Form 990, Part I, Line 5 and Part V, Line 2aHFNY does not file any year-end employee tax filings because it outsources its human resources, payroll, and employee benefits administration to an outside professional employer organization. Accordingly, all employment documentation, including required payroll records, deductions forms, hiring documentation, and year end tax filings are prepared and maintained by the outside professional employer organization and not by HFNY. HFNY reimburses the professional employment organization for the employees' compensation and the reimbursements are reported on 990 Part VII, Section A and 990 Part IX, Lines 5-10.
Form 990, Part VI, Section A, line 8bThere are no committees with the authority to act on behalf of the governing body. Therefore, this question was answered no in accordance with the instructions.
Form 990, Part VI, Section B, line 11bForm 990 is prepared by an independent CPA firm and reviewed in detail by the organization's top management. The reviewed Form 990, not including Schedule B, is then provided to the board of directors prior to filing with the IRS.
Form 990, Part VI, Section B, line 12cThe organization has a written conflict of interest policy that provides that a person in a position of authority over the organization, such as an officer, director, or manager, may not benefit financially from a decision he or she could make in such capacity, including indirect benefits to family members or businesses with which the person is closely associated. Annually, our Board of Directors reviews and discloses any potential conflicts and signs the policy. The Chief Executive Officer and Executive Committee review the disclosures for potential conflicts. Should any potential conflicts of interest be disclosed, the board member or officer would be asked to refrain from participation in any deliberation or decision with regard to matters affected by the relationship.
Form 990, Part VI, Section B, line 15aThe compensation review of the Chief Executive Officer requires a review of compensation determinations by disinterested persons, the use of salary surveys to benchmark compensation for similar persons in functionally comparable organizations, and a contemporaneous substantiation of the deliberation and decision. The process was last completed during the fiscal year. 15b - The organization does not compensate any other officers or key employees. Therefore, this line was answered "no" in accordance with the instructions.
Form 990, Part VI, Section C, line 19The organization's governing documents, conflict of interest policy, and audited financial statements are available upon request.
