Schedule H, Part V, Section B, Line 3ETHE SIGNIFICANT HEALTH NEEDS ARE A PRIORITIZED DESCRIPTION OF THE SIGNIFICANT HEALTH NEEDS OF THE COMMUNITY AND IDENTIFIED THROUGH THE CHNA.
Schedule H, Part V, Section B, Line 5 Facility , 1Facility , 1 - Carle Hoopeston Regional Health Center. The Vermilion County Community Health Needs Assessment is a collaborative undertaking by the Vermilion County Executive group (Carle Hoopeston Regional Health Center, OSF Sacred Heart Medical Center, United Way of Danville Area, Vermilion County Mental Health Board, and Vermilion County Public Health Department). Through this needs assessment, collaborative community partners have identified numerous health issues impacting individuals and families in the Vermilion County region. Several themes are prevalent in this health-needs assessment - the demographic composition of the Vermilion County region, the predictors for and prevalence of diseases, leading causes of mortality, accessibility to health services and healthy behaviors. Results from this study can be used for strategic decision-making purposes as they directly relate to the health needs of the community. The study was designed to assess issues and trends impacting the communities served by the collaborative, as well as perceptions of targeted stakeholder groups. This study includes a detailed analysis of secondary data to assess information regarding the health status of the community. In order to perform these analyses, information was collected from numerous secondary sources, including publicly available sources as well as private sources of data. Additionally, primary data were collected for the general population and the at-risk or economically disadvantaged population. Areas of investigation included perceptions of the community health issues, unhealthy behaviors, issues with quality of life, healthy behaviors and access to medical care, dental care, prescription medications and mental-health counseling. Additionally, demographic characteristics of respondents were utilized to provide insights into why certain segments of the population responded differently. Ultimately, the identification and prioritization of the most important health-related issues in the Vermilion County region were identified. The collaborative team considered health needs based on: (1) magnitude of the issue (i.e., what percentage of the population was impacted by the issue); (2) severity of the issue in terms of its relationship with morbidities and mortalities; (3) potential impact through collaboration. The 2022 Community Health Needs Assessment was approved and adopted by Board of Trustees on October 27, 2022. The 2023-2025 Community Benefit Implementation Plan was adopted and approved by the Board of Trustees on October 27, 2022.

Schedule H, Part V, Section B, Line 6b Facility , 1Facility , 1 - Carle Hoopeston Regional Health Center. Vermilion County Health Department (Danville, Vermilion County, Illinois) Vermilion County Mental Health Board (Danville, Vermilion County, Illinois) United Way of Danville, Illinois Area (Danville, Vermilion County, Illinois)
Schedule H, Part V, Section B, Line 11 Facility , 1Facility , 1 - Carle Hoopeston Regional Health Center. Health Needs Selected Using a modified version of the Hanlon Method, four significant health needs were identified and determined to have equal priority: 1. Violence 2. Income/Poverty 3. Healthy Behaviors 4. Behavioral Health Behavioral Health: Description of Priority Area: Behavioral health issues continue to be an issue across the county. Lack of resources, funding, and stigma contribute to the issue in Vermilion County. According to County Health Rankings, the ratio of the county population in Vermilion County to the number of mental health providers in 2023 was 260:1, a slight increase from recent years. This number is better than the State of Illinois national ratio of 320:1. The number of suicides in Vermilion County has increased every year for the past four years in Vermilion County, with 19 in 2023, the number of 211 calls in Vermilion County continue to increase, and aggregate data on drug-related overdose deaths maintains high in Vermilion County. When asked to identify health concerns other than chronic disease, survey respondents identified mental health as a priority. When combined with drug/substance abuse, the behavioral health priority area was deemed essential to creating a healthier community. By educating community partners on early identification of behavioral health conditions, increasing awareness of behavioral services and decreasing the stigma associated with seeking services, behavioral health improvement will be brought to the forefront of health priority in Vermilion County. Implementation Plan - Anticipated Impact & Plan to Evaluate: Carle Hoopeston Regional Health Center, in partnership with the Carle Health system, will pursue these initiatives to address behavioral health in Vermilion County: 1. Institute mental health medical care in Carle primary care offices in Vermilion County. 