Schedule H, Part V, Section B, Line 3ETO BETTER TARGET COMMUNITY RESOURCES ON THE SERVICE AREA'S MOST PRESSING HEALTH NEEDS, THE HOSPITAL PARTICIPATED IN A GROUP DISCUSSION WITH ORGANIZATIONAL DECISION MAKERS AND COMMUNITY LEADERS TO PRIORITIZE THE SIGNIFICANT COMMUNITY HEALTH NEEDS WHILE CONSIDERING SEVERAL CRITERIA: ALIGNMENT WITH ASCENSION HEALTH STRATEGIES OF HEALTHCARE THAT LEAVES NO ONE BEHIND; CARE FOR THE POOR AND VULNERABLE; OPPORTUNITIES FOR PARTNERSHIP; AVAILABILITY OF EXISTING PROGRAMS AND RESOURCES; ADDRESSING DISPARITIES OF SUBGROUPS; AVAILABILITY OF EVIDENCE-BASED PRACTICES; AND COMMUNITY INPUT. THE SIGNIFICANT HEALTH NEEDS ARE A PRIORITIZED DESCRIPTION OF THE SIGNIFICANT HEALTH NEEDS OF THE COMMUNITY AS IDENTIFIED THROUGH THE CHNA. SEE SCHEDULE H, PART V, LINE 7 FOR THE LINK TO THE CHNA AND SCHEDULE H, PART V, LINE 11 FOR HOW THOSE NEEDS ARE BEING ADDRESSED.
Schedule H, Part V, Section B, Line 5 Facility , 1Facility , 1 - Ascension St. John Hospital. Ascension Southeast Michigan Community Health was responsible for leading the Community Health Needs Assessment (CHNA) process for all Ascension Southeast Michigan hospitals. The Center for Population Health, Southeastern Michigan Health Association (SEMHA) was contracted to provide extensive local, national, state, and regional health data and statistics to assist the CHNA steering committee in prioritizing the needs of our service areas. Ascension Southeast Michigan began working on the CHNA on July 1, 2018 (FY 2019, Tax Year 2018). At the onset of our work, we convened our internal CHNA steering committee. The CHNA Steering Committee members included the VP of Community Health, the directors of community health, and two community health leads. The Association for Community Health Improvement (ACHI), Community Health Assessment toolkit, and IRS guidance was used as a guide for the process of completing the FY 2019 CHNA. Next, the steering committee agreed to hire the Center for Population Health to gather the secondary data for the Ascension Southeast Michigan service areas. The committee had several face-to-face meetings to define the community, compose the community health survey, distribution and implementation of the survey, and instruct the data consultant on the secondary data needs. The survey was distributed widely throughout the Ascension Southeast Michigan service area via the Ascension Southeast Michigan Community Health department. Paper and online surveys were sent to community members and key stakeholders from June 2018 - December 2018. The public survey was administered at the following: physician offices, community health fairs, community health wellness centers (Southfield, Riverview/Detroit, and Livingston), and the Annual Christmas store located at Ascension Providence Hospital, Southfield campus. The key stakeholders' survey was administered at the following, local public health department leaders, faith-based organization leaders, principals, and superintendents from the schools where our school-based health centers are located. Federally Qualified Health Center leaders, hospital presidents, other southeast Michigan health system community benefit leaders (i.e., Beaumont, Henry Ford), Ascension Southeast Michigan associates, and Ascension Southeast Michigan leadership. On April 1, 2019, a prioritization meeting of key stakeholders convened to provide guidance and oversight in the development of the CHNA priorities. The meeting attendees included the directors from the local health departments, Detroit, Macomb County, Oakland County, Livingston County, and St. Clair County. Also, individuals from a variety of health professions such as public health, physicians, nurses, finance, health planning, communications, behavioral health, and faith-based leaders. The secondary data, as well as primary data (community survey), enabled the meeting attendees to gain further insight into the needs and gaps in the hospital service area. To select the top health priorities, we utilized the Hanlon Method approach. The Hanlon method lists those health needs to be viewed as priorities based on baseline data, numeric values, and feasibility factors. The health departments provided public health experts about current health issues and trends in their populations with a focus on the uninsured and underinsured, low income, minority, senior citizen, and child populations. After completing the Hanlon Method prioritization with internal and external key stakeholders, the CHNA steering committee conducted a final prioritization assessment based on the following relevant factors for each Hanlon Method priority: 1. Feasibility/ability to make a measurable impact 2. List of known/existing evidence-based programs and/or other interventions 3. Potential partners/partnership opportunities The CHNA Steering committee finalized the top three priorities as the following: Obesity Reduction and Diabetes Prevention including high blood pressure, high cholesterol, diabetes, physical activity and fruit and vegetable consumption. Mental Health/Substance Abuse Prevention including depression, suicide, stress and anxiety, and substance abuse intervention. Improving Maternal/Infant Health including infant mortality prevention, adequate prenatal care, breastfeeding, safe sleep, and smoking cessation. The three priorities were approved by the Ascension Southeast Michigan Board of Trustees on June 17, 2019.
