Form And Line ReferenceFormAndLineReferenceDescExplanationExplanationTxt
Schedule H, PART V, SECTION B, LINE 5The process for preparing the 2022-2024 Community Health Needs Assessment was an inter-organizational and community collaborative process, initiated with the goal of developing an assessment that was reflective of the needs of the community including the clinical and social determinants of health. The 2022-2024 Community Health Needs Assessment involved a primary data collection strategy in conjunction with the evaluation of secondary data. Community input on health priorities in Westchester County was gathered through two collaborative surveys and evaluated along with secondary data to capture an up-to-date high-level view of the health status of Westchester County. Through the process of the electronic/paper survey supplemented by the telephone survey, data was able to be collected and documented from a broad range of the population allowing an analysis of participation by age, race, ethnicity, and income. Montefiore New Rochelle, as part of the Montefiore Health System, participated in the Community Health Needs Assessment Survey Collaborative led by the Greater New York Hospital Association (GNYHA). The Community Health Needs Assessment Survey Collaborative included multiple health systems and hospital across New York State that agreed to work with GNYHA to develop and administer an electronic and paper-based survey across service areas with GNYHA assuming responsibility for analyzing and reporting the data collected. The surveys of Westchester County residents took place during the Spring and Summer of 2022. The survey was disseminated through multiple distribution points including our hospitals and clinics, community-based organizations, and government partners. Montefiore New Rochelle Hospital also participated in the Mid-Hudson Region Community Health Assessment Collaborative with the Westchester County Department of Health and other hospitals and health systems in Westchester County. Local Health Departments and regional hospitals in the mid-Hudson Region partnered in a collaborative to create a regional survey to assess the community health needs and community assets from each county of the Mid-Hudson region working with the Siena College Research Institute in the Spring of 2022 to administer a region-wide telephonic survey and provider focus groups for each county. Survey participants were contracted via landline telephone, cell phone, an online panel and online recruitment from each county at various in-person events and other community partnerships to enhance representation and meet budget constraints. The online recruitment from each county included distributing the survey URL to community partners, promoting the survey on social media, and providing access to the survey at community events. Montefiore plays a leadership role in a variety of critical community and public health efforts, including membership in the Westchester County Department of Health's Hospital Planning Team and Advisory Committee meetings, board membership and strategic partnership with the local Boys and Girls Club and youth Community Outreach Program for youth development activities, and the Montefiore New Rochelle Women, Infants and Children Program for Westchester and Putnam counties. Participation in these efforts give Montefiore trust-based access to the communities we serve, which we continue to foster by participating in, and presenting at, community sponsored events, Community Board meetings and local conferences. The Hospital has not received any written comments concerning it most recently conducted CHNA and implementation strategies.
Schedule H, Part V, Section B, Line 6AMontefiore New Rochelle Hospital's CHNA was conducted along with Montefiore Mount Vernon Hospital. The reporting data between the two facilities are essentially the same, even though demographically distinct, due to the close proximity of each facility to the other, the high migration of patients between facilities and the sharing of employees. Both organizations are also members of the Montefiore Health System, the premier academic health system and the University Hospital for Albert Einstein College of Medicine, serving the 3.1 million people living in the New York City region and the Hudson Valley.