3. Establish and launch a new telehealth service focused on Behavioral Health for residents in Vermilion County, allowing residents to connect with world-class care from their home. 4. Provide donations and in-kind support to community and school-based programs that improve access to mental health programming. Evaluation of Prior Impact: In response to the identified health need, Carle Hoopeston Regional Health Center took the following actions: - While the X Waiver is no longer technically an application requirement under the State of Illinois, Carle Hoopeston Regional Health Center continues to provide significant addiction medication recovery options for the Vermilion County population. - In 2023, Carle Hoopeston Regional Health Regional Health Center maintained a psychiatry residency program, working to respond to this critical need. - Carle Hoopeston Regional Health Center, in collaboration with the Carle Health system, established a mobile health/behavioral health mobile unit specifically focused on Vermilion County, beginning in 2023, and expanding into 2024. - Carle is supporting a funding increase in staff at Crosspoint Human Services, a local behavioral health provider, with funds won in an Illinois HFS grant. - Carle Hoopeston Regional Health Center, in partnership with the Carle Health system, provided Progressive Ag Safety Days in 2023, where patients and community members can learn about farm and rural living safety. - Carle Hoopeston Regional Health Center, in partnership with the Carle Health System Center for Rural and Farm Safety, wrote and distributed through Vermilion County Farm Bureaus Wellness Works articles every month in 2023, providing updates and information on farm safety and rural community behavioral health safety. - CHRHC completed 2 ECHO OUD cohorts to address rural MOUD desserts. - CHRHC finalized details for Mobile Unit design and assembly, with delivery in 2024. - Conducted training / education for Georgetown - Ridge Farm school district staff development day. - CHRHC distributed Narcan throughout the county in partnership with public health, and undertook work to promote drug take back day activities with local partners. - CHRHC facilitated Vermilion County ROSC (Recovery Oriented System of Care) development and formation in 2023. - In 2023, CHRHC hired behavioral health outreach staff dedicated to Vermilion County. Violence: Description of Priority Area: National data shows that gun violence has been a chronic issue over the past decade and that some cities have seen recent surges in shootings. There is evidence of the harmful effects of violence on child development, the longterm health of affected populations, and the economic development of entire communities. Over the last several years, the number of violent crimes, especially gunrelated, have increased in Vermilion County. Respondents of the community health survey of 2022 identified Violence as a priority in Vermilion County. In most recent data, there were 20 firearm related deaths per 100,000 people; this is significantly higher than the Illinois and national average of 13 and has increased in recent years. Implementation Plan - Anticipated Impact & Plan to Evaluate: Carle Hoopeston Regional Health Center, in partnership with the Carle Health system, will pursue these initiatives to reduce substance violence in the community: 1. Continue screening at points of entry into Carle systems in Vermilion County for signals of abuse, neglect, gun safety, and more violence related indicators to identify and act proactively on signals of violent households and make linkages to supportive community services. 2. Investigate and advocate for seniors experiencing physical, social isolation, or financial abuse through partnership with Carle Health CRIS Healthy Aging Center. 3. Continue to provide in-kind support from Carle Health leadership to participate in the Vermilion County Violence Coalition. Evaluation of Prior Impact In response to the identified health need, Carle Hoopeston Regional Health Center took the following actions: -Ensured routine and thorough screenings at points of entry for signals of abuse, neglect, gun safety, and violence. -Through partnership with the CRIS Center for Health Aging, Carle Hoopeston Regional Health Center investigated and advocated for seniors experiencing physical, social isolation, or financial abuse. -Carle Hoopeston Regional health Center provided $3,750 to organizations address violence in communities, including the Survivor Resource Center and DARE. Income/Poverty: Description of Priority Area: According to recent Census data, over 18% percent of Vermilion County residents live in poverty, and 13% of Vermilion County residents are expected to experience food insecurity. Research has shown people living in poverty can face greater barriers accessing medical care, less likely to have health insurance, have less access to healthy foods, which contribute to higher rates of obesity and chronic disease. Poor health can limit one's ability to work, reduce economic opportunities, inhibit educational attainment, and lead to medical debt and bankruptcy. Residents identified income/poverty as a high area of concern as did community stakeholders who described their focus on educational advancement and increased awareness of community resources that offer assistance.

