FieldValueIRS element (for citing)
Description1The Foundation is committed to providing grants & capacity building to U.S.-based organizations working to improve heart health in the U.S. through its Connections for Cardiovascular Health (CCH) / CCH Next Generation program for its Grant Awardees ("Awardees") and other interested parties. Evaluation services from university partners provide basis for understanding program outcomes & where capacity building needs to occur. Awardees submit program reports 3 times throughout the program cycle, & receive evaluations at each point, including recommendations for mid-course corrections to address program challenges. At mid-year, Awardees may also submit action plans to ensure they meet program goals by year-end. Awardees have access to Foundation staff & university partners to provide program assistance & collaboration for use of innovation & technology. At year-end, all 11 CCH-funded programs met or exceeded their objectives based on evaluations assessed in light of pandemic restrictions.Description1Txt
Expenses1$182,690Expenses1Amt
Description2The Learning Series & Mentorship program are 2 segments that provide additional capacity building & learning opportunities to Awardees. For the Learning Series, the Foundation held an Orientation Webinar for CCH Next Generation Awardees in August with a focus on program reporting, evaluation & communications, a Virtual Awards Recognition & Roundtable Event in September bringing together current & previous Awardees to learn from one another, & a Storytelling & Communications Webinar in November to help Awardees bring awareness to their programs. The Mentorship program has evolved out of 10 years of lessons learned & the successful launch of a pilot mentorship program in 2019. Peer-to-peer learning is fostered through pairing newly-funded organizations with longstanding Awardees to help them launch successful programs & work through implementation barriers through regular engagements & leveraging a curriculum developed by the Foundation, its university evaluation partners & the mentors.Description2Txt
Expenses2$172,048Expenses2Amt
Description3The Foundation held an open call for applications for our CCH Next Generation program, which leverages 10 years of learnings from the CCH program to support applicants in developing programs for potential funding through its Effective Program Practices Guide. As part of this, the Foundation held an informational webinar to support capacity building for potential applicants and to help them prepare an evidence-based program for the online application. The Foundation's Effective Program Practice Guide was shared as part of the application process and informational webinar.Description3Txt
Expenses3$30,005Expenses3Amt
Description4The Foundation shared its lessons learned through dissemination and communications activities such as social media, e-mail distribution to the Foundation's mailing list of over 2,000 individuals, and two presentations at the American Public Health Association 2020 Annual Meeting & Expo. The Foundation also released a 10-year Special Report, which can be found on our website, that reflects on 10 years of lessons learned from the CCH program.Description4Txt

New applicants have the opportunity to modify or adapt these learnings highlighted in the Effective Program Practices Guide when designing and implementing their own innovative, community-based heart health programs. The intent of the grants is to support innovative initiatives or programs at the community level to help improve cardiovascular health in the community during the grant year and beyond. Our aim is for funded programs to maximize the benefit to participants and to work toward sustainability beyond our funding. To qualify for a Foundation grant, an organization is required to be a U.S.-based, nonprofit organization with a 501(c) designation or a public school, government entity or municipal institution that is eligible to accept tax-deductible, charitable contributions. Organizations that are 501(c)(3) and also have an IRS designation as a 509(a)(3) are ineligible for funding. The nonprofit organization must be engaged in charitable work at the community level or otherwise. The program must address the Foundation's Connections for Cardiovascular HealthSM Next Generation mission and meet five key criteria:1. Address patient cardiovascular health issues within the United States and its territories2. Recognize and work to address an unmet need related to cardiovascular health in the community3. Respond to the urgency around addressing cardiovascular health issues, including cardiovascular disease or conditions contributing to cardiovascular disease4. Improve the quality of participants and non-professional caregivers' lives in connection with the services provided and work done5. Demonstrate a clear plan for long-term program sustainability, to further improve cardiovascular health after Foundation grants are expendedFoundation grant applications that meet the criteria and describe innovative initiatives with clearly defined outcomes, measurable results and processes will be considered for funding.Organizational Criteria:1. The organization and its program must be based in the United States and its territories.2. The Foundation provides grants to organizations that are: o Nonprofits with a 501(c) designation. (Organizations that have a 509(a)(3) designation are ineligible for funding.) o Public schools o Municipal institutions (e.g., city health department, municipal hospital or county board of health).3. Applying organizations will be required to provide 