Form 990, Part VI, Line 8: Explanation of No Contemporaneously Documentation of Meetings	The Organization does not have any active committees with authority to act on behalf of the governing body.
Form 990, Part VI, Line 11b: Form 990 Review Process	Form 990 is prepared by an independent Enrolled Agent and reviewed in detail by the Organization's top management. The reviewed Form 990 is then provided to the board of directors prior to filing with the IRS.
Form 990, Part VI, Line 12c: Explanation of Monitoring and Enforcement of Conflicts	The Organization requires all officers and board members to annually complete and sign a conflict of interest questionnaire. The board Chairman is responsible for reviewing the signed statements and ensuring that interested persons are in compliance with the conflict of interest policy. Should any potential conflicts of interest be disclosed, the board member or officer would be asked to refrain from participation in any deliberation or decision with regard to matters affected by the relationship.
Form 990, Part VI, Line 15a: Compensation Review & Approval Process - CEO, Top Management	When the new CEO was hired in 2015, a compensation study was performed. Compensation for CEO positions of similar size and mission type organizations were considered. The CEO's recommended compensation was presented and approved by the independent board. The process was documented in the board minutes.
Form 990, Part VI, Line 15b: Compensation Review and Approval Process for Officers and Key Employees	In the past several years ther have been no raises provided to staff due to severe financial constraints. In 2019, raises were provded for staff members who changes positions, but were paid less than the previous postion holder. Also, after many years of no raises, cost-of-living raises were provided for some stafff.
