Schedule H, Part V, Section B, Line 5-South Peninsula HospitalSouth Peninsula Hospital conducted a community health needs assessment (CHNA) for the southern Kenai Peninsula in 2023, in cooperation with MAPP of SKP Health Coalition. This CHNA was conducted in compliance with IRS Section 501(r), and identified health needs of the residents of the South Kenai Peninsula Hospital Service Area of the Kenai Peninsula Borough. The final assessment was adopted by the SPH Board of Directors on June 28, 2023, and made available to the public on the organization's website www.sphosp.org free of charge. The complete CHNA report can be found in the separate document link titled CHNA on the South Peninsula Hospital website www.sphosp.org. Numerous presentations were made of the report findings from June through October, including at meetings of the hospital's Board of Directors, Homer City Council, SPH general hospital staff, SPH general medical staff, Rotary and MAPP of the Southern Kenai Peninsula local community health coalition made up of twelve partnering local agencies representing the eight dimensions of wellness
Schedule H, Part V, Section B, Line 11-South Peninsula HospitalIn our efforts to address mental health needs in the last year, SPH hired a Mental Health Nurse Practitioner and Counselor via a state behavioral health grant as part of a plan for integrated behavioral health for women at Seaside Women's Care; and recruited an additional full-time Psychiatric Mental Health Nurse Practitioner at our outpatient Serene Waters Behavioral Health, increasing the number of full-time psychiatric providers there to three. We continued to address youth needs by participating in Planet Youth Homer, a data-driven model based on the Icelandic Prevention Model, to help identify and address youth needs across the community. We improved our incident stress debriefing program and added contracted positions for a staff chaplain. SPH continued to address the finding of lack of affordable housing by breaking ground on a 26-unit housing complex, offering efficiency, one bedroom and two bedroom units for SPH traveling nurses, visiting MDs and as a launch pad for new hires to the community. We sent representatives to the local community housing forum, our health coalition coordinator served on a borough-wide task force to address housing solutions, and staff contributed to the City's comp plan revisions via meeting attendance and interview. SPH continued to address substance use, misuse and abuse by supporting Megan's Place Needle exchange by providing a free location, advocacy assistance, and support. We Have a representative on the board of Kachemak Bay Recovery Connection, the local nonprofit with a mission of recovery ready community. SPH provides support as part of an inkind match for state funding and provides administrative support for additional grant acquisition and management. We hopes that the adoption of Epic electronic health record ensuring all staff will have access to up to date resources to assist those needing connection to addiction care upon discharge of ER and Acute Care. We continued with community trainings on overdose response, implementing a protocol to allow staff to distribute Narcan kits to patients and community members; training Acute Care staff on Narcan administration and kit distribution procedures, and provided overdose response trainings at numerous community events and highly popular music festivals. For the finding of lack of childcare, SPH finalized renovations of an existing building to be an SPH employee childcare center. We hired and trained several staff, purchased furnishings and equipment, developed curriculum and policies and applied for state licensure. Three safe-sitter classes were offered throughout the year, including on in the nearby Russian village. SPH staff and MAPP staff were active in a local childcare solutions workgroup, and sponsored the first "networking" breakfast that brought together childcare providers across the service area. In responding to the need to improve physical health of local residents, we contracted a social media manager to do more regular health screening promotions and wellness tips; Worked on a plan to merge OB/GYN and Midwifery as one clinic, in one location. Set to move in July 2025. We increased marketing for the under-utilized Family Care Clinic and walk in laboratory; we opened the Renew Clinic, staffed by a full-time plastic surgeon, and dedicated care team, to expand access to specialized physical health services in the community; we added a full time Nurse Practitioner and Nurse to the Seaworthy Functional Medicine team and started promoting the "Medicare Wellness Visits" at Homer Medical Center. We continued to support access to vaccines, offering free vaccines at health fair blood draws, health fair and numerous times at the local food pantry. We integrated clinic data with state vaccine tracking to reduce need for manual entry, and provided "roving" vaccine stations with