1) a copy of their current IRS 990 form or documents that establish eligibility to receive charitable donations and 2) audited financials for the past fiscal year. (If audited financials are not available, unaudited financial statements may be submitted. The CCH program preference is to provide funding to organizations that have both a current audited financial statement and IRS Form 990 available.)4. After submitting the application, organizations cannot make substantive changes to their tax status. Once selected for a grant award, organizations cannot reassign, transfer or credit their grant to another entity, including any related entity, without prior written approval by the Foundation. Any changes in an organization's tax status after submitting the application or being selected for a grant award must be reported immediately and may impact the organization's eligibility to receive the grant.Funding Criteria:The Foundation's preference is to support new organizations with innovative programs to expand the reach of the Connections for Cardiovascular HealthSM Next Generation program. In addition, the Foundation maintains our commitment to support sustainability by providing reduced funding to support a limited number of previously-funded organizations with programs that demonstrate positive outcomes. The Foundation will only accept one application per organization for a single year of funding. There is no guarantee of funding. Applicants who are approved for a Foundation grant award must meet program objectives. 1. The CCH Next Generation open call for applications will provide program grant funding to organizations that have never been funded before by the Foundation's Connections for Cardiovascular HealthSM program. CCH Next Generation may provide limited grant funding to previously-funded organizations on an invitation basis to support dissemination, sustainability and/or mentorship activities.2. Through evaluating results from community-based programs from Connections for Cardiovascular HealthSM, the AstraZeneca HealthCare Foundation and its evaluation partners have identified six innovative approaches commonly used to improve cardiovascular health at the community level. Applying organizations will need to align with one of six innovative approaches as the basis of their Connections for Cardiovascular HealthSM Next Generation program or identify their own innovative approach. The six innovative approaches include: 1. Leveraging access for uninsured and underserved participants to improve cardiovascular knowledge and health 2. Bringing programs to participants 3. Educating children to serve as heart health ambassadors 4. Improving cardiovascular health through food-based programs 5. Using health coaches/promotors to improve cardiovascular health 6. Providing culturally sensitive program interventions to maximize participant outcomes 3. Applicants are encouraged to consult the AstraZeneca HealthCare Foundation's Effective Program Practices Guide as a means of informing the proposed program's design relative to the selected innovative approach. 4. Program Grant funding requests must be between $125,000 and $150,000. 5. CCH Next Generation welcomes applications from every state in the nation and has a preference for funding applications from the top 20 states by cardiovascular disease mortality (Alabama, Arkansas, Georgia, Iowa, Indiana, Kentucky, Louisiana, Maryland, Michigan, Missouri, Mississippi, Nevada, New York, Ohio, Oklahoma, Pennsylvania, South Carolina, Tennessee, Texas, West Virginia). 6. Additional preference will be given to applications from states where the Foundation has never previously funded a program (Alabama, Arkansas, Iowa, Indiana, Nevada, Ohio, Oklahoma).7. Programs should be designed with funding for equipment less than 30% of the total budget for equipment that is integral to the specific program design and not for equipment that might support other ongoing, routine programs run by the organization. Equipment costs over 30% of the total funding request will not be considered. Incidental equipment and costs necessary to complete or sustain a project are allowed. 8. The Foundation will not fund vehicle purchases. 9. Costs over 10% in the budget category "Other" will not be considered.10. Any budget expenses allocated for healthcare provider compensation must only support a portion of each healthcare provider's annual salary for their work on the program and cannot be in addition to the healthcare provider's annual salary for the 16-month grant timeframe paid for by the applying organization. Program Design: 1. Exclusions. The Foundation does NOT fund programs that focus exclusively on the following: o Capital investments and unsolicited capital campaigns o Media/awareness campaigns o Hospital software systems, hospital in-patient programs or enhancement of existing hospital services o Professional education and/or training for healthcare professionals that is more than incidental to the program o Research or clinical trials o Healthcare provider/cardiologist salaries o Faith-based programs that are NOT open to the community-at-large2. The Foundation does NOT support: o Cardiovascular initiatives outside the United States or its territories o Individuals o For-profit organizations o Endowments o Journals or advertising o Political causes, lobbying, fraternal or social organizations o Religious organizations whose activities are not open to the general public o Nonprofit organizations that discriminate on the basis of age, race, color, religion, national origin, gender identity, sexual orientation, marital status, military service, veteran