FieldValueIRS element (for citing)
Description1The AstraZeneca HealthCare Foundation is committed to providing grants and capacity building to US-based organizations working to improve heart health in the United States through its Connections for Cardiovascular Health (CCH) program for its Grant Awardees and other interested organizations. Evaluation services from our university partners provide the basis for understanding program outcomes and where capacity building needs to occur. Grant Awardees have access to Foundation staff and university partners to provide program assistance. Grant Awardees submit program reports three times throughout the program cycle, and receive evaluations at each point, which can include recommendations to address program challenges. At mid-year after receiving the evaluation, Grant Awardees may also be required to submit action plans to ensure they meet program goals by year-end. At the end of the year, all 11 funded programs met or exceeded their objectives based on their evaluations.Description1Txt
Expenses1$147,880Expenses1Amt
Description2In April, the Mentor-Mentee Learning Forum served to launch the Foundation's pilot mentorship program and foster peer-to-peer learning amongst six Grant Awardees. The mentorship program pairs newly-funded organizations with longstanding Grant Awardees to help them launch successful programs. The Learning Forum included activities such as evaluation consultations with our university partners as well as workshops on mentorship, communications, sustainability and evaluation and the opportunity to see the CCH Grant Awardee co-host's program in action.Description2Txt
Expenses2$289,968Expenses2Amt
Description3Capacity building and sustainability are key elements of the Foundation's CCH program. Activities supporting these focus areas included regular engagement calls to support Grant Awardee communications and social media activities, to further share information on their programs, lessons learned, and demonstrate their impact via program outcomes. The Foundation also supported sustainability through engaging with university consultants via a webinar to help the Grant Awardees develop program sustainability plans, culminating in an in-person workshop and consultations on the plans in November to mark the CCH program's 10th year anniversary. Grant Awardees had access to our university evaluation partners to further develop and complete their program sustainability plans.Description3Txt
Expenses3$47,015Expenses3Amt
Description4The Foundation developed its Effective Program Practices Guide as an educational resource designed to help organizations develop their own community health programs. The Guide includes general guidelines for building an effective community health program and six innovative program approaches commonly used by CCH Grant Awardees for other organizations to adopt or modify. The Foundation shared lessons learned through dissemination activities such as: a webinar presentation through the Society for Public Health Education, social media, e-mail distribution to the Foundation's mailing list of over 2,000 interested parties, and additional email outreach through partner organizations. Dissemination efforts focused on helping to build capacity for eight Grant Awardees to submit abstracts to the American Public Health Association 2019 Annual Meeting & Expo. Six Grant Awardee abstracts were accepted for presentations (2 poster, 5 oral), and the Foundation's oral presentation, and 2 short films.Description4Txt

2019 Connections for Cardiovascular HealthSM Criteria:Part XV, Line 2d2019 Connections for Cardiovascular HealthSM Criteria:The AstraZeneca HealthCare Foundation's Connections for Cardiovascular HealthSM (CCH) program awards Foundation grants annually to U.S.-based, nonprofit 501(c)(3) organizations or similar nonprofit organizations working to improve cardiovascular health in their communities. The intent of the grants is to support innovative initiatives or programs at the community level to help improve cardiovascular health in the community during the grant year and beyond. Our aim is for funded programs to maximize the benefit to participants and to work toward sustainability beyond our funding. To qualify for a Foundation grant, an organization is required to be a U.S.