Schedule H, Part V, Section B, Line 3ETHE PRIORITIES CONSIST OF FOCUS AREAS THAT IMPACT 6 OF THE 8 HEALTH CONCERNS FOR ULSTER COUNTY OUTLINED IN THE MID-HUDSON REGIONAL COMMUNITY HEALTH ASSESSMENT 2022-2024. THE NEW YORK STATE PREVENTION AGENDA OUTLINES TWO OTHER PRIORITY AREAS THAT WERE NOT SELECTED AS PRIORITIES FOR HEALTHALLIANCE HOSPITAL 2022-2024 COMMUNITY SERVICE PLAN: PROMOTE A HEALTHY AND SAFE ENVIRONMENT AND PREVENT COMMUNICABLE DISEASES. ALTHOUGH ALL OF THESE MERIT FOCUS FOR IMPROVING POPULATION HEALTH, THEY WERE NOT SELECTED DUE TO THE LIMITED AMOUNT OF RESOURCES AVAILABLE TO ADDRESS THESE ISSUES AND THE RELATIVE SEVERITY OF NEED DEMONSTRATED FOR THE PRIORITY AREAS THAT WERE CHOSEN.
Schedule H, Part V, Section B, Line 5 Facility , 1Facility , 1 - HEALTHALLIANCE HOSPITAL. HealthAlliance Hospital participated in two community health needs assessment workgroups. The first, led by the Ulster County Departments of Health and Mental Health (UCHMH), includes members of the established Healthy Ulster Council. Representatives from UCHMH, HealthAlliance Hospital, Ellenville Regional Hospital, Cornell Cooperative Extension, Live Well Kingston, American Lung Association, TFAC (Tobacco Free Action Communities) in Ulster, Dutchess, and Sullivan, and the Institute for Family Health, regularly meet to build on existing strengths, share services with one another, and create an integrated system of chronic disease prevention. In 2022, the focus shifted to the community health assessment (CHA) and monthly meetings were spent discussing assessment timeline, survey distribution strategies, data review, Prevention Agenda priority selection, and best-practices intervention presentations to be included in the members' community health improvement plan (CHIP) or community service plan (CSP). While all members reviewed and analyzed secondary data sources, e.g., federal, state, regional and local data, to identify trends and patterns that influence or result in poor physical and behavioral health outcomes for Ulster County residents, primary data collection and analysis activities were conducted by Siena College Research Institute (SCRI) under contract with the seven counties' Departments of Health, including Ulster County. SCRI conducted a Mid-Hudson Regional Community Health Assessment survey and collected 5,699 responses from the region's adult residents from March - May 2022. The overall sample was weighted by age, gender, reported race/ethnicity, income and county using the 2015-2020 American Community Survey (ACS) 5-year estimates to ensure statistical representativeness. Respondents were contacted in English and Spanish via landline telephone, cell phone, an online panel, online recruitment from each county and at various in-person events, and other community partnerships to enhance representation. HealthAlliance Hospital further facilitated survey distribution efforts to ensure all segments of the county's population were provided a chance to voice their health needs and concerns, especially those underserved and underrepresented. The hospital posted the community survey link on its public-facing webpage and publicized the survey opportunity through its social media platforms, including Instagram and Twitter/X. HealthAlliance also disseminated the survey to all employees for their participation and input; partnered with Ulster County Chamber of Commerce and utilized the snow-ball method to distribute to all of their members, who in turn encouraged their employees to complete the survey; conducted community outreach at community events, including the popular Hudson Valley Renegades baseball games in the Minor League, the Dutchess/Ulster Heart Walk, and the Kingston Women's Bike Fest; partnered with People's Place, a not-for-profit agency in Ulster County, to ask their patrons to participate in the survey as the agency serves a traditionally underserved and low-income population; and promoted the survey link via a QR code (e.g. printed and publicly posted large posters with the QR code) for ease of participation. Over a two-month period, a total of 647 Ulster County adult residents completed the community survey. The county-wide sample of 647 was also weighted by age, gender, reported race/ethnicity, and income to ensure statistical representativeness. In addition to participating in the Mid-Hudson Regional Community Health survey, a Community Partner Survey was also conducted by Siena College in the spring and early summer of 2022. The survey collected data from health and human services providers who serve underrepresented