Part V, Section B, Line 5: The assessment was achieved through the co-leadership of VVV Consultants LLC, McPherson County Public Health along with input from other county hospitals, Lindsborg Community Hospital and Mercy Hospital in Moundridge. The McPherson County CHNA process included gathering opinions from the public and area sources through use of a survey and secondary research to uncover historical community health statistics for the primary service area. A survey instrument comprised of 16 questions was developed by the leadership of the participating hospitals, the public health department and VVV Consultants LLC. The electronic survey was made available through the county through various means including media, social media, hospital websites, direct email, and was publicized through said sources as well as newspaper and radio. A town hall and focus group was held to provide input regarding the results of the survey. Attendees included representatives from health care, law enforcement, education, civic leadership, public health, local industry, senior services and the Chamber of Commerce, among others. A total of 228 survey responses were received with each community in the county represented in the respondent group. McPherson Hospital, Inc. Part V, Section B, Line 6b: McPherson County Health Department McPherson Hospital, Inc. Part V, Section B, Line 11: The annual update below pertains to the CHNA and implementation strategy completed for the 2021 tax year (fiscal year 06/30/2022), which is the third annual update for the three-year cycle.This update reflects progress on the Community Health Needs Assessment (CHNA) adopted by the boards of the three McPherson County, Kansas hospitals in June 2022. The McPherson Hospital-specific Implementation Strategy was adopted by the McPherson Hospital Board of Trustees on October 24, 2022. A status update for each identified initiative during the July 1, 2024 through June 30, 2025 reporting period is outlined below.It is important to note that limited resources can make implementation of certain strategies challenging without dedicated funding. While maintaining a strong focus on managing operating expenses, the hospital continues to experience variable and unpredictable revenue streams.1. Mental Health IssuesThe Community Mental Health Task Force meets quarterly to review current statistics, emerging trends, and collaborative strategies to better serve the community. Impact is monitored as activities, initiatives, and inter-agency approaches are developed to support appropriate patient care.The Emergency Department continues its contract with a third-party provider offering one-on-one observation for patients requiring immediate mental health support and possible transportation. This service helps maintain a safer environment for patients, staff, and law enforcement, while allowing medical teams and first responders to remain available for other community needs. This resource may be utilized for admissions from other departments as needed.McPherson Hospital continues to utilize the Columbia Suicide Severity Rating Scale (CSSRS) in the Emergency Department to assess suicide risk. The screening is completed for all ED patients and hospital admissions. Initial tracking demonstrated 100 percent compliance, and ongoing spot checks are conducted to ensure continued adherence.Immediate debriefings are conducted following emotionally challenging events to assess staff well-being and provide support. The Director of EMS and the Safety Officer have educated nursing directors and house supervisors on effective debriefing practices. Debriefings occur after codes, requested traumas, and other qualifying events. Documentation is reviewed by the Safety Officer and the Executive Director of Nursing and Acute Care Services, with referrals to additional resources, including the Employee Assistance Program, offered as appropriate.The hospital continues to offer an internal Employee Assistance Program focused on supporting employee well-being while also increasing awareness and understanding of mental health issues. The program is introduced during new employee orientation and promoted as a confidential resource for a wide range of personal and professional challenges. While usage data is not tracked due to confidentiality, leadership engagement indicates the program is being utilized as intended.2. Drug AbuseThe Community Drug Abuse Taskforce, which met quarterly prior to the COVID-19 pandemic, has not reconvened due to ongoing logistical challenges. This initiative remains part of the hospital's implementation strategy should the group re-form in the future. McPherson Hospital continues to support community drug abuse education efforts, including collaboration with local law enforcement.The hospital remains fully functional in its use of the Kansas Tracking and Reporting of Controlled Substances (K-TRACS) database within the Emergency Department. Integration with the electronic health record allows providers to review prescribing histories and make informed clinical decisions regarding controlled substances.Based on ongoing evaluation of patient care, a formal non-narcotic order set has not been deemed necessary. Providers continue to use clinical judgment and remain mindful that narcotics are not always the safest or most appropriate pain management option.McPherson Hospital remains 100 percent compliant with the State of Kansas requirement for electronic prescribing of controlled substances. Waivers for printed prescriptions are used only in the event of system downtime.3. ObesityThe hospital continues to support community fitness