Schedule H, Part V, Section B, Line 3EThe significant health needs are a prioritized description of the significant health needs of the community and identified through the CHNA.
Schedule H, Part V, Section B, Line 5 Facility , 1Facility , 1 - Jupiter Medical Center ("JMC"). In conducting its most recent community health needs assessment ("CHNA"), Jupiter Medical Center obtained input from 9 leaders representing public health, healthcare organizations, social services, and community leaders through key stakeholder interviews and focus groups. Primary input was also obtained by conducting an online community health survey distributed to members of the community. The process for gathering and analyzing this data spanned from October 2021 to June 2022. JMC consulted with various community organizations in conducting this CHNA, including: -FoundCare, Inc. -Health District of Palm Beach County -Healthier Jupiter -Limestone Creek Community -MyClinic -Palm Beach County Health Department -Palm Health Foundation -Town of Jupiter The stakeholders provided valuable feedback regarding 1) The health and quality of life in the community; 2) Underserved populations and communities of need; 3) Barriers to improving health; and 4) Most important health and quality of life issues. In order to develop a broad understanding of community health needs, JMC conducted a community survey during January and February of 2022. A link to the survey was distributed via e-mail, social media and word of mouth to the community at-large. The survey inquired about 1) health issues that most impact the community; 2) weaknesses in the community; and 3) what would improve the quality of life in the community, in addition to obtaining information about respondents' health habits and behaviors. A total of 2,111 surveys were completed and included members of underserved and minority populations. Populations with the most serious unmet healthcare needs identified in the CHNA process include low-income/uninsured individuals, Black and Hispanic populations, migrant workers and workers in service industries. Secondary data was assessed, including demographics, socioeconomic indicators, and key health indicators.
Schedule H, Part V, Section B, Line 11 Facility , 1Facility , 1 - Jupiter Medical Center ("JMC"). Based on the information gathered through the 2021 (FYE 9/30/2022) Community Health Needs Assessment, Jupiter Medical Center chose the needs below to address over the next three years: 1) Access to Services 2) Access to Primary Care Providers 3) Access to care for Chronic Diseases JMC has undertaken the following activities to address Access to Services: 1) Epitomizing JMC's commitment to providing patients with the entire continuum of leading-edge surgical care, the Johnny and Terry Gray Surgical Institute began serving patients in early 2024, less than two years after construction on the facility first began. This 90,000 square foot facility features the latest in hybrid technology to accommodate complex surgeries with real-time consultations and a comprehensive approach to care. The facility's sixteen "smart" operating rooms accommodate complex surgeries, while the two hybrid suites offer surgeons the ability to transition from a minimally invasive surgery to an open procedure, enhancing the patient experience and improving outcomes. The "operating rooms of the future" use state-of-the-art equipment that is digitally integrated. The surgical team is able to access and transmit a patient's images from the operating room to additional specialists for real-time consultation, resulting in a multi-disciplinary, comprehensive approach to care. The Institute provides a spacious reception area for loved ones along with private consultation rooms to meet the surgeon before or after a procedure. Recognizing the importance of real-time communication, leading-edge technology is integrated into the Institute's design, providing seamless updates from the care team to a patient's family. JMC further enhanced its surgical capabilities in Spring 2024, becoming one of the nation's first facilities to acquire the da Vinci 5 Robotic Surgical System and bringing JMC's surgical robot count to six. During FYE 9/30/2024, JMC celebrated the milestone of 10,000 robotic procedures, highlighting JMC's commitment to providing minimally-invasive, complex care. 2) To increase Emergency Department capacity, JMC completed the expansion of the Emergency Department in January 2023 by adding eleven rooms and increasing capacity by 30%. The renovated facility also includes eight Fast Track bays to treat patients with non-life-threatening ailments more quickly. The renovation and expansion features advanced clinical equipment, spacious patient and triage areas, and expansive waiting areas for loved ones. The center also features cutting-edge technology, including a dedicated CT scanner with True Fidelity technology that utilizes Artificial Intelligence to perform deep learning image reconstruction. The collective improvements will improve quality outcomes and decrease wait times. 3) To enhance the hospital's capacity for providing comprehensive inpatient care, JMC is constructing a new 92-bed patient tower on the main JMC campus. JMC received final approval for this project from the Town of Jupiter in April 2023 and plans to open in early 2026. This new patient tower will feature an Oncology Floor, a Cardiac Floor, and two Orthopedic Floors, in addition to housing specialized simulation labs for critical care, medical/surgical, and labor and delivery/NICU environments. 