Form 990, Part VI, Line 11b: Form 990 Review Process	The form 990 is prepared by an independent accounting firm and reviewed internally by the COO and accounting manager. A final draft is sent to the Board's Audit & Finance comittee prior to the filing with the Internal Revenue Service.
Form 990, Part VI, Line 12c: Explanation of Monitoring and Enforcement of Conflicts	Members of the Board are required to read and sign a conflict of interest policy. Employes sign the employee handbook and conflict policy upon hire.
Form 990, Part VI, Line 15a: Compensation Review & Approval Process - CEO, Top Management	The compensation determination process for top officials includes an annual review by the Executive Committee to determine the CEO's compensation. The compensation process for all other employees includes an annual review to determine compensation.
Form 990, Part VI, Line 19: Other Organization Documents Publicly Available	The organization makes its governing documents, conflict of interest policy, and financial statements available to the public upon request.
Form 990, Part XII, Line 2: Change of Oversight or Selection Process	THE ORGANIZATION'S EXECUTIVE COMMITTE IS RESPONSIBLE FOR OVERSIGHT OF THE AUDIT, INCLUDING SELECTION OF THE INDEPENDENT ACCOUNTANT. THE PROCESS IS CONSISTENT WITH PRIOR YEARS.