Carle Hoopeston Regional Health Center will address food insecurity, will work on establishing community access to healthy food access, and work towards healthy foods rx. 13. Expand prescription for nutrition program and food access through Carle's Mobile Health Clinic. Evaluation of Prior Impact In response to the identified health need, Carle Hoopeston Regional Health Center took the following actions: -CHRHC hosted multiple healthcare-focused career/job fair in Vermilion County in 2023. It was well attended by teachers and students in Vermilion County. This will be an annual event. -Carle Hoopeston Regional Health Center continued to work on establishing mentoring programs with local school districts. This was not completed in 2023, but work underway. -Carle Health completed and began moving into the Carle at the Riverfront project, a significant investment in downtown Danville, in Vermilion County. This commitment to economic development and community impact has had a visible halo effect in downtown and the surrounding communities in Vermilion County. Healthy Behaviors: Description of Priority Area: Like many communities in the United States, obesity and obesity related illnesses continue to be a concern in Vermilion County. Obesity is associated with poorer mental health outcomes, reduced quality of life, and the leading cause of death in the U.S. and worldwide, through contributing to heart disease, stroke, diabetes and some types of cancer. According to the most recent County Health Rankings for Vermilion County, 38% of adults in Vermilion County are obese, higher than the Illinois average of 34%. In the 2022 CHNA survey, respondents indicated that being overweight was the second most important health issue and was rated as the most prevalently diagnosed health condition. Research strongly suggests that obesity is a significant problem facing youth and adults nationally, in Illinois, and within Vermilion County. A healthy lifestyle, comprised of regular physical activity and balanced diet, has been shown to increase physical, mental and emotional well-being. In Vermilion County, 31% of adults are subject to physical inactivity, while access to healthy exercise opportunities sit at 68%, lower than both Illinois and the US. A healthy lifestyle, comprised of a proper diet, has been shown to increase physical, mental, and emotional well-being. Consequently, nutrition and diet are critical to preventative care. Over two-thirds (69%) of residents report no consumption or low consumption (1-2 servings per day) of fruits and vegetables per day. Implementation Plan - Anticipated Impact & Plan to Evaluate: Carle Hoopeston Regional Health Center will pursue these initiatives to increase healthy behaviors, and reduce obesity: 1. Encourage providers to give out nutrition Rx and physical activity Rx. 2. Provide secure and healthy food options through Congregate Meal Program and Meals on Wheels through partnership with Carle Health CRIS Healthy Aging Center. 3. Provide comprehensive assessments providing homecare aides, emergency home response services, automated medication dispense as well as referrals to other CRIS services through the Care Coordination Unit through partnership with Carle Health CRIS Healthy Aging Center. 4. Provide Wellness Works articles related to health at all Farm Bureaus on a monthly basis through Carle Health Rural Health and Farm Safety. 5. Offer education on critical farm and rural safety by providing Progressive Ag Safety Days within Vermilion County through Carle Health Rural Health and Farm Safety. 6. Inform Vermilion County of significant health priorities and awareness efforts through bulletin inserts in faith communities through Carle Health Faith Community Health. 7. Continue to distribute food boxes to families in need under the Carle Community Health Initiative. 8. Increase awareness of overall health and wellness through Carle and Health Alliance health-based social media platforms. 9. Donations to community and school-based programs that encourage physical activity and nutritional education. Evaluation of Prior Impact: In response to the identified health need, Carle Hoopeston Regional Health Center took the following actions: -Carle Hoopeston Regional Health Center worked to develop plans for a Demonstration Kitchen, where patients and community residents can observe lessons and learn how to cook healthy meals using fresh, healthy ingredients. The Demonstration Kitchen will be completed in 2024. -Purposeful design by Carle Hoopeston Regional Health Center and Carle Health in the Carle and the Riverfront project was a walkway on campus, where community members can exercise and walk in a safe and well-lit area. -Carle Hoopeston Regional Health Center supported the build out of a community garden in 2023, providing access to nutritious foods and an opportunity to learn about growing food for patients and the community. Carle is exploring whether or not the community garden will yield enough produce to help support the Mobile Market, but will continue to support the community with healthy food access either way. NEEDS CARLE IS NOT ADDRESSING: While there were a number of additional needs identified by the data and input from community leaders, Carle does not have the resources to adequately address all of them. The issues that will not be addressed in this implementation plan were prioritized as such because the needs were not as significant; they are not central to the hospital's mission; and Carle has determined it does not have the ability to make a measurable impact in those areas. Carle will not focus on addressing community health challenges related to cancer, dental health, language barriers, and STIs outside of our regular and constant efforts to provide top clinical care to those we serve.

