$1,471,898$14,673COMMUNITY EDUCATION - AS THE COMMUNITY'S Nonprofit HOSPICE, AGRACE IS DEDICATED TO INCREASING AWARENESS AND UNDERSTANDING OF MATTERS RELATED TO serious illness, DEATH, DYING, CARE GIVING, and GRIEF. WITH OUR GUIDANCE, OUR NEIGHBORS ARE BETTER PREPARED TO MAKE DECISIONS ABOUT THE CARE THEY OR THEIR LOVED ONES RECEIVE. In September 2021, Agrace began hosting free, twice-monthly advance care planning workshops to help people learn how to put their health care preferences in writing. Every adult should have advance directives, even healthy people. IN 2021, 148 NURSING AND MEDICAL STUDENTS, RESIDENTS AND FELLOWS, PHARMACY STUDENTS AND STUDENTS FROM OTHER HEALTH CARE PROFESSIONS PARTICIPATED IN ROTATIONS AT AGRACE. SINCE THE INITIATION OF THE AGRACE ROTATION PROGRAM IN 1995, APPROXIMATELY 2,650 STUDENTS, INTERNS, RESIDENTS, FELLOWS AND WORKING HEALTH CARE PROFESSIONALS FROM INSTITUTIONS OF HIGHER LEARNING HAVE COMPLETED AN EDUCATIONAL EXPERIENCE AT AGRACE. AGRACE RECEIVES NO REIMBURSEMENT FOR THESE ROTATIONS WITH THE EXCEPTION OF AN ANNUAL STIPEND FROM THE UW-MADISON SCHOOL OF PHARMACY.
$1,124,176$14,673SPIRITUAL & GRIEF SERVICES - AGRACE'S GRIEF support PROGRAM PROVIDES SERVICES TO PATIENTS, FAMILY MEMBERS AND COMMUNITY MEMBERS. IN 2021, AGRACE PROVIDED GRIEF SERVICES TO 3,068 INDIVIDUALS - BOTH CHILDREN AND ADULTS. The Agrace Grief Support Center in Fitchburg, Wis., hosts grief support groups and one-on-one support. During 2021, our grief specialists and counselors provided support via phone and virtual meetings with limited in-person availability due to the ongoing COVID pandemic. Still, 447 adults and children were served through community grief support throughout Agrace's service area. IN ADDITION TO INDIVIDUAL COUNSELING, AGRACE PROVIDES GRIEF SERVICES THROUGH GRIEF SUPPORT GROUPS, HOLIDAY REMEMBRANCE PROGRAMS, AND THE COMMUNITY TRAUMATIC DEATH RESPONSE TEAM. THESE PROGRAMS ARE OFFERED FOR A NOMINAL FEE, AND GRIEF SUPPORT GROUPS ARE OPEN TO ANYONE IN THE COMMUNITY EXPERIENCING GRIEF DUE TO A DEATH, NOT JUST TO PATIENTS AND FAMILIES SERVED BY AGRACE. Based on community need, in 2021 Agrace held an LGBTQ+ Spouse/partner loss group, a COVID-loss support group and a Spanish-language grief support group, in addition to many other types of offerings. BUTTERFLY AND BRICK MEMORIALS ARE AVAILABLE FOR INDIVIDUALS TO MEMORIALIZE OR HONOR THE LIFE OF A LOVED ONE. ALTHOUGH PROVIDING GRIEF SUPPORT IS REQUIRED BY STATE AND FEDERAL REGULATIONS, GRIEF SUPPORT SERVICES ARE NOT COVERED BY INSURANCE, MEDICARE, OR MEDICAID REIMBURSEMENT. AGRACE PROVIDES THESE SERVICES BECAUSE THE ORGANIZATION BELIEVES THEY ARE CRITICAL TO HELPING FAMILIES RETURN TO FUNCTIONAL LIVING AFTER THEIR LOSS.

