| Form And Line Reference | Schedule D, Part X, Line 2 FIN 48 (ASC 740) footnote | FormAndLineReferenceDesc |
| Explanation | Schedule D, Part X, Line 2 requires that the organization provide the text of the footnote to the organization's financial statements that reports the organization's liability for uncertain tax positions under ASC 740. ASC 740 addresses the accounting for uncertainty in income taxes recognized in an entity's financial statements and prescribes a threshold of more-likely-than-not for recognition and derecognition of tax positions taken or expected to be taken in a tax return. The adoption of ASC 740 by Bon Secours Mercy Health, Inc. on September 1, 2007 did not have a material impact on BSMH's consolidated financial statements. As the organization does not conduct a separate audit of its financial statements, below is the related statement from the Bon Secours Mercy Health, Inc. consolidated audited financial statements: The Company and most of its subsidiaries (including certain joint venture entities) are exempt from federal income taxes under Section 501(c)(3) of the Internal Revenue Code of 1986, as amended. Their related income is exempt from federal income tax under Section 501(A). Some of the subsidiaries are taxable entities, and some of the income of the tax-exempt entities is subject to taxation as unrelated business taxable income. The Company and its subsidiaries file U.S. federal income tax returns, and they also file in various state and foreign jurisdictions. The Company accounts for uncertain tax positions in accordance with ASC Topic 740, Income Taxes. The Company accounts for uncertainty in income tax positions by applying a recognition threshold and measurement attribute for financial statement recognition and measurement of a tax position taken or expected to be taken in a tax return. The Company has determined that no significant unrecognized tax benefits or liabilities exist as of December 31, 2023. Accounting for uncertainty in income taxes, ASC Topic 740-10 prescribes a comprehensive model for how an organization should measure, recognize, present and disclose in its financial statements uncertain tax positions that an organization has taken or expects to take on a tax return. The Company is subject to routine audits by taxing jurisdictions. There are no current audits in progress. As of December 31, 2023, the Company has no uncertain tax positions. As of December 31, 2023 and 2022, the Company's taxable subsidiaries had net operating loss carryforwards for Federal income tax purposes of $182,444 and $169,279, respectively, which are available to offset future Federal taxable income subject to various limitations. The Company had net operating loss carryforwards for state income tax purposes of $119,717 and $113,528 as of December 31, 2023 and 2022, respectively, which are available to offset future state taxable income subject to various limitations. The Company accounts for income taxes under the asset and liability method. Under this method, deferred tax assets and liabilities are recognized for the estimated future tax consequences attributable to differences between the financial statement carrying amounts of existing assets and liabilities and their respective tax bases. Deferred tax assets and liabilities are measured using enacted tax rates expected to be in effect during the year in which those temporary differences are expected to be recovered or settled. The effect on deferred tax assets and liabilities of a change in tax rates is recognized in income in the period that includes the enactment date. Interest and penalties related to income taxes are accounted for as income tax expense. As of December 31, 2023, the Company has placed a partial valuation allowance of $83,168 and $18,945 on the gross deferred Federal and state tax assets, respectively. As of December 31, 2022, the Company has placed a partial valuation allowance of $156,538 and $103,396 on the gross Federal and state deferred tax assets, respectively. When determining the valuation allowance for deferred tax assets, the Company annually considers whether it is more likely than not that these amounts will not be utilized in future periods. There was a decrease in the valuation allowance for gross deferred Federal and state tax assets of $73,370 and $84,451, respectively, as of December 31, 2023. There was an increase in the valuation allowance for gross deferred federal tax assets of $1,354 and a decrease in the valuation allowance for gross deferred state tax assets of $10,532 as of December 31, 2022. | ExplanationTxt |