Schedule H, Part I, Line 7 OTHER COMMUNITY BENEFITSCASH AND IN-KIND DONATIONS CARLE HOOPESTON REGIONAL HEALTH CENTER CONTRIBUTED OVER $23,000 TO CHARITABLE ORGANIZATIONS IN THE COMMUNITY VIA CASH AND IN-KIND DONATIONS, INCLUDING PROGRAMS AIMED TO HELP YOUTH, HEALTH IMPROVEMENT, HEALTH EDUCATION AND HEALTH PROFESSIONS EDUCATION IN 2023. A SIGNIFICANT PORTION OF CHRHC'S CASH DONATIONS, OVER $10,000 - WENT TOWARDS HEALTH IMPROVEMENT IN 2023. THE LARGEST OF THIS CATEGORY, $5,000 WAS DONATED TO THE HOOPESTON MULTI AGENCY TO ENHANCE THE MULTITUDE OF IMPORTANT COMMUNITY BENEFIT SERVICES THEY OFFER. THE ORGANIZATION ADDRESSES NEEDS FOR DISABLED AND ELDERLY PATIENTS, INCLUDING RIDES THROUGHOUT HOOPESTON AND DANVILLE TO HELP MAINTAIN THEIR HEALTH AND LIFESTYLE, AND ASSISTING CLIENTS IN COORDINATING CARE APPOINTMENTS AND FINDING RESOURCES.
Schedule H, Part II Community Building ActivitiesCOMMUNITY-BUILDING ACTIVITIES: CHRHC'S SUPPORT OF COMMUNITY-BUILDING ACTIVITIES IS AN IMPORTANT PART OF CONTRIBUTING TO THE ECONOMIC VIABILITY OF THE COMMUNITY. A SIGNIFICANT PORTION OF THESE ACTIVITIES IS FOCUSED ON COMMUNITY SUPPORT AND ECONOMIC DEVELOPMENT, INCLUDING CASH, IN-KIND DONATIONS AND BUDGETED EXPENDITURES FOR THE CITY, BUSINESS ASSOCIATIONS AND OTHER PROGRAMS IN HOOPESTON, TOTALING OVER $5,000 IN 2023. WHILE NOT PAID FOR BY CHRHC BUT BY THE CARLE FOUNDATION, CARLE EMPLOYEES WERE ALSO ACTIVE ON COMMUNITY BOARDS AND COMMISSIONS IN 2023 IN THE CHRHC SERVICE AREA, BRINGING LEADERSHIP TO THE COMMUNITIES, INCLUDING: ROTARY CLUBS, VERMILION ADVANTAGE, AND SPEAKING ON COMMUNITY PANELS.