4) To increase JMC's general capacity to serve its community, construction of a new 53,000 square foot hospital facility began in 2023. This facility, referred to as the Neighborhood Hospital at Avenir, will feature 24-hour emergency services with 10 emergency room beds, 29 inpatient beds, operating rooms, a diagnostic laboratory, and imaging services. This expansion will provide the community with conveniently-located expanded access to essential emergency and surgical care, as well as inpatient nursing and rehabilitation services. With plans to open in 2026, the Neighborhood Hospital at Avenir will also include dedicated medical office space for both primary and specialty care providers, featuring services such as: -Primary Care (traditional and concierge) -Specialty Care (Cardiology, Oncology, Pulmonology and more) -Women's Imaging -Physical Therapy -Phlebotomy JMC has undertaken the following activities to address Access to Primary Care Providers: 1) To increase the number of primary care physicians in Jupiter Medical Center Physician Group, JMC is actively recruiting physicians both locally and outside of the immediate service area. The number of physicians employed by the Jupiter Medical Center Physician Group nearly doubled from 2022 to 2023, expanding the strata of individuals cared for by Jupiter physicians. 2) JMC continues to partner with HealthyMe (formerly MyClinic), a clinic that provides free comprehensive primary care services to patients who demonstrate a financial need. HealthyMe served more than 4100 individuals in 2023. JMC provides the Executive Director and Business Manager for HealthyMe. 3) The Neighborhood Hospital at Avenir, JMC's new hospital facility on track to open in 2026, will include medical office space for primary care providers. JMC has undertaken the following activities to address Access to care for Chronic Diseases: 1) In November 2022, JMC began offering next-day oncologist appointments for newly diagnosed cancer patients. Patients can call the Anderson Family Cancer Institute for a next-day appointment with a cancer specialist, empowering them and their families to jump start their journey to beat cancer. Since opening the Anderson Family Cancer Institute in March 2020, JMC has hired oncologists who specialize in certain types of cancers, including breast and gynecologic, head and neck, genitourinary, lung, gastrointestinal, blood, and rare neuroendocrine cancers. In 2023, JMC welcomed five physicians to the cancer team, building upon JMC's expertise across medical, surgical and radiation oncology. Technology has also been added to JMC's oncology department, including a Varian TrueBeam linear accelerator, to expand the radiation treatment options available to cancer patients. In 2023, JMC expanded its imaging and diagnostic capabilities to ensure the highest accuracy in cancer detection. At the Margaret W. Niedland Breast Center, JMC launched 3D Contrast Enhanced Mammography, a groundbreaking technology that identifies breast cancer earlier. The technology offers more detailed imaging and allows for more flexibility for women with pacemakers or those who cannot tolerate an MRI. Additionally, JMC hosts a Cancer Wellness Program in which various JMC departments collaborate to assist patients in managing fatigue, exercise/rehabilitation, and nutrition. JMC also supplements clinical cancer care through support groups, health fairs and screenings, and other educational community events. In January 2024, JMC announced an innovative new partnership with My Gene Counsel, a digital health company specializing in scaled genetic counseling and testing. This collaboration marks a pioneering step for JMC as it becomes the first clinical site in the region to integrate digital tools for all patients who have been seen for genetic counseling. My Gene Counsel's Living LabReport will provide JMC patients with updated genetic counseling information as new disease details are discovered, precision medicines are developed, guidelines evolve, and genetic variants are reclassified. The availability of these services at JMC breaks down barriers to care and provides every patient with cancer, or a family history of cancer, the opportunity to have genetic testing that could positively impact their cancer journey. In response to a growing demand for access to specialized and emerging treatments for urological conditions, JMC prioritized the creation of a dedicated urological care center. In June 2024, the Barb and Joe Charles Center for Urology was opened in the Anderson Family Cancer Institute on the hospital's main campus. The center operates on a multidisciplinary model so patients can receive the best quality care in an environment that prioritizes personalized treatment and superior outcomes. In September 2024, JMC became one of the first in the region to administer a first-in-class gene therapy for bladder cancer. This therapy is the first and only FDA-approved gene therapy for non-muscle invasive bladder cancer, benefitting patients who are unresponsive to standard treatments and providing a path for more targeted and effective interventions.