Schedule H, Part V, Section B, Line 7aThe website that the hospital facility makes its CHNA report widely available to the public: https://www.montefiore.org/documents/communityservices/MNR-MMV_2022-CHNA-C SP-Report__FINAL.pdf
Schedule H, Part V, Section B, Line 10aThe website that the hospital facility's most recently adopted implementation strategy is posted: https://www.montefiore.org/documents/communityservices/MNR-MMV_2022-CHNA-C SP-Report__FINAL.pdf
Schedule H, Part V, Section B, Lines 11Through the process of completing and reviewing data obtained through the primary and secondary sources, engaging with community stakeholders and key partners and a review of resources available within the Medical Center and through its partnerships, an implementation strategy and community service plan was developed to address the significant needs identified. Three priorities were reselected for the 2022-2024 Community Health Assessment and Community Service Plan after considering community feedback, provider input, and the resources available to the Hospital: 1) Prevent Chronic diseases, 2) Promote healthy women, infants, and children, and 3) Prevent communicable Diseases. The COVID -19 pandemic required a shift in priorities and programming for the 2019-2021 selected priorities allowing the hospital to revisit these priorities for the 2022-2024 plan. Montefiore has elected to retain the three priority areas selected from the last reporting cycle. Although Westchester County has continued to be among the top five in many health outcomes and has shown improvements along with the rest of New York State, the rates for conditions identified in the communities of New Rochelle remains higher in most cases than the countywide and statewide averages. In addition, New Rochelle has hotspots where populations identify as a cultural/racial/ or ethnic minority show disparate rate of impact for many health conditions. As such, the hospital has chosen priorities areas from the NYS Prevention Agenda that address some of the health disparities highlighted in the secondary data. The first priority area selected was to prevent chronic Disease with a specific focus on preventive Care and Management. The hospital will continue their work on chronic disease prevention and care through continuing implementation of the Diabetes Self-Management Education Program while expanding implementation of a team-based care model involving care coordination from multiple specialties to support patients with diabetes. Our goal is to promote evidence-based care to prevent and manage chronic diseases including asthma, arthritis, cardiovascular disease, diabetes and prediabetes and obesity. The targeted community is mainly low-income individuals that includes a high proportion of Hispanic and non-Hispanic black. The second priority area selected was to promote healthy women, infants, and children with a focus on perinatal and infant health. The hospital will address this need by providing participates of the New Rochelle W.I.C. Program with needed resources, support, and education to increase initiation and duration of breastfeeding. Our goal is to increase the number of infants that are breastfeed with an objective of increasing the percentage of infants who are exclusively breastfed in the hospital by 10%. The third priority area selected is to prevent communicable disease focusing on vaccine preventable diseases. We plan to support outreach efforts through education and advocacy of the importance of HPV vaccination with parents of children and adolescents age 9-13 and provide vaccinations with the goal of increasing the percentage of NYS 13-year-old adolescents with a complete HPV vaccine series by 10% to 37.4%. The targeted community is mainly low-income individuals including a large Hispanic or non-Hispanic black population. Despite each of the different methods and approaches to primary data collection in gathering community input, there was a consistent focus on obesity and related behaviors and outcomes among the main community health concerns. Obesity and its related behaviors have significant impact in chronic disease; therefore, it is intended that the programs that are detailed specifically for the reduction of obesity will also affect the prevalence of Diabetes, hypertension, asthma, cancer, and cardiovascular disease in Westchester County. Montefiore has a vast portfolio of programs and services that address a majority of the significant community health needs identifies in the community health needs assessment. The breadth and depth of the programs and service vary, but each address a need identifies in the community. While the programs selected for the Community Health needs assessment and implementation strategy does not address all health concerns, there are existing and planned programs across Montefiore Health system that address these community-identified priorities. In addition to the multiple resources that have been developed at Montefiore independently and through partnership with other organizations, there continues to be a need for community-based programs and resources that can augment Montefiore's programs and services. Montefiore continues to engage in cross-sector partnerships with government, community organizations, hospitals, and businesses to address the health and social needs of our patient population. Since the previous version of this report in 2019, Montefiore has continued to expand and has a goal to scale screening and referrals of patients for unmet social needs. This includes use of the internet database platform www.nowpow.com, to connect patients to needed resources, which has been a challenge for the health care sector in the past. This online tool is a much more comprehensive and practical alternative to the homegrown referral guides that many health care providers have had to use, which were difficult to search and keep-up to date, a problem that is largely addressed by the online version. Many Montefiore sites have been introduced to this new online resource and work is underway to integrate this kind of solution more seamlessly into the various workflows across the ambulatory, ED and inpatient setting. The use of an internet database will allow Montefiore to connect patients to important community resources provided outside of the health system by many of our community partners to address community needs such as housing (quality and affordability), transportation, employment, and education. As Montefiore is an organization that works with complex health needs and whose community faces multi-factorial crisis that impact overall health, we at Montefiore understand the importance of addressing the social determinants of Health as part of our mission of advancing the health of the communities that we serve.
Schedule H, Part V, Section B, Line 13HFamily size is factored into the eligibility criteria for financial assistance.
Schedule H, Part V, Section B, Lines 15d & 15eThe Medical Center has internal resources available for assistance with the FAP application process. The organization's FAP provides addresses, locations and phone numbers for offices within Montefiore to assist with completing applications. The financial aid and Medicaid staff at these locations are also certified application counselors that assist with Market place and Medicaid applications.

