Two projects are included: (1) Portable Dentistry - Over the past 2 years, more than 10,000 children have been reached and more than 45 Early Education and Care Organizations and 4 school systems continue to use the BEST methodology; (2) Fluoride Varnish Treatment ("FVT") Program - a key partner in statewide effort to secure Medicaid reimbursement for this preventive dental measure, BEST is leading as one of two entities, which have developed a specific model for translating this policy into action. YEAH IS ENGAGED IN AN EFFORT TO INFLUENCE POLICIES THAT IMPACT YOUTH REPRODUCTIVE HEALTH AND EDUCATION in five areas: (1) Advocacy-Creating a unified voice for those working on adolescent reproductive health; (2) Data collection- Collection and dissemination of local reproductive health indicators for youth; (3) Community awareness-An active grassroots campaign to give parents the tools they need to help their youth avoid teen pregnancy; (4) Public policy-Eliminate disparities in adolescent reproductive health outcomes that disproportionately impact vulnerable populations and (5) Networking-Create a network for community agencies working on youth health and education. 314337 10000 THE LIVE WELL SPRINGFIELD COALITION IS A COLLABORATIVE EFFORT TO ADDRESS CHILDHOOD OBESITY. Three projects include: (1) Mason Square Food Justice Project-a multi-year effort to address the health inequities in Black populations; (2) Farm-to- Preschool and Families-a pilot program for nutrition education and food access, including 9 early education and care providers in the research and testing phase; and (3) FIT+ -a 3-year diabetes prevention project targeting center-city neighborhoods. The coalition makes local grants to neighborhood-based teams at a grass-roots level with the aim of increasing community mobilization and provides training and technical assistance to these groups as they engage in practical health promotion activities and address larger public policy issues. 70168 The PVAC is a collaborative effort to bring about an alignment of local interests and initiatives to address asthma. The following is a typical partnership fashioned by the coalition: PVAC, BMC HealthNet Plan and Baycare Health Partners are working together using Healthy Homes principles and community health workers ("CHWs") and nurses to identify, remediate and prevent of environmental exposures in the home that trigger asthma.
