FieldValueIRS element (for citing)
Description1Program Reporting & Evaluation:Grant Awardees submit program reports three times throughout the program cycle, and receive evaluations at each point, which can include recommendations to address program challenges. The evaluation services provided by our university partners help to identify program outcomes and areas that may need addressed. After receiving the evaluation, Grant Awardees may also be required to submit action plans and progress reports to ensure they meet program goals by year-end. Grant Awardees have access to Foundation staff and university partners to provide program assistance.Description1Txt
Expenses1$134,657Expenses1Amt
Description2Mentorship Program:The Mentorship Program includes a webinar learning series and regular consultations that provide capacity building and learning opportunities to Grant Awardees. Educational webinars cover various topics to support the community-based programs, such as reporting and evaluation, sustainability, communications and dissemination. The Mentorship Program has continued to evolve out of over ten years of lessons learned and the successful launch of the pilot Mentorship Program in 2019. Peer-to-peer learning is fostered through the pairing of newly-funded organizations with long-standing Grant Awardee(s), with support from Foundation staff and university partners, to help them launch successful programs and work through implementation barriers.Description2Txt
Expenses2$107,805Expenses2Amt
Description3Grant Application Review Process:The Foundation Grant Review Committee reviewed and vetted grant applications using an Excel spreadsheet to ensure that each application adhered to requirements, had appropriate plans in place and a clearly defined budget. All grant applications were recommended by the Foundation Grant Review Committee for approval by the Board.Description3Txt
Expenses3$114,127Expenses3Amt
Description4Communications and Dissemination:The Foundation shares its lessons learned and aims to amplify the work of Grant Awardee programs through dissemination and communications activities such as social media, its Annual Report, e-mail distribution to the Foundation's mailing list of over 2,000 individuals and a mentorship program toolkit for other philanthropic and nonprofit organizations, which is currently being developed. Additionally, the Foundation supports Grant Awardees in their efforts to share lessons learned and further build awareness for their programs through providing educational resources, tips and templates for activities such as social media, press releases, program landing pages, program toolkits, videos and webinars.Description4Txt

2021 Connections for Cardiovascular HealthSM Next Generation Criteria:Part XV, Line 2d2021 Connections for Cardiovascular HealthSM Next Generation Criteria:The AstraZeneca HealthCare Foundation's Connections for Cardiovascular HealthSM (CCH) program awards Foundation grants annually to U.S.-based, nonprofit 501(c)(3) organizations or similar nonprofit organizations working to improve cardiovascular health in their communities.CCH Next Generation focuses on sharing nearly 10 years of Foundation and Grant Awardee learnings from the field so new applicants have the opportunity to modify or adapt these learnings highlighted in the Effective Program Practices Guide when designing and implementing their own innovative, community-based heart health programs. The intent of the Program Grants is to support innovative initiatives or programs at the community level to help improve cardiovascular health in the community during the grant year and beyond. Our aim is for funded programs to maximize the benefit to participants and to work toward sustainability beyond our funding. One way the Foundation maintains our commitment to program sustainability and sharing lessons learned is by providing Mentor Grants. These grants support a limited number of previously-funded organizations with programs that consistently demonstrated positive outcomes so they can help recently funded organizations implement their programs. The intention of 2021 grant funding is two-fold:1. To provide second year CCH Next Generation funding to organizations that received Program Grants in 2020 with programs that demonstrate positive outcomes and to focus on sustaining the program and sharing lessons learned and best practices on program resiliency through a program toolkit, video or webinar.2. To provide limited grant funding to previously-funded organizations on an invitation basis to support mentorship activities and program sustainability through CCH Mentor Grants.Organizational Criteria:Invited organizations need to maintain their status as a U.S.-based, nonprofit organization with a 501(c) designation of a public school, government entity or municipal institution that is eligible to accept tax-deductible, charitable contributions. Organizations that are 501(c)(3) and also have an IRS designation as a 509(a)(3) are ineligible for funding.Funding Criteria:Organizations must be invited to apply for a single year of funding. There is no guarantee of funding. Each application will be reviewed and vetted to confirm its eligibility and will need to achieve a "meets expectations or "exceeds expectation" on its 2020 Mid-Year Evaluation to be eligible for funding. Organizations that receive a "Partially Meets Expectations or "Does Not Meet Expectations" ranking on their 2020 Mid-Year Evaluation will be reviewed by the Foundation Grant Review Committee and Board of Trustees.Applicants who are approved for a Foundation grant award must meet program objectives. - Program Grant funding requests must be between $125,000 and $150,000. - Organizations that are awarded Program Grants in 2021 (Year Two) may be eligible for a third year of continued funding in 2022 (Year Three), based on performance at the mid-year mark, availability of funds and Board approval. - Mentor Grants up to $55,000 are comprised of: o Support for the continuation of the organization's CCH-funded program and o Support to serve as a mentor to CCH Next Generation Grant Awardees and engage regularly with mentees and the Foundation to share lessons learned to help CCH Next Generation programs based on learnings from the field. - Programs should be designed with funding for equipment less than 30% of the total budget for equipment that is integral to the specific program design and not for equipment that might support other ongoing, routine programs run by the organization. Equipment costs over 30% of the total funding request will not be considered. Incidental equipment and costs necessary to complete or sustain a project are allowed. - The Foundation will not fund vehicle purchases. - Costs over 10% in the budget category "Other" will not be considered. - Any budget expenses