1. PART 1, LINE 7:THE COST-TO-CHARGE RATIO METHODOLOGY WAS UTILIZED TO CALCULATE THE AMOUNT included IN THE TABLE. THE CALCULATION OF THIS RATIO WAS DERIVED FROM RATIO OF PATIENT CARE COST-TO-CHARGES. In calculating the patient care cost for the RATIO, the organization reduced its operating expenses for its non-patient care costs and cost of its community benefits and building activities not RELYING on the RCC factor for costing purposes.
PART III, SECTION A, LINE 2:THE COST OF BAD DEBT EXPENSE WAS ESTIMATED BASED ON THE CHARGES, APPLIED TO THE RATIO OF TOTAL PATIENT CARE EXPENSES TO TOTAL CHARGES FOR ALL SERVICES RENDERED. ANY PAYMENTS OR DISCOUNTS were EXCLUDED FROM BAD DEBT EXPENSE.
PART III, SECTION A, LINE 3:THE ESTIMATED AMOUNT OF BAD DEBT EXPENSE (AT COST) ATTRIBUTED TO PATIENTS under THE ORGANIZATION'S CHARITY CARE POLICY WAS BASED ON RESULTS OF predictive analysis. BAD DEBT SHOULD BE INCLUDED AS A COMMUNITY BENEFIT BECAUSE THE organization provides MUCH NEEDED HEALTH CARE SERVICES INDISCRIMINATELY to THE COMMUNITY-AT-LARGE WITHOUT REGARD TO WHETHER OR NOT THE PATIENT HAS INSURANCE AND IF THE BILL WILL EVER BE PAID.
PART III, SECTION A, LINE 4:AS REPORTED IN MONTEFIORE HEALTH SYSTEM'S AUDITED CONSOLIDATED FINANCIAL statements, BAD DEBT EXPENSE IS DESCRIBED AS FOLLOWS: "Subsequent changes to the estimate of the transaction price (determined on a portfolio basis when applicable) are generally recorded as adjustments to patient service revenue in the period of the change. For the years ended December 31, 2022 and 2021, changes in the Health System's estimates of expected payments for performance obligations satisfied in prior years were not significant. Portfolio collection estimates are updated based on collection trends. Subsequent changes that are determined to be the result of an adverse change in the patient's ability to pay (determined on a portfolio basis when applicable) are recorded as bad debt expense. Bad debt expense for the years ended December 31, 2022 and 2021 was not significant".
PART III, SECTION B, LINE 8:MEDICARE REVENUE AND ALLOWABLE COSTS REPORTED IN PART III, SECTION B, WERE derived FROM THE MEDICARE COST REPORT. MEDICARE ALLOWABLE COSTS REPORTED ON Part III, LINE 6, PER SCHEDULE H INSTRUCTIONS, WERE DERIVED USING THE MEDICARE COST REPORT COST-FINDING METHODOLOGY, WHICH APPORTIONS ROUTINE COSTS TO Medicare USING DAYS AND ANCILLARY COSTS TO MEDICARE USING DEPARTMENTAL RATIOS OF COSTS TO CHARGES (RCCS). THE FOLLOWING TABLE REPRESENTS A RECONCILIATION OF MEDICARE REVENUE AND COSTS INCLUDED ON LINES 5, 6, AND 7 IN PART III TO THE TOTAL ACTUAL MEDICARE REVENUE AND costs of THE HOSPITAL. AS HIGHLIGHTED IN THIS TABLE, INCLUDING THE adjustments described BELOW WOULD HAVE INCREASED THE MEDICARE SHORTFALL TO $21,520,965. REVENUE COSTS SHORTFALL PART III, LINES 5-7: $34,239,024 $43,488,715 (9,249,691) ADD COST not included: - 7,919,167 (7,919,167) ADD MEDICARE DME: 3,743,007 6,381,000 (2,637,993) ADD SUBSIDIZED HLTH SRVS 1,740,524 3,454,638 (1,714,114) ADJUSTED MEDICARE SHORTFALL: $39,722,555 $61,243,520 (21,520,965) The Medicare shortfall should be included as a community benefit, since the organization is rendering much needed health care services mainly to a frail elderly population living on a fixed income with great health needs.
