Ct MRI Imaging services Carilion Clinic, the sole member of the Hospital, includes the community benefit report for the Hospital in a system-wide annual report. Ratio of cost-to-charges was used to calculate the expense. Bad debt expense subtracted from total expenses for the calculation is 3,900,839. The financial statements do not include a footnote on bad debt. The audited financial statements include a provision for bad debt. The provision is calculated based on historical bad debt experience and managements evaluation of the collectibility of the accounts receivable. Balances are written off after all reasonable collection efforts have been made. The bad debt expense reported on line 2 was calculated using the cost-to-charges ratio applied to the provision for bad debt reported in the financial statements. The estimated amount of bad debt expense, reported on line 3, attributable to patients eligible for charity care was determined using external credit reporting data to identify the patients that were eligible and applying the cost-to-charges ratio to the charges of the patients so identified. Sufficient information was not available to make a definitive determination of their eligibility. Patient account balances are reduced by discounts allowed and payments. Medicare allowable costs are determined from the Medicare cost report using the cost-to-charges ratio. The Hospital does not consider a Medicare shortfall as a community benefit. When an uninsured self pay patient registers for medically necessary services, the patients account is electronically marked as being potentially eligible for charity care. Carilion Clinics Eligibility Assistance Service EAS screens the patient to determine if charity care is appropriate. The patient must apply for all entitlement programs for which he or she may be eligible i.e. Medicaid, Social Security Disability, etc. EAS assists the patient with the applications. If the patient does not qualify for one of the programs then the Federal Poverty Guidelines FPG are used to determine eligibility for charity care. A patient whose account balance is less than 1,500 and is at or below 100 of FPG is automatically given a 100 discount of their account. For balances greater than 1,500 the patient must complete a Financial Assistance Application and provide appropriate documentation. A sliding scale of discounts is available for patients with incomes up to 400 of FPG. An underinsured patient can apply for charity care. Applications are available at all Patient Access/Registration points, offices of EAS, Carilion Clinics Internet website or by calling for an application. Carilion Medical Center uses demographic information and data on patients treated to determine community health needs. Additionally, information from public databases compiled on the Virginia Atlas of Community Health website is reviewed for the entire geographic area served by Carilion Medical Center. A community health needs assessment is scheduled for completion in compliance with recent guidelines related to quantifying community benefit. The charity care policy is posted in all patient registration areas. At the time of registration all uninsured self pay patients are given a brochure explaining the charity care policy and how to apply. Also, see comments for Part III line 9b. Carilion Medical Center, comprised of two hospitals, Carilion Roanoke Memorial Hospital and Carilion Roanoke Community Hospital, is located in Roanoke, Virginia. Roanoke is a 1,185 square mile valley located in southwest Virginia near the Blue Ridge and Allegheny Mountains. Carilion Medical Center primarily serves the almost 250,000 residents of Roanoke City and County, Salem, Botetourt County, Craig County and the Town of Vinton. Approximately 20 of adults under age 64 and 10 of children who live in the Roanoke Valley are uninsured. Additionally, 11 of residents live below the poverty level. Carilion Roanoke Memorial Hospital is one of the largest hospitals in the state of Virginia with 703 beds and an additional 60-bed Neonatal Intensive Care Unit and pediatric emergency department. With a level 1 trauma center and childrens hospital, complete with pediatric emergency room, Carilion Roanoke Memorial Hospital treats residents throughout southwest Virginia. In addition to offering high-tech services, the hospital is also home to ten residency programs and nine fellowship programs affiliated with the University of Virginia. Carilion Medical Center hosted the Back to School Blast, an event coordinated by the Rescue Mission of Roanoke Valley and other local non-profits and corporate sponsors to provide backpacks, school supplies and free haircuts for low-income children. Grant funding was provided for several projects including Smith Mountain Good Neighbors to support a summer day camp program for children in Franklin and Bedford Counties who live near or under the poverty level and Greater Roanoke Transit Company for a free trolley service for the community that operates in a continuous loop with stops that include Carilion Medical Center and Downtown Roanoke. Goals include reducing the carbon footprint and enhancing the local economy by providing easier access to downtown. Carilion Medical Center collaborated with local school systems, Goodwill Industries of the Roanoke Valley, the Department of Rehabilitative Services, Blue Ridge Behavioral Health, Blue Ridge Independent Living Center and local parent representatives to offer Project SEARCH. The goal of this one year high school transition program is to provide employment and educational opportunities for individuals with significant disabilities and to find long term employment in skilled positions for its participants. All eleven participants were placed in positions within Roanoke based Carilion Clinic affiliates and the program finished with an overall attendance rate of 96, demonstrating the dedication of these students to finding a job. Participants gained more independence, learned vital job skills and self-advocacy. More than 2,700 children learned about healthcare careers through Caring Careers, an educational program for school aged children provided onsite at local schools and community centers. Carilion Medical Center provides support of the local EMS crews through replenishment of medications used for patient care or product with an expired use by date. With a teen pregnancy rate in Roanoke City of more than 2.5 times the statewide average, Carilion Medical Center provides a teen pregnancy prevention program that partners with local schools to provide educational programs and assist with administration of risk behavior assessments and presentations of results. Carilion Medical Center staff worked with Childrens Trust of Roanoke Valley to create the SW Virginia Alliance for Safe Babies, a coalition focused on awareness and education to reduce infant injury and deaths due to Abusive Head Trauma and SIDS. Financial support was provided to Childrens Trust of Roanoke Valley for their regional program that provides child friendly interview room and forensic interviewer to perform interviews of children to be used by multi-disciplinary team investigating alleged child abuse cases provides training for multi-disciplinary team members, and coordination of referral sources for abused children and their family. Carilion Medical Center serves all patients regardless of their ability to pay. The Hospitals governing Board is elected annually and the majority of members are neither