allocated for healthcare provider compensation must only support a portion of each healthcare provider's annual salary for their work on the program and cannot be in addition to the healthcare provider's annual salary for the 13-month grant timeframe paid for by the applying organization.Program Design: - Exclusions. The Foundation does NOT fund programs that focus exclusively on the following: o Capital investments and unsolicited capital campaigns o Media/awareness campaigns o Hospital software systems, hospital in-patient programs or enhancement of existing hospital services o Professional education and/or training for healthcare professionals that is more than incidental to the program o Research or clinical trials o Healthcare provider/cardiologist salaries o Faith-based programs that are NOT open to the community-at-large- The Foundation does NOT support: o Cardiovascular initiatives outside the United States or its territories o Individuals o For-profit organizations o Endowments o Journals or advertising o Political causes, lobbying, fraternal or social organizations o Religious organizations whose activities are not open to the general public o Nonprofit organizations that discriminate on the basis of age, race, color, religion, national origin, gender identity, sexual orientation, marital status, military service, veteran status or disability.- Program Design. Invited organizations will need to apply with the same program they applied for, and were awarded funding for, in 2020.* - Measurable Outcomes and Participant Goals. The Foundation requires that all programs: o Collect and measure program outcomes. o Maintain or exceed their 2020 CCH Next Generation Program Grant reached, screened, educated and tracked participant goals or 2020 CCH Mentor Grant reached and tracked participant goals. o Reached participants must be new to the program, and up to 50% of participants screened, educated and tracked may be participants who were tracked during the 2020 grant year and are continuing in the program. Organizations will be need to record a baseline and follow-up measure(s) for each of the continuing participants during the grant year. o Maintain or exceed their 2020 CCH Next Generation Program Grant or 2020 CCH Mentor Grant goals that are connected to a community cardiovascular health need(s). o Demonstrate measurement outcomes at both the mid-year (March 2022) and year-end (October 31, 2022) timeframes following the grant award. o Report on program outcomes for approximately half of the planned unduplicated tracked participants for at least one clinical measure or measure for standardized testing (e.g., Progressive Aerobic Cardiovascular Endurance Run test, etc.) by March 2022. o Have baseline and follow-up data for at least one clinical measure (e.g., body mass index, blood pressure, weight, etc.) or measure for standardized testing (e.g., PACER test, etc.) for each participant counted toward the total tracked. (It is preferred to include baseline and follow-up data for all clinical and behavioral measures for each participant, when possible). o Dissemination. Organizations receiving a Program Grant in 2021 will need to develop and disseminate one of the following items during the grant year to share lessons learned and best practices on program resiliency from their experiences implementing their CCH Next Generation-funded program: a program toolkit, program video or webinar. Program Execution: - Grant Awardees need to ask the Foundation's consideration and approval prior to making a major change in program objectives, budget or scope. Some examples include, but are not limited to, program location, target audience, number of participants reached, screened, educated and tracked, and behavioral and/or clinical measures. - Award recipients will be required to submit regular program reports and other requirements to the Foundation according to deadlines and processes set forth in the Letter of Agreement. Note: Criteria for assessing applications is subject to change and may be updated by the AstraZeneca HealthCare Foundation from year to year. Application cycles are planned on a year-by-year basis. *Unless prior approval received from the AstraZeneca HealthCare Foundation for program design and/or outcome revisions.
Part XV, Line 3aThe AstraZeneca HealthCare Foundation's (AZHCF) overall mission is to promote public awareness and education of healthcare issues and support nonprofit organizations consistent with its charitable purpose. AZHCF's signature program, Connections for Cardiovascular Health (CCH), was launched in 2010 to provide grants and capacity building to nonprofit and other tax-exempt organizations working in innovative ways to help improve cardiovascular health in communities across the United States. The Connections for Cardiovascular HealthSM Next Generation program launched in 2020, builds on ten years of lessons learned and aims to expand to areas of the country with the greatest need and where our funding has not previously reached.Since the inception, CCH and CCH Next Generation funded programs have collectively reached over 1.7 million individuals and tracked over 70,000 participants for progress. Nine organizations that received CCH Next Generation funding in the third quarter of 2020 and implemented their programs through the fourth quarter of 2021 reached over 17,000 individuals and tracked over 2,500 new participants for progress toward improved cardiovascular health. The Foundation's Grant Awardees receive ongoing educational and peer support through our Mentorship Program, which aims to help organizations further build capacity as they implement their programs. The Mentorship Program includes regular consultations and an educational webinar series guided by a curriculum developed by Foundation staff, our university evaluation partners and mentors. Webinars in 2021 focused on program reporting and evaluation to demonstrate impact; communications to share program stories; and program planning and implementation to help address barriers and plan for the continuation of programs. Topics included: - Reporting & Evaluation o The Role of Evaluation in Community-Based Programming o Meeting Mid-Year Reporting Expectations o Meeting Year-End Reporting Expectations- Communications: o Crisis Communications Planning o Storytelling, Key Message Development & Video o Strategic Integrated Communications & Storytelling o Social Media Analytics o Visual Storytelling through Landing Pages & Canva o Program Dissemination- Program Planning & Implementation o Planning for the Remainder of the Grant Year o Behavioral Health o Orientation: A Look Ahead at What's to Come for Year Two o Program SustainabilityFor more information on CCH Next Generation, visit our website: https://www.astrazeneca-us.com/sustainability/healthcare-foundation. html