populations, including low-income, veterans, persons experiencing homelessness, the aging population, the LGBTQIA+ community, and people with a mental health diagnosis or those with a substance use disorder (SUD). HealthAlliance Hospital participated as a respondent among the final sample of 40. HealthAlliance Hospital also joined the Greater New York Hospital Association's (GNYHA) 2022 Community Health Needs Assessment (CHNA) Collaborative, which supported participating hospital members' primary data collection efforts to meet the requirements of the NYS CSP by gathering information on community health needs and engaging with community members. A diverse group of GNYHA member hospitals across the state participated in the Collaborative, including community and safety net hospitals, small health systems, and large academic medical centers. GNYHA developed the CHNA survey with members' input in multiple stages through a collaborative and iterative process, made the survey available in 11 languages on paper and online, collected the data and analyzed the results, and created custom reports for each participating hospital. The survey used validated questions from existing surveys such as the CDC, BRFSS (Behavioral Risk Factor Surveillance System), and the NYC Department of Health and Mental Hygiene's Community Health Survey (NYC CHS). Once the CHNA survey was designed, all recruitment activities were tasked among participating hospital members. HealthAlliance Hospital, along with other participating collaborative members, engaged and enlisted adult participants using similar strategies as the Mid-Hudson Regional Community Health survey mentioned above. The Collaborative efforts resulted in a final sample of more than 17,600 responses; 331 of which were from Ulster County.
Schedule H, Part V, Section B, Line 6a Facility , 1Facility , 1 - HEALTHALLIANCE HOSPITAL. The CHNA was conducted with HealthAlliance Hospital (both Broadway and Mary's Avenue Campuses) and Ellenville Hospital. Please note in 2022, HealthAlliance Hospital was consolidated into one campus on Mary's Avenue.
Schedule H, Part V, Section B, Line 6b Facility , 1Facility , 1 - HEALTHALLIANCE HOSPITAL. HealthAlliance Hospital participated in two community health needs assessment workgroups. The first, led by the Ulster County Departments of Health and Mental Health (UCHMH), includes members of the established Healthy Ulster Council. Representatives from UCHMH, HealthAlliance Hospital, Ellenville Regional Hospital, Cornell Cooperative Extension, Live Well Kingston, American Lung Association, TFAC (Tobacco Free Action Communities) in Ulster, Dutchess, and Sullivan, and the Institute for Family Health. Schedule H, Part V, Section B, Line 11 Facility , 1Facility , 1 - HEALTHALLIANCE HOSPITAL. Based on the findings at the conclusion of the two CHNA workgroup assessments, along with an internal review process to determine HealthAlliance's areas of expertise and available resources, HealthAlliance Hospital aligned its action plans with the NYS 2019-2024 Prevention Agenda by targeting two public health prevention priorities: I. Prevent Chronic Diseases Focus Area 4: Chronic Disease Preventive Care and Management II. Promote Healthy Women, Infants and Children Focus Area 1: Maternal and Women's Health For Priority I, HealthAlliance Hospital, through its outpatient Diabetes Education Center, has promoted an evidence-based care intervention to prevent and manage diabetes and prediabetes patients by 1) increasing the proportion of people with diabetes who get formal diabetes education by 10% by December 2024 and 2) decreasing the percentage of adult members with diabetes whose most recent HbA1c (hemoglobin A1C) level indicated poor control (>9%) by 10% by December 2024. To achieve these goals, the Center, which is recognized by the American Diabetes Association (ADA), has been providing Intensive Lifestyle Interventions education program for patients with type 2 diabetes to improve their glycemic control and reduce their risk factors for cardiovascular disease. The Center's Certified Diabetes Care and Education Specialists (CDCES) were anticipated to serve a minimum of 220 patients annually through the development of individualized management plans that fit their patients' lifestyles, beliefs, and culture. In the years 2022-2023, the CDCES served 228 patients and 207 respectively, averaging to an amount of 217.5 patients annually. The CDCES has been working to integrate the ADCES' (Association of Diabetes Care & Education Specialists) seven self-care behaviors: Healthy Coping, Healthy