Schedule H, Part III, Line 2 Bad debt expense - methodology used to estimate amountTO COMPUTE AND CONVERT FINANCIAL ASSISTANCE, UNREIMBURSED MEDICAID, MEANS-TESTED PROGRAMS, AND BAD DEBT CHARGES TO COST; A CONSISTENT GAAP (GENERALLY ACCEPTED ACCOUNTING PRINCIPLES) BASED COST-TO-CHARGE RATIO WAS USED ACROSS ALL PAYERS. ALTHOUGH THE METHODOLOGY WAS SIMILAR TO WORKSHEET #2, FOR SIMPLICITY PURPOSES CERTAIN IMMATERIAL VALUES WERE OMITTED. OTHER COMMUNITY BENEFITS COSTS WERE REPORTED AT THE ACTUAL EXPENSE INCURRED. PATIENT RECEIVABLE PAYMENTS AND RELATED DISCOUNTS WERE RECORDED AT ACTUAL AMOUNTS AT THE TIME OF PAYMENT RECEIPT. A SEPARATE GAAP-BASED PROVISION FOR ESTIMATED BAD DEBTS AND DISCOUNTS WAS RECOGNIZED FOR ACCOUNTS IN PROCESS AND PENDING ADJUDICATION AND PAYMENT OCCUR. ACCOUNTS DETERMINED ELIGIBLE FOR FINANCIAL ASSISTANCE WERE PROCESSED IMMEDIATELY FOR FINANCIAL ASSISTANCE DISCOUNT WITH NO COLLECTION EFFORT. FOR ACCOUNTS WITH INSUFFICIENT INFORMATION AND DOCUMENTATIONTO DETERMINE FINANCIAL ASSISTANCE ELIGIBILITY, THE HOSPITAL CONSULTED WITH A VARIETY OF ALTERNATIVE SOURCES TO HELP DETERMINE AN INDIVIDUAL'S FINANCIAL MEANS (OR LACK OF MEANS) TO PAY. BASED ON RELATED TRENDS, THE HOSPITAL FURTHER DEVLOPED A GENERAL ESTIMATE OF FINANCIAL ASSISTANCE WHICH CONTINUED TO RESIDE WITHIN BAD DEBTS. This narrative also pertains to Schedule H, Part I, Line 7 EXPLANATION OF COSTING METHODOLOGY USED FOR CALCULATING LINE 7 TABLE.
Schedule H, Part III, Line 3 Bad Debt Expense MethodologyCARLE HOOPESTON REGIONAL HEALTH CENTER USES 50% AS A GENERAL ESTIMATE OF BAD DEBT EXPENSE ATTRIBUTABLE TO PATIENTS ELIGIBLE UNDER THE ORGANIZATION'S FINANCIAL ASSISTANCE POLICY.
Schedule H, Part III, Line 4 Bad debt expense - financial statement footnoteAS A RESULT OF ADOPTING ASU 2014-09, THE HEALTH SYSTEM CONTINUED TO MAINTAIN AN ALLOWANCE FOR BAD DEBTS RELATED TO PERFORMANCE OBLIGATIONS SATISFIED PRIOR TO JANUARY 1, 2018. THESE ACCOUNTS HAVE ALL BEEN FULLY RESOLVED, THEREFORE THE ALLOWANCE FOR BAD DEBTS HAS DECLINED TO $0. PLEASE SEE SCHEDULE H, PART III, LINE 2 METHODOLOGY USED TO ESTIMATE BAD DEBT FOR 990 REPORTING PURPOSES.
Schedule H, Part III, Line 8 Community benefit & methodology for determining medicare costsTHE NUMERATOR (TOTAL EXPENSE) AND DENOMINATOR (TOTAL GROSS CHARGES) OF THE SIMPLE RATIO OF PATIENT CARE COST TO CHARGES IS ADJUSTED BY ELIMINATING NON-PATIENT CARE THAT GENERATES OTHER REVENUE, BAD DEBT EXPENSE, MEDICAID, AND OTHER PROVIDER TAXES AND THE TOTAL COST OF COMMUNITY BENEFIT ACTIVITIES AND PROGRAMS. ALSO, ANY GROSS PATIENT CHARGES FOR PROGRAMS NOT RELYING ON THE RATIO ARE ELIMINATED FROM BOTH THE NUMERATOR AND DENOMINATOR OF THE RATIO. THESE ADJUSTMENTS ARE INTENDED TO ELIMINATE ANY POTENTIAL FOR DOUBLE COUNTING OF COMMUNITY BENEFIT EXPENSES. THE RESULTANT RATIO ALIGNS WITH SCHEDULE H REQUIREMENTS. ILLINOIS LAW DEFINES GOVERNMENTAL-SPONSORED INDIGENT HEALTH CARE AS THE UNREIMBURSED COST OF MEDICARE, MEDICAID, AND OTHER FEDERAL, STATE, OR LOCAL INDIGENT CARE PROGRAMS. WHEN THERE IS A SHORTFALL, WE DO BELIEVE THIS IS A COMMUNITY BENEFIT BECAUSE, AS A HOSPITAL, WE ARE STEPPING UP TO CARRY THE BURDEN OF THE GOVERNMENT, ASSURING CARE TO SENIORS, AND THOSE LESS FORTUNATE DEMOGRAPHICS THAT HAVE EXPERIENCED INCREASING COSTS OVER THE PAST DECADE WHILE LIVING ON FIXED INCOMES.