Form 990, Part I, Line 1 BRIEF MISSIONlives in the comprehensive, compassionate care we provide to patients and families while aging or seriously ill. SIGNIFICANT MISSION-RELATED ACTIVITY TOOK PLACE IN 2021: In 2021, Agrace served a total of 4,477 individual patients and clients through its Hospice Care, Supportive Care, Age at Home and Adult Day Center programs. EXPANDING MISSION AND VISION: Agrace announced plans to expand its service area to include Waukesha County. In 2021, Agrace prepared to open a new location at 1227 Corporate Center Drive, Suite 202, in Oconomowoc, Wis. as a home base for staff. Adult Day Center Opens: In July 2021, Agrace opened its doors to a freestanding Adult Day Center located at 1702 W. Beltline Highway, Madison. The Center is a safe, supportive place for older adults who need companionship and help with daily activities. In 2021, the Center provided daytime care to 53 clients. Agrace offers Free Advance Care Planning: Advance care planning is a process of talking with your family about what health care you would want if you could not speak and putting your preferences in writing. Every adult should have advance directives, even healthy people. Starting in September, Agrace began hosting free advance care planning workshops twice a month. Agrace's specially trained employees meet with anyone in the community either one-on-one or with their family or loved ones. Agrace Updates Mission, Vision, Values: Every few years, Agrace creates a new forward-looking strategic plan. At the same time, we review our mission, vision, and values to make sure they continue to fit our growing organization. These statements must describe our purpose and our people now and also guide our future business decisions, including hiring. Agrace is only as good as our people-and we need to choose staff who will be worthy of trust, respect, and admiration. With the approval of our Board of Directors, Agrace made some updates to our mission, vision, and values: Our mission is what we do: Providing personalized care and support, where and when you need it, for life's changing health needs. Our vision tells where we want to go: Building a future where every person experiences extraordinary care and guidance while aging or seriously ill. Our values must be shared by every person at Agrace. We believe these three values set us apart from many other organizations: Belonging - We respect and honor everyone's worth, building communities where people can be their true selves. Resourcefulness - We are an agile organization full of creative problem solvers. Heart-We bring authentic care and connection to those we serve. Cardiac Program: To offer more support for patients with advanced heart disease, Agrace has collaborated with the American Heart Association on an Advanced Cardiac Care Program. Agrace is one of the first hospices in the country to join this effort. As part of the program, our clinical staff receive specialized training on how to manage the symptoms of advanced heart disease and keep our patients comfortable-wherever they receive care. With Agrace, patients who have heart disease can continue treatments that control their symptoms and improve their quality of life. The Advanced Cardiac Care Program's care guide can help prevent the repeat hospital and emergency room visits that are a huge source of stress for patients and families.
Form 990, Part III, Line 1 ORGANIZATION'S MISSIONAGRACE HOSPICECARE PROVIDES CARE TO PATIENTS WHO MEET CERTAIN CRITERIA UNDER THE ORGANIZATION'S CHARITY CARE POLICY WITHOUT CHARGE OR AT AMOUNTS LESS THAN ESTABLISHED RATES. SUCH PATIENTS ARE IDENTIFIED BASED ON FINANCIAL INFORMATION OBTAINED FROM THE PATIENT AT THE TIME THEY ENROLL IN THE PROGRAM OR SUBSEQUENT CHANGES TO THEIR FINANCIAL CIRCUMSTANCES. BECAUSE THE ORGANIZATION DOES NOT PURSUE COLLECTION OF AMOUNTS DETERMINED TO QUALIFY AS CHARITY CARE, THEY ARE NOT REPORTED AS REVENUE. AGRACE HOSPICECARE'S MISSION, "Providing personalized care and support, where and when you need it, for life's changing health needs ," IS FULFILLED BY THE COMPREHENSIVE, COMPASSIONATE CARE WE PROVIDE TO PATIENTS AND FAMILIES FACED WITH LIFE-LIMITING ILLNESS, HELPING THEM TO LIVE AS FULLY AS THEY CAN UNTIL THE END OF LIFE. IN 2021, THE ORGANIZATION PROVIDED CHARITY CARE OF APPROXIMATELY $531,922. MOST PATIENTS AND FAMILIES WANT TO RECEIVE CARE AT HOME. Most OF AGRACE SERVICES ARE DELIVERED IN PATIENTS' HOMES OR IN HOMELIKE SETTINGS SUCH AS COMMUNITY-BASED RESIDENTIAL FACILITIES, ASSISTED LIVING FACILITIES AND NURSING HOMES. AGRACE ALSO MAKES HOSPICE SERVICES AVAILABLE TO PATIENTS IN HOSPITAL SETTINGS. AGRACE PROVIDES ALL LEVELS OF CARE TO OUR PATIENTS AND MAINTAINS TWO INPATIENT UNITS FOR ACUTE, RESIDENTIAL AND RESPITE CARE, as well as hospice memory care.
Form 990, Part III, Line 4d Description of other program services(Expenses $ 1,471,898 including grants of $)(Revenue $ 14,673) COMMUNITY EDUCATION - AS THE COMMUNITY'S Nonprofit HOSPICE, AGRACE IS DEDICATED TO INCREASING AWARENESS AND UNDERSTANDING OF MATTERS RELATED TO serious illness, DEATH, DYING, CARE GIVING, and GRIEF.
Form 990, Part III, Line 4d Description of other program services(Expenses $ 1,124,176 including grants of $)(Revenue $ 14,673) SPIRITUAL & GRIEF SERVICES - AGRACE'S GRIEF support PROGRAM PROVIDES SERVICES TO PATIENTS, FAMILY MEMBERS AND COMMUNITY MEMBERS.