Eating, Being Active, Taking Medication, Monitoring, Reducing Risk, and Problem Solving, into the intervention. The program has also focused on providing additional support to Medicaid patients to address their social determinants of health (SDOH) needs. For Priority II, by December 2024, HealthAlliance Hospital's goal is to reduce low-risk C-section (Cesarean delivery) to less than 25% from the rate of 30% at the time of the assessment. Through WMCHealth Institute for Women's Health and Wellness, the new Center for Women's Health Equity and HealthAlliance Hospital has been able to attain this goal with a multi-strategic approach. Those strategies include: 1) train all Obstetrics staff with the Relias performance modules, which promote vaginal birth and fetal heart rate monitoring; 2) offer expectant mothers prenatal obstetrics consultations and patient education opportunities to raise awareness about how to prepare for a healthy pregnancy and promote natural childbirth; 3) implement an updated workflow process among providers to utilize the revised C-section checklists to enable prenatal record and ultrasonography review; 4) implement Peer-to-Peer Discussions prior to any unscheduled, non-urgent C-sections; and 5) collaborate with community partners and enhance community outreach efforts to increase the number of enrolled expectant mothers into prenatal care, as well as linking Substance Use Disorders (SUD) mothers through HealthAlliance Hospital's widely successful Neonatal Abstinence Syndrome (NAS) program to address their SUD and SDOH needs. Over the course of 2023, HealthAlliance reduced the rate of low-risk Cesarean delivery to 20.8%. Efforts will continue to maintain the Cesarean delivery rate underneath the stated goal of 25% for 2024. The NYS Prevention Agenda outlines three other priority areas that were not selected as public health priorities for the HealthAlliance Hospital 2022-2024 CSP: Promote a Healthy and Safe Environment, Promote Well-Being and Prevent Mental and Substance Use Disorders, and Prevent Communicable Diseases, as a result of the public input and internal ranking system process. Beyond the two selected public health priorities, HealthAlliance Hospital acknowledges the wide range of significant health needs that were identified but cannot be addressed by the hospital due to the level of priority and the lack of expertise and resources. 2022 - 2024 COMMUNITY HEALTH NEEDS ASSESSMENT IMPLEMENTATION PLAN NYS prevention agenda priority area: Prevent chronic diseases Focus area 4: Chronic Disease Preventive Care and Management Prevention agenda goal 4.3: Promote evidence-based care to prevent and manage chronic diseases including asthma, arthritis, cardiovascular disease, diabetes and prediabetes and obesity. Objective 4.3.1: Increase the proportion of people with diabetes who get formal diabetes education (Healthy People 2030 - D-06) by 10% by December 2024 Evidence based intervention: Diabetes Management: Intensive Lifestyle Interventions for Patients with type 2 Diabetes to improve glycemic control and reduce risk factors for cardiovascular disease (https://www.thecommunityguide.org/findings/diabetes-intensive-lifestyle-interventions-patients-type-2-diabetes). Disparities Addressed: Low-income, racial/ethnic minorities, seniors and disadvantaged urban and rural communities NYS prevention agenda priority area: Prevent chronic diseases Focus area 4: Chronic Disease Preventive Care and Management Prevention agenda goal 4.3: Promote evidence-based care to prevent and manage chronic diseases including asthma, arthritis, cardiovascular disease, diabetes and prediabetes and obesity. Objective 4.3.2: Decrease the percentage of adult members with diabetes whose most recent HbA1c level indicated poor control (>9%) by 10% by December 2024 Evidence based intervention: Diabetes care and education specialists (DCES) will serve approximately 220 participants annually by December 2024 and develop individualized management plans that fit their lifestyles, beliefs and cultures. The DCES will ensure participants understand how to use various diabetic prevention devices like glucometers, insulin pens, pumps and continuous glucose monitoring devices; and use the information from these devices and participants' lifestyles to identify patterns and opportunities for better diabetic management. Furthermore, the specialists will help participants to develop coping skills to address the daily challenges of the disease, reduce the risks for complications, decrease costs by reducing or eliminating the need for medications