Schedule H, Part III, Line 9b Collection practices for patients eligible for financial assistanceEFFECTIVE JANUARY 1, 2016, CARLE COMBINED THE CARLE COMMUNITY CARE DISCOUNT PROGRAM WITH THE CARLE HOOPESTON REGIONAL HEALTH CENTER CHARITY PROGRAM TO MAKE ONE PROGRAM: THE CARLE FINANCIAL ASSISTANCE PROGRAM (CFAP). CFAP PROVIDES DISCOUNTED OR FREE CARE TO THOSE WHO NEED IT WITHIN THE CARLE FOUNDATION, INCLUDING CARLE HOOPESTON REGIONAL HEALTH CENTER. NOT ONLY DOES THIS RESULT IN A MORE TIMELY APPLICATION PROCESS, BUT MOST IMPORTANTLY, IT MEANS PATIENTS RECEIVE THE SAME DISCOUNT AT THE HOOPESTON HOSPITAL AND CLINICS IF THEY MIGHT NEED AN ADDITIONAL LEVEL OF SPECIALTY OR HOSPTIAL CARE. WITH THIS PROGRAM, CARLE HOOPESTON REGIONAL HEALTH CENTER HAS A ROBUST PRESUMPTIVE ELIGIBILITY PROCESS. WE PRESUME ELIGIBILITY FOR VERIFIED HOMELESS, DECEASED WITH NO ESTATE, MENTAL INCAPACITATION, RECIPIENTS OF WIC (WOMEN, INFANTS AND CHILDREN NUTRITION PROGRAM), SNAP (SUPPLEMENTAL NUTRITION ASSISTANCE PROGRAM), LIHEAP (LOW INCOME HOME ENERGY ASSISTANCE PROGRAM), ILLINOIS FREE LUNCH AND BREAKFAST PROGRAM, RECEIPT OF GRANT ASSISTANCE FOR MEDICAL SERVICE, FRANCES NELSON HEALTH CENTER (PROMISE HEALTHCARE FQHC) DISCOUNT REFERRALS, MEDICAID TITLE XIX, XXI, AND IN-NETWORK MEDICAID MANAGED CARE PLANS. IN ADDITION, WE UTILIZE A VENDOR, EXPERIAN INFORMATION SOLUTIONS, INC., TO PROACTIVELY IDENTIFY PATIENTS WHO MAY BE PRESUMPTIVELY QUALIFIED FOR ASSISTANCE - NOT ONLY FOR PUBLIC PROGRAMS LIKE MEDICAID, BUT ALSO BASED ON A NUMBER OF KEY FINANCIAL INDICATORS, INCLUDING CREDIT HISTORY, DEMOGRAPHICS, AND GROSS INCOME. IDENTIFIED PATIENTS MAY BE PRESUMED ELIGIBLE AND AUTOMATICALLY ENROLLED IN THE PROGRAM, OR THEY MAY BE CONTACTED AND ENCOURAGED TO APPLY FOR ASSISTANCE. PATIENTS WHO ARE NOT DEEMED PRESUMPTIVELY ELIGIBLE WOULD NEED TO REQUEST AND COMPLETE AN APPLICATION. IF THE PATIENT DOES NOT REQUEST OR COMPLETE AND RETURN THE APPLICATION, THEN THE BALANCE IS DEEMED THEIR RESPONSIBILITY TO PAY. FOR PATIENTS THAT DO NOT QUALIFY FOR CFAP, AND WHO MAY BE UNINSURED OR UNDERINSURED, CARLE HOOPESTON REGIONAL HEALTH CENTER HAS OTHER DISCOUNT OPTIONS AVAILABLE, SUCH AS PROMPT PAY, ILLINOIS UNINSURED DISCOUNT, AND CAPPED DISCOUNT - WHERE A PATIENT'S OUT-OF-POCKET MEDICAL EXPENSES ARE LIMITED TO 40% OF THEIR ANNUAL GROSS INCOME IF THEY EARN AT OR BELOW 400% OF THE FEDERAL POVERTY LEVEL. ONCE ALL APPLICABLE DISCOUNTS HAVE BEEN APPLIED, WE MAKE EVERY ATTEMPT TO WORK WITH THE PATIENT AND SET UP PAYMENT ARRANGEMENTS ON THE REMAINING BALANCE DUE. THE CURRENT MINIMUM IS 5% OF THE TOTAL BALANCE DUE OR $25.00 A MONTH. IF THEY CANNOT MEET THESE GUIDELINES, OUR IN-HOUSE BILLING STAFF MEMBERS WORK WITH THEM TO SET UP A TEMPORARY, SHORT-TERM PAYMENT ARRANGEMENT UNTIL THEY CAN MAKE THE MINIMUM PAYMENT. IF THEY ARE UNABLE TO MAKE PAYMENT ON THE BALANCE DUE, THEN THE BALANCE MAY BE LISTED WITH AN OUTSIDE COLLECTION AGENCY. WHEN THE ACCOUNT IS STILL IN-HOUSE, THE MINIMUM NOTIFICATION IS MONTHLY ITEMIZED STATEMENTS. IF A PATIENT DOES NOT RESPOND, THE ACCOUNT IS GIVEN A FINAL NOTICE, EITHER BY LETTER OR PHONE, AND SENT TO AN OUTSIDE COLLECTION AGENCY. CARLE HOOPESTON REGIONAL HEALTH CENTER WILL NOT FILE COLLECTION SUIT LIENS ON A PRIMARY RESIDENCE, NOR DO WE AUTHORIZE AN AGENCY TO DO SO-CALLED "BODY ATTACHMENTS." THE AGENCIES ARE AWARE OF OUR CFAP AND HAVE BEEN ADVISED TO REFER THE PATIENTS BACK TO CARLE HOOPESTON REGIONAL HEALTH CENTER IF THEY DEEM THE PATIENT IS UNABLE TO PAY. BILLING STAFF WILL THEN MAKE ONE MORE ATTEMPT TO WORK WITH THE PATIENT TO SEE IF HELP IS AVAILABLE. IF STAFF DETERMINES THAT THE PATIENT MIGHT QUALIFY, WE SEND THE PATIENT A CFAP APPLICATION AND INSTRUCT THE AGENCY TO PUT A HOLD ON THE ACCOUNT; THE AVERAGE HOLD IS 60 DAYS. IF THE PATIENT IS THEN APPROVED FOR DISCOUNTS AT 100% FOR FINANCIAL ASSISTANCE, THE BALANCE IS ADJUSTED AND THE ACCOUNT IS CLOSED WITH THE AGENCY. IF THE PATIENT IS APPROVED FOR LESS THAN 100%, THE ADJUSTMENTS ARE REPORTED TO THE AGENCY AND THE AGENCY WILL BEGIN COLLECTION EFFORTS ON ANY REMAINING BALANCE.

Schedule H, Part VI, Line 2 Needs assessmentCarle primarily addresses healthcare needs in our community by being a part of the aforementioned CHNA in partnership with Vermilion County Public Health Department, OSF Healthcare, and United Way of Vermilion County. However, Carle is acutely aware of the need for Access to Care, making it a mainstay of our community benefit efforts. We have a strong financial assistance program based on a philosophy of doing the right thing for the community and patients, balanced by a careful stewardship of the community's resources. While Access to Care was not selected as a priority for the 2022 CHNA conducted in conjunction with community partners, critical access to care will continue to be a priority for Carle Hoopeston Regional Health Center. As a tax-exempt organization, Carle Hoopeston Regional Health Center provides care to patients regardless of their ability to pay for that care or source of payment. We also recognize that some patients need help paying their bills. Carle's Financial Assistance Program provides discounts or free care to those who need it. Carle's generous Financial Assistance Program, consistently and diligently applied, has resulted in our ability to reach many people over the years. In addition to charity care, Carle supports a wide range of programs and services to increase community capacity, health care work force expansion, and social services that provide complementary healthcare-related services. Carle Hoopeston Regional Health Center has, and will continue to, pursue these initiatives to improve access to care in partnership with the Carle Health system: 1. Offer a charity care program and communicate the availability of that program. 2. Recruit more providers into the Carle system, thereby expanding access/capacity. 3. Support local community clinics to ensure added local capacity for health care. 4. Donate to existing community health programs. 5. Participate in population health initiatives that actively manage the health of members. 6. Promote prescription affordability as a 340B provider. 7. Enhance access-related initiatives that will improve patient access and ability to interface more efficiently for needed services - Patient Contact Center, Scheduling Center, Prescription Refill request process, virtual visits and more. 8. Continue to provide outreach care access and education through the Carle Community Health Initiative's Faith Community Health and Rural Health and Farm Safety services.