Schedule H, Part V, Section B, Line 11 Facility , 2Facility , 2 - Jupiter Medical Center ("JMC") (cont.). 2) JMC expanded the Structural Heart Program in 2023, giving patients more options for heart and vascular care that are less invasive and require less recovery time. Adding to its expertise in innovative Structural Heart procedures, JMC now offers transcatheter aortic valve replacement (TAVR), a minimally invasive procedure used to replace the aortic valve in people with severe aortic stenosis. The multidisciplinary group of experts of JMC's Structural Heart Program have treated conditions such as aortic valve stenosis, aortic insufficiency, mitral valve regurgitation, tricuspid valve regurgitation, hypertrophic cardiomyopathy, and holes in the heart, including Atrial Septal Defect (ASD) and Patent Foramen Olvale (PFO). Additional physicians were added to this team of experts during 2023. JMC's Atrial Fibrillation (AFib) Center of Excellence focuses on providing relief to patients through a host of modern techniques, offering various options to help patients manage Afib and reduce long-term reliance on blood thinners. In 2023, JMC expanded the electrophysiology program to integrate minimally invasive cutting-edge treatments for abnormal heart rhythms. This robust program now includes a multidisciplinary team of specialists experienced in a wide range of pharmaceutical, minimally invasive, and surgical options. Included in the skillset of the JMC Afib Center of Excellence team is the WATCHMAN implant, a minimally-invasive procedure that offers an alternative to long-term blood thinner medication. In 2024, JMC completed its 200th WATCHMAN implant procedure. 2024 also saw the JMC cardiac team perform the region's first robotic mitral valve replacement surgery, a minimally-invasive procedure replaces damaged mitral valves with prosthetic ones. JMC provides several minimally-invasive valve replacement options, including the Harmony transcatheter pulmonary valve (TPV), the first FDA-approved system explicitly designed for severe pulmonary regurgitation. The benefits of the JMC Afib Center of Excellence are felt even beyond the local community served by JMC, as this center serves as a national teaching and training site for cardiology and electrophysiology physicians. JMC was among the first in the region to provide Barostim Baroreflex Activation Therapy, the world's first FDA-approved heart failure device utilizing neuromodulation to alleviate symptoms in heart failure patients. Highlighting JMC's dedication to leading the way in cardiac care, JMC was also the first in Palm Beach County to adopt Boston Scientific's AGENT drug-coated balloon for in-stent restenosis (ISR) and the first in the tri-county area to implement the Shockwave C2 Coronary Lithotripsy balloon for intra-coronary lithotripsy. For patients with AFIB seeking alternatives to long-term anticoagulants, JMC electrophysiologists offer the Amplatzer Amulet LAA Occluder, which permanently closes a primary stroke pathway. 3) JMC continues its partnership with Lox Farms Fresh Rx program to increase availability to and consumption of fresh produce among cancer and cardiac patients in the community. Qualified participants receive a weekly bag of produce for 12 weeks along with educational materials (in English and Spanish) related to food storage, recipes, and resources. Other health needs identified through the CHNA process that are not specifically being addressed are described below, along with an explanation of why these needs are not being specifically addressed. The following needs are addressed through Access to Services and Treatment of Chronic Diseases. These needs received a relatively low priority per community input obtained through key stakeholder interviews and the community survey. 1) Affordability of health care 2) Obesity 3) Preventative care 4) Food insecurity/low food access The following needs are addressed through JMC's comprehensive services and/or educational outreach. These needs also received a relatively low priority per community input obtained through key stakeholder interviews and the community survey. 1) Adult Mental Health 2) Asthma in Children 3) Health Equity 4) Pediatric Mental Health 5) Unintentional Injuries 6) Youth Vaping The following needs are outside the scope of services provided by JMC, but opportunities may exist for JMC to support organizations addressing these needs. 1) Pedestrian/Biker Safety 2) Poverty Among Children 3) Violence/Safety 4) Substance Use 5) Transportation

Schedule H, Part V Joint venture that operates a hospital facilityThe organization participates in a joint venture that operates a hospital facility. However, under the final regulations of section 501(r), the organization does not treat this facility as a hospital facility for section 501(r) compliance purposes because it does not have sufficient control to ensure that the facility furthers an exempt purpose under section 501(c)(3). Accordingly, the organization treats its share of income as unrelated business income under section 513. The facility is not included in Part V, Sections A or B of Schedule H, but is disclosed here for completeness and transparency.