| Schedule H, Part V, Section B, Line 5 Facility A, 1 | Facility A, 1 - MERCY HEALTH - SPRINGFIELD REGIONAL MEDICAL CENTER. Springfield Regional Medical Center participated in a regional Community Health Needs Assessment (CHNA) process through participation with the Health Collaborative and Greater Dayton Area Hospital Association as well as a local Community Health Needs Assessment (CHNA) process coordinated by the Clark County CHNA/CHIP Stakeholder Group ("Stakeholder Group "Group"). Because the regional assessment included Cincinnati, Dayton, Springfield, and Urbana, we wanted to make sure the data collected, and priorities selected had the appropriate mix of localized data and health prioritization. As part of our regional CHNA data collection process, The Health Collaborative supported in providing tools for community input. Utilizing these tools, the needs assessment used a mixed-method approach to data collection, including secondary quantitative data and primary quantitative (Regional CHNA community and provider surveys) and qualitative (focus groups and interviews) data. Secondary data collection, beginning in January of 2021, sought to understand the greatest health conditions of the region, including prevalence and impact on community members. These results informed the creation of survey items that were organized around a set of cocreated research questions. Each data collection strategy adhered to a recruitment plan to ensure a representative sample of community members, voices of marginalized populations, and providers across the health and social services sectors were captured. All results are summarized for the region, which includes the Cincinnati Metropolitan Statistical Area (MSA), Dayton-Kettering MSA (including Clark County, which is not part of the Dayton MSA but is similar in that it borders the Dayton MSA and is not a rural county), and other rural counties in the geographic service area that are predominately rural and not included in other MSAs. Overall, the scope of data collection included 8,321 community surveys available in five languages, 859 provider surveys, and 5 focus Groups. The community surveys represented 26 counties, males, females, ages 18-65+, Black/African American, Multiracial, Asian, American Indian, Alaskan Native, White, and Hispanic/Latino populations. The provider surveys represented behavioral health, education, emergency medical services, faith-based organizations, federally qualified health centers, justice/corrections, medical care (adult, geriatric, pediatric), oral health, organizations addressing health related social needs and social determinants of health, pharmaceutical, and public health departments. The focus Groups were facilitated by the Clark County Combined Health District with sessions held with Latino Coalition, Springfield Promise Neighborhood, Family youth initiatives, and Restored Life Ministries. These events included Messiah Community (monthly education), YMCA kids programming (monthly nutrition), Senior Center Monthly Meal, Youth Center Monthly Meal, Legacy Apartments (monthly nutrition and blood pressure checks), Rotary Manor (monthly nutrition and blood pressure checks) Cherry Arbors (monthly blood pressure checks); and seven sessions as part of the 2023 Aging Mastery Program at the Urbana Senior Center. * In an effort to support area students with educational opportunities, Urbana Hospital successfully launched an internship program with students from Ohio Hi-Point Community Technical College. Through this program, ten students received valuable internship experience and one student was offered a full-time position. * To improve public transportation for residents and patients in the service area, Urbana Hospital participated and collaborated in four local and regional transportation meetings during 2023. * During 2023, Urbana Hospital conducted Social Determinants of Health Screenings and had 458 outpatient patients report transportation as being a challenge. Urbana Hospital used this information at local and regional transportation meetings to drive solution-oriented discussion, capacity building, troubleshooting, and began working through community partnerships to scale transportation accessibility. * In preparation of future strategic updates for local and regional transportation, Urbana Hospital has began planning the communication and rollout of transportation support. Behavioral Health (Mental Health, Addiction, Trauma) Implementation Activities: Goal: Improve mental health, addiction, and trauma access through the development of coordinated care, pathways, partnerships, and support programming in Champaign County. 2023: Strategies and Outcomes: * In an effort to meet the number of inpatient or outpatient depression screening referrals with community capacity, Urbana Hospital has collaborated with four local organizations in the behavioral health space. The collaborators included Urbana Paramedicine, the Champaign County CORE substance abuse and Mental Health Coalition, Champaign County Senior Center, and the Champaign County Youth Center. * During 2023, Urbana Hospital supported the re-launch of Mended Hearts, a community lead regional support group for former patients and their families who have had a physical cardiovascular issue resulting in the need for physical and emotional trauma and recovery. The program came to a pause when the pandemic occurred, and in its re-launch, Mended Hearts held nine support groups and served 85 people. * To help expand clinicians entering the Mental and Behavioral Health space locally, Urbana