Schedule H, Part V, Section B, Line 6a Facility , 1Facility , 1 - Ascension St. John Hospital. The community health needs assessment was conducted with the following facilities: 1. Ascension River District Hospital 6. Ascension Brighton Center for Recovery
Schedule H, Part V, Section B, Line 11 Facility , 1Facility , 1 - Ascension St. John Hospital - Part 1. The priority needs identified in the 2019 CHNA are Obesity reduction and Diabetes Prevention, Mental Health/Substance Abuse, and Improving Maternal/Infant Health . For each selected priority, strategies have been developed that focus on outreach and education, evidence-based interventions, or advocacy. Prioritized Need: Obesity reduction and Diabetes Prevention. There are three goals for the prioritized need: obesity reduction and Diabetes Prevention. (1): Increase access to weight reduction programs/services for individuals living with excess weight (overweight and obesity) (*Overweight and obesity definition: Weight that is higher than what is considered as a healthy weight for a given height is described as overweight or obese. Body Mass Index, or BMI, is used as a screening tool for overweight or obesity.) The implementation strategy for Goal (1): Utilize the Ascension Southeast Michigan Wellness centers to implement physical activities through exercise classes, creation of dedicated walk paths on hospital campuses for associates and the public, and partnerships with other fitness organizations to increase awareness, knowledge, and behaviors for preventing and reducing obesity. (2): To identify pre-diabetic adults (age 18 years and older) and provide education, programs and services to delay the onset of type 2 diabetes. The implementation strategy for Goal (2): Implement the Centers for Disease Control and Prevention (CDC) Diabetes Prevention Program (DPP) as a community wide education and evidence-based intervention that prevents and reduces the complications of diabetes in communities, including those that are diverse and underserved. Goal (3): Identify children living with obesity in the Tri-county area (Oakland, Macomb, and Wayne) through a Ascension Southeast Michigan school-based health center. The implementation strategy for Goal (3): Implement 5-2-1-0 as an age-appropriate community wide education and evidence-based intervention that improves awareness, knowledge and behaviors for preventing and reducing obesity in communities, including those that are diverse and underserved. The obesity reduction and diabetes prevention priority area is being addressed by educating school-aged children on the prevention of obesity and educating adults about obesity and diabetes prevention by providing pre-diabetes education classes and offering physical activity classes through our wellness centers. Ascension Southeast Michigan Community Health department partnered with the National Kidney Foundation to teach the Center for Disease Control (CDC) Pre-Diabetes program. The Diabetes Prevention Program (DPP) is a structured, year-long, and group-based program that meets weekly for four months, then monthly for the remainder of the year. The goal of the DPP is to make it easier for people with pre-diabetes to participate in affordable, high-quality lifestyle change programs to reduce their risk of type 2 diabetes and improve their overall health. Evidence from the Diabetes Prevention Program demonstrated pre-diabetes risk could be reduced by 5-7% when those at high risk make modest lifestyle changes, explicitly losing 5-7% body weight and being physically active 5 days a week for at least 30 minutes. In FY 2020 (Tax year 2019), 5 DPP cohorts were completed, and 73% of adult participants decreased their weight by 5-7%, which is the recommendation from the CDC for reducing the risk of pre-diabetes. For school-aged children, an evidence-based program called 5-2-1-0 (5 or more servings of fruits and vegetables, 2 or fewer hours of recreational screen time, 1 or more hours of physical activity, and 0 sweetened beverages), which is a community-based, multi-setting childhood obesity prevention program designed for children up to the age of 18 was administered to 215 students in FY 2020. The six-week curriculum aims to: limit unhealthy choices for snacks and provide healthy options; limit or eliminate sugary drinks and increase water consumption; prohibit the use of food as a reward; provide opportunities to get physical activity every day and limit recreational screen time. At the end of the program, the students increase their knowledge of healthy eating and physical activity by 98% and increase their healthy diet and exercise by 96%. Prioritized Need: Mental Health/Substance abuse prevention. There are two goals for the need: mental health/substance abuse prevention. (1): Provide access to mental health programs/services for children, youth, and adults. The implementation strategy for Goal (1): Increase access to mental health programs/services for children, youth, and adults, through the Ascension Southeast Michigan Community health school-based health centers and referrals to Eastwood clinics from the Ascension Southeast Michigan hospitals