Schedule H, Part I, Line 7g Subsidized Health ServicesNone of the subsidized health services reported on Line 7g are attributable to physician clinics.
Schedule H, Part I, Line 7 Costing Methodology used to calculate financial assistanceTHE COSTS IN THE TABLE WERE COMPUTED USING THE ORGANIZATION'S COST-TO-CHARGE RATIO. THIS RATIO WAS DETERMINED USING IRS SCHEDULE H, WORKSHEET 2. THE AMOUNTS REPORTED AT LINE 7 ARE COMPUTED ON THE BASIS OF THE IRS SCHEDULE H WORKSHEETS FOR EACH RESPECTIVE LINE ITEM.
Schedule H, Part III, Line 2 Bad debt expense - methodology used to estimate amountFOR FINANCIAL STATEMENT PURPOSES, THE ORGANIZATION HAS ADOPTED ACCOUNTING STANDARDS UPDATE NO. 2014-09 (TOPIC 606). IMPLICIT PRICE CONCESSIONS, WHICH INCLUDE BAD DEBTS, DIRECTLY REDUCE THE AMOUNT OF PATIENT SERVICE REVENUE REPORTED ON THE ORGANIZATION'S AUDITED FINANCIAL STATEMENTS. THEREFORE, BAD DEBTS ARE INCLUDED IN NET PATIENT REVENUE AND BAD DEBT EXPENSE IS NOT SEPARATELY REPORTED AS AN EXPENSE.
Schedule H, Part III, Line 3 Bad Debt Expense MethodologyDue to the fact that the organization has adopted Accounting Standards Update No. 2014-09 (Topic 606) for financial statement purposes, the organization reports $0 bad debt expense because bad debts are included in net patient revenue. Therefore, the amount of bad debts attributable to patients eligible under the organization's financial assistance policy is also $0.
Schedule H, Part III, Line 4 Bad debt expense - financial statement footnoteJUPITER MEDICAL CENTER INC. IS INCLUDED IN CONSOLIDATED FINANCIAL STATEMENTS. BELOW IS THE TEXT OF FOOTNOTE REGARDING BAD DEBT EXPENSE: The initial estimate of the transaction price is determined by reducing the standard charge by any contractual adjustments, discounts, and implicit price concessions. Subsequent changes to the estimate of the transaction price are generally recorded as adjustments to patient service revenue in the period of the change. For the years ended September 30, 2024 and 2023, no significant adjustments to revenue were recognized due to changes in the estimates of implicit price concessions for performance obligations satisfied in prior years. Subsequent changes that are determined to be the result of an adverse change in the patient's ability to pay are recorded as bad debt expense.
Schedule H, Part III, Line 8 Community benefit & methodology for determining medicare costsJupiter Medical Center, Inc. utilizes the Medicare cost report to report the total revenue received from Medicare and the Medicare allowable costs. Jupiter Medical Center, Inc. provides services under governmental programs as a community benefit and as part of its mission; in many cases, this results in accepting payment for those services at less than cost. For this reason the organization believes the shortfall on Medicare services should be considered community benefit.