Hospital developed and launched a regional Behavioral Health Clinical Internship Program. The program had three interns for its launch year who provided mental and behavioral health care to 219 patients. * Urbana Hospital launched Behavioral Health telemedicine for psychiatry to further expand Mental Health services and support in Clark and Champaign Counties. Additionally, Urbana Hospital made pediatric psychiatry services through the telemedicine available in the Emergency Department and had 100% of consults fielded by a provider within eight hours. Chronic Disease: COPD, Heart Disease/Congestive Heart Failure, Cancer (with specific focus on Breast, Lung/Bronchus, and Skin/Melanoma) Implementation Activities: Goal 1: Increase community engagement and participation with prevention and education programming. Goal 2: Enhance screenings as well as services accessible to Champaign County communities in the areas of COPD, heart disease/congestive heart failure, and cancer (with specific focus on breast, lung/bronchus, and skin/melanoma). 2023 Strategies and Outcomes: * During 2023, Urbana Hospital participated in 91 local and regional events by providing topic-based learning outreach around health equity barriers and other health specific concerns. * Throughout the year, Urbana Hospital began work on identifying a Dermatology partner to assist with future Skin Cancer Screenings for Champaign County. * Urbana Hospital in partnership with Mercy Health Springfield, hosted nine educational and awareness events to promote access to lung cancer and breast cancer screenings. Through these events 890 mammogram screenings and 1,472 lung cancer screenings were conducted, a 25% increase in total screenings from 2022. * During 2023, Urbana Hospital expanded the cardiovascular service line to include electrophysiology, cardiac, and vascular services by adding two physicians. With the additional physicians, Urbana Hospital has expanded services available to the community by building the regional pathway for Cardio-oncology as a specialty service support. * In order to develop a cardiovascular/wound care treatment crossover for the care of patients with overlapping vascular, amputation, chronic wounds, diabetes, and connected cardiac catheterization needs, Urbana Hospital has started a best practice development study, began work on effective strategies to prevent lower limb amputation, started looking for a center of excellence, and began a new partnership with a doctor from Grandview Hospital to help give hyperbaric patients from Dayton to Springfield a place for local care. * During the year, Urbana Hospital launched revised clinical guidelines for better patient outcomes for both COPD and Congestive Heart Failure. Access to Care: Focus on primary care, women's health, appropriate point of care: (non-emergent options: urgent, virtual) Implementation Activities: Goal: Grow capacity and access points for Primary Care and Women's Health. Have more options for an appropriate point of care for non-emergency such as urgent and virtual care. 2023 Strategies and Outcomes: * During 2023, Urbana Hospital hired a new OB/GYN Oncology provider and launched a Women's Health OB/GYN office with surgical support to serve Champaign County. * Urbana Hospital recruited six new physicians to the region who started to offer non-emergent care models for greater local access to care. * To expand access and availability to receive care, Urbana Hospital developed and launched same-day appointment scheduling in the Primary and Specialty Care settings. | Schedule H, Part I, Line 7g Subsidized Health Services | COMMUNITY MERCY HEALTH PARTNERS (CMHP) OPERATES THE DPM REACH PROGRAM SUBSTANCE ABUSE AND COPD CLINIC. THE AMOUNT OF SUBSIDIZED HEALTH SERVICES ATTRIBUTABLE TO THE PHYSICIAN CLINIC IS $273,195. |
| Schedule H, Part I, Line 7 Bad Debt Expense excluded from financial assistance calculation | 21607651 |
| Schedule H, Part I, Line 7 Costing Methodology used to calculate financial assistance | Cost of financial assistance at cost was calculated with a cost to charge ratio using worksheet 2. The cost related to Medicaid patients was determined using Bon Secours Mercy Health's cost accounting system and included both inpatients and outpatients for traditional Medicaid and Medicaid managed care plans. For subsidized services Bon Secours Mercy Health's cost accounting system used to determine cost related to the specific service excluding traditional Medicaid and Medicaid managed care patients. Costs for charity and bad debt accounts are deducted using a ration of cost to charge specific to that subsidized service. Costs for other programs reflect the direct and indirect costs of providing those programs. |
