Part IV, Line 2b:Recipient trust funds for children under state care.
Part V, Line 4:During the year ended June 30, 2024, the donor of an endowment totaling $2,000,000 originally intended to be held in perpetuity provided notice to the Health Center that transferred the variance power to the Health Center's Board of Directors. The Board of Directors released the restrictions on the fund and transferred the amounts to net assets without donor restrictions. The funds are for use in the programs of The Children's Center, Inc. which include the provision of health care services to medically fragile children. The audited financial statements include a prior year adjustment of ($247,693). The adjustment is included in investment earnings or losses.
Part X, Line 2:The Health Center believes that it has appropriate support for any tax positions taken affecting its annual filing requirements, and as such, does not have any uncertain tax positions that are material to the financial statements. The Health Center would recognize future accrued interest and penalties related to unrecognized tax benefits and liabilities in income tax expense if such interest and penalties are incurred.

The Children's Center, Inc.Part V, Section B, Line 5: Feedback was obtained from the Hospital's Board President, physicians at neighboring hospitals, various community leaders, and parents of patients. These individuals were interviewed to gain insight into how the Hospital currently serves the special needs pediatric population and how care could be improved. Based on responses from interviews, the Hospital gained insight on barriers to improving the health and quality of life for the residents of the primary community, opinions on types of services that are needed to address important health issues in the community, and further specialization of services for complex health issues.
The Children's Center, Inc.Part V, Section B, Line 11: After analyzing primary causes for inpatient hospitalization as well as reviewing health needs identified through Key Stakeholder interviews, the following health needs were identified: 1. Expand Outpatient Services by creating more capacity and accessibility for pediatric patients, both in-person and virtual visits. 2. Increase the number of students receiving health and safety education by expanding the number of participating school partnerships in the Healthy Schools Oklahoma program. 3. Improve family communication and understanding by providing additional resources for non-English native languages such as Spanish or Vietnamese. The Children's Center plans to address all needs identified and the following actions were taken during fiscal year end 2024:1. A new Outpatient tower will house a robust rehabilitation center of excellence. As a leader in pediatric therapy services, this expansion will allow us to serve more patients who need physical therapy, occupational therapy, speech therapy, recreational therapy, music therapy as well as many support services that allows a child to thrive through assistive technology.2. Healthy Schools of Oklahoma brings over 26 years of school health experience and knowledge to the Hospital, reinforcing the importance of keeping Oklahoma's children healthy and safe. Healthy Schools provides funding, curricula, training, evaluation tools and support to participating elementary schools focusing on physical activity, nutrition, injury prevention, tobacco use prevention and oral health education. Further, Safe Kids Oklahoma provides dedicated and caring staff, operations support, and other resources to assist in achieving our common goal: keeping your kids safe. We serve the community by implementing evidence-based programs.3. Bethany Children's added a Certified Healthcare Interpreter to the Care Coordination team as a resource for non-English speaking families. This resource has been an essential benefit so far and the Hospital is looking to expand the team as we grow to serve more non-English speaking families.
The Children's Center, Inc.Part V, Section B, Line 13b: Patients whose family income is above 400% of the federal poverty guidelines are eligible to receive services at amounts no greater than the amounts generally billed to Oklahoma Medicaid.
The Children's Center, Inc.Part V, Section B, Line 13h: There are instances when a patient may appear eligible for charity care discounts, but there is no financial assistance form on file due to a lack of supporting documentation. Often there is adequate information provided by the patient or through other sources, which could provide sufficient evidence to provide the patient with charity care assistance. In the event there is no evidence to support a patient's eligibility for charity care, Bethany Children's could use outside agencies in determining estimate income amounts for the basis of determining charity care eligibility. Once determined, due to the inherent nature of the presumptive circumstances, the only discount that can be granted is a 100% write off of the account balance. Presumptive eligibility may be determined on the basis of individual life circumstances that may include:1. State-funded prescription programs;2. Homeless or received care from a homeless clinic;3. Participation in Women, Infants and Children programs (WIC);4. Food stamp eligibility;5. Subsidized school lunch program eligibility;6. Eligibility for other state or local assistance programs that are unfunded (e.g., Medicaid spend-down);7. Low income/subsidized housing is provided as a valid address
The Children's Center, Inc.Part V, Section B, Line 16j: The policy is also provided to patients and families upon admission to the Hospital facility.

Part I, Line 3c:Patients whose family income is above 400% of the federal poverty guidelines are eligible to receive services at amounts no greater than the amounts generally billed to Oklahoma Medicaid. There are instances when a patient may appear eligible for charity care discounts, but there is no financial assistance form on file due to a lack of supporting documentation. Eligibility for other state or local assistance programs that are unfunded (e.g., Medicaid spend-down);7. Low income/subsidized housing is provided as a valid addressPatients complete a Hospital Financial Assistance Application to help determine the discounted or free services in which they are eligible. The Application is reviewed with supporting documentation and based on internal guidelines a decision on the applicable discount is made.
Part I, Line 7:There was no charity care as 90% of patients are covered under Medicaid. Unreimbursed Medicaid on line 7b was calculated using the costing methods to prepare the cost reports. Community health improvement services are reported based on actual expenses recorded in the Organization's general ledger.
Part III, Line 2:The amount reported on line 2 represents implicit price concessions. The Children's Center determines its estimate of implicit price concessions based on its historical collection experience with this class of patients.
Part III, Line 3:The estimated amount of the Organization's implicit price concessions attributable to patients eligible under the Organization's charity care policy is calculated based on the percentage of individuals living below the poverty level in Bethany, OK in 2023. 18.9% can reasonably be considered a community benefit as it would have been written off to charity care.
Part III, Line 4:The footnote to the Organization's financial statements can be found on pages 13 and 14 of the attached audited financial statements.
Part III, Line 9b:Normal collection procedures will be followed for all patients unless Bethany Children's Financial Assistance Application Form is completed and submitted to Bethany Children's along with required documents listed on the application. The Children's Center does not pursue extraordinary collection activities for 120 days from the date of service while the patient is applying for financial assistance. A 30-day notice would be provided to the patient before any collection activities would begin. A patient has a total of 240 days to apply for financial assistance. After a determination of financial assistance any personal portion due would follow normal collection procedures.
Part VI, Line 2:In addition to the community health needs assessment, the Hospital Executive team has a strategic plan to address the needs of the community. Also, patient satisfaction surveys are distributed at various Hospital events.
Part VI, Line 3:Notification about charity available from Bethany Children's, which shall include a contact number, shall be disseminated by various means, which may include, but are not limited to, the publication of notices in patient bills and by posting notices in hospital hall rooms, in the Conditions of Admission form, admitting departments, hospital business offices, and at other public places as Bethany Children's may elect.
