$1,237,848$0$1,250COMMUNITY EDUCATION - AS THE COMMUNITY'S HOSPICE, AGRACE HOSPICECARE IS DEDICATED TO INCREASING AWARENESS AND UNDERSTANDING OF MATTERS RELATED TO DEATH, DYING, CARE GIVING, GRIEF AND LOSS. WITH OUR GUIDANCE, OUR NEIGHBORS ARE BETTER PREPARED TO MAKE DECISIONS ABOUT THE CARE THEY OR THEIR LOVED ONES RECEIVE. IN A TYPICAL YEAR, MORE THAN 155 NURSING AND MEDICAL STUDENTS, RESIDENTS AND FELLOWS, PHARMACY STUDENTS AND STUDENTS FROM OTHER HEALTH CARE PROFESSIONS PARTICIPATED IN ROTATIONS AT AGRACE. SINCE THE INITIATION OF THE AGRACE ROTATION PROGRAM IN 1995, APPROXIMATELY 2,515 STUDENTS, INTERNS, RESIDENTS, FELLOWS AND WORKING HEALTH CARE PROFESSIONALS FROM INSTITUTIONS OF HIGHER LEARNING HAVE COMPLETED AN EDUCATIONAL EXPERIENCE AT AGRACE. AGRACE RECEIVES NO REIMBURSEMENT FOR THESE ROTATIONS WITH THE EXCEPTION OF AN ANNUAL STIPEND FROM THE UW-MADISON SCHOOL OF PHARMACY.
$1,111,515$0$13,538SPIRITUAL & GRIEF SERVICES - AGRACE HOSPICECARE'S SPIRITUAL & GRIEF COUNSELING PROGRAM PROVIDES SERVICES TO PATIENTS, FAMILY MEMBERS AND COMMUNITY MEMBERS. IN 2020, AGRACE HOSPICECARE PROVIDED GRIEF SERVICES TO 2,075 INDIVIDUALS - BOTH CHILDREN AND ADULTS. The Agrace Grief Support Center in Fitchburg, Wis., hosts grief support groups and one-on-one support. During 2020, our grief specialists and counselors provided support via phone and virtual meetings while the Center was closed due to the COVID pandemic. Still, 347 adults and children were served through community grief support throughout Agrace's entire service area. IN ADDITION TO INDIVIDUAL COUNSELING, AGRACE HOSPICECARE PROVIDES GRIEF SERVICES THROUGH GRIEF SUPPORT GROUPS, HOLIDAY REMEMBRANCE PROGRAMS, AND THE COMMUNITY TRAUMATIC DEATH RESPONSE TEAM. THESE PROGRAMS ARE OFFERED FOR A NOMINAL FEE, AND GRIEF SUPPORT GROUPS ARE OPEN TO ANYONE IN THE COMMUNITY EXPERIENCING GRIEF DUE TO A DEATH, NOT JUST TO PATIENTS AND FAMILIES SERVED BY AGRACE HOSPICECARE. BUTTERFLY AND BRICK MEMORIALS ARE AVAILABLE FOR INDIVIDUALS TO MEMORIALIZE OR HONOR THE LIFE OF A LOVED ONE. ALTHOUGH PROVIDING GRIEF SUPPORT IS REQUIRED BY STATE AND FEDERAL REGULATIONS, GRIEF SUPPORT SERVICES ARE NOT COVERED BY INSURANCE, MEDICARE, OR MEDICAID REIMBURSEMENT. AGRACE PROVIDES THESE SERVICES BECAUSE THE ORGANIZATION BELIEVES THEY ARE CRITICAL TO HELPING FAMILIES RETURN TO FUNCTIONAL LIVING AFTER THEIR LOSS.