PART III, SECTION C, LINE 9B:FOR PATIENTS WHO ARE KNOWN TO QUALIFY FOR FINANCIAL ASSISTANCE, ALL collection activity IS HALTED AND THE PATIENT IS REFERRED FOR FINANCIAL aid. If an ACCOUNT IS IN COLLECTION AND THE PATIENT REQUESTS FINANCIAL AID OR IF the AGENCY DETERMINES THAT THE PATIENT IS ELIGIBLE FOR FINANCIAL ASSISTANCE, the ACCOUNT IS REFERRED BACK TO THE HOSPITAL WHERE THE PATIENT IS PROVIDED ASSISTANCE WITH COMPLETING AN APPLICATION FOR ASSISTANCE. The organization also uses predictive analysis to assist in charity care determinations in the absence of completed financial aid applications. Financial screening/Presumption Charity uses financial information that is contained in a patient's credit report and other patient specific attributes to estimate a patient's income level and where they are in relation to the Federal Poverty Level to qualify under the Hospital's Charity Care Policy. Presumptive eligibility may also be based on prior FAP eligibility or enrollment in certain specified means-tested public programs to presumptively determine if an individual is FAP eligible. Full Financial aid is also granted to patients with outstanding self-pay bills and current Medicaid coverage and patients who are homeless. Ambulance reports can be a source of reference if it is documented in the report that the patient is undomiciled.
2. NEEDS ASSESSMENT:In addition to collaborative input from the local Department of Health and clinical partners, Montefiore works closely with its communities. Working with a variety of organized partnerships and collaboratives and other providers in lower Westchester as well as extensive work with representatives of the affected communities has helped Montefiore to identify health care needs and determine the appropriate configuration of services. Beyond the formal structures that Montefiore established to gain input from the communities it serves, Montefiore's Office of Community and Population Health participates in a variety of informal organized partnerships and collaboratives, working with other providers in Westchester, the Westchester County Department of Health, community-based organizations and others, using a community level approach that involves relevant community based organizations interested in planning and developing initiatives aimed at improving the health of the residents of Westchester County.
3. PATIENT EDUCATION OF ELIGIBILITY FOR ASSISTANCE:ALL INTAKE, REGISTRATION, AND COLLECTION AGENCY STAFF IS TRAINED ON THE HOSPITAL'S FINANCIAL AID POLICY AND HOW TO PROVIDE PATIENTS WITH ASSISTANCE. MONTEFIORE NEW ROCHELLE HOSPITAL HAS A POLICY THAT ESTABLISHES GUIDELINES FOR THE BILLING OFFICE TO FOLLOW WHEN WORKING WITH INDIVIDUALS WHO ARE HAVING DIFFICULTY PAYING THEIR MEDICAL BILLS. A REFERRAL IS MADE TO THE HOSPITAL'S FINANCIAL AID OFFICE AND A FINANCIAL COUNSELOR WILL HELP THE PATIENT APPLY FOR FREE OR LOW-COST INSURANCE. IF THE FINANCIAL COUNSELOR DETERMINES THAT THE PATIENT DOES NOT QUALIFY FOR LOW-COST INSURANCE, THE COUNSELOR WILL HELP THE PATIENT APPLY FOR A FINANCIAL AID DISCOUNT BASED ON INCOME LEVEL. THE HOSPITAL MAKES ITS FINANCIAL AID POLICY KNOWN TO THE PUBLIC BY PROVIDING WRITTEN INFORMATION AVAILABLE IN ENGLISH, SPANISH, Arabic, Haitian-CREOLE, ITALIAN AND PORTUGUESE INCLUDING BROCHURES AT ALL PATIENT SERVICE AREAS, INFORMATION POSTED ON THE INTRANET AND INTERNET, AND INFORMATION SENT OUT ON PATIENTS' BILLS. THERE ARE ALSO SIGNS POSTED AT ENTRANCEWAYS, IN THE emergency DEPARMENT, ADMITTING OFFICE, BILLING AND MEDICAID OFFICES AND OTHER REGISTRATION AND WAITING AREAS ADVISING PATIENTS OF THE AVAILABILITY OF FINANCIAL AID.