status or disability.3. The Foundation seeks programs that are focused on measurable results. The program should demonstrate clear, defined and designated outcomes, measurements and processes, and include clinical outcome goals or outcome goals for standardized testing (e.g., Progressive Aerobic Cardiovascular Endurance Run test, etc.). To support these efforts, it is recommended the organization utilize: a) Healthcare providers, or trained community health workers/health coaches as appropriate, to collect clinical metrics. b) An outside evaluator to analyze and evaluate data, which may include staff who are internal to the organization with the necessary expertise but who are not involved in the program's operations, management or delivery. c) Provide an innovative approach. The program should find innovative ways relevant to the program, population and goals to maximize benefits for the participants and impact to the community. d) Demonstrate sustainability. Programs should be able to continue beyond the potential grant period and potential AstraZeneca HealthCare Foundation funding. e) Measurable Outcomes and Participant goals. Grant applicants will need to: o Collect and measure program outcomes. o Be designed to reach at least 400 unduplicated participants by year-end. o Be designed to track at least 200 unduplicated participants by year-end. o Demonstrate measurement outcomes at both the mid-year (March 2021) and year-end (November 30, 2021) time frames following the grant award. o Report on program outcomes for approximately half of the planned unduplicated tracked participants by March 2021.Program Execution:1. Organizations that are awarded Mentor Grants in 2020 (Year One) may be eligible for a second year of continued funding in 2021 (Year Two), based on performance at the mid-year mark, availability of funds and Board approval. 3. 4. The Foundation will not fund vehicle purchases. 5. Costs over 10% in the budget category "Other" will not be considered. 6. Any budget expenses allocated for healthcare provider compensation must only support a portion of each healthcare provider's annual salary for their work on the program and cannot be in addition to the healthcare provider's annual salary for the 16-month grant timeframe paid for by the applying organization. Program Design:1. Program Design. Invited organizations applying for program funding will need to apply with the same program they were previously awarded funding for in 2019.*2. Part XV, Line 3aThe AstraZeneca HealthCare Foundation's (AZHCF) overall mission is to promote public awareness and education of healthcare issues and support nonprofit organizations consistent with its charitable purpose. AZHCF's signature program, Connections for Cardiovascular Health (CCH), was launched in 2010 to provide grants and capacity building to nonprofit and other tax-exempt organizations working in innovative ways to help improve cardiovascular health in communities across the United States. The Connections for Cardiovascular HealthSM Next Generation program launched in 2020, building on ten years of lessons learned and aims to expand to areas of the country with the greatest need and where our funding has not previously reached. In 2020, the CCH program marked its 11th year of grantmaking with the launch of its next iteration of the program, CCH Next Generation. Since the inception, CCH and CCH Next Generation funded programs have collectively reached over 1.7 million individuals and tracked over 67,000 participants for progress. Eleven organizations that received funding in the third quarter of 2019 and implemented their programs through the fourth quarter of 2010 reached over 11,000 individuals and tracked over 2,000 new participants for progress toward improved cardiovascular health. The AZHCF is also committed to sharing lessons learned for CCH Grant Awardees. Sharing key learnings is pivotal so that other organizations can use these insights in designing and implementing their own, innovative community-based cardiovascular health programs. AZHCF's Special Report, released in 2020, highlights the first decade of the CCH program through stories of the organizations and programs we have funded. The Foundation presented with longstanding CCH Grant Awardee, Catherine's Health Center (Grand Rapids, MI), on best practices for program sustainability and the benefits of health coaching models during two sessions at the American Public Health Association's 2020 Virtual Annual Meeting: o Sustainability of Cardiovascular Health Programs: Making Community Programs Tick After Funding Disappears o Health Coaches/Promotors: An Asset for Enhancing Retention and Recruitment and Facilitating the Feedback Loop Between Program Participants and ProvidersVisit our website's "Lessons Learned" section for more information: www.astrazeneca-us.com/foundation. The Foundation's Grant Awardees also have the opportunity to receive ongoing educational support with a virtual Learning Forum webinar series that aims to help organizations further build capacity as they implement their programs. Sessions in 2020 included: o An Orientation Webinar with a focus on reporting and evaluation to demonstrate program impact and communications to share program stories o A Virtual Awards Recognition and Roundtable on World Heart Day to bring together current and previous Grant Awardees to learn from one another, elevate awareness of cardiovascular disease and share ways to address it at the community level. Also included were two Learning Forums with sessions focused on reporting and evaluation, communications, sustainability and mentorship.