Form 990, Part I, Line 1 BRIEF MISSIONAND FAMILIES FACED WITH LIFE-LIMITING ILLNESS, HELPING THEM TO LIVE AS FULLY AS THEY CAN. SIGNIFICANT MISSION-RELATED ACTIVITY TOOK PLACE IN 2020: Living wage for Agrace employees: As a large employer in southern Wisconsin, Agrace is committed to ensuring staff who rely on their employment with Agrace to support themselves and their families are earning livable wages. Investing in the financial security and quality of life of our staff is a natural extension of our mission, vision, and values. To that end, Agrace adopted pay practices in early 2020 to continue to pay every benefit-eligible employee a living wage. Additionally, when some Agrace Certified Nursing Assistants were temporarily called off work due to the pandemic, Agrace continued to pay them based on their normal hours. EXPANDING MISSION AND VISION: IN 2020, Agrace worked to expand our reach into the underserved and rural Prairie du Chien area of Crawford County. Adding to our service area brings local residents better access to high-quality end-of-life and supportive care, a move passionately supported by our staff who hail from and currently live in this area. Virtual Visits keep Agrace alongside patients and clients during pandemic: As COVID-19 prevented Agrace from entering some patients' homes and long-term care facilities, virtual visits using smartphones, iPads or laptop computers, video chats let our staff see our patients-and our patients see their loved ones. Agrace paused in-person grief support groups during the COVID outbreak, we continued to offer one-on-one grief support over the phone as well as virtual grief support groups. Cultural education: Agrace's manager of diversity and inclusion coordinated opportunities for staff and volunteers to learn more about the cultural and end-of-life traditions of the Hmong and Amish communities of southern Wisconsin. Memory Care Suites milestone: Agrace's Hospice Memory Care Suites at the Fitchburg campus marked its first anniversary in December. The Agrace Hospice Memory Care Suites are a homelike, 12-room secure unit that is specially designed to be comforting and safe for hospice patients who have dementia or other memory care needs. A new name for supportive care: When people are seriously ill and need ideas to help them feel and cope better, they don't want to be intimidated by more medical jargon. To help them embrace our palliative care service, Agrace changed the name from PalliaHealth to Agrace Supportive Care.
Form 990, Part III, Line 1 ORGANIZATION'S MISSIONAGRACE HOSPICECARE PROVIDES CARE TO PATIENTS WHO MEET CERTAIN CRITERIA UNDER THE ORGANIZATION'S CHARITY CARE POLICY WITHOUT CHARGE OR AT AMOUNTS LESS THAN ESTABLISHED RATES. SUCH PATIENTS ARE IDENTIFIED BASED ON FINANCIAL INFORMATION OBTAINED FROM THE PATIENT AT THE TIME THEY ENROLL IN THE PROGRAM OR SUBSEQUENT CHANGES TO THEIR FINANCIAL CIRCUMSTANCES. BECAUSE THE ORGANIZATION DOES NOT PURSUE COLLECTION OF AMOUNTS DETERMINED TO QUALIFY AS CHARITY CARE, THEY ARE NOT REPORTED AS REVENUE. AGRACE HOSPICECARE'S MISSION, "PARTNERING WITH PATIENTS AND FAMILIES TO IMPROVE QUALITY OF LIFE THROUGHOUT SERIOUS ILLNESS," IS FULFILLED BY THE COMPREHENSIVE, COMPASSIONATE CARE WE PROVIDE TO PATIENTS AND FAMILIES FACED WITH LIFE-LIMITING ILLNESS, HELPING THEM TO LIVE AS FULLY AS THEY CAN UNTIL THE END OF LIFE. IN 2020, THE ORGANIZATION PROVIDED CHARITY CARE OF APPROXIMATELY $411,322. MOST PATIENTS AND FAMILIES WANT TO RECEIVE CARE AT HOME. MORE THAN 90 PERCENT OF AGRACE HOSPICECARE SERVICES ARE DELIVERED IN PATIENTS' HOMES OR IN HOMELIKE SETTINGS SUCH AS COMMUNITY-BASED RESIDENTIAL FACILITIES, ASSISTED LIVING FACILITIES AND NURSING HOMES. AGRACE HOSPICECARE ALSO MAKES HOSPICE SERVICES AVAILABLE TO PATIENTS IN HOSPITAL SETTINGS. AGRACE HOSPICECARE PROVIDES ALL LEVELS OF CARE TO OUR PATIENTS AND MAINTAINS TWO INPATIENT UNITS FOR ACUTE, RESIDENTIAL AND RESPITE CARE.
Form 990, Part III, Line 4d Description of other program services(Expenses $ 1,237,848 including grants of $ 0)(Revenue $ 1,250) COMMUNITY EDUCATION - AS THE COMMUNITY'S HOSPICE, AGRACE HOSPICECARE IS DEDICATED TO INCREASING AWARENESS AND UNDERSTANDING OF MATTERS RELATED TO DEATH, DYING, CARE GIVING, GRIEF AND LOSS.
Form 990, Part III, Line 4d Description of other program services(Expenses $ 1,111,515 including grants of $ 0)(Revenue $ 13,538) SPIRITUAL & GRIEF SERVICES - AGRACE HOSPICECARE'S SPIRITUAL & GRIEF COUNSELING PROGRAM PROVIDES SERVICES TO PATIENTS, FAMILY MEMBERS AND COMMUNITY MEMBERS.