and emergency room visits, and help find and access cost-savings programs. Disparities Addressed: Low-income, racial/ethnic minorities, seniors and disadvantaged urban and rural communities NYS prevention agenda priority area: Promote Healthy Women, Infants and Children Focus area 1: Maternal & Women's Health Prevention agenda goal 1.2.1: Reduce Maternal Mortality and Morbidity Objective 1.2.1: Decrease low-risk C-section (Cesarean delivery) rate to less than 25%. Evidence based intervention: * Enroll and train Obstetrics (OB) staff in an interactive online training platform, Relias Performance Modules, which promote vaginal birth and fetal heart rate monitoring; * Offer expectant mothers prenatal obstetrics consultations, including anesthesia consultation to evaluate high-risk stratified patients, as well as patient educations opportunities to raise awareness about how to prepare for a healthy pregnancy and to promote natural, vaginal childbirth; * Necessitate the usage of the revised C-section (Cesarean section) checklists, such as the labor induction and C-section scheduling forms, as part of a standardized process and workflow among providers to enable prenatal record and ultrasonography review; * Implement peer-to-peer discussion prior to unscheduled, non-urgent C-sections to ensure evidence-based care practice guidelines, in accordance with the American College of Obstetricians and Gynecologists (ACOG), are properly followed; and * Collaborate with community partners and enhance community outreach efforts to increase the number of expectant mothers enrolls into prenatal care, as well as linking opioid use disorders patients through HealthAlliance Hospital's successful Neonatal Abstinence Syndrome (NAS) Project to address some of their SDOH needs Disparities Addressed: Low-income, racial/ethnic minorities

Schedule H, Part I, Line 7g Subsidized Health ServicesTHE COSTS ON LINE 7G DO NOT INCLUDE ANY COSTS ATTRIBUTABLE TO PHYSICIAN CLINICS.
Schedule H, Part I, Line 7 Costing Methodology used to calculate financial assistanceTHE HOSPITALS' COSTING METHODOLOGY WAS BASED UPON WORKSHEET 2 OF THE IRS 990 INSTRUCTIONS. THE COST-TO-CHARGE RATIO WAS USED FOR THE VARIOUS SUB-LINE ITEMS OF LINE # 7. The hospital changed the way it is reporting financial assistance at cost as it relates to its presumptive charity. In 2023, the hospital changed to only include accounts with service dates from 2023, as those are the accounts for which it has costs in the current year. Any presumptive related to 2022 or prior were excluded from the 2023 return.
Schedule H, Part III, Line 2 Bad debt expense - methodology used to estimate amountThe amount reported on line 2 is equal to the provision for bad debt expense per the audited financial statements (AFS). The explanation of the methodology used to estimate this amount can be found in footnote #2 on page 19 of the attached Audited Financial Statements.
Schedule H, Part III, Line 4 Bad debt expense - financial statement footnoteThe text of the footnote that describes bad debt expense can be found on page 19 of the attached Audited Financial Statements.
Schedule H, Part III, Line 8 Community benefit & methodology for determining medicare costsThe costs on line 6 were taken directly from the cost report costs (D & E & I-series, where applicable) plus lab fee schedule costs and payments less subsidized services & Medicare losses which were taken from Exhibit 46 of the New York State Institutional Cost Report. For 2023 there was a shortfall from Medicare. Hospital costs were up but reimbursement from Medicare was down although inpatient volume was up. This is primarily due to outlier payments being down. The hospital should consider this loss a community benefit.
Schedule H, Part III, Line 9b Collection practices for patients eligible for financial assistanceThe credit and collection policy for self-pay balances provides that, in some cases, non-insured patients that do not qualify for Medicaid qualify for charity care. In those instances, the credit and collection department will follow the guidelines as set forth in the charity care policy. The charity care policy provides that the financial counselor evaluates the information provided in patients' charity care applications in conjunction with federal income poverty guidelines to determine the write-off percentage. If the patient/guarantor qualifies for charity care based on the sliding scale, the patient will be required to pay the outstanding amount. A payment plan is established not to exceed 10% of the guarantor's gross monthly income.