| Schedule H, Part II Community Building Activities | Community Mercy Health Partners (CMHP) addresses various community concerns including health improvement, poverty, workforce development, and access to health care. CMHP hospitals conduct community health education and support groups, health fairs and screenings for the communities served. CMHP hospitals work with state and local leadership to address community needs and provide healthcare services to the poor and underserved. CMHP is committed to addressing the social determinants of health (SDOH) and social needs for patients and communities throughout our footprint. With a deep understanding of both areas, the team is working systematically across the ministry and in our local communities to ensure we respond holistically and impactfully. CMHP addresses SDOH through its Community Health Needs Assessment (CHNA), Community, investment strategy, advocacy/public policy partnerships, and cross sector solutions. CMHP addresses social needs through SDOH patient assessment, health education/promotion programs, capacity building of local community resources, and closed loop referral system. CMHP hospitals provide programs to improve the physical surroundings and housing in the communities served. Inadequate housing has a negative impact on the health of residents in the area by leading to violence in the neighborhoods. A robust economy positively impacts residents covered by health insurance and improves the capacity of the community to support health services. Additional detail regarding CMHP's community building activities and the promotion of health of its communities can be found in the 2023 Community Health Annual Report prepared by Bon Secours Mercy Health available at https://www.mercy.com/about-us/mission/giving-back. |
| Schedule H, Part III, Line 2 Bad debt expense - methodology used to estimate amount | The provision for bad debts is based upon management's assessment of historical and expected net collections considering historical business and economic conditions, trends in health care coverage, and other collection indicators. Net patient accounts are reduced by an allowance for doubtful receivables based upon the hospital's historical collection experience adjusted for current environmental risks and trends for each major payor source. Significant provision is made for self-pay patient accounts in the period of service based on past collection experience. The hospital's concentration of credit risk related to net patient accounts is limited due to the diversity of patients and payors. Net patient accounts consist of amounts due from governmental programs (primarily Medicare and Medicaid), private insurance companies, managed care programs and patients themselves. Net patient service revenue for services provided to patients who have third-party payor coverage is recognized based on contractual rates for services rendered. The hospital recognizes a significant amount of patient service revenue at the time services are rendered even though it does not assess the patient's ability to pay. As a result, the provision for bad debts is presented as a deduction from patient service revenue (net of contractual provisions and discounts). Amounts recognized are subject to adjustment upon review by third-party payors. For uninsured patients that do not qualify for charity care, the hospital recognizes revenue when services are provided. Based on historical experience, a significant portion of the hospital's uninsured patients will be unable or unwilling to pay for services provided. Thus, the hospital records a significant provision for bad debts related to uninsured patients in the period the services are provided. Any discounts applied to self-pay patients would be deemed either Charity or a contractual adjustment. Bad debt would be based on the balance after the charity or contractual adjustment that is deemed uncollectable following a reasonable collection effort. |
| Schedule H, Part III, Line 3 Bad Debt Expense Methodology | The hospital's financial assistance policy does not permit the cost of patients who are uncooperative or unable to be located to be reclassified from bad debt to financial assistance. The hospital's financial assistance policy requires an application and supporting documentation. The hospital also utilizes available resources (e.g. technology solutions, service organizations, etc.) to obtain such information as credit score to assist in determining whether a patient is presumed eligible for financial assistance pursuant to the BSMH Financial Assistance Policy. Therefore, zero dollars are being reported on Part III, Line 3 as amounts included in bad debt that could be attributable to patients eligible under the hospital's financial assistance policy. The hospital follows the Catholic Health Association of the United States policy document, Community Benefit Program, A Revised Resource for Social Accountability ("CHA guidelines") for determining community benefit. The CHA guidelines recommend that hospitals not include bad debt expense as community benefit. |