Schedule H, Part III, Line 9b Collection practices for patients eligible for financial assistanceJupiter Medical Center has a collections policy which is followed for patients who are known to qualify for financial assistance, which includes review of traditional financial assistance information, including submitted tax returns, sources of income, available credit and other demographic information. JMC has instituted a financial assistance policy which allows for a full write off of account balances without a patient application for care based upon review of available consumer credit scores and certain demographic information gathered at the time of registration. All collection activity ceases on patient encounters when the patient is believed to qualify for financial assistance.

Schedule H, Part VI, Line 2 Needs assessmentJupiter Medical Center works closely with and supports HEALTHYME (FORMERLY MYCLINIC), an independent not-for-profit free clinic that provides a wide range of medical services to residents of Palm Beach County with limited financial resources, in responding to needs of low-income and uninsured. JMC also conducts many health fairs, support groups and education events throughout the community where residents can provide insight into community health needs. Lastly, a Community Health and Benefit Committee of the Board of Directors was formed during the FY19 to include community members and board members. The committee is focused on identifying and addressing community health needs.
Schedule H, Part VI, Line 3 Patient education of eligibility for assistanceWhen Jupiter Medical Center registers a patient, insurance/guarantor information is sought. If a patient does not have insurance, JMC employs financial counselors who are trained in helping patients qualify for financial assistance under the federal, Hill-Burton program, State of Florida Medicaid assistance, local Palm Beach County Health District care along with advising on JMC's financial assistance policies. Financial counselors advise the patient on completion of paperwork necessary to receive assistance. In addition, JMC has partnered with a financing institution to provide patients of additional means the ability to finance patient care at a 0% interest rate provided that payments are made over a 12 month period. Further, the organization provided its financial assistance policy and plain language summary on its website, and upon request at the hospital facility. In addition, individuals were notified about the FAP by being offered a copy of the plain language summary and by the conspicuous public displays that demonstrated information on financial assistance.
Schedule H, Part VI, Line 4 Community informationJMC is located in Jupiter, Florida. A majority of the patients served by the hospital reside in northern Palm Beach County and southeast Martin County. Palm Beach County makes up 1,970 square miles and Martin County makes up 543 square miles. Palm Beach County is the state's third largest county by population while Martin County is the 32nd largest county out of 67 counties in the state of Florida. According to the most recent available U.S. Census data dated April 1, 2020, the population for both counties has experienced significant growth since 2010 with reported population percentage increases of 13.04% for Palm Beach County and 7.88% for Martin County from April 1, 2010 to April 1, 2020. The elderly population (Age 65+) makes up 23.54% of JMC's Primary Service Area and 28.65% of JMC's Secondary Service Area. These percentages are consistent with the populations in Palm Beach and Martin Counties, but they are significantly higher than state and national percentages. The Caucasian population makes up 77.25% of the population, with Hispanic/Latino population accounting for 11.65%, Black/African American 6.14%, and all others at less than 5%.
Schedule H, Part VI, Line 5 Promotion of community healthJMC's governing body is comprised of lay persons and physicians who reside in the organization's primary service area who are neither employees nor contractors, nor family members thereof. JMC extends medical staff privileges to all qualified physicians in the community for some or all of its service lines. JMC constantly looks at community needs in order to determine what service line should be expanded and equipment reinvestments should be made to better serve our primary service area. For example, JMC's hospitalist program was created in response to JMC's finding that many hospital patients did not have a primary care physician. JMC also continues to invest in oncological services as new oncologists quickly reach capacity. The organization offers medical education to the community and has several ongoing medical research studies.
Schedule H, Part VI, Line 6 Affiliated health care systemJUPITER MEDICAL CENTER, INC IS AN AFFILIATE OF JUPITER HEALTH, INC. AND JUPITER MEDICAL CENTER PHYSICIANS GROUP, INC.
