organization's staff conducts desk and on-site program, file, fiscal, and civil rights monitoring of all grant recipients at least annually. A report is provided to each grant recipient noting all findings, observations, and best practices. Grant recipients are required to respond and provide corrective action for all reported findings. The Wisconsin Department of Workforce Development and the U.S. Department of Labor also conduct on-site as well as desk monitoring of most grant recipients. Form 990, Part VI, Section B, line 11 The prepared Form 990 is reviewed and approved by the members of the organization's governing body before the return is filed with the IRS. The distribution of the return and the vote to approve are generally conducted electronically. Form 990, Part VI, Section B, line 12c All directors, officers, and employees are covered by the organization's conflict of interest policy. Article XII of the organization's bylaws contains the organization's conflict of interest policy. The policy calls for full disclosure by directors with discussion by the governing body in the event a conflict of interest is disputed. After discussion by the governing body, the chair has the authority to make the final determination of whether a conflict of interest exists. In addition, all directors annually complete a "Declaration of Affiliations" statement that is designed to identify interests that could give rise to conflicts. The completed statements are kept on file with the executive director. The organization's employees also complete a statement disclosing interests that could give rise to conflicts. These statements are updated annually and are also kept on file with the executive director. The executive director has the authority to make the final determination of whether a conflict of interest exists with regards to an employee. If an interest involves the executive director, the chair will make the final determination. The executive director is charged with regularly monitoring interests of the directors and employees that could give rise to conflicts. Any director with a conflict of interest is prohibited from participating in the governing body's discussion on the transaction unless directly asked by the chair in the course of the discussion. A director must also abstain from voting on any motion is which he/she has a conflict of interest. Form 990, Part VI, Section C, line 19 The organization makes its governing documents, conflict of interest policy, and financial statements available to the public upon request. These documents are also posted on the organization's website.