4. COMMUNITY INFORMATION:The lower Westchester facilities of Montefiore New Rochelle Hospital has identified Westchester County as their primary service area. Montefiore is the largest health service provider in New Rochelle providing critical care services including community-based primary care and specialty ambulatory services. The City of New Rochelle is the anchor city for the Long Island Sound Region in Westchester County and has approximately 81,587 residents. Since 2000, the population of New Rochelle has grown by 3.4% compared to Westchester County (5.8%) and New York State (4.2%). New Rochelle is ethnically diverse. Its population is 40.1% non-Hispanic white, 32.9% Hispanic, 17.9% non-Hispanic black, 4.8% Asian and 4.3% other. Almost one-third (28.1%) of its residents are foreign-born. Among the population, 39.9% speak another language other than English at home. Over one-tenth (10.3%) of New Rochelle's population lives below the poverty line as compared to 9.4% for Westchester County. While a smaller proportion of individuals live in poverty in Westchester County than in New York State overall, those who are black (14%) and Hispanic (15.1%) are more likely to be living in poverty than those who are white (5.6%). The median household income in New Rochelle is $87,159, slightly lower than the median household income of $105,387 for Westchester County, and higher than the median income for New York State ($74,314). While Westchester County remains among the healthiest counties in New York State, several of its individual municipalities continue to have significant health gaps. Portions of lower Westchester, specifically Mount Vernon, Yonkers, New Rochelle, and White Plains are "hot spots" for various health outcomes, such as asthma and preterm births in the County. Additionally, certain groups, such as some racial/ethnic minorities or those with less education, experience poorer health outcomes

ExplanationAs it relates to Compensation from either Montefiore Medicine Academic Health System, Inc. or Montefiore Health System, Inc. The Compensation Committee reviews and approves all forms of compensation and benefits provided to all officers and certain key employees with the assistance of a national independent compensation consulting firm. The consulting firm assists the Compensation Committee with its decision-making process to ensure executive compensation levels are reasonable and appropriate relative to those of other similar organizations. Base salaries in Column B(i) are determined based on competitive market practices for comparable positions with similar size organizations and scope of responsibilities. Bonus and incentive compensation in Column B(ii) is based on the achievement of performance goals. The executives' compensation program has a significant pay-at-risk component to ensure the alignment of pay and organizational performance. Goals are set in advance in areas such as quality of care, patient satisfaction, community services and financial performance. Compensation is at risk if the goals established by the Compensation Committee are not met. Other reportable compensation in column B(III) includes distributions from the supplemental executive retirement plan. In a manner designed to qualify for the "rebuttable presumption of reasonableness" the Compensation Committee of the Board of Trustees expressly reviewed and approved these retirement benefit arrangements for senior executives in a manner that qualified under the intermediate sanctions rules of the Federal tax law, and in recognition of (a) the executives' years of service to the organization and (b) the significant contributions to enhancing the ability of the organization to achieve its charitable mission in a manner consistent with financial solvency. Accordingly, this benefit should be viewed as applying to years of service for the organization. POOLED SUPPLEMENTAL EXECUTIVE RETIREMENT PLAN DISTRIBUTIONs based on multiple years of service and paid out in 2022 as taxable income: Susan Green-Lorenzen, R.N. - $871,397; Philip O. Ozuah, M.D., PH.D. - $632,224; Christopher Panczner - $558,708; Colleen Blye - $810,464. The Health System also funds a second defined contribution SERP plan. An officer of the organization currently participates in this plan. Contributions are at a fixed percentage of base salary or total cash compensation (base salary plus annual incentive award). A notional balance is developed for each participant that is the total of contributions less distributions (interest is not accrued). On January 1 following the third, sixth and ninth anniversaries of each participant's participation date, 50% of the remaining SERP account becomes vested and is paid out if the participant is employed on that date. On January 1 following the twelfth anniversary of each participant's participation date, the remainder of the SERP account becomes fully vested and is paid out. If a participant reaches age 65 before their twelfth anniversary of their participation date, their SERP account balance will become fully vested on the later of the first of the month following their 65th birthday or the third anniversary of their participation date. SERP account balances will also become fully vested upon the earliest of death or disability while employed by Montefiore, involuntary termination without cause or if Montefiore chooses to terminate the plan. During 2022, there were no distributions from the plan for the participating officer. Since the 2022 contribution accrual will be paid out prior to March 15, 2023, there was no deferred compensation from this plan reported in column (C) for the participating officer, Philip O. Ozuah, M.D., PH.D.ExplanationTxt