and wellness initiatives, impacting hundreds of participants annually. During this reporting period the hospital co-sponsored select local fitness events and continued to provide wellness programming for staff. Hess Fitness Center (HFC) staff teaches a weekly class at the Cedars living facility in McPherson as well as 4 classes per week at HFC that focuses on balance, mobility, functional exercise as well as mental and social skills.The hospital supported virtual fitness and wellness efforts encouraging families to stay active together. Educational messaging was shared through social media, highlighting the importance of physical activity and heart health.Free weekly yoga classes continue to be offered at the hospital's fitness center on Tuesdays. Participation averages approximately 10 to 12 individuals per week. These classes support flexibility, balance, stress reduction, and overall wellness.Validated food insecurity screening questions are incorporated into screenings for all inpatients on Med/Surg and ICU. While food insecurity affects an estimated 11 percent of McPherson County residents, recent clinical priorities have emphasized malnutrition awareness. Screenings are supported through physician and dietitian consultations, and information on free community food resources is displayed in exam rooms throughout the clinics.Beginning in early 2025, our Registered Dietitian launched a "Healthy Habits" class focused on practical, sustainable lifestyle changes. The class is designed to provide education on implementing easy, attainable habits related to nutrition, meal planning, physical activity, sleep, and overall wellness. The goal is to equip participants with realistic tools they can apply in daily life to support long-term health and weight management. McPherson Hospital, Inc. Part V, Section B, Line 16j: We assist applicants in filling out the form. 1 1 - McPherson Medical & Surgical Associates 1000 Hospital Drive McPherson KS 67460 Physician clinic, family & internal medicine, surgery and hospitalists Part I, Line 7g: Subsidized health services community benefit expenses include $9,531,141 of expenses attributable to physician clinics that have net operating losses. Part III, Line 4: See Note B (page 10) of the audited financial statements regarding the allowance for implicit price concessions. Part III, Line 8: Amounts were obtained from the cost report filed with Medicare for the period ending June 30, 2025. The total amount of shortfall between Medicare revenue received and relating allowable costs is considered community benefit. Part III, Line 9b: Patients who have qualified for financial assistance are considered qualified for six months. Updated information must be provided for any subsequent account balances. Patients who have qualified for partial financial assistance are to make payment arrangements for any remaining balance according to the policies that apply to other patients. Part III, Line 3: McPherson Hospital, Inc. is unable to determine the amount of implicit price concessions (bad debt expense) attributable to patients eligible under its charity care policy for the year covered by this return. Suggested methods for estimation were either not in place or proved to be unreliable. For example, the number of financial assistance applications distributed but not returned was not tracked. Using demographic data to estimate an amount did not provide reasonable results. Rather than provide an estimate that cannot be substantiated, McPherson Hospital Inc. has chosen to report zero dollars as implicit price concessions (bad debt expense) attributable to charity care patients. Part III, Line 2: The Hospital's implicit price concessions (bad debt) at cost was determined by applying an overall ratio of cost to charges. Part VI, Line 2: McPherson Hospital, Inc. does not have a formal community needs assessment process, with the exception of the process utilized for the CHNA. Part VI, Line 3: All self-pay patients are screened by a contractor for eligibility under federal, state, or local government programs. Notice of the Hospital's financial assistance program is posted in patient registration areas and on the Hospital's website. The availability of financial assistance is also communicated to patients with the first billing statement and at any time during the collection process if an inability to pay is expressed by the patient/guarantor. Part VI, Line 4: McPherson Hospital, Inc.'s primary service area is McPherson, Kansas, a rural community of approximately 14,068. According to 2025 census information, the per capita income for McPherson was $34,490. The percentage of individuals below poverty level was 11.9 percent. For the year ending June 30, 2025, 12 percent of the Hospital's inpatient admissions were Medicaid recipients and 6 percent were uninsured. Larger hospitals in Hutchinson, Salina, and Newton also provide services to the residents of McPherson. The distances between these facilities and McPherson Hospital, Inc. are approximately 23, 29, and 28 miles respectively. There are no federally-designated medically underserved areas or populations in McPherson. Part VI, Line 5: McPherson Hospital, Inc.'s governing body is comprised of individuals who reside in the primary service area. All Board members, with the exception of the CEO and two physicians, are not employed by the Hospital. Annually, all trustees sign a conflict of interest statement and disclose any potential conflicts of interest. With limited exceptions for hospital-based services, the Hospital extends medical staff privileges to all qualified physicians in the community.