and physician offices. (2): Decrease youth risk factors for suicide, depression and substance abuse. The implementation strategies for Goal (2): Strategy (1) Implement the Rapid Assessment for Adolescent Preventive Services (RAAPS) as a suicide risk screening and provide mental health education, counseling, and referral for youth in partner schools. Goal (2) Strategy (2): Implement Red Flags mental health education, counseling, and referral for youth in partner schools. The Mental Health/Substance Abuse prevention need is being addressed by increasing access to mental health programs/services for children and adults. For youth in particular, the focus is to screen them for depression and suicide. The Ascension Southeast Michigan School Based Health Centers utilizes the Rapid Assessment for Adolescent Preventive Services (RAAPS) as a risk screening for depression and suicide for students. In turn, based on the results students are counseled or referred externally for mental health treatment. In FY 2020 (Tax year 2019), 815 students were screened, 61 students counseled for depression, 20 students counseled for suicide, and 3 students referred for external mental health treatment. To increase access to mental health services, we expanded into 3 school-based health centers. Also, our Eastwood clinics that specialize in mental/behavioral health services converted over 35% of referrals to scheduled appointments. Prioritized Need: Improving Maternal and Infant Health. There is one goal for the need: improving maternal and infant health. Goal: Improve the health and well-being of women and infants. The implementation strategies for the goal are: Strategy (1) Implement the Maternal Infant Health Program (MIHP), which provides evidence-based services to improve awareness, knowledge, and behaviors for preventing maternal mortality and infant mortality for communities, including vulnerable and at-risk populations. Strategy (2) Increase access and participation to parenting classes for pregnant women and mothers with infants less than a year. The improving maternal and infant health priority is utilizing the Maternal Infant Health Program (MIHP) to address the needs of mothers and infants in southeast Michigan areas. The Maternal Infant Health Program, is an evidence-based approach to promoting the importance of healthy pregnancies, nutrition and breastfeeding and infant safety. In FY 2020 (Tax Year 2019), we worked to ensure mothers received adequate prenatal care (7 to 13 prenatal visits). 69% of our mothers achieved adequate prenatal care. Also, we also increased their knowledge on breastfeeding and safe sleep practices. The parenting classes serve at-risk pregnant women and their families in an effort to produce healthy maternal outcomes, as well as reduce the number of infant deaths at birth and during the first year of life. In FY 2020 we increased participation in our parenting classes over 25%. Needs That Will Not Be Prioritized In our FY 2019 CHNA planning, other needs were present from our primary and secondary data. However, after completing the Hanlon method the following are needs we decided we would not prioritize in this CHNA. Access to care was identified as a need by the Hanlon Method prioritization. However, the CHNA steering committee confirmed through the final prioritization assessment that it is a low priority need. Due to MI Medicaid expansion, a large amount of the population in the Southeast Michigan area have health insurance and a primary care provider. Also, it would be challenging to make a measurable impact on health insurance cost. Also, the utilization of a primary care provider, and routine check-ups are behavioral activities that can be encouraged through education from the main three priorities. However, we continue to collaborate with a local Federal Qualified Health Center to support access to specialty and dental care. A sub-category under Access to care is adequate prenatal care that will be addressed under the Improving Maternal/Infant Health priority.
Schedule H, Part V, Section B, Line 11 Facility , 2Facility , 2 - Ascension St. John Hospital - Part 2. Healthy behaviors priority included the sub-categories of smoking and safe sleep identified from the Hanlon Method prioritization. Based on the CHNA steering committee final prioritization assessment, this is a priority that coincides with the subcategories of the main three priority areas. It would be more feasible to address healthy behaviors under the top three priority areas. Other health issues identified by the Hanlon Method that were not selected as a top priority were cancer, cardiovascular disease, arthritis, and asthma. These health issues were identified but not selected as a priority because of the following: These health issues ranked lower on the Hanlon Method scoring than other health issues. The CHNA steering committee evaluated each issue and concluded that there is an inability to measure impact for cancer, cardiovascular disease, and arthritis. The health issue of Asthma continues to be addressed in our school-based health centers through our Asthma Camp and Deep Breathing programs.