| Schedule H, Part III, Line 4 Bad debt expense - financial statement footnote | BON SECOURS MERCY HEALTH'S (BSMH) AUDITED FINANCIAL STATEMENTS DO NOT CONTAIN A FOOTNOTE THAT DESCRIBES BAD DEBT EXPENSE. BSMH ELECTED TO EARLY ADOPT ASU 2011-07. ACCORDINGLY, BAD DEBT EXPENSE IS REFLECTED AS A DEDUCTION FROM REVENUE RATHER THAN AS AN OPERATING EXPENSE. NOTES TO CONSOLIDATED FINANCIAL STATEMENTS, 2. SIGNIFICANT ACCOUNTING POLICIES, (d) NET PATIENT ACCOUNTS AND NET PATIENT SERVICE REVENUE (PAGES 10 & 11) STATES Patient receivables are recorded at net realizable value based on certain assumptions determined by payor class. For third party payors including Medicare, Medicaid, and commercial insurance, the net realizable value is based on the estimated contractual reimbursement percentage, which is based on current contract prices or historical paid claims data by payor. For self-pay receivables, which includes patients who are uninsured and the patient responsibility portion for patients with insurance, the net realizable value is determined using estimates of historical collection experience. These estimates are adjusted for estimated conversions of patient responsibility portions, expected recoveries and any anticipated changes in trends. |
| Schedule H, Part III, Line 8 Community benefit & methodology for determining medicare costs | The hospital follows the Catholic Health Association of the United States policy document, Community Benefit Program, A Revised Resource for Social Accountability ("CHA guidelines") for determining community benefit. The CHA guidelines recommend that hospitals not include Medicare losses as community benefit. The hospital's cost accounting system was used to determine the Medicare amounts in Part III. |
| Schedule H, Part III, Line 9b Collection practices for patients eligible for financial assistance | Patients known to qualify for charity care or financial assistance are not sent to a collection agency. The organization repeatedly offers patients access to financial help during their hospital stays and after, as well as with each billing notice. Bills are sent to a collection agency as a last resort and only: when patients have the ability to pay some portion of their healthcare expenses but refuse to do so; when patients refuse to work with the organization to determine if they qualify for free or discounted care via federal, state, local or hospital assistance programs; when the organization is unable to locate the patient or person responsible for the bill. Patients that are presumed to be eligible for financial assistance based on individual life circumstances will be provided 100% financial assistance. Patients determined to have presumptive financial assistance eligibility will not be required to meet income criteria, asset eligibility criteria, or fill out a financial assistance application. CMHP utilizes available resources (e.g. technology solutions, service organizations, etc.) to obtain information such as credit scores to assist in determining a patient's presumed eligibility. |

Community Mercy Health Partners' (CMHP) primary service area includes Clark and Champaign counties, which have a total population of approximately 175,000. The residents of the CMHP primary service area are generally older, poorer and have worse health statistics than state and national averages. Unemployment has also risen in our community during the last several years, and we serve a growing number of uninsured and underinsured patients. CMHP owns two of the three hospitals in this geographic area. The third hospital (not owned by CMHP) is a limited-service for-profit hospital that specializes in outpatient and short-stay surgery, and it does not have an emergency department. Springfield City Proper is predominantly a manufacturing community with jobs still rooted in that industry today. As a county the primary demographics are comprised of residents who are: White, Non-Hispanic (84.1%), African American (8%), Hispanic (3.4%) and Two or more races (3.6%). 47% of our residents are married while 29% have never married or are Divorced at 13.7%. 45% of our married households have families while 14% are Single, female parent families. 28.9% of our residents are individuals living alone. Clark County has higher than average health risk factors and higher averages of individuals with disabilities, sexually transmitted diseases, and chronic diseases. Additionally, babies in Clark County are more likely to be born below average birth weight and pre-term in comparison with the state averages. Nearly 20% of Clark County women who are pregnant smoke during their pregnancy and only about 66% of Clark County women receive adequate (or better) prenatal care. Champaign County is home to the City of Urbana. Access to care is an issue here as in many rural counties, with low numbers of primary care, mental health, and preventable hospital stays are higher than the Ohio average. The percentage of adults who smoke and the number of poor physical and mental health days are also higher than the U.S. and Ohio averages. Champaign County has community strengths, as well. Among these strengths are their socio-economic standing with positive high school graduation rates aligned with the state average, lower unemployment than the state average, and higher median household income than the state average. This also translates to generally lower behavioral risk factors across the board.