-based, nonprofit organization with a 501(c) designation or a public school, government entity or municipal institution that is eligible to accept tax-deductible, charitable contributions. Organizations that are 501(c)(3) and also have an IRS designation as a 509(a)(3) are ineligible for funding. Grant applicants must be invited to apply for funding. The nonprofit organization must be engaged in charitable work at the community level or otherwise. The program must address the Foundation's Connections for Cardiovascular HealthSM mission and meet five key criteria:1. Address patient cardiovascular health issues within the United States and its territories2. Recognize and work to address an unmet need related to cardiovascular health in the community3. Respond to the urgency around addressing cardiovascular health issues, including cardiovascular disease or conditions contributing to cardiovascular disease4. Improve the quality of participants and non-professional caregivers' lives in connection with the services provided and work done.5. Demonstrate a clear plan for long-term program sustainability, to further improve cardiovascular health after Foundation grants are expended.Foundation grant applications that meet the criteria and describe innovative initiatives with clearly defined outcomes, measurable results and processes will be considered for funding.Organizational Criteria:1. The organization and its program must be based in the United States and its territories.2. The Foundation provides grants to organizations that are: o Nonprofits with a 501(c) designation. (Organizations that have a 509(a)(3) designation are ineligible for funding.) o Public schools o Municipal institutions (e.g., city health department, municipal hospital or county board of health). o Government Entities3. Applying organizations will be required to provide 1) a copy of their current IRS 990 form or documents that establish eligibility to receive charitable donations and 2) audited financials for the past fiscal year. (If audited financials are not available, unaudited financial statements may be submitted. The CCH program preference is to provide funding to organizations that have both a current audited financial statement and IRS Form 990 available.)4. After submitting the application, organizations cannot make substantive changes to their tax status. Once selected for a grant award, organizations cannot reassign, transfer or credit their grant to another entity, including any related entity, without prior written approval by the Foundation. Any changes in an organization's tax status after submitting the application or being selected for a grant award must be reported immediately and may impact the organization's eligibility to receive the grant.Funding Criteria:The intention of 2019 CCH Program Grant funding is two-fold: to support a limited number of previously-funded programs that demonstrate positive outcomes and to work towards program sustainability beyond CCH funding. The Foundation will only accept one application per organization for a single year of funding. There is no guarantee of funding:1. Organizations that received their first year of CCH funding in November 2018 may apply for funding up to $125,000 in support of their previously CCH-funded program and to work towards program sustainability beyond CCH-funding.2. Organizations that have received more than one year of previous CCH funding may apply for funding up to $50,000 in support of their previously-funded CCH program and to work towards program sustainability beyond CCH-funding.3. Programs should be designed with funding for equipment less than 30% of the total budget for equipment that is integral to the specific program design and not for equipment that might support other ongoing, routine programs run by the organization. Equipment costs over 30% of the total funding request will not be considered. Incidental equipment and costs necessary to complete or sustain a project are allowed.4. The Foundation will not fund vehicle purchases.5. Costs over 10% in the budget category "Other" will not be considered.Program Design: 1. Exclusions. The Foundation does NOT fund programs that focus exclusively on the following: o Capital investments and unsolicited capital campaigns o Media/awareness campaigns o Hospital software systems, hospital in-patient programs or enhancement of existing hospital services o Professional education and/or training for healthcare professionals that is more than incidental to the program o Research or clinical trials o Healthcare provider/cardiologist salaries o Faith-based programs that are NOT open to the community-at-large2. The Foundation does NOT support: o Cardiovascular initiatives outside the United States or its territories o Individuals o For-profit organizations o Endowments o Journals or advertising o Political causes, lobbying, fraternal or social organizations o Religious organizations whose activities are not open to the general public o Nonprofit organizations that discriminate on the basis of age, race, color, religion, national origin, gender identity, sexual orientation, marital status, military service, veteran status or disability.3. Program Design. Invited organizations will need to apply with the same program they applied for, and were awarded funding for, in 2018. The program design will need to remain consistent with their approved and funded 2018 CCH Program Grant application, unless they received prior approval from the AstraZeneca HealthCare Foundation to revise their program design and/or outcomes.4. The Foundation seeks programs that: o Are focused on measurable results. The program should demonstrate clear, defined