Schedule H, Part VI, Line 2 Needs assessmentCommunities are dynamic systems in which multiple factors interact to impact the quality of life and health status. In addition to the formal CHNA conducted every three years, the Ascension Southeast Michigan Community Health department helps to lead a community round table whose purpose is to assess needs within the community, prioritize action and work in partnership to address identified challenges. The coalition works closely with its member organizations, which come from multiple sectors of the community, including local government, business, education, faith communities, public health, health care providers, and other social and human service organizations. Also, the coalition works closely with other coalitions as well as the local and state health departments to stay abreast of changing needs within the community and to identify evidence-based and promising practices to address these needs. Ascension Southeast Michigan Community Health assesses the health care needs of the communities by utilizing data from local, national, state, and hospital. The data and statistics were obtained from internal as well as external sources to identify health-specific trends. The reports from these sources included but not limited to: County Data (for all 5 counties we serve Wayne, Oakland, Macomb, Livingston, St. Clair and City of Detroit), Michigan Behavioral Risk Factor Surveillance System (MiBRFSS), Hospital Data (for Providence Hospital, Southfield and Novi Campus, Macomb-Oakland Hospital, Warren and Madison Heights campus, River District, and St. John Hospital), Emergency data and Ambulatory data. In addition to data, other ways Ascension St. John Hospital as part of Ascension Southeast Michigan Hospitals identifies the needs of the community include: Feedback from community partners on trends observed in the population seeking care/services in their agencies. Reports and other white papers received from local health departments, and other nonprofit health organizations such as the kidney foundation, American heart association greater Detroit area health council etc. Assessments of social determinant of health needs performed in resident clinics. Feedback from board members and advisory councils on trends and observations of community needs. Feedback from multidisciplinary Ascension associates such as case managers, strategic planning, etc. These sources aid in Ascension Southeast Michigan Community Health and Ascension Southeast Michigan hospitals development of programs and services throughout the five counties we serve.
Schedule H, Part VI, Line 3 Patient education of eligibility for assistanceAscension St. John Hospital as part of Ascension Southeast Michigan hospitals communicate with patients in multiple ways to ensure that those who receive services are aware of the hospital's financial assistance program as well as their potential eligibility for local, state, or federal programs. Signs are prominently posted in each service area, and bills contain a formal notice explaining the hospital's charity care program. Also, the hospital employs financial counselors, health access workers, and enrollment specialists who consult with patients about their eligibility for financial assistance programs and help patients in applying for any public programs for which they may qualify.
Schedule H, Part VI, Line 4 Community informationAscension St. John Hospital is part of Ascension Southeast Michigan Hospitals. Ascension Southeast Michigan hospitals, is a non-profit Catholic health system comprising five hospitals, including one specialty hospital located in a five-county area of southeastern Michigan. The health system campuses are organized as follows: Ascension St. John Hospital, Ascension Macomb-Oakland Hospital, Warren Campus, Madison Heights campus, Ascension River District Hospital, Ascension Brighton Center for Recovery Hospital, and Ascension Providence Hospital, Southfield Campus, Novi campus. Southeastern Michigan makes up slightly half of the state's population, with the majority concentrated in Metro Detroit. Ascension Southeast Michigan hospitals serve a five-county region of southeast MI that includes: Livingston, Macomb, Oakland, St. Clair, and Wayne County (city of Detroit). The total population for all five counties, including Detroit, is a little over 4 million. The percentage of persons living below poverty ranges from 5.2% to 34.5%. Overall, the population is quite diverse, made up of white, black, Hispanic or Latin, and multi-racial people.