Schedule H, Part VI, Line 2 Needs assessmentFor the community-based strategies, HealthAlliance will continue to actively participate in a work group consisting of the public health department and health partners for the purposes of maintaining engagement with local partners over the during the period of this CHNA. Initially, meetings will be held throughout the year; however, frequency will be revisited throughout the timeframe to ensure that the meetings are meeting the needs of all partners. Schedule H, Part VI, Line 3 Patient education of eligibility for assistanceThe hospital informs the patient of the availability of charity care several ways: 1. notice is on every patient bill with an application on the back of the bill 2. on the hospital website 3. all members of the patient accounting department are trained to screen patients for charity care when discussing bills and concerns regarding inability to pay 4. fliers are posted in registration areas of the hospital 5. registration clerks will identify patients without a pay source and refer the patient to a financial counselor and hand the patient a financial assistance form 6. financial counselors assist patients in securing Medicaid and screen for charity care 7. the collection agencies that HAHV engages with are aware of the charity care policy and assist patients in applying up to 180 post-placement with them HealthAlliance is committed to: * providing access to quality healthcare services with compassion, dignity and respect for those we serve, particularly the poor and the underserved in our communities. * caring for all persons, regardless of their ability to pay for services. * assisting patients who cannot pay for part or all of the care they receive. * balancing needed financial assistance for some patients with broader fiscal responsibilities in order to sustain viability and provide the quality and quantity of services for all who may need care in a community. In accordance with American Hospital Association recommendations, HealthAlliance adopted the following guiding principles when handling the billing, collection and financial support functions for our patients: * provide effective communications with patients regarding hospital bills * make affirmative efforts to help patients apply for public and private financial support programs * offer financial support to patients with limited means * implement policies for assisting low-income patients in a consistent manner * implement fair and consistent billing and collection practices for all patients with patient payment obligations HealthAlliance effectively communicates with patients regarding patient payment obligations. Financial counseling is provided to patients about their payment obligation and hospital bills. Information on hospital-based financial support policies and external programs that provide coverage for services are made available to patients during the pre-registration and registration processes and in response to patients seeking financial assistance. Information regarding the financial assistance is also provided by the onsite financial counseling staff in both inpatient and outpatient areas. Patient accounting also supports the financial counseling program by providing patients with information and applications while handling customer service calls. Our financial counselors and county Medicaid workers also provide guidance regarding the financial assistance program when necessary. Financial assistance is available on the back of every hospital bill as well as the hospital website at www.hahv.org. Financial counselors make affirmative efforts to help patients apply for public and private programs for which they may qualify and that may help them obtain and pay for healthcare services. The patient does not need to have an account with the hospital; they just have to be in a situation in which they are in need of insurance coverage. Every effort is made to determine a patient's eligibility prior to or at the time of admission or service. However, determination for financial support can be made during any stage of the patient's stay after stabilization or collection cycle. HealthAlliance offers financial support to patients with limited means. This support is available to uninsured and underinsured patients who do not qualify for public programs or other assistance. Notification about financial assistance, including contact information, is available through the hospital website, hospital posters and flyers, financial assistance applications and hospital statements. The financial assistance policy (FAP) and application are available on our website at https://www.hahv.org/financial-assistance and are supplied in both English and Spanish as well as plain language summary format. If additional assistance is needed, HealthAlliance offers translation services for patients. HealthAlliance has an established written policy for the billing, collection and support for patients with payment obligations. The organization makes every effort to adhere to the policy and is committed to implementing and applying the policy for assisting patients with limited means in a professional, consistent manner. Educated staff members work closely with patients (including those working in patient registration and admitting, financial assistance, customer service, billing and collections) about these policies with an emphasis on treating all patients with dignity and respect regardless of their insurance status or their ability to pay for services. All patient registration staff receives in-service training regarding the financial assistance program. Patient accounting also receives information about the program and how to handle patients seeking financial assistance. Financial counselors receive in depth training to handle financial assistance requests, process applications, and manage outcomes. HealthAlliance provides several options for patients who either do not have insurance coverage or who have inadequate insurance coverage. Our many programs have been very successful and many have taken advantage of our offerings. Financial counseling offers a patient the opportunity to speak to a financial counselor to determine what assistance can be accessible based on family income. Patients often shop around for the most economical healthcare and we offer price quotes. This allows the patient to know their financial obligation up front and understand the competitive pricing being offered. Despite millions of dollars of charity care distributed to needy patients every year, a consistent challenge is patients not utilizing the programs we offer. Often, they don't want to provide the necessary financial information to initiate the financial aid process. Continual outreach and education will be ongoing to minimize confusion and improve utilization. HealthAlliance is committed to providing the best possible healthcare and is very sensitive to low income patients' ability to pay. The call center offers the convenience of calling one telephone number for all billing inquiries regardless of which hospital the patient is calling about.