and designated outcomes, measurements and processes, and include clinical outcome goals or outcome goals for standardized testing (e.g., Progressive Aerobic Cardiovascular Endurance Run test, etc.). To support these efforts, it is recommended the organization utilize: a) Healthcare providers, or trained community health workers/health coaches as appropriate, to collect clinical metrics. b) An outside evaluator to analyze and evaluate data, which may include staff who are internal to the organization with the necessary expertise but who are not involved in the program's operations, management or delivery. o Provide an innovative approach. The program should find innovative ways relevant to the program, population and goals to maximize benefits for the participants and impact to the community. o Demonstrate sustainability. Programs should be able to continue beyond the potential grant period and potential AstraZeneca HealthCare Foundation funding. o Measurable Outcomes and Participant goals. Grant applicants will need to: a) Collect and measure program outcomes. b) Maintain or exceed their 2018 CCH Program Grant reached and tracked participant goals. c) Maintain or exceed their 2018 CCH Program Grant programmatic goals that are connected to a community cardiovascular health need(s). d) Demonstrate measurement outcomes at both the mid-year (May 2020) and year-end (November 2020) time frames following the grant award. e) Report on program outcomes for approximately half of the planned unduplicated tracked participants by May 2020. f) Demonstrate quality and robustness in measurement and evaluation efforts.Program Execution:1. Prior written approval from the Foundation is required from Grant Awardees prior to making a major change in program objectives, budget or scope. Some examples include, but are not limited to, program location, target audience, number of participants reached and tracked, and behavioral and/or clinical measures.2. Award recipients will be required to submit regular program reports and other requirements to the Foundation according to deadlines and processes set forth in the Addendum to the Letter of Agreement. These requirements include: o Three-month progress report submitted by January 24, 2020 (covers progress from November 2019 through January 2020) o Mid-year progress report, sustainability efforts and program outcomes submitted by May 29, 2020 (covers progress from November 2019 through May 2020) o Comprehensive year-end report, sustainability efforts and program outcomes submitted by November 13, 2020 (covers progress from November 2019 through October 31, 2020)Note: Criteria for assessing applications may be changed or updated by the AstraZeneca HealthCare Foundation from year to year. Application cycles are planned on a year-by-year basis.
Part XV, Line 3aThe AstraZeneca HealthCare Foundation's (AZHCF) overall mission is to promote public awareness and education of healthcare issues and support nonprofit organizations consistent with its charitable purpose. AZHCF's signature program, Connections for Cardiovascular Health (CCH), was launched in 2010 to provide grants and capacity building to nonprofit and other tax-exempt organizations working in innovative ways to help improve cardiovascular health in communities across the United States. In 2019, the CCH program marked its 10th year of grantmaking. Since the inception, CCH-funded programs have collectively reached over 1.7 million individuals and tracked over 65,000 participants for progress. Eleven organizations that received funding in the third quarter of 2018 and implemented their programs through the fourth quarter of 2019 reached over 18,500 individuals and tracked over 5,500 new participants for progress toward improved cardiovascular health. The AZHCF is also committed to sharing lessons learned for CCH Grant Awardees and other interested organizations. Grant funding dedicated for dissemination activities in 2019 resulted in CCH Grant Awardees presenting at 10 state and national conferences. This includes seven organizations who presented at the American Public Health Association (APHA) 2019 Annual Meeting & Expo, through oral presentations, poster sessions and the Global Public Health Film Festival.Additional efforts to support capacity building for CCH Grant Awardees included two Learning Forums, with sessions focused on reporting and evaluation, communications, sustainability and mentorship. Mentorship was a new component this year with a mentorship pilot program, where three newly-funded organizations were paired with several longstanding CCH Grant Awardees, to help them get their programs off to successful starts and provide guidance along the way.The AZHCF shared its own lessons learned through the development and distribution of the Effective Program Practices Guide, a webinar with the Society for Public Health Education, and our own presentation and short film highlighted at the APHA 2019 Annual Meeting & Expo alongside our Grant Awardees.