the hospital for employee ease of vaccinations. We added the use of video to social media to share pertinent messages from subject matter experts related to health screenings. Examples include colonoscopy screening, heart health, kidney health, etc. And SPH sponsored video health educational segments at local movie theater pre-movie screenings. We hired a second full time nutritionist, and launched a free weekly community walking group at the SPARC to support physical activity and wellness for all community members. To address the growing need of an aging population, we continue with project planning for infusion clinic expansion, finalizing the drawings and sponsoring community meetings; we opened plastic surgery clinic and recruited dermatology; Launched an annual senior-centered wellness fair to improve access to health education, screenings, and community resources. SPH expanded collaboration with the Senior Center to better coordinate services and strengthen supports for older adults in the service area. SPH sponsored Senior Day at the Kenai Peninsula State Fair to increase outreach and health messaging to the senior population. In responding to barriers to care, SPH acute care nurses initiated data collection as part of CMS Social Determinants of Health with all hospitalized patients. We launched a media campaign to promote our financial assistance program, and finalized guidelines for a Patient Assistance fund to help outpatient travel with free taxi vouchers as needed. We continued a relationship with Homer Kachemak Bay Rotary to offer greatly reduced blood draws as part of annual health fair, with 714 individuals getting reduced tests, and 400 people attending the one day educational fair. We updated campus and outpatient building signage to improve wayfinding and enhance the overall patient experience. To address social isolation, our Community Health & Wellness offerings included a community wide steps challenge, the addition of free weekly walks, and the expansion of Wellness Wednesdays. We expanded our volunteer program and the developed of a companion program for our swing bed patients. SPH regularly sponsors local programs and offerings that make for robust social offerings in the community. SPH continued to address the worker shortage by hosting high school career tours, the University of Alaska healthcare boot camp, and consolidating HR efforts to recruit and serve. The addition of childcare and housing will be a big attraction for new employees.
Schedule H, Part V, Section B, Line 16j-South Peninsula HospitalFinancial Assistance information is provided on patient statements and as financial navigators call patients to discuss bills, financial assistance program information is shared. SPH also offers free consults through the financial navigators for seniors accessing and signing up for Medicare. SPH's financial aid counselor office is located in the main entrance of the hospital for easy access and financial aid counselors are represented at community events.

Schedule H, Part III, Section A, Line 4Bad Debts and charity care are subtracted from patient service revenue to arrive at net patient revenue. Percentage of self-pay write-off's that are charity care times the cost to charge ratio to get the true cost.
Schedule H, Part III, Section B, Line 8This data was obtained from the CMS website on our Provider Statistical & Reimbursement Report for the tax year. We believe the shortfall in line 8 should be considered community benefit due to the provision of care to Medicare patients regardless of their ability to pay. Further the hospital is not allowed to collect from the contractual write-off's from patients despite the deficit in the cost to provide the services to this population.
Schedule H, Part III, Section C, Line 9bThe facility has a debt collection policy. The policy allows for any funds sent to bad debt that are eligible for financial assistance to be pulled back from a collection agency.
Schedule H, Part VI, Line 2South Peninsula Hospital uses a collaborative effort within state and local agencies and also community members and focus groups to identify the community health needs.
Schedule H, Part VI, Line 3The hospital employs 5 financial navigators that are available to meet with patients to discuss bills, payment plans, and financial assistance options. The counselors can also assist patients with completing the charity care application. Additionally, a copy of the FAP is available at all registration areas, displayed prominently in the emergency check in area and waiting rooms, and published on our website.
Schedule H, Part VI, Line 4The hospital serves the Southern Kenai Peninsula Service Area located in the Kenai Peninsula Borough. That service area encompasses several communities in South Central Alaska to include Anchor Point, Diamond Ridge, Fox River, Fritz Creek, Happy Valley, Homer, Kachemack City, Nanwalek, Nikolaevsk, Ninilchick, Port Graham, and Seldovia for an estimated population of 20,000.