Schedule H, Part VI, Line 5 Promotion of community healthAs part of Ascension Southeast Michigan Hospitals, Ascension St. John Hospital promotes the health of its communities by striving to improve the quality of life within the community. Research has established that factors such as economic status, employment, housing, education level, and built environment can all be powerful social determinants of health. Additionally, helping to create higher capacity within the community to address a broad range of quality of life issues also impacts health. Ascension Southeast Michigan Hospital administrators and staff have provided leadership and participated on collaborative initiatives with Greater Detroit Area Health Council, Wayne County Health Department, The City of Detroit Department of Health and Wellness Promotion, Michigan Crime Victim Services Commission, The Michigan Department of Health and Human Services, Michigan Primary Care Association, area Federally Qualified Health Centers, and other community organizations to identify community needs and address community problems. Ascension Southeast Michigan Hospitals have a demonstrated history and strong foundation built upon its Mission, Vision, and Values. It is this spirit that leads the institution and drives health care services. Often patients, family members, friends, and the like are pleased with our position of care - to heal the body, mind, and spirit, have experienced it first-hand, and wish to show their appreciation by making a gift to support the care of others that are vulnerable. The spiritual essence that's woven into the fabric of our daily operations allows members of the community to comfortably and directly impact the lives of others through philanthropy. We work to create an ethical legacy for emphasizing the critical value of community and encourage the compassionate generosity of others to make gifts that enhance the exemplary care that we have been recognized for in the health care system. The hospitals promote the health of the community in a variety of ways, either directly providing services or in partnership and/or collaboration with other organizations serving the community. We directly serve the community through 25 school-based health centers in the Southeast Michigan area. The schools are selected based on a minimum of 70% of students receiving free or reduced lunch, indicating a lower family income. Services to these students include physical and psychosocial support. We partner with a mobile dental service to provide onsite dental services to improve access to needed care. The school team consists of a nurse or nurse practitioner, a medical assistant, and a mental health therapist. There are special programs for students identified with asthma including an annual free asthma camp for students. Noting that heart disease is a leading cause of death in the entire hospital service area, the health system has an annual heart health fair at selected hospitals to provide onsite health screening and assessment by a team. Grief support and support for victims of crime are provided to children and youth through a special initiative at our Detroit based hospital. Partnerships with over 80 churches are a vehicle that provides health education and screening to the general community through the faith community nursing initiative. There are four community wellness centers offering enhanced fitness and other physical and educational sessions offered to the general population of three counties in the service area. In FY 2020, the four community wellness centers had over 17,000 encounters. Also, monthly programming and health education classes are offered at seven parks and recreation centers and senior centers in our service areas. Ascension has instituted a nationally recognized initiative to help eradicate human trafficking. Our team of specifically trained master's level social workers are subject matter experts in the human trafficking. As content experts, they provide training to Ascension emergency department staff, nurses on the inpatient units, case managers and social workers on how to identify possible victims, initiate human trafficking protocols and protect themselves from retaliation. We partner with other agencies serving the general community to promote health, including Gleaners food bank and Forgotten Harvest to address food insecurity. We partner with the local federally qualified health centers via participation in the Southeastern Michigan FQHC council and membership in the Michigan primary care association. We partner with the local chapter of the Kidney Foundation to provide diabetes prevention and diabetes management education in several community locations. Other partnerships/collaboration include Detroit Wayne Mental Health Authority, the local sane (sexual assault network), several domestic violence shelters, Southeastern Michigan infant mortality reduction task force, Greater Detroit Area Health Council, Detroit, Wayne, Oakland, St. Clair and Macomb health departments, youth connection and others to promote community health with particular attention to those who are poor and vulnerable. The health system publishes and distributes over 80,000 bimonthly educational newsletters called "CareLink" targeting adults 55 and older, which includes informative articles as well as a schedule of educational classes on topics such as CPR, diabetes, preventing falls, heart health, and other health topics. All classes are open to the public, and the majority are free of charge. Further, the health system has two mobile units that provide mammography services to uninsured and underserved women and a heart and vascular mobile unit that provides a basic heart profile at churches, community centers, corporations, and other locations. The structure of the health system calls for a statewide governing board that includes members from the local hospital service area. Further, we have several advisory committees composed of residents and agencies serving the community, such as the parent and separate student advisory groups for the school-based health centers, the local mission committee, the local foundation boards, and other services specific advisory groups. Medical staff by-laws for each hospital guide offering privileges to qualified physicians in the communities surrounding each of the hospitals. Each teaching hospital has a medical education department for the training of residents and fellows. Each site hosts an annual research day. This long standing tradition, which has been in existence for over 80 years at some of our sites, brings clinicians and graduate medical students together to discuss current medical research being conducted at the hospital. This work encompasses everything from retrospective medical chart review to analysis of the latest therapeutics and medical devices. The primary goal is to teach residents and fellow researchers how to research to improve patient care and advance the practice of medicine. Examples of this work include the following; "lack of screening for intimate partner violence in primary care settings"; an effort to reduce 30-day readmission rates in chronic obstructive pulmonary disease"; and "telemedicine" and "tighter blood pressure control" to name a few. Also, the medical staff participates in an annual summer biomedical exposure program for 20 area college students. The program introduces the students to the broad field of medicine. In all, the department supports ongoing research by residents, fellows, and physicians and provides Research and EIB review as well as oversight and guidance to remain in compliance with current rules and regulations.
