responsibility. If a patient is insured under a government program, staff informs them about any financial responsibility they have, and ask whether the patient has further questions about their government benefits. If a patient is uninsured, staff provides education about government programs that may be available, and including appointment-setting with the on-site state Medi-Cal worker. If a patient is uninsured and unable to meet any portion of their financial responsibility, staff connects them with government programs, and if appropriate, helps them apply for AHGLs hospital assistance program. AHGL communicates to the public through the website, application form and plain language, the FAP, application form and plain language summary (PLS) are available free of charge in registration areas. Notices about financial assistance are posted in high traffic registration areas.
Part VI, Line 4 - Community InformationThe population in Los Angeles County has grown to 10,231,037, an increase of 4.2% in 8 years, which is lower than the overall proportional growth in California standing at 5.6%. With a state population of 39.5 million people, one in four Californians live in Los Angeles County. In 2018, the AHGL service area population was estimated at 329,014 residents. In particular, ZIP codes 91203Glendale (8.3%) and 91020Montrose (5.9%) experienced the highest percentage of growth. In 2018, residents in the AHGL service area were, on average, slightly older (42.0 years old) than the whole of Los Angeles County (38.2 years old). Overall, in the AHGL service area, a higher percentage of residents were represented in older age groups than the rest of Los Angeles County. In 2017, a majority of the population living in the AHGL service area was ethnically either White (51.2%) or Hispanic/Latino (28.2%). Los Angeles County had a higher percentage of Hispanic/Latino residents (48.4%) and a lower percentage of White residents (28.3%) than in the AHGL service area. The Black/African-American population in the AHGL service area (1.7%) was less than one quarter that of Los Angeles County (8.5%). The Asian population in the service area (15.9%) was slightly higher than in Los Angeles County (14.4%). The AHGL service area consists of geographically concentrated ethnic communities that contribute to the areas vibrancy and community-based assets. For example, among the approximate 200,000 residents in the City of Glendale, 72,000 have Armenian heritage. In particular, the foreign bornpopulation skews heavily towards people of Armenian (82.9%) or Persian (7.5%) ancestry. There is, however, a large Latino population (18.2%) primarily of Mexican heritage. 2017 data show a 79.6% high school graduation rate as opposed to an 83.3% rate in California. In 2018, a lower percentage of families in the AHGL service area lived below poverty (10.4%) than families in Los Angeles County (13.5%). Similarly, the percentage of families living below poverty with children (7.0%) was significantly lower than Los Angeles County (10.2%). The average household income of residents in the AHGL service area ($99,577) was higher than that of Los Angeles County ($96,019) by 3.7%. Household sizes in the AHGL service area ranged from 2.5 and 3.1. The AHGL service area had a slightly smaller average household size (2.7) than the average for L.A. County (3.0). The AHGL service area population varies in its income power. The median household income in the service area ranged between $114,010 in Glendale 91208 and 91205 ($61,682).
Part VI, Line 4 - Community Building ActivitiesThe hospital is involved in numerous community building activities which promote the health of the communities it serves. Numerous community concerns are addressed, including health improvement, education, poverty, workforce development and access to care. We also encourage our employees to serve on community collaboration boards, health advocacy programs, and physical improvement projects to promote the health of the communities we serve. In addition, we work with neighborhood programs, including schools, work sites and safety net providers to promote health and wellness and prevent disease. These activities are not included elsewhere on Schedule H.
Part VI, Line 5 - Promotion of Community HealthOur hospitals mission is, Living Gods love by inspiring health, wholeness and hope. Our community benefit work is rooted deep within our mission and merely an extension of our mission and service. We have also incorporated our community benefit work to be an integral component of improving the triple aim. The Triple Aim concept broadly known and accepted within health care includes:1) Improve the experience of care for our residents.2) Improve the health of populations.3) Reduce the per capita costs of health care. Our strategic investments in our community are focused on a more planned, proactive approach to community health. The basic issue of good stewardship is making optimal use of limited charitable funds. Defaulting to charity care in our emergency rooms for the most vulnerable is not consistent with our mission. An upstream and more proactive and strategic allocation of resources enables us to help low income populations avoid preventable pain and suffering; in turn allowing the reallocation of funds to serve an increasing number of people experiencing health disparities.Hospitals and health systems are facing continuous challenges during this historic shift in our health system. Given todays state of health, where cost and heartache is soaring, now more than ever, we believe we can do something to change this. These challenges include a paradigm shift in how hospitals and health systems are positioning themselves and their strategies for success in a new payment environment. This will impact everyone in a community and will require shared responsibility among all stakeholders. As hospitals move toward population health management, community health interventions are a key element in achieving the overall goals of reducing the overall cost of health care, improving the health of the population, and improving access to affordable health services for the community both in outpatient and community settings. The key factor in improving quality and efficiency of the care hospitals provide is to include the larger community they serve as a part of their overall strategy. Population health is not just the overall health of a population, but also includes the distribution of health. Overall health could be quite high if the majority of the population is relatively healthyeven though a minority of the population is much less healthy. Ideally such differences would be eliminated or at least substantially reduced.Community health can serve as a strategic platform to improve the health outcomes of a defined group of people, concentrating on three correlated stages:1) The distribution of specific health statuses and outcomes within a population; 2) Factors that cause the present outcomes distribution; and 3) Interventions that may modify the factors to improve health outcomes.Improving population health requires effective initiatives to: 1) Increase the prevalence of evidence-based preventive health services and preventive health behaviors, 2) Improve care quality and patient safety, and 3) Advance care coordination across the health care continuum. We will work together with our community to ensure the community health improvements are identified and then targeted for programs to influence behaviors to obtain improved health within the whole community.
Part VI, Line 6 - Affilated Health Care SystemThe hospital is a member of Adventist Health, a health care system which provides healthcare services in diverse markets within the Western United States. A member hospital may share some services with other member hospitals in its geographic area, such as clinical, management and support services. Using today's technology, hospitals outside the geographic area can provide support through remote services such as telepharmacy and robotics surgery. The Corporate Office provides important shared administrative support for member hospitals' rural health clinics and home care agencies, quality of care, other clinical needs, financing and risk management, and shared clinical and financial information technology. As many experienced and new physicians search for alternatives to independent practice, there is also corporate administrative support for hospital affiliated medical groups that engage physicians through employment or other contracts. This provides stability and growth of qualified physicians across many specialties, which is very important to make healthcare services available and to maintain and improve health within the communities served by all member hospitals.

Form 990, Part VI, Line 12c: Explanation of Monitoring and Enforcement of ConflictsDuring the first quarter of each year, the annual conflict of interest questionnaire is sent to board members, hospital officers, key employees and department directors for completion and signature. The questionnaire is accompanied by a letter of explanation, to illustrate examples of a conflict and to remind the recipient that if any perceived conflict should arise before the next annual questionnaire, he/she is to notify the CEO immediately. The Corporate General Counsel distributes, collects, and reviews for signatures the COIs for all applicable Corporate employees and the board. In addition, the hospital's administration is responsible for keeping record to ensure all hospital-based individuals with director and above positions have submitted their COIs. Conflicts are documented and reviewed with the Board.
Form 990, Part VI, Line 15a: Compensation Review & Approval Process - CEO, Top ManagementThe hospital board of directors has established a Compensation Committee to overseethe executive compensation program. This committee is composed of independentdirectors with no conflicts of interest.
