All assistance reported in Part III is provided directly to individuals served by the organization's programs.Eligibility criteria for each form of assistance are established in advance in writing, based on the requirements of the funding source supporting the program together with program guidelines approved by management. Assistance is limited to individuals who meet those criteria and who participate in, or are referred through, the organization's programs.Each request for assistance is documented on an intake or application form completed by program staff, recording the recipient's eligibility, the type and amount of assistance, and the program and funding source to which it is charged. Requests are reviewed and approved by a program manager before any disbursement is made.Disbursements are processed through the organization's accounting system, coded to the applicable program and grant, and supported by the approved documentation. Noncash assistance consists of gift cards, which are tracked on a log recording card value, date issued, and recipient acknowledgment of receipt.