Schedule H, Part VI, Line 4 Community informationIn 2023, HealthAlliance of the Hudson Valley, a member of the Westchester Medical Center Health Network (WMCHealth), operated a 177-hospital-bed health care system comprising HealthAlliance Hospital (HAH) in Kingston, NY, and Margaretville Hospital in Margaretville, NY. It also operated Mountainside Residential Care Center, an 82-bed nursing home in Margaretville adjacent to Margaretville Hospital. Historically, HealthAlliance Hospital in Kingston was comprised of two campuses, HAH Mary's Avenue Campus and HAH Broadway Campus. In December 2022, those two facilities merged into one hospital at the Mary's Avenue location, completing a 113 million dollar expansion and enhancement. The HealthAlliance Broadway will be transformed into a Health Village for outpatient services. On the reimagined Broadway campus, in the heart of Kingston, HealthAlliance will collaborate with community partners in the health village to provide preventative and primary care services in one convenient destination. HealthAlliance is guided by the needs of its patients and their families. HealthAlliance delivers the best health care of the highest value in a safe, compassionate environment; invests in innovative technologies and leading-edge therapies to advance health care delivery; and improves the overall health and well-being of the diverse communities it serves. The new HealthAlliance Hospital features the addition of a new, two-story, 79,000-square-foot building adjoining the current facility, as well as a full renovation of 48,000 square feet of space within the existing hospital. Key components of the new 162-bed facility include private rooms and a newly constructed: emergency care center, including emergency behavioral health; critical care unit; family birthing center; advanced imaging center, caregiver center, lobby and registration. Plus remodeled centers for ambulatory surgery, infusion therapy and endoscopy. The combined project on Mary's Avenue and Broadway, known as the Healthy Neighborhood Initiative, are expected to cost $134.9 million. HealthAlliance defines its primary service area (PSA) by a federal definition that consists of the top 75% of hospital discharges from the lowest number of contiguous zip codes. Due to the geographical location of acute care hospitals under HealthAlliance, there are two distinct primary service areas that lie within Ulster and Delaware counties, though not encompassing all of each county. Although defined as two service areas, HealthAlliance regards it as a single primary service area for operational community need development. According to the 2020 Census, the population for Ulster County was 178,371 with populations concentrated in the cities of Kingston, New Paltz and Saugerties. Patients from adjacent counties also visit the hospital or one of our outpatient locations for services that may not be available in their respective communities. Ulster County's percentage of adult smokers from 2016 to 2018 decreased from 15.6% to 14.7%, and further to a rate of 11.8% in 2021. In addition, Ulster County reported a higher percentage of cigarette smoking among those with poor mental health compared to those who indicated not having poor mental health (34.6% vs 13.4%). Although cigarette use has been decreasing over time, the use of electronic vapor products, also known as e-cigarettes, has increased dramatically. Ulster County is monitoring the use of vaping, especially among the young people of the county. According to the NYSDOH, e-cigarettes are by far the most used tobacco product by high school students in New York State. Among high school students, nearly 1 in 5 (18.7 percent) reported vaping nicotine compared to a record low of about 1 in 50 (2.1 percent) who reported smoking cigarettes in 2022. There is some evidence that current vaping among youth could lead to future cigarette use. The data formerly showed a surge in binge drinking, going from 10.5% of adults reporting binge drinking in 2013-2014, to more than double that at 22.2% in 2016. At the time, Ulster County showed the highest rates of binge drinking compared to the region and New York State. Ulster county has shown improvement on this front with rates dropping to 18.5% in 2018, and further to 12.2% in 2021. Ulster County now has the third lowest binge drinking rate in the region, and is underneath the statewide average of 14.9% Lastly, authorities in Ulster County have been focusing attention on the prevalence of opioid-related overdoses and deaths. Overdoses have been rising since 2014, more than doubling during that period. Overdose deaths in Ulster County increased 407% between 2014 and 2021, but have begun to fall in recent years. The continued rise in overdoses coupled with the decline in overdose fatalities does not represent an easing of the opioid epidemic, but rather the effectiveness of harm reduction strategies, including widespread distribution of naloxone and fentanyl test strips.