Schedule H, Part VI, Line 3 Patient education of eligibility for assistanceOur practice is to look at each patient's financial status in relation to our Carle Financial Assistance Program and the criteria of the Uninsured Patient Discount Act, and to provide the patient with the deepest discount available. By expanding the presumptive eligibility screening processes and determining the financial status of patients up-front, we have been able to pinpoint those needing assistance early in the process, minimizing bad debt and optimizing our ability to help. Staff is also diligent in following up with patients during hospitalization and after discharge if there's any reason to believe the patient could benefit from financial assistance, and we auto-qualify certain patient populations for Carle Financial Assistance Program, such as the homeless, WIC, SNAP (Supplemental Nutrition Assistance Program), Medicaid, Low Income Home Energy Assistance Program (LIHEAP), and Township Assistance recipients. Communicating that Financial Assistance is Available CARLE HOOPESTON REGIONAL HEALTH CENTER has made a concerted, continuous effort to be sure that people have access to information that will help them with their medical bills. These include: -Advertising Carle Financial Assistance Program; continued presence in appropriate community publications; and on-site via displays throughout the hospital and clinics -Simplified application form, including a version in Spanish, that contains information regarding the Carle Financial Assistance Program -Publication of a Plain Language Summary and all other financial assistance-related information on https://carle.org/patients-visitors/financial-assistance -Information about the Carle Financial Assistance Program on all statements, collection letters and Hospital admission packets -Carle Financial Assistance Program information and applications at all registration points, Hospital main lobby and carle.org -Meetings with local legislators to help them assist constituents with healthcare needs, including financial assistance
Schedule H, Part VI, Line 4 Community informationThe primary service area of Carle Hoopeston Regional Health Center is defined as the geographical boundary of Vermilion County, Illinois. While some residents of surrounding counties utilize CHRHC's services, our most recent CHNA targeted Vermilion County residents, who make up the majority of CHRHC patients. There are 4 other hospitals also serving the community: Gibson Area Hospital, OSF Sacred Heart Medical Center, Iroquois Memorial Hospital, and Riverside Medical Center. Pockets of extreme poverty exist throughout this region, with rates highest around the city of Danville. The programs within our community benefit plan generally have impact upon all the targeted communities, with certain programs directed at specific populations. Vermilion County's population as of 2022 is 72,337, predominantly white (81.8%). The percentage of individuals living in poverty is 19.7%, with 6.5% of the population under 65 not having health insurance. Persons living in poverty per capita is 19.7%, and 6.5% of the population under 65 years of age are without health insurance.
Schedule H, Part VI, Line 5 Promotion of community healthCarle Hoopeston Regional Health Center has an open medical staff and is governed by an 11-member community board that serves on a volunteer basis, with 7 voting members and 4 non-voting staff members. Surplus funds are reinvested into the organization for technology replacement and advancement, clinical service expansion and quality improvement. Funds are also invested into our communities to meet identified health needs which contribute to the overall well-being of the residents of the communities we serve.
Schedule H, Part VI, Line 6 Affiliated health care systemCarle Hoopeston Regional Health Center is a Critical Access Hospital committed to improving quality of life in northern Vermilion County and the surrounding area. CHRHC integrated with The Carle Foundation in 2012 to provide more access to primary and specialty care. Though CHRHC falls under the umbrella of The Carle Foundation, it maintains a separate board of directors and leadership. While CHRHC's CHNA and community benefit reporting is separate from Carle Foundation Hospital, it follows the same guidelines and principles as Carle. CHRHC trustees are involved in the CHNA and implementation plan, and serve on human services agency boards and committees to provide support to help address identified community needs in Vermilion County.