employees nor contractors of the Hospital. Medical staff privileges are extended to qualified providers. In addition to clinical care, the hospital works to achieve its mission through the education of health professionals and the community. Any surplus funds are reinvested in new technology, clinical initiatives, education and charitable efforts. This includes providing free, discounted and subsidized care as well as critical medical services that operate at a loss. Carilion Roanoke Memorial Hospitals Emergency Department has specially trained nurses who deliver immediate crisis intervention to victims of sexual assaults and/or abuse. They offer victims a unique, thorough and objective medical and legal examination, comprised of both a physical examination and forensic evidence collection and documentation. Clinical Forensic Nurse Examiners are the link between the judicial system, law enforcement, community resources, and the laboratories that process the evidence. Carilion Medical Center provides financial and in-kind support annually to the Mission of Mercy Dental event in Roanoke. More than 1,100 residents of southwest Virginia received free dental services from volunteer dentists and Carilion Medical Center employees provided free medical screenings including health and medication histories, blood pressure, blood coagulation rates, and blood glucose levels for all patients. Carilion Medical Centers community health education department serves as host of the local chapter of the National Safe Kids Coalition and provides education on injury prevention to the community and other providers. In 2010, 311 individuals received car seat education or car seat safety checks at 46 events. In addition, Carilion Medical Centers Safe Kids Coalition coordinator provided training, through a program offered by the Virginia Department of Health on proper car seat installation for other health and safety providers, to 197 during 32 training events free of charge. Carilion Medical Center provides education to the public about health risks and steps that can be taken to improve health. Approximately 33,000 people were touched during more than 450 community events that include regularly scheduled health screenings such as blood pressure, blood glucose and cholesterol as well as seasonal stroke, vascular, prostate and facial sun damage detection screenings. Physicians on Foot, a program to encourage physical activity was introduced to the community in 2010 with a local primary care physician leading participants on a two mile walk along a local greenway near the Roanoke River every Saturday morning. In-kind support was provided to Roanoke Adolescent Health Partnership, a unique collaboration of the Roanoke City Health Department, Roanoke City Public Schools, Roanoke Redevelopment and Housing Authority, and Blue Ridge Behavioral Healthcare, to provide clinical services at local public schools. Carilion Medical Center physicians provided clinical services for 600 visits during 2010. Financial and in-kind support is provided to Child Health Investment Project CHIP of Roanoke Valley for operational expenses related to their early childhood home visiting program as they pair low-income children, ages birth to kindergarten-entry, with a community health nurse and family case manager for health care coordination help accessing needed medical services, assistance with the management of chronic conditions, and preventive services like fluoride dental varnish developmental education, kindergarten preparation and regular child assessment and monitoring. Grant funding was provided to several organizations including Voices for Virginias Children for a Campaign for Childrens Mental Health to transform the childrens mental health system in Virginia Rx Partnership for operations of the public-private partnership that solicits free drugs in bulk from participating pharmaceutical companies and arranges for their distribution to licensed pharmacies serving uninsured clients Mental Health America of Roanoke Valley to increase access to mental health services for adults with no health insurance coverage or inadequate insurance for mental health care The Rescue Mission of Roanoke for capital renovations to their clinic for the homeless that provides medical, psychiatric and dental health services and Refugee and Immigration Services for a program to improve refugee and immigrant health and understanding of American health care practices. Carilion Medical Center is wholly owned by Carilion Clinic, a not-for-profit healthcare organization serving nearly one million people in Virginia through hospitals, outpatient specialty centers and advanced primary care practices. Led by multi-specialty physician teams with a shared philosophy that puts the patient first, Carilion is committed to improving health outcomes for every patient while advancing the quality of care through medical education and research. To advance the coordination of care and quality, an electronic medical record has been implemented in all Carilion Clinic affiliated hospitals and physician practices, allowing physicians to easily and efficiently collaborate through the use of shared patient records. Additional improvements in medical programs and services include a new nationally certified Stroke Alert protocol that speeds lifesaving care to stroke patients the introduction of hypothermia therapy to southwest Virginia for the treatment of cardiac arrest patients certification by the National Committee for Quality Assurance NCQA of 11 Carilion Clinic Family Medicine practices as Patient-Centered Medical Homes, a new model of care designed to keep patients healthy and prevent illness and the introduction of MyChart, an online tool that gives patients secure access to their health information. To advance education of health professionals, Jefferson College of Health Sciences, within Carilion Medical Center, is a professional health sciences college offering the Master of Science in Nursing, Physician Assistant and Occupational Therapy and 10 baccalaureate and associate healthcare programs. As one of few private healthcare colleges in the Southern United States, the students have demonstrated excellent licensure pass rates in professional programs. Approximately 1,000 undergraduate students and 80 graduate students are enrolled annually with 87 receiving some type of financial aid. Through a unique public-private partnership, Virginia Tech and Carilion Clinic recently opened the Virginia Tech Carilion School of Medicine and Research Institute in Roanoke. The School welcomed an inaugural class of 42 students in August 2010. The Research Institute opened in September 2010 with a mission to advance the health of the nation and the world by making major scientific advances in understanding human health and developing new diagnostics, treatments and cures for disorders that limit the quality of life and the realization of full human potential. This is done through biomedical and behavioral research. An additional benefit to the community is Carilion Clinics economic contribution to the region. As the areas largest employer, jobs are provided for nearly 11,000 residents of the region, with a payroll of more than 625 million in 2010. With respect to Carilion Clinic, the Compensation Committee reviews the compensation of the President and Chief Executive Officer, Executive Vice Presidents, which include the Chief Financial Officer, the Chief Medical Officer and the Chief Operating Officer.