Schedule H, Part VI, Line 5 Promotion of community healthPlease see narrative for Schedule H, Part V, Section B, Line 11 for a complete description of how the organization's hospital facilities further its exempt purposes by promoting the health of the community. In addition, the COVID-19 pandemic created a greater need and focus on community health care. HealthAlliance of the Hudson Valley and the WMCHealth Network continued the leading role in the Hudson Valley on the dissemination of information surrounding COVID-19, educating the community, providing community testing, and vaccination. In all of our efforts we focused on the hardest to reach individuals and communities. We coordinated closely in all of our COVID-19 messaging, outreach, and direct care with New York State Department of Health, Delaware County Department of Health, and Ulster County Department of Health, other hospitals, healthcare providers, and Federally Qualified Health Centers. We provided detailed and ongoing information and messaging for community organizations, employers, and municipal governments. We participated at all levels of governments and with numerous community organizations on COVID-19 planning, preparedness, and response efforts. In January 2023, WMCHealth launched a network wide Equity, Inclusion and Diversity (EID) Steering Committee. The Committee serves to advise and actively engage in advancing activities to meet the organization's equity, inclusion and diversity priorities and goals. The committee will make recommendation with the objective of advancing EID best practices across the network aligned with the following priorities including but not limited to: * Promote inclusive practives to foster a welcoming climate for staff, patients, and visitors. * Advance initiatives to support culturally responsive patient-centered care. * Develop approached aimed at addressing disparities in care and health outcomes of surrounding communities. In June 2023 with the help of a $750,000 grant from New York State, WMCHealth and HealthAlliance of the Hudson Valley established the Center for Women's Health Equity, the first of its kind based in New York States' Hudson Valley Region. The aim of the Women's center is to reduce the disparities of heath among women in our region and advance health equity across the continuum of care. Additionally, HAHV's clinical and community relations teams provide ongoing outreach, education and programming services to the communities it serves. HAHV participated in more than 100 community events in 2023 which included such things as: hosting a wide variety of cancer support groups, providing bike helmet safety fittings, lending clinical expertise to community meetings and roundtables on health care issues (i.e. substance abuse, behavioral health care, pre-natal care, diabetes, etc.). A heart health community conversation in collaboration with the American Heart Association that also included a hands-only CPR demonstration. Free health screenings were offered at public events including Prostate Cancer screening and Blood Pressure screening. We also distributed information on public health issues such as Lyme Disease and Wound Care at numerous community events. Healthy cooking classes were offered to our patients and the communities to assist individuals in taking an active role in their ongoing health and wellness. HAHV also supports local charities in their mission of health and wellness in the communities.
Schedule H, Part VI, Line 6 Affiliated health care systemHealthAlliance of the Hudson Valley (HAHV) is the parent company to affiliate members HealthAlliance Mary's Avenue Campus, Broadway Campus, Margaretville Memorial Hospital, a rural critical access hospital and Mountainside Residential Care Center, a skilled nursing facility. This integrated healthcare system is committed to providing quality and compassionate medical care for our patients, their families and our community. In addition to service consolidation between the two Kingston hospitals, HAHV also committed to streamlining operations and improving care at the other affiliate locations. In September of 2020, the consolidation of HAHV Broadway Campus to the Mary's Ave Campus' operating license. The newly renovated, consolidated hospital opened at Mary's Ave in December of 2022. The Broadway campus will be transformed into a Health Village for outpatient services. On the reimagined Broadway campus, in the heart of Kingston, HealthAlliance will collaborate with community partners in the health village to provide preventative and primary care services in